The Roles of Nurses in the Selection of Treatment Methods by Patients Informed of the Diagnosis of Cancer : A Questionnaire Survey in Patients
Get PDF本研究は告知を受けたがん患者が治療選択の意思決定を行う際、看護師がどの程度援助していると認識しているのか明らかにするために行った。その結果、がん告知をされ手術療法を受けた患者は、医師からの治療方法の説明や希望確認は十分あったと答え、治療方法決定に一番影響を与えていたのも医師であった。そして、患者は看護師に対してやさしい言葉や態度で接してくれ、自分の話を聞いてくれる"見守ってくれる"存在とは認識しているものの、患者に積極的に話しかけ、自分の権利を擁護してくれる存在としての認識が薄かった。これは、看護師が患者の意思決定への役割をきちんと果たせていない結果とも推測された。患者の意思決定における看護師に必要な役割は対象の受容と理解、具体的な説明、患者の権利を保障することであると考えられた。This study was performed to clarify nurses' awareness of the degree of their support when patients informed of the diagnosis of cancer make decisions on the selection of treatment methods. As a result, patients who were informed of cancer and underwent surgery answered that physicians gave adequate explanations to them, confirmed their intentions, and affected them most markedly when making decisions on the selection of treatment methods. The patients considered that nurses' words and attitudes were kind to them, and nurses "observed" them, listening to them. However, they only slightly regard nurses as persons who talk to them and protect their rights. These results suggest that nurses are not playing their role in patients' decision-making as an ethical issue. The roles of nurses in patients' decision-making may be the acceptance and understanding of the subject, provision of concrete explanations, and the securing of patients' rights.13KJ00004797611原

Get PDFA短期大学の看護学生と看護教員で、平成14年度から「新見まごころネットワーク」として健康・生活相談に関するソフトの開発、運用マニュアルを作成し、平成15年7月から運用している。今回、情報処理教育のカリキュラムである医療情報Aの科目において、看護学生が「まごころネット」ヘアクセスし、その学びを内容分析の手法を用いて明らかにした。その結果、看護学生がHPへアクセスすることにより、看護学生が在宅高齢者との関わりを通して、高齢者理解が深まること、在宅高齢者が学生に生活上の知恵を伝えられる場になること、看護職あるいは看護学生と在宅高齢者がITを使ってコミュニケーションをとることができるので、疾病予防につながることや地域での高齢者を見守ることができること、さらには地域の高齢者の生きがいになること、高齢者同士の交流の場にもなること、地域全体の健康意識の向上につながることなど地域看護活動の役割や重要性について学ぶことができることが明らかになった。The nursing students and instructors at a junior college developed software for health/welfare consultation called "Niimi Magokoro Network" and a manual for its use in 2002, and began to use them in July 2003. In this study, nursing students accessed the "Niimi Magokoro Network" in Medical Information A as a subject in the information processing education curriculum, and their learning was clarified by the content analysis method. A Discussion on the Acceptance of Disability in Terms of Changes in Patients' Consciousness and the Family Relationship : Supports for in Elderly Patients in the Transition Period to Treatment at Home
Get PDF著者が訪問看護を行った高齢者への関わりの振り返りを行い,入院中から退院後の生活にかけて,高齢者への看護のあり方の課題を得ることを目的に考察を加えた。疾病による障害を経験した高齢者が,どのような生きがいの変化を体験しているのか,また,生きがいに影響している因子との関連性があるのかを明らかにした。その結果,患者は退職という社会的な役割変化を経験し始める時期に発症することで,身体的な障害を受けると同時に,社会と家庭の両方において役割喪失を体験し,発症前の自己像との違いに葛藤している状態であった。しかし,家族から患者の気持ちを受け止めている発言がみられたことで,患者の表情や言葉に変化が見られ,障害受容につながっていると思われた。よって,障害を有する患者本人のありのまま全てを受け入れるか否かが,家族にとっての障害受容ではないかと考えられる。そして,それを受容した家族から受けるサポートによって,患者は障害受容ができると考えられる。このことから,専門職は,患者本人の障害受容と家族の受容の,2者に対しての支援を考えることが必要であることが示唆された。The purpose of this discussion was to reflect on our home-visit nursing program and the involvement of elderly patients, and clarify tasks for appropriate nursing of the elderly from the hospitalization period to the period after discharge. We evaluated changes in something to live for in elderly patients with disability due to disease, and factors affecting what they lived for. As a result, since patients developed disease near the retirement age when changes in their social role occur, they not only developed physical disability but also experienced the loss of roles both in society and at home, and experienced a conflict between their own images before and after the onset of the disease. However, after their families verbally expressed their acceptance of the patients' feelings, the patients' facial expressions and words changed, leading to their acceptance of the disability. Whether families accept all aspects of disabled patients is important. Support by families who have accepted patients may consequently enable patients to accept their own disabilities. Therefore, specialists should provide support to both patients and their families to facilitate this.10KJ00005818548原

Get PDF新見公立短期大学看護学科(以下「本学」)の取組「電子カルテ教育システムによる看護基礎教育」が文部科学省平成19年度「現代的教育ニーズ取組支援プログラム」(現代GP)に選定された。取組の背景として,国では病院等に電子カルテの導入を推進していることがある。しかし,本学学生が在学中に行う実務実習で,これらを積極的に活用することは極めて困難である。また,これを看護基礎教育に本格的に導入している教育機関は,国内ではほとんどみられない。そこで本取組では,電子カルテを実質的に体験させるための新たなシミュレーションシステム開発を企画した。従来の紙を媒体とした看護基礎教育に加え,システム上に設定したモデル患者に,学内端末から学生がアクセスし,看護記録を作成し,看護上の問題を評価して看護計画・処置等を入力する。教員は,ネットワークを介して,その内容を評価・添削し,指導を行うなど,オンラインで随時,個別的かつ双方向的に運用する計画である。看護教育の中に情報教育を多面的・有機的に取り込み,情報スキルを活用したカリキュラム構築を図り,さらに教育改善のための資料を収集できるシステムとして発展させることを計画としている。23KJ00005818561報
The effectiveness of and future tasks regarding electronic chart-based training systems as a new training tool : Through the development of nursing processes in patients with respiratory disturbance
Get PDF新見公立短期大学看護学科(以下本学)の取組『電子カルテ教育システムによる看護基礎教育』が文部科学省平成19年度『現代的教育ニーズ取組支援プログラム』(現代GP)に選定された。取り組みの背景としては、国では病院などで電子カルテの導入を推進していることがある。そこで電子カルテシステムを導入している病院において、基礎看護学実習で学生が電子カルテを活用してよかったこと及び困ったことについて明らかにし、学内で電子カルテシステムを活用した教育システムの課題と有効性を検討した。さらに現在行っている教材としての電子カルテ教育システムの取組の一端として取り組みの実際とその評価を行ったので報告する。Basic nursing education using an electronic chart-based training system, an initiative by the Department of Nursing Science, Niimi College (hereinafter called the College) was selected as a