    The Relationship between Passive Smoking and Caries Experience in Children Aged 1-12 Years and Influencing Factors

    Objective: The aim of this study was to evaluate the relationship between passive smoking by parents and dental caries in children aged 1-12 years and the factors affecting this relationship. Materials and Methods: The examination data of 900 children aged 1-12 years who were admitted to Zonguldak Bülent Ecevit University, Faculty of Dentistry, Department of Pediatric Dentistry between December 2020 and May 2021 for various reasons and underwent routine examinations and met the study criteria were retrospectively analyzed. Sociodemographic characteristics, passive smoking status and dmft/DMFT scores of the children were recorded. The relationship between passive smoking status and dmft/DMFT scores and the factors affecting this relationship were analyzed. Results: The results of A statistically significant correlation was found between passive smoking status and dmft/DMFT scores of children (p=0.000). At the same time, a significant relationship was found between passive smoking and income level (p0.05). Conclusion: Passive smoking is thought to be a risk factor in caries formation. Therefore, it is important to reduce exposure to passive smoking in order to protect both oral and dental health and general health of children</jats:p

    Türkiye’de Vergi Yükü Ölçümü: Yapısal Eşitlik Modeli Analizi

    Türkiye’de bireyler, genellikle vergi yükünün yüksekliğinden şikâyet ederler. Çalışma, matematiksel olarak ölçülebilen objektif vergi yükü ve literatürde ölçülmesinin oldukça zor olduğu ifade edilen subjektif vergi yükünü ölçmeyi amaçlamıştır. Bu amaç doğrultusunda Yapısal Eşitlik Modelinden yararlanılan çalışmada, objektif vergi yükü üzerinde; gelir düzeyi ve ödenecek verginin, subjektif vergi yükü üzerinde ise vergi adaleti, vergi karmaşıklığı, hükümete güven ve vatandaşlık bilincinin etkili olduğu sonucuna varılmıştır. Çalışma, subjektif vergi yükünün (%58) objektif vergi yükünden (%25, %33) yüksek olduğunu ortaya koyması bakımından literatürdeki çalışmalardan ayrılmaktadır. Bu bağlamda çalışma, literatüre özgün ve önemli katkılar sunmaktadır.</jats:p

    XERISCAPE LANDSCAPE STUDY AT ZONGULDAK BÜLENT ECEVİT UNIVERSITY

    Xeriscape is a landscape design approach used in regions where water is limited and vegetation must adapt to these conditions. This approach, which is based on water conservation and low maintenance, is of great importance in arid regions or areas where water resources are scarce. Rock gardens offer both aesthetic and ecologically sustainable solutions as an example of xeriscape. In this context, a rock garden project was produced in the garden of the Engineering building in Zonguldak Bülent Ecevit University (BEUN) Farabi Campus. An exemplary planting design project, evaluated within the scope of the transformation of an intensively used area into an xeriscape in order to contribute to the creation of sustainable urban green texture on the campus, was addressed within the scope of climate-compatible applications.</jats:p

    A Holistic Approach to Nursing Students’ Changing Life and Anxiety in the Pandemic: A Descriptive Cross Sectional Study Utilizing Positive Psychotherapy

    The life balance model of positive psychotherapy states that human lives have four dimensions: body, achievement, relationships, and spirituality. When these dimensions are in balance, people are healthy and productive. This study aimed to determine the effects of the COVID-19 pandemic on nursing students’ anxiety levels and life balance. This cross-sectional and descriptive study was conducted with 584 nursing students studying at 3 different universities in Turkey. The participants experienced significant changes in the life balance model’s dimensions of achievement, body, relationships, and spirituality before and during the pandemic. The participants’ anxiety increased with changes in these dimensions. Based on these results, it would be beneficial to support students in order to meet the needs in the life dimensions that students neglect. In addition, it is thought that the life balance model, which is being used for the first time in nursing, can be developed as a tool in the nursing practice.</jats:p

