    OPPORTUNITY LOSS ANALYSIS OF DISCHARGE AGAINST MEDICAL ADVICE (DAMA) AT INPATIENT UNIT OF TYPE A HOSPITAL IN SURABAYA

    Background: Patients who Discharge Against Medical Advice (DAMA) will experience the opportunity loss. DAMA will result in opportunity loss for patients to get treatment and cure their illness. For hospitals, DAMA patients will cause the opportunity loss for hospitals to provide services to patients.Aim: The purpose of this research was to analyze the opportunity loss as the result of DAMA patient. Methods: The method used in this research was a descriptive research. The samples were chosen from the total sample of inpatient population who did DAMA as many as 77 medical records. The inclusive samples were doctors who were responsible for other doctors and dentists as well as inpatient medical records. Meanwhile, the exclusive samples were doctors who are not responsible for other doctors and dentists as well as inpatient medical records. Data analysis used Microsoft Excel to see the total and percentage of DAMA patients.Results: The number of DAMA patients was 77 patients. The highest number of DAMA patients was those in grade 3 as many as 25%. It happened because the grade 3 was cheaper than others. As many as 65% of DAMA patients paid hospital fees using general payments. Patients felt free to discharge themselves from the hospitals because they funded on their own. The majority of DAMA patients were type 2 DM patients amounted to 12% because they assumed they could carry out self-care at home.Conclusions: DAMA patients caused opportunity loss and no Social Security Agency for Health (BPJS Kesehatan) guarantees. Therefore, the hospital has to form a policy to prevent patients from DAMA, notify and ask patients' reasons of doing DAMA, so the hospital can use these as information for hospital service evaluation. Keywords: Opportunity loss, DAMA, Hospital

    BOARDING TIME AND PATIENT SATISFACTION AT THE EMERGENCY DEPARTMENT

    Background: Delays of inpatient transfer from the Emergency Department (ED) will affect delayed care, overcrowd at ED, and patient's disappointment. Such patient transfer delay from the Emergency Departement (ED) also occurs at Haji General Hospital, Surabaya.Aims: This study analyzed the relationship  between inpatient boarding time from Emergency Departement (ED) and patient satisfaction at Haji General Hospital, Surabaya.Methods: This was an observational research using a cross-sectional approach. The population was 980 ED patients who decided to be hospitalized. Out of 980 ED patients, the study only involved 83 respondents selected by simple random sampling. The instruments used were observational sheet and questionnaires to collect data which were then analyzed with a chi-square test.Results: Average boarding time was 142 minutes 25 seconds (minimum = 60 minutes; maximum = 290 minutes). Almost all respondents (81.3%) were satisfied about fast boarding time. Inpatient boarding time at the Emergency Departement (ED) was significantly related to patient satisfaction (p = 0.058 <α = 0.1). The faster inpatients move to ED, the more satisfied they are.Conclusion: The average patient boarding time from the Emergency Departement (ED) to Inpatient Unit was slow. Respondents were satisfied with the quality of services even though patient boarding time was slow. The hospital should pay more attention to evaluate the minimal standard of boarding time, ensure the availability of beds,  and improve the quality of facilities and services to enhance patient satisfaction. Keywords: Boarding time, Emergency Department, Inpatient, Satisfaction

