    Surabaya had more than 1.600 pharmacists in 2016. However, based on the data there were 144 pharmacists that did not have practice licenses in hospitals and primary health care in Surabaya. It meant that the implementation of The Regulation of Health Minister Number 889 Year 2011 was ineffective. The purpose of this research was describing the pratice lincesing of pharmacist in hospitals and primary health care in Surabaya and the factors that influenced the problem. This research was an observational descriptive study with cross sectional method. Primary data were collected through observation, interview and discussion with the staff in charged in Surabaya Public Health Office. While the secondary data could be achieved from official documents, reports, and archives. Comparing the number of pharmacists in the hospitals or primary health care with the minimum manpower standard of pharmacist based on The Regulation of Health Minister Number 56 Year 2014, the result showed that 33 hospitals had not fulfilled the standard minimum number of pharmacists which mean they had not made their practice licenses. Therefore, the given recommendation was strengthening Public Health Office's role to direct warning and punishment toward hospitals and pharmacists.Keywords: pharmacist, practice license, hospital and primary health car

    Pengaruh Kepuasan Kerja Terhadap Komitmen Karyawan Rumah Sakit Mata Undaan Surabaya

    Preliminary survey result showed the number of employees knowledge about the event held by Undaan Eye Hospital in Surabaya reached 52,8%. It means more than 20% employees are not aware of activity outside the work which that number can affect to decreasing employee commitment to the organization. Job satisfaction factors divided into 5 there are pay satisfaction, promotion, coworker, supervisor and the job characteristic. The aim of this study is to analyze the influence job satisfaction to employee commitment. Observational analytic was used in this research with the cross sectional design. The data was obtained through questionnaire with simple random sampling involving 62 employees as the sample of this research. The data obtained were analyzed using logistic regression test univariate (α=0,05) to examine the influence of Job satisfaction to employee commitment. The test result indicates that job satisfaction significantly influenced employee commitment at significance value of 0,02. Job satisfaction factors that there are influences toward employee commitment are employee's pay satisfaction (sig=0,001), promotion (sig=0,005), supervisor (sig=0,004) and job characteristic (sig=0,001). On the other hand, the employee's coworker satisfaction does not influence the employee commitment.Keywords: employee commitment, job satisfaction, job satisfaction factors

    Perencanaan dan Pengadaan Obat di Puskesmas "X” Berdasarkan Permenkes Nomor 74 Tahun 2016

    Imperfect and inaccurate drugs and medical consumables planning and procurement likely affect their sustainability in Public Health Center (Puskesmas). This paper aims to study the mechanism of drugs planning and procurement implemented by Puskesmas "X". This was a descriptive research using a qualitative approach and observational method. This study found there were two sources of drug financing and medical consumption in Puskesmas "X", using the Revenue and Expense Budgeting of the local administration and capitation funds from National Health Coverage program. Drugs planning was financed by the local government revenues and expenses budgeting scheme. Performed under the basis of annual drug usage report. The plannings were conducted at the end of the year to project next year drugs' consumption. Drugs procurement process was performed by preparing drugs consumption report and ordering sheet paper which then were submitted to drug logistics unit at Puskesmas "X". Afterward, the procurement process was performed by accessing e-catalog as the legal online system provided by the national government. This research concluded that the implementation of the planning and procurement of drugs in Puskesmas has fulfilled and followed Health Ministry Regulation Number 74 Year 2016.  Keywords : drugs planning, drugs procurement, regulationpublic health cente

    Hubungan Karakteristik Individu Perawat dengan Insiden Keselamatan Pasien Tipe Administrasi Klinik di Rumah Sakit Umum Haji Surabaya

