    Les traitements traditionnels et les compléments alimentaires chez le patient atteint de cancer: État des lieux en Tunisie

    Contexte : Les médecines alternatives et complémentaires connaissent à nos jours un véritable essor dans le monde entier, surtout chez le patient atteint d’un cancer qui cherche à améliorer son bien-être physique et moral. Objectifs : Réaliser un état des lieux des traitements traditionnels et des compléments alimentaires chez le patient atteint de cancer et évaluer l’incidence de ces approches sur la prise en charge anticancéreuse. Méthodologie : Il s’agit d’une étude prospective transversale à visée descriptive réalisée à l’hôpital de jour d’oncologie d’un centre hospitalier Tunisien. La collecte des données s’est déroulée du 1er février au 30 mars 2022. Résultats : Chez la population à l’étude, la prévalence d’utilisation des médecines complémentaires était de 71 %, avec une prédominance féminine et une moyenne d’âge de 54 ans. La phytothérapie occupait la première place (69,7 %); avec des pourcentages qui variaient entre 20 % et 40 %, on trouvait les produits d’origine animale, le régime alimentaire, la massothérapie et les compléments alimentaires; dans une moindre mesure on a noté la médecine des ventouses (12,4 %) et les thérapies du corps-esprit (rokia charria, activité sportive, etc.), à 10 %. Les plantes les plus consommées qui ont été recensées sont la Nigella sativa L. (44,9 %) suivie de l’Ephedra foeminea Forssk. (42,7 %) et du Curcuma longa L. (32,6 %). Parmi les interactions herbe-médicament recensées, 33,4 % ont été classées contre-indiquées par rapport à 56,7 % qui ont été classées comme bénéfiques. Conclusions : L’enquête a révélé une prévalence importante de l’utilisation de la médecine douce, ce qui nécessite une grande vigilance et des études plus approfondies. Objectives: To provide an overview of the use of traditional treatments and dietary supplements by cancer patients and to assess the impact of these approaches on cancer care. Methods: This prospective cross-sectional descriptive study was conducted at the oncology day hospital of a Tunisian medical centre. Data were collected from February 1 to March 30, 2022. Results: In the study population, the prevalence of complementary medicine use was 71%; users were predominantly female, and average age was 54 years. Herbal medicine was most commonly used (69.7%); animal-derived products, diet, massage therapy, and dietary supplements were also common, with percentages ranging between 20% and 40%. Cupping therapy (12.4%) and mind-body therapies (such as rokia charia and physical activity; 10%) were noted to a lesser extent. Of the plants identified, the most frequently consumed was Nigella sativa L. (44.9%), followed by Ephedra foeminea Forssk. (42.7%) and Curcuma longa L. (32.6%). Among the identified herb-drug interactions, 33.4% were classified as contraindicated, while 56.7% were classified as beneficial. Conclusions: The survey revealed a significant prevalence of alternative medicine use, which requires careful attention and further research.     Describing and Evaluating the Clinical Pharmacist’s Role in a Canadian Multiple Sclerosis Clinic

     Background: The current approach to treatment of multiple sclerosis (MS) involves use of disease-modifying therapies to slow progression of the disease, as well as the symptomatic management of fixed neurological deficits. Although pharmacists are uniquely positioned to support MS care teams with all aspects of medication management, their presence is rare among MS ambulatory care teams in Canada. Objectives: To document the pharmacist’s contributions and to evaluate the impact of the pharmacist’s role following creation of a clinical pharmacist position in a Canadian MS clinic within a large, urban, university-affiliated, tertiary care centre. Methods: This study was conducted in 2 parts: a prospective, descriptive case study of the clinical pharmacist’s role and a retrospective assessment of medication-related patient calls before and after implementation of the pharmacist position. Results: The pharmacist performed a variety of clinical activities, with the greatest proportions of time spent on patient care (63.3%), drug access research (15.7%), and development and review of internal documents (9.0%). Patient care primarily involved conducting patient assessments, making medication recommendations, and assisting patients with medication-related issues. The proportion of medication- related issues resolved remained similar at 92.9% before and 95.7% after implementation of the clinical pharmacist (p = 0.48). The median time to resolve medication-related issues was reduced from 4.1 to 2.9 days (p = 0.016) with pharmacist involvement. Conclusions: Pharmacists can support MS care teams through a variety of medication-related clinical activities aligned with their scope and expertise. The presence of a pharmacist on the MS care team significantly