    Dose optimization of piperacillin/tazobactam, cefepime, and ceftazidime for carbapenem-resistant Pseudomonas aeruginosa isolates in Türkiye

    Although CRPA may test susceptible to other β-lactams such as ceftazidime (CAZ), cefepime (FEP), and piperacillin/tazobactam (TZP), reduced potency has been observed. We assessed the adequacy of EUCAST Susceptible (S) or Susceptible Increased Exposure (SIE)/(I) doses for CAZ, FEP, and TZP against CRPA clinical isolates.CRPA isolates were collected from patients at three Turkish hospitals. CAZ, FEP, and TZP MICs were determined using broth microdilution. Monte Carlo simulations were performed to determine the probability of target attainment (PTA) for a free time above the MIC (fT > MIC) targets for various doses of each agent against isolates defined as susceptible. fT > MIC targets were 70% for CAZ or FEP and 50% for TZP. Cumulative fraction of response (CFR) was calculated. Optimal PTA and CFR was 90% target achievement.The percentages of isolates SIE/I to CAZ, FEP, and TZP were 49,8%, 47%, and 31,8% respectively. Reduced potency was noted with 54,1% of CAZ-S isolates having MICs of 4 or 8 mg/L. Of the FEP and TZP-S isolates, MICs at the breakpoint (8 and 16 mg/L, respectively) were the mode with 45,2 and 53,9% of isolates for each, respectively. At an MIC of 8 mg/L for CAZ, the EUCAST standard dose was insufficient (CFR of 85%). 3 h infusions of EUCAST SIE doses were required for 90% PTA at MIC of 8 mg/L and an optimized CFR of 100%. For FEP, the SIE dose of 2 g q8h 0.5 h infusion of was effective (CFR 96%), utilization of an extended 3 h infusion further optimized the PTA at 8 mg/L (CFR 99%). For TZP, the standard dose of 4.5 q6h administered as a 0.5 h infusion was inadequate (CFR 86%). A standard TZP dose with an extended infusion (4.5 g q8h over 4 h) and the SIE dose 4.5 g q6h 3 h infusion resulted in CFRs > 95%.These data support the EUCAST SIE breakpoints for FEP and TZP. To optimize PTA at the SIE breakpoint for CAZ, prolonged infusion is required

    Bankaların Finansal Performans ve Borç Ödeme Gücü Açısından Değerlendirilmesi

    Bu çalışma ile bankalarının finansal performansları ve ödeme güçleri 2021-2023 yılları için TOPSIS ve Bankometer modelleri ile analiz edilmiştir. Araştırma sonucunda banka performanslarının 2022 yılında düştüğü ortaya konulmuştur. Bu düşüşün bankaların finansal varlıkları ile kullandırılan kredilerde bir azalma ve kısa vadeli yükümlülüklerde bir artış nedeniyle ortaya çıktığı görülmüştür. 2023 yılında ise banka performansları tekrar yükselişe geçmiştir. Kur korumalı mevduat uygulaması ve politika faizindeki artışla Türk Lirasına geçiş sürecinin banka performanslarına da olumlu olarak etki ettiği düşünülmektedir. Ek olarak banka performansları açısından en önemli rasyonun Likit Aktifler/Kısa Vadeli Yükümlülükler olduğu anlaşılmıştır. Ayrıca ilgili dönemlerde tüm bankaların borç ödeme güçlerinin yüksek olduğu tespit edilmiştir. Finansal performans ile ödeme gücü arasında tam bir uyumun olmadığı, ancak performans puanı yüksek olan bankaların ödeme güçlerinin de yüksek olduğu çalışmanın önemli sonuçlarındandır.</jats:p

