Abstract PO4-19-04: AXSANA – EUBREAST-3: An international prospective multicenter cohort study to evaluate different surgical methods of axillary staging in clinically node-positive breast cancer patients treated with neoadjuvant chemotherapy
Abstract Background The optimal surgical staging procedure of the axilla in patients who convert from a clinically positive (cN+) to a clinically negative node status (ycN0) through neoadjuvant chemotherapy is still controversial. Widely diverse techniques such as full Axillary Lymph Node Dissection (ALND), Targeted Axillary Dissection (TAD), Targeted Lymph Node Biopsy (TLNB) and Sentinel Lymph Node Biopsy (SLNB) alone are given preference in different international guidelines. So far, no comparative data on the oncological outcome or the morbidity of the different procedures are available. Further research is needed to safely de-escalate the extent of axillary surgery in this patient group. Trial design The AXSANA study is an international prospective cohort study including cN+ patients converting to ycN0 status and treated with different axillary staging techniques according to the standard at their treating institution. The study was initiated by the EUBREAST network. The trial includes patients with cT1-4c tumors, who present initially with axillary lymph node metastasis scheduled for neoadjuvant chemotherapy. According to an amendment in 2020 the inclusion of patients with highly suspicious nodes without confirmation using a minimally invasive biopsy is allowed. All patients converting to ycN0 status undergo follow-up for 5 years irrespectively of the ypN status. Primary endpoints: Invasive disease-free survival, axillary recurrence rate and health-related quality of life (HRQoL). HRQoL are evaluated using four standardized questionnaires (EORTC QLQ-C 30, EORTC QLQ-BR 23, Lymph ICF and SOC-13) at baseline and after 1, 3 and 5 years after surgery. Secondary endpoints are the feasibility and performance of different axillary staging techniques (detection rate, number of removed lymph nodes and association with complications, arm morbidity and quality of life, operating time and use of clinical and economic resources); impact of learning curve, and the detailed mapping of surgical and oncological treatment standards in different countries. The impact on different regional treatment strategies (radiotherapy, ALND) in patients with ypN0(i+), ypN1(mi) and ypN1 is assessed. Current status of the study: 4336 patients from 284 study sites and 26 countries were enrolled in the study between June 2020 and June 2023. Among 3722 patients with a defined surgical concept 1631 women were scheduled for TAD, 1483 for ALND, 469 for SLNB, 25 for TLNB and 109 for other procedures. A target lymph node was marked in 2309 patients, most frequently using clips/coils (1850, 80.1%), followed by magnetic seeds (219, 9.5%), carbon ink (171, 7.4%), radar marker (80, 3.5%), radioactive seeds (4, 0.2%) and other techniques (24, 1.0%). DUYGUSAL ZEKANIN ÖRGÜTSEL İLETİŞİM ÜZERİNDEKİ ETKİLERİ: BELEDİYELERDE BİR ÇALIŞMA
Communication is at the core of human life. Municipalities, on the other hand, are institutions that provide public services and work in direct contact with people living in their regions. With technological developments, it has become inevitable for institutions to improve themselves in terms of the concept of communication. In this context, the development of internal and external relations and communication between employees has made it necessary for the emotional intelligence factor to come into play. In this research, the effects of emotional intelligence on organizational communication were investigated. The universe of the study consists of Kozlu Municipality in Zonguldak province and employees of Zonguldak Municipality. The survey form method was used as the data collection method in the study. The first part of the survey includes the Sociodemographic Form, which includes personal information, the second part includes the Organizational Communication Scale, and the third part includes the Rotterdam Emotional Intelligence Scale. In the study, the effects of the sub-dimensions of the Rotterdam Emotional Intelligence Scale (appraisal of own emotions, appraisal of others' emotions, control of own emotions, control of others' emotions) used as an emotional intelligence scale on organizational communication were evaluated.The study concluded that emotional intelligence has effect on organizational communication. While the sub-dimensions of the emotional intelligence scale, namely, appraising one's own emotions, appraising others' emotions, and controlling others' emotions, were not found to be related to organizational communication, the sub-dimension, controlling one's own emotions, was found to be related to organizational communication.