reduced turnaround times for resolving medication-related issues, improving the efficiency and timeliness of care. Bien que les pharmaciens occupent une position unique pour soutenir les équipes de soins de SP dans tous les aspects de la gestion des médicaments, leur présence est rare parmi les équipes de soins ambulatoires en SP au Canada. Objectifs : Documenter les contributions du pharmacien et évaluer l’incidence potentielle de son rôle après la mise en place d’un poste de pharmacien clinicien dans une clinique canadienne de SP au sein d’un grand centre de soins tertiaires urbain affilié à une université. Méthodologie : Cette étude a été menée en 2 parties : une étude de cas prospective et descriptive du rôle du pharmacien clinicien et une évaluation rétrospective des appels des patients liés aux médicaments avant et après la mise en place du poste de pharmacien. Résultats : Le pharmacien effectuait diverses activités cliniques, la plus grande proportion de temps étant consacrée aux soins aux patients (63,3 %), à la recherche sur l’accès aux médicaments (15,7 %) et à l’élaboration et à l’examen de documents internes (9,0 %). Les soins aux patients consistaient principalement à évaluer les patients, à formuler des recommandations en matière de médicaments et à aider les patients confrontés à des problèmes liés aux médicaments. La proportion de problèmes liés aux médicaments résolus est restée similaire, soit 92,9 % avant et 95,7 % après la mise en œuvre du pharmacien clinicien (p = 0,48). Le délai médian nécessaire pour résoudre les problèmes liés aux médicaments a été réduit de 4,1 à 2,9 jours (p = 0,016) avec la participation du pharmacien. Conclusions : Les pharmaciens peuvent soutenir les équipes soignantes de SP grâce à diverses activités cliniques liées aux médicaments, adaptées à leur portée et à leur expertise.     How Patient-Centred Are Inhaler Device Choices? A Survey of Canadian Prescribers

    Background: The choice of inhaler device type can play a crucial role in managing asthma and chronic obstructive pulmonary disease (COPD). With various devices available, differences in choice and application may lead to confusion for both prescribers and patients. Furthermore, improper use of a device may lead to suboptimal or inadequate treatment. Objectives: The primary objective was to identify factors that prescribers consider when selecting an inhaler device for a patient. The secondary objective was to evaluate the rankings of these factors, including identification of which factors had greater importance and frequency for prescribers’ choice of inhaler device for patients. Methods: A 10-question online survey was developed and distributed in late 2021 to prescribers (physicians, nurse practitioners, and pharmacists) in western Canada in an outpatient setting. Prescribers were asked to use their own words to describe the factors they considered important and were then asked to rank the stated factors in order of importance for 2 scenarios: an 83-year-old woman with COPD and a 21-year-old man with asthma. The results were examined qualitatively and quantitatively. Recurring themes were identified, and each response was categorized on the basis of its corresponding theme. Results: In all, 82 respondents completed the survey (yielding a total of 164 responses across the 2 scenarios). Overall, prescriber experience (84/164, 51%), cost (84/164, 51%), patient ease of use (59/164, 36%), and other patient considerations (49/164, 30%) were the factors most frequently mentioned. The prescriber’s experience was most often mentioned as a factor for scenario 1 (COPD), whereas cost was most often mentioned for scenario 2 (asthma). In both scenarios, prescriber experience was the highest-ranked factor. Conclusions: When determining the appropriate type of inhaler device, respondents frequently prioritized their own experience, as well as cost and ease of use. However, many respondents ranked prescriber experience higher than all other factors. Étant donné la diversité des dispositifs disponibles, les différences de choix et d’application peuvent prêter à confusion tant pour les prescripteurs que pour les patients. De plus, la mauvaise utilisation d’un appareil peut conduire à un traitement sous-optimal ou inadéquat. Objectifs : L’objectif principal consistait à identifier les facteurs pris en compte par les prescripteurs lors de la sélection de l’inhalateur pour un patient. L’objectif secondaire consistait à évaluer le classement de ces facteurs, notamment l’identification des facteurs les plus importants et des inhalateurs les plus fréquemment choisis par les prescripteurs. Méthodes : Un sondage en ligne de 10 questions a été préparé et distribué fin 2021 aux prescripteurs (médecins, infirmières praticiennes et pharmaciens) de l’ouest du Canada en milieu ambulatoire. Les prescripteurs devaient, dans leurs propres mots, décrire les facteurs qui leur semblaient importants avant de les classer