    Patient safety incidents type of clinic administration are incidents that occur in the process of patient identification, handover, agreement, informed consent, queuing, referral, admission, discharge of patients from inpatient, transfer of care, division of tasks, response of emergency. Based on patient safety incidents reports of RSU Haji, there were 12 patient safety incidents type of clinic administration (standard 0% or zero accident) during January 2014-June 2016 in inpatient unit. This research aimed to analyze the correlation between nurse's individual characteristics with patient safety incidents type of clinic administration. This research was an observational descriptive study with cross sectional design. Questionnaires were distributed to 48 nurses that divided into 6 inpatient units. The results of this research showed that the lower the nurse's knowledge about patient safety and patient safety incidents type of clinic administration, the higher the nurse's work fatigue, the lower the nurse's motivation toward to the application of patient safety program, the higher the tendency of patient safety incidents type of clinic administration occured in inpatient unit of RSU Haji Surabaya. The conclusion of this research are knowledge, work fatigue, motivation of nurse have a correlation with patient safety incidents type of clinic administration in inpatient unit of RSU Haji Surabaya.  Keywords: patient safety incidents, clinic administration, nurse, hospita

    Kualitas Fungsional dan Kepuasan Pasien terhadap Perawat dan Dokter Gigi di Rumah Sakit

    Background: The quality of healthcare is an important aspect to be implemented by hospitals as a healthcare facility. Doctors and nurses are human resources that directly provide services to patients. Interpersonal communication between physicians and nurses with patients may influence patients' satisfaction as an output of service quality.Aim: This study aimed to identify the functional quality of nurses and dentists on patients' satisfaction at Dental Polyclinic of Dr. R. Sosodoro Djatikoesoemo Bojonegoro Hospital.Method: This research was a quantitative descriptive and observational research. The data were gathered from 68 respondents. Primary data were collected through a questionnaire disseminated for patients at Dental Polyclinic from 14th to 26th of May 2018. The data were processed and analyzed by using 2x2 position matrix.Results: The results show that the functional quality of nurses is in quadrant IV (3.18; 3.21), and functional quality of doctors is in quadrant II (3.48; 3.46).Conclusion: This study concludes the nurses' functional quality still must be improved and promoted to the patients regarding to their competence, assurance (3S application and nurse's hospitality), and empathy (nurses' tranquility) while overall dentists' functional quality (competence, assurance, and empahty) is maximized.Keywords: dentists, functional quality, hospital, nurses, satisfactio

    Pendekatan PDSA Untuk Perbaikan Proses pada Indikator Sasaran Keselamatan Pasien di Rumah Sakit

    Background: Patient safety is an indicator of hospital service quality. A hospital in Surabaya identified six indicators of patient safety goals. There are two indicators which can not achieve the standard, namely effective communication and infection risk reduction.Aims: This study aims to identify the process improvement that can be done to increase indicator performance by using PDSA cycle.Method: A descriptive observational design was used in this study with a case study and participatory approach. There were 5 subjects selected by purposive sampling. Interview and observation were used to collect data that then were analyzed descriptively. The validity of data was done by triangulation of method, source, and theory.Results: The PDSA results indicated that the cause of the poor indicators performance of both patient safety goals is the poor compliance of the health staffs in carrying out read-back procedure and hand hygiene as written in SOP. It was caused by the lack of knowledge and motivation of the health staffs in implementing the SOP.Conclusion: In conclusion, process improvement can be done by socializing read-back SOP and hand hygiene as well as supervision conducted periodically by managers. Plan stage is one step which should be improved. Commitment in implementing the improvement planning is necessary.     Pengaruh Informasi Pelayanan Prolanis dan Kesesuaian Waktu terhadap Pemanfaatan Prolanis di Pusat Layanan Kesehatan Unair

    Background: The Chronic Disease Management Program (Prolanis) was a promotive and preventive program developed by BPJS Kesehatan in collaboration with Primary Health Care (FKTP). Target ratio of prolanis patient visit (RPPB) to FKTP in safe zone was at least 50% every month. Average range of RPPB at UnairHealth Service Center (PLK Unair) from January to July of 2017 was 15% out of 62 registered participants, still below the target of the safety zone determined by BPJS Kesehatan.Aims: This study aimed to analyze the influence of enabling factors and need factor for the utilization ofProlanis at PLK Unair.Method: This research was a correlational-analytic research by using cross sectional research design.Results: The result of this study showed that there was a significant influence between prolanis information (p=0,020) and time conformity (p=0,008) towards the utilization of prolanis in PLK Unair. Meanwhile, BPJS class, distance, travel time, availability of transportation, type of transportation, social relations, health workers,health facilities, waiting time, and need has no significant effect.Conclusion: The conclusion of this research is the utilization of prolanis in PLK Unair influenced by prolanis information and time conformity of the patients. PLK Unair needs to recollect the information of their patients, redesign the socialization media and conduct home visit as well as discuss the schedule of prolanis activitieswith the patients.Keywords: enabling factor, need factor, prolanis, utilization of health service

    Background: Infection cases are the major cause of the high rate of morbidity and mortality in the world. Nosocomial infections cause 1.4 million deaths every day in the world. Even the number of nosocomial infections in Indonesia also increases. Therefore, prevention is very necessary to be done. One effective prevention is hand-washing. However, health staffs including midwives still had under 50% compliance to apply Standard Operating Procedure of hand-washing.Aims: The aim of this research was to analyze the influence of individual factors (motivation, knowledge, and attitude) and organizational factors (training, availability of facilities, supervision) on midwives' compliance in applying Standard Operating Procedure of hand-washing.Method: The research was conducted at RSU Bakti Mulia, Banyuwangi. This was an observational research with an analytical and cross-sectional design. The population of the research was 40 midwives in the midwifery unit. The sample size was thirty-three midwives in the midwifery unit determined by Lemeshow formula withm simple random sampling. Data were collected by observation and questionnaire.Results: The results showed that all variables had p values (0,001) < α (0.05). Individual and organizational factors have a significant and positive effect on midwives' compliance in the Standard Operating Procedure of hand-washing.Conclusion: It can be concluded that the higher individual factors (motivation, knowledge, attitude) and the better organizational factors (training, availability of facilities, supervision) are, the higher midwives' compliance in Standard Operating Procedure of hand washing is.     Pengaruh Pengawasan dan Kondisi Kerja terhadap Keterlibatan Kerja Karyawan Rumah Sakit Mata Undaan Surabaya

    Preliminary survey result showed the employee job involvement rate of Rumah Sakit Mata Undaan Surabaya reached 65,26%. It means that the employee's job involvement rate was low (standard 80%). Job involvement consists of three factors; support from co-workers, employee's participation in decision making, and the dependence of the tasks. The aim of this study was to analyze the influence of supervision and working condition of employee job involvement. The observational analysis was used in this research with the cross-sectional design. The data were obtained through questionnaires with simple random sampling as many as 62 employees as the sample. The obtained data were analyzed with logistic regression test univariate (α=0,05) to examine the influence of supervision and working condition of the employee's job involvement. The test result indicated that supervision significantly influenced employee's job involvement at significance value was0,000201. Better supervision made better employee's job involvement. On the other hand, working condition did not influence the employee's job involvement (significance value was 0,097), but cross-tabulation result indicated that better working condition made better employee's job involvement. It can be concluded that supervision affects job involvement while working condition does not affect the job involvement.  Keywords: job involvement, supervision, working condition

    Pencapaian Standar Nasional Pelayanan Kesehatan Peduli Remaja pada Posyandu Remaja di Surabaya

    Since 2010, Surabaya has a teen integrated service post (Posyandu) to solve teenagers' problem, but there were still a lot of teenagers' problem in Surabaya, such as early marriage. This study aims to determine about National Standard achievement of teen health care services at integrated service posts in Surabaya. This research used a cross-sectional design with a qualitative descriptive approach. The interview was conducted in 15 teen integrated service posts in Surabaya, and the respondents were the coordinator of teenage integrated service post. The results showed that the assessments of services and networking standard were difficult to achieve since teenagers' interest and socialization were not well organized. Also, there were no specific guidelines for teenagers, so the officers should guess and make their own guidelines. Lots of teen integrated service posts in Surabaya did not complete the form of evaluation and did not record and report their activities. The lack of health management in teen integrated service post influences the achievement of PKPR National Standard. Keywords: health care service, integrated service, National standard, teenage.