    IMPLEMENTATION OF THEORY OF CONSTRAINT ON WAITING TIME OF PRESCRIPTION SERVICE

    Background: Medical services provided within the Indonesian National Health Insurance programme or Jaminan Kesehatan Nasional (JKN) have been continuously improving for the past several years. One of them is pharmaceutical service in the Pharmacy Store for JKN patients at one hospital with the highest number of visitors in Surabaya. However, one preliminary study points out that patient waiting time for prescription services in the Pharmacy Store for JKN patients is longer than the national standard.Aim: This study aims to investigate the causes for such delay in prescription services as one of pharmaceutical services in the Pharmacy Store for JKN patients at one hospital in Surabaya.Methods: This is an observational descriptive research with a reference to the Theory of Constraints. The research was conducted from August 2017 to February 2018. The data were obtained from observations, interviews with one pharmacist and four pharmacy technicians, and waiting time for prescription service.  Results: The study finds that the longer waiting time is caused by inefficient division of jobs and workers and the lack of worker's discipline to work on time. The behaviors and habits of pharmacist and pharmacy technicians in drug preparation greatly affect patient waiting time, since the services still highly depend on manual human labor.Conclusions: Patient experience in prescription drug services is significantly influenced by the time spent waiting for the service provider.     EFFECT OF ORGANIZATIONAL CULTURE INTENSITY ON JOB SATISFACTION

    Background: Organizations need to pay attention to the job satisfaction of their members to achieve their goals. In 2011, the Airlangga Healthcare Center surveyed job satisfaction with the target of 80%. However, the target has not been achieved.  Job satisfaction can be affected by several factors, and one of them is the culture within the organization.Aim: This study aims to analyze the effect of organizational culture intensity towards job satisfaction of the Airlangga Healthcare Center's employees.Method: This study was conducted in November 2011 – June 2012 with a cross-sectional research design. The sample of the study used the population total technique. The independent variable of the research was the organizational culture intensity. While the dependent variable was job satisfaction. The data were analyzed descriptively by employing cross-tabulation analysis between organizational culture intensity and job satisfaction.Results: The most influential aspect of the organizational culture was cooperation that reached 73.9%. Job satisfaction aspects with a very satisfying level were job and co-worker aspects that reached 78.3%. The most influential aspect of organizational cultures that affect job satisfaction is trust that amounted to 83.3%. However, the organizational cultures affect job satisfaction with a percentage of  92.3% overall.Conclusion: The research concludes that the organizational culture intensity is a factor that affects job satisfaction of the employees at the Airlangga Healthcare Center. The recommendation for the organization is to improve the togetherness aspect of organizational cultures.Keywords: organizational culture intensity, job satisfaction, organization.

    EVALUATION OF LINEAR ACCELERATOR UTILIZATION FOR CA MAMMAE RADIOTHERAPY AT A PRIVATE HOSPITAL

    Background: Radiotherapy is an important cancer therapy in Indonesia. For hospitals which have provided radiotherapy tools for more than five years, they need to evaluate its utilization and influence on patients' condition.Aim: This study aims to analyze the use of Linac for radiating breast cancers in one of a type-C private hospital in Central Java by using Health Technology Assessment.Method: This study is an observational and descriptive study with an in-depth interview. There were 72 medical record documents examined. Furthermore, the researchers calculated the profits from the financial feasibility of tool investment gained by the hospital. This study involved six Key Informants and four triangulation informants.Results: This study shows that in terms of effectivity aspect, one Linac can prolong patient waiting time about 2-4 weeks. Such a long waiting time may cause disease progression to increase. Meanwhile, seen from the technical characteristics, the tool is not well-maintained by the internal and external parties. It causes the tool's performance worse. In terms of the economic aspect, the tool has lasted for 7.5 years, but it technically has been utilized for ten years. Therefore, the hospital needs to supply more radiation tools.Conclusion: The Linac utilization in a year increased, and the ca mammae patient visits were high. In addition to those aspects, the profits gained from the health services were high as well. The hospital should add radiation tools to improve the radiation capacity and decrease patient waiting time.Keywords: linac, economic evaluation, Ca Mammae

    Evaluasi Sistem Pelaporan Insiden Keselamatan Pasien di Rumah Sakit

    Background: Incident reporting systems are designed to obtain information about patient safety and used for organizational and individual learning.Aims: The objective is to evaluate the implementation of patient safety incident reporting system at a hospital of Surabaya.Method: This study was an observational descriptive research supported by qualitative data. This study used Health Metrics Network (HMN) model.Results: The results of the input evaluation show that there was a policy that regulates the incident report, but its implementation was still not appropriate with no direct funding. However, facilities were provided for reporting. There were socialization for employees who have different understanding and responsibility, organizational structure of the patient safety team, problem solving method which had not used PDSA (Plan, Do, Study, Action), and computerized technology.Conclusion: The process evaluation shows that the indicators were in line with the rules. The data sources were in accordance with the guidelines. Data collection, process, presentation, and analysis were in line with the theory. The output evaluation shows the submission of incident reports had not been timely. Moreover, the report was complete and suitable to the existing guidelines, and it had been used for decision-making. It is required for the hospital to revise the guidebook of incidence reporting and improve the human resource skill.Keywords: evaluation, incident, patient safety, reportin

    Lack of patient visits to FKTP causes capitation not being optimally utilized. The purpose of this research is to analyze the causes of the unfullfilled capitation payment indicators based on fulfillment of service commitment at FKTP. The type of research used is observational with cross sectional design. Approach in the research is quantitative and qualitative. The research used total sampling, where population is secondary data covering result of indicator achievement from 62 Puskesmas, 31 DPP, and 97 Klinik Pratama in Surabaya. This research concluded that indicator of AK and RPPB at FKTP still not fulfill target of safe zone. AK can be caused by the lack of participants' knowledge of FKTP services, limited resources, and BPJS Kesehatan online application that sometimes have interruptions. FKTP needs to provide promotions and education to participants', add resources and provide training to staff, advocate with health offices, and cooperation with other parties to help provide facilities. Whereas the target indicator of RPPB has not been achieved due to lack of socialization of Prolanic.     Background: Social justice is a condition of fulfilling material, spiritual and social needs of citizens to live properly and to develop themselves in carrying out the social functions. The social welfare rate in Indonesia was still low amounted to 62.8 in 2016. Children with mental illness problems experience social welfare. The government has provided support both strategic support and operational support for their lives.Aim: The objective of the research is to analyze the government's support for fulfilling child's health rights for those who suffer from mental illness.Method: This research was descriptive. The data were collected by using policy review and observation. Policy review was used to discover the strategic support given by government to fulfill health needs of mentally illed children. Meanwhile, the observation was to find out government's operational support for Kalijudan Surabaya Technical Implementation Unit (UPT) in Basic Social Services.Results: The results show that there is a good strategic support for the children with mental illness. The operational support has been implemented, but not in accordance with the existing regulations. However, Kalijudan Surabaya Technical Implementation Unit (UPT) in Basic Social Services has provided the right of health by cooperating with related parties.Conclusion: It can be concluded that the strategic support given includes Law Number 8 Year 2016 about disabled people, Health Ministry Law Number 25 Year 2014 about child's health support, and Surabaya Mayor's Law Number 2 Year 2013 about organization of Kalijudan Surabaya Technical Implementation Unit (UPT) at Basic Social Service Department.     Perhitungan Konsumsi Obat Untuk Logistik Medik di Rumah Sakit Islam Jemursari Surabaya

    Inaccurate drugs planning may cause excessive budget, stagnant, and stockout. This research aims to analyze drugs planning in medical logistics of Rumah Sakit Islam Jemursari Surabaya. This research is a descriptive study using cross sectional design. Primary data were obtained through observations and interviews. Secondary data were obtained through drugs planning and drugs consumption data from November to December 2016. Afterward, data were processed and found that 40% drugs item on November 2016 have greater amount of consumption than their planning and 65% drugs item on December 2016 have greater amount of consumption than their planning. The method of drugs planning that used in medical logistics of Rumah Sakit Islam Jemursari Surabaya was consumption method, but it was not calculate the average of drugs consumption. After the calculation based on Keputusan Menteri Kesehatan Nomor 1121 Tahun 2008, there is only 20% drugs item on November 2016 which have greater amount of consumption than their planning and 20% drugs item on December 2016 which have greater amount of consumption than their planning. This research showed that the calculation of drugs consumption for medical logistics in Rumah Sakit Islam Jemursari Surabaya need repairement.Keywords: consumption method, drugs planning, hospita