par ordre d’importance dans le cadre de deux scénarios : une femme de 83 ans atteinte de MPOC et un homme de 21 ans avec de l’asthme. Les résultats ont fait l’objet d’un examen qualitatif et quantitatif. Des thèmes récurrents ont été identifiés et chaque réponse a été catégorisée en fonction du thème correspondant. Résultats : Au total, 82 répondants ont répondu au sondage (total de 164 réponses dans les 2 scénarios). Dans l’ensemble, l’expérience du prescripteur (84/164, 51 %), le coût (84/164, 51 %), la facilité d’utilisation pour le patient (59/164, 36 %) et d’autres considérations en rapport avec le patient (49/164, 30 %) étaient les facteurs déterminants les plus fréquemment mentionnés. Pour le scénario 1 (MPOC), l’expérience du prescripteur était le facteur le plus souvent mentionné, alors que le coût l’était pour le scénario 2 (asthme). Dans les deux scénarios, l’expérience du prescripteur était le facteur le plus important. Conclusions : Lors de la détermination du type d’inhalateur approprié, les répondants ont souvent donné la priorité à leur expérience personnelle, ainsi qu’au coût et à la facilité d’utilisation. Cependant, de nombreux répondants ont accordé une note plus élevée à l’expérience du prescripteur qu’à d’autres facteurs.     True Rate of Allergy among Pediatric Inpatients with Penicillin Allergy Labels (TRIAL)

    Background: Penicillin allergy is a common drug allergy diagnosis in pediatric patients; however, upon appropriate allergy testing, many of these patients are found not to have a true allergy. For patients with a reported allergy, alternative antibiotics are prescribed, which are less effective, more toxic, or more expensive. There is a lack of data evaluating allergies in hospitalized children and comparing allergy assessments conducted by pediatric allergists and pharmacists. Objective: To estimate the percentage of pediatric patients admitted with reported penicillin allergy who did not have a true penicillin allergy. Methods: This single-centre prospective cohort study included inpatients between 6 months and 17 years of age, with a documented penicillin allergy, who were admitted to the general pediatric and oncology units of a tertiary care children’s hospital between November 2019 and March 2023. The allergy history, evaluation, and risk categorization were performed by pharmacists. The history was reviewed with the allergist, and the patient was then referred, underwent skin testing, or received oral amoxicillin challenge with monitoring for 1 hour. Results: Thirty patients were included, of whom 29 (97%) had delabelling of their penicillin allergy. Four patients (13%) had delabelling on the basis of history alone, without risk assessment. Twenty-five (83%) of the patients were assessed as having low risk; 24 of these had delabelling following oral challenge, and 1 did not complete the oral challenge because of transfer to another hospital. One patient (3%) wasassessed as having moderate risk, with delabelling on the basis of results of skin testing and oral challenge. The pharmacist’s and allergist’s risk assessments were in agreement in 29 (97%) of the 30 cases. Conclusions: Pediatric patients, including those with oncologic malignancies, are often mislabelled as having a penicillin allergy. Pharmacists are able to accurately determine true allergy risk and delabel penicillin allergies for pediatric patients in the hospital setting. Pour ceux présentant une allergie signalée, des antibiotiques alternatifs sont prescrits, moins efficaces, plus toxiques ou plus coûteux. Peu de données permettent d’évaluer les allergies chez les enfants hospitalisés et de comparer les évaluations des allergies réalisées par les allergologues pédiatriques et les pharmaciens. Objectif : Estimer le pourcentage de patients pédiatriques admis avec une allergie à la pénicilline signalée, mais qui n’avaient pas de véritable allergie à la pénicilline. Méthodologie : Cette étude de cohorte prospective monocentrique comprenait des patients hospitalisés âgés de 6 mois à 17 ans, présentant une allergie documentée à la pénicilline, qui ont été admis dans les unités de pédiatrie générale et d’oncologie d’un hôpital pour enfants de soins tertiaires entre novembre 2019 et mars 2023. Les antécédents, l’évaluation et la catégorisation des risques de l’allergie ont été renseignés par les pharmaciens. L’anamnèse a été revue avec l’allergologue, et le patient a ensuite été référé, a subi un test cutané ou a reçu une provocation orale à l’amoxicilline avec surveillance pendant 1 heure. Résultats : Sur 30 patients inclus, 29 (97 %) ont vu un désétiquetage de leur allergie à la pénicilline. Quatre patients (13 %) ont bénéficié d’un désétiquetage sur la seule base de leurs antécédents, sans évaluation des risques. Vingt-cinq (83 %) patients ont été évalués comme présentant un faible risque; 24 d’entre eux ont bénéficié d’un désétiquetage à la suite d’une provocation orale, et 1 n’a pas terminé la provocation orale en raison d’un transfert vers un autre hôpital. Un patient (3 %) a été évalué comme présentant un risque modéré, avec un désétiquetage basé sur les résultats des tests cutanés et de la provocation orale. Les évaluations des risques par le pharmacien et l’allergologue concordaient dans 29 (97 %) des 30 cas. Conclusions : Les patients pédiatriques, y compris ceux atteints de cancers malins, sont souvent étiquetés à tort comme ayant une allergie à la pénicilline. Les pharmaciens sont en mesure de déterminer avec précision le risque réel d’allergie et de désétiqueter les allergies à la pénicilline chez les patients pédiatriques en milieu hospitalier.     