İletişim, insan yaşamının temelinde yer almaktadır. Belediyeler ise kamu hizmetlerine bakan kuruluşlar olmasından dolayı bulundukları bölgelerde yaşayan insanlarla birebir temas halinde çalışmaktadır. Teknolojik gelişmelerle birlikte kurumların kendilerini iletişim kavramı anlamında geliştirmesi kaçınılmaz olmuştur. Bu bağlamda kurum içi ve kurum dışı ilişkilerin ve çalışanlar arasındaki iletişimin gelişmesi duygusal zeka faktörünün devreye girmesini gerekli kılmıştır. Bu araştırmada, duygusal zekanın örgütsel iletişim üzerindeki etkileri incelenmeye çalışılmıştır. Araştırmanın evrenini Zonguldak ilindeki Kozlu Belediyesi ve Zonguldak Belediyesi bünyesindeki çalışanlar oluşturmaktadır. Araştırmada veri toplama yöntemi olarak anket formu yöntemi kullanılmıştır. Anket formunun ilk bölümünde katılımcıların kişisel bilgilerinin yer aldığı Sosyodemografik Form, ikinci bölümünde Örgütsel İletişim Ölçeği ve üçüncü bölümünde ise Rotterdam Duygusal Zeka Ölçeği yer almaktadır. Çalışmada duygusal zeka ölçeği olarak kullanılan Rotterdam Duygusal Zeka Ölçeği’ nin alt boyutlarının (kendi duygularını değerlendirme, başkalarının duygularını değerlendirme, kendi duygularını kontrol, başkalarının duygularını kontrol) örgütsel iletişim üzerindeki etkileri değerlendirilmiştir. Araştırmada duygusal zekanın örgütsel iletişim üzerinde etkili olduğu sonucu elde edilmiştir. The effect of Annona muricata (Graviola) on the prevention of brain damage due to ionizing radiation in rats
In this study, it was aimed to evaluate the effect of ethanol extract of Annona Muricata (AM) leaves in the prevention of brain damage caused by ionizing radiation (IR). This study was conducted in the Experimental Animal Research Unit of a university with 28 adults female Wistar Albino rats. The experimental groups were as follows: Control group (n = 8), AM group (n = 6), IR group (n = 8), AM + IR group (n = 6). In the IR group, astrocyte hypertrophy, microglial reaction and inflammatory reaction levels were significantly higher than the control and AM groups (P < 0.001). Edema was significantly higher in the IR group compared to the control group (P=0.001). The MDA of the IR group was significantly higher compared to the control group and AM group (P=0.031, P=0.006, respectively). The MDA of the AM + IR group was significantly higher than the AM group (P=0.039). Our findings show that histomorphology and oxidant damage caused by IR can be ameliorated using AM, as demonstrated by the comparison of the controls to AM + IR recipients, which showed similar histomorphology and oxidant damage levels
SUÇU ETKİLEYEN SOSYOEKONOMİK FAKTÖRLERİN ROBUST YÖNTEMLER İLE BELİRLENMESİ: AVRUPA BİRLİĞİ ÖRNEĞİ
Toplumlarda güvensizliğe yol açan suç; toplumu ve ülke ekonomisini etkilemekle birlikte, toplumda hem psikolojik hem de parasal maliyetlere yol açabilmektedir. Suç konusu sürdürülebilir kalkınmanın sağlanması için de önem arz etmektedir. Ülkelerde ve kurumlarda barış ve adaletin sağlanması Birleşmiş Milletler’in sürdürülebilir kalkınma amaçlarındandır. Mevcut çalışmada Avrupa Birliğine üye ülkeler için şiddet ve mülkiyet suçlarının sosyoekonomik belirleyicilerinin tespit edilmesi amaçlanmıştır. Bu amaçla uygulamada Robust yöntemler kullanılmıştır. Bu yöntemler genellikle aykırı gözlemlerin olduğu veri setlerinde uygulanmaktadır. Aykırı gözlemler, tahmincileri etkileyerek güvenilir olmayan tahmin sonuçlarına neden olabilmektedirler. Çalışmada Robust Değer Atama Yöntemi; şiddet ve mülkiyet suç türlerine ilişkin eksik gözlemler içeren bazı ülkelerin eksik gözlemlerinin atanmasında kullanılırken; Robust Seyrek Temel Bileşenler Analizi veri setinin boyutunu indirgemede kullanılmıştır. Bileşen yükü 0,50 ve üzerinde olan 5 değişkenin Robust Regresyon Analizinde kullanılmasına karar verilmiştir. Şiddet suçunun bağımlı değişken olarak kullanıldığı model için Robust Regresyon Analizi sonucuna göre Fransa, Belçika ve İsveç aykırı gözlemler olarak tespit edilmiştir. Kişi başına reel gayri safi yurtiçi hasıla, işsizlik oranı ve gini katsayısı ile yüz bin kişiye düşen şiddet suç sayısı arasındaki ilişki pozitif yönlü iken yükseköğrenime kayıt oranı ve 15-24 yaş genç nüfus oranı ile yüz bin kişiye düşen şiddet suç sayısı arasındaki ilişki ise negatif yönlü olarak tespit edilmiştir. Tüm bu bağımsız değişkenler %5 anlamlılık düzeyinde istatistiksel olarak anlamlı bulunmuştur. Mülkiyet suçunun bağımlı değişken olarak kullanıldığı model için ise sadece İrlanda aykırı gözlem olarak tespit edilmiştir. Kişi başına reel gayri safi yurtiçi hasıla ile yüz bin kişiye düşen mülkiyet suç sayısı arasındaki ilişki ise pozitif yönlü ve %5 anlamlılık düzeyinde istatistiksel olarak anlamlı bulunmuştur.</jats:p
Yüksek Teknoloji İhracatında Karşılaştırmalı Rekabet Gücü Analizi: Öncü ilk 40 Ülke Örneği
İhracata dayalı büyüme stratejileri çerçevesinde ekonomik büyüme ve kalkınmanın önemli araçlarından birisi teknoloji üretimi ve ihracatıdır. Teknolojinin günlük yaşamın bir parçası olduğu günümüzde hemen hemen tüm ülkeler teknoloji üretimi ve ihracatıyla uğraşmaktadır. Bu kapsamda teknoloji üretimi ve ihracatında ayrıştırıcı unsurun, yüksek teknolojili ürün üretimi ve ihracatı olduğu söylenebilir. Bu çalışmada yüksek teknoloji ihracatında öncü 40 ülke örneklem olarak belirlenmiş, ulaşılan en güncel verilerle 2012- 2019 yılları arası ülkelerin sektördeki rekabet güçleri Açıklanmış Karşılaştırmalı Üstünlükler Endeksi ve Simetrik Açıklanmış Karşılaştırmalı Üstünlükler Endeksi ile incelenmiştir. Araştırma sonucunda yüksek teknolojili ürün ihracatında Çin ve Çin özel yönetim bölgesi Hong Kong’un başta olmak üzere Asya ülkelerinin karşılaştırmalı üstünlüğe veya diğer ifade ile rekabet gücüne sahip olduğu; AB üye ülkelerinden ise Fransa, Macaristan ve Çekya dışında karşılaştırmalı üstünlüğü sürdüren ülkenin olmadığı belirlenmiştir. Dissatisfaction Levels in Turkish Residential Buildings and Recommendations for Improving Quality
Prognostic role of systemic immune-inflammation index versus other cardiac markers in acute myocarditis in young adults
Aim: Myocarditis, an inflammatory disease of the myocardium, can range from asymptomatic cases to severe forms such as fulminant myocarditis. The systemic immune-inflammation index (SII) has emerged as a potential biomarker for various inflammatory diseases. This study aimed to determine the effect of SII on the prognosis of young adults with acute myocarditis and compare it with other cardiac markers.Methods: We retrospectively analyzed patients aged 18-40 years who were admitted to the emergency department with a diagnosis of acute myocarditis between January 2014 and January 2024. Patients were divided into non-fulminant and fulminant myocarditis groups based on diagnostic criteria.Results: SII, troponin I and N-terminal pro-brain natriuretic peptide (NT-proBNP) levels were significantly higher in the fulminant myocarditis group (p < 0.001 for all). Logistic regression analysis identified SII and NT-proBNP as independent predictors of fulminant myocarditis but not for troponin I (p = 0.064). The optimal cutoff value for SII in diagnosing fulminant myocarditis was 1020, with a sensitivity of 91% and specificity of 83%, outperforming troponin I. Patients with SII ≥1020 had a significantly higher risk of adverse outcomes.Conclusion: The SII enables early detection of adverse outcomes and is an independent predictor of prognosis in young adults with myocarditis
Abstract PO4-18-12: MELODY: A prospective non-interventional multicenter cohort study to evaluate different imaging-guided methods for localization of malignant breast lesions (EUBREAST-4/iBRA-NET, NCT 05559411)
Abstract Background: In the last decades, the proportion of breast cancer patients receiving breast-conserving surgery has increased, reaching 70-80% in developed countries. In case of non-palpable lesions, surgical excision requires some form of breast localization. While wire-guided localization has long been considered gold standard, it carries several limitations, including logistical difficulties, the potential for displacement and patient discomfort, and re-excision rates reaching 21%. Other techniques (radioactive seed or radio-occult lesion localization, intraoperative ultrasound, magnetic, radiofrequency and radar localization) have been developed with the aim of overcoming these disadvantages. However, comparative data on the rates of successful lesion removal, negative margins and re-operations are limited. In the majority of studies, the patient’s perspective with regard to discomfort and pain level has not been evaluated. The aim of MELODY (MEthods for LOcalization of Different types of breast lesions) is to evaluate different imaging-guided localization methods with regard to oncological safety, patient-reported outcomes, and surgeon and radiologist satisfaction. Methods: The EUBREAST and the iBRA-NET have initiated the MELODY study to assess breast localization techniques and devices from several perspectives (NCT05559411, http://melody.eubreast.com). MELODY is a prospective intergroup cohort study which enrolls female and male pts. requiring breast-conserving surgery and imaging-guided localization for invasive breast cancer or DCIS. Multiple or bilateral lesions and neoadjuvant chemotherapy are allowed. Primary outcomes are: 1) Intended target lesion and/or marker removal, independent of margin status on final histopathology, and 2) Negative resection margin rates at first surgery. Secondary outcomes are, among others: rates of second surgery and secondary mastectomy, Resection Ratio (defined as actual resection volume divided by the calculated optimum specimen volume), duration of surgery, marker dislocation rates, rates of marker placement or localization failure, comparison of patient-reported outcomes, rates of “lost markers” and diagnostician/radiologist’s and surgeon’s satisfaction as well as the health economic evaluation of the different techniques. Target accrual: 7,416 patients. Enrollment started in January 2023. The study will be conducted in 30 countries and is supported by the Oncoplastic Breast Consortium (OPBC), AWOgyn, AGO-B and SENATURK.