    Hasta Değerlendirmeleri: Halkla İlişkiler Perspektifinden Nitel Bir Analiz

    Amaç: Çalışmanın amacı özel bir sağlık hizmeti sunucusuna başvuran hastalar tarafından hizmet sunum sürecine ilişkin yapılan değerlendirmeleri halkla ilişkiler perspektifinden incelemek ve hasta beklentilerini araştırmaktır. Yöntem: Çalışmada betimsel araştırma yaklaşımı kullanılarak 81 hasta değerlendirmesi nitel veri analizine tabi tutulmuştur. Veri analizinde MAXQDA 2022 yazılımı kullanılmıştır. Veri analiz sürecinde doküman incelemesi yaklaşımı kullanılarak veri ilk aşamada kodlama işlemine tabi tutulmuş sonrasında ise elde edilen kodlar birleştirilerek temalar elde edilmiştir. Bulgular: Hasta değerlendirmelerinin analizi sonucunda 4 tema ve 19 kod belirlenmiştir. Hatalı uygulamalar (işlem, iletişim, gereksiz işlem ve sistem hataları), personel tutum ve davranışları (ilgisizlik, sert davranışlar, hassasiyet göstermeme, işlem sırasında gereksiz davranışlar ve personelin olumsuz konuşma tarzı), tedavi sonuçları (sonucu beğenmeme, kurumu tercih etmeme, tedaviden vazgeçme, güvensizlik, pişman olma ve olumsuz tavsiye) ve tedavi maliyetleri (gereksiz ödemeler, yüksek fiyatlar, fiyat-performans dengesizliği ve ticarethane hissi vermesi) ile ilgili değerlendirmeler belirlenen temalardır. Hasta değerlendirmelerine ilişkin bulgular, hastaların sağlık hizmeti deneyimine ilişkin olumsuz bir tutum geliştirdiğini ve beklentilerinin karşılanmadığını göstermiştir. Bu durum bir yandan hasta açısından karşılanması gereken beklentileri ortaya koyarken diğer yandan ise sağlık hizmeti sunucusu açısından iyileştirmeye açık yönleri ortaya koymuştur. Sonuç: Genel olarak değerlendirildiğinde, ağız ve diş sağlığı alanında işletmecilik perspektifinden hizmet sunum sürecinin yürütülmesi yaklaşımının güçlendirilmesine ihtiyaç duyulduğu söylenebilir. Çalışma bulgularının başta ağız ve diş sağlığı hizmeti sunucuları olmak üzere sağlık hizmeti alanında karar vericiler açısından faydalı olacağı düşünülmektedir.</jats:p

    Inter‐rater reliability in the assessment of consciousness in patients receiving palliative care in intensive care: A prospective cross sectional observational study

    AbstractBackgroundThe Glasgow Coma Scale (GCS) is one of the methods that has validity for evaluating the consciousness levels of patients in the literature and is accepted by health authorities.AimThe purpose of this study was to evaluate the inter‐rater reliability of GCS in intensive care patients receiving palliative care.Study DesignA prospective cross sectional observational study. The study was conducted in a general intensive care unit with 20 beds with patients receiving palliative care. In the unit, 18 nurses worked in two shifts, day and night. Each patient's primary palliative care nurse and two additional researchers were given one minute to independently record the patient's GCS total and subscale scores. All observations were completed within 5 min as there could be significant changes in the patient's GCS score during observations.ResultsA total of 258 assessments were completed. For the GCS total scoring, a moderate agreement was found between palliative care nurses and the first researcher‐observer (49.0%) and also between palliative care nurses and the second researcher‐observer (47.7%). In addition, there was a substantial agreement between the first and second researchers (78.9%) and also between all observers (61.5%) (all p = .001).ConclusionsAlthough there was a near‐perfect agreement between the two researcher‐observers, we found only moderate agreement among all observers (palliative care nurses and two researcher‐observers) in the evaluation of GCS total and subscale scores.Relevance to Clinical PracticeWe found that lack of knowledge and training on the standardized use of GCS is still a problem for palliative and intensive care units. Because of the diversity of patients requiring GCS assessment in palliative care units, refresher training programs and hands‐on workshops on consciousness assessment should be organized regularly for more experienced nurses.</jats:sec