Antimicrobial Use among Adult Inpatients in Northern Canada, 2019–2021

    Background: Antimicrobial use data from inpatients in northern Canada suitable to inform stewardship programs are limited. Objective: As a special project of the Canadian Nosocomial Infection Surveillance Program, to describe antimicrobial use for inpatients in northern Canadian acute care hospitals. Methods: Participating acute care hospitals serving adult or mixed adult and pediatric populations in northern Canada submitted annual data on the use of all systemic antimicrobials from 2019 to 2021. Patient-day denominators were also submitted. Northern Canada was defined as the territories and Statistics Canada’s provincial north. Data were analyzed in terms of defined daily doses per 1000 patient days (DDD/1000pd), as per the Anatomical Therapeutic Chemical classification system. Antimicrobials were categorized using the World Health Organization’s AWaRe (Access/ Watch/Reserve) classification system. Results: Each year, 42–47 hospitals participated. More than 90% of participating hospitals were in Alberta or British Columbia. There was large variation in overall antimicrobial use between hospitals (e.g., interquartile range 429 to 779 DDD/1000pd in 2021). From 2019 to 2021, there was a 49% relative increase in antimicrobial use, from 401 to 596 DDD/1000pd (p = 0.11). Over the same period, the use of third- and first-generation cephalosporins increased by 80% and 64%, respectively; antimicrobials in the “Reserve” category increased from 0.4% to 2% of overall use. Conclusions: This study represents the largest collection of antimicrobial use data for inpatients in northern Canada to date. From 2019 to 2021, there was an increase in antimicrobial use of 195 DDD/1000pd, largely driven by increases in the use of third- and first-generation cephalosporins. The findings should be interpreted with caution, as results may not be generalizable to all northern hospitals. Objectif : Projet spécial de Programme canadien de surveillance des infections nosocomiales visant à décrire l’utilisation d’agents antimicrobiens chez les patients hospitalisés dans des hôpitaux de soins aigus dans le nord du Canada. Méthodologie : Les hôpitaux de soins aigus ayant participé au projet et qui servent une population adulte ou une population mixte composée d’adultes et d’enfants dans le nord du Canada ont soumis leurs données annuelles concernant l’utilisation de tous les agents antimicrobiens systémiques de 2019 à 2021. Les dénominateurs de jours-patients ont également été soumis. Le nord du Canada était défini comme les territoires et le nord provincial de Statistique Canada. Les données ont été analysées en termes de doses journalières définies par 1000 jours-patients (DJD/1000 jours-patients), selon le Système de classification anatomique, thérapeutique et chimique. Les agents antimicrobiens ont été répartis à l’aide du système de classification AWaRe de l’Organisation mondiale de la Santé : accès aux antibiotiques; antibiotiques à surveiller; antibiotiques de réserve. Résultats : Chaque année, entre 42 et 47 hôpitaux ont participé. Plus de 90 % de ces hôpitaux se situaient en Alberta ou en Colombie-Britannique. De grandes variations quant à l’utilisation d’agents antimicrobiens de manière générale ont été observées d’un hôpital à l’autre (par exemple, l’intervalle interquartile était de 429 à 779 DJD/1000 jours-patients en 2021). Entre 2019 et 2021, on a observé une augmentation relative de 49 % de l’utilisation d’agents antimicrobiens, qui est passée de 401 à 596 DJD/1000 jours-patients (p = 0,11). Pendant la même période, l’utilisation des céphalosporines de troisième et de première génération a augmenté respectivement de 80 % et 64 %; les agents antimicrobiens de la catégorie « Réserve » ont quant à eux augmenté, passant de 0,4 % à 2 % de l’utilisation globale. Conclusions : Cette étude constitue la plus grande collecte de données sur l’utilisation d’agents antimicrobiens chez les patients hospitalisés dans le nord du Canada à ce jour. Entre 2019 et 2021, on a observé une augmentation de 195 DJD/1000 jours-patients de l’utilisation de ces agents, une augmentation principalement due à une hausse de l’utilisation des céphalosporines de troisième et de première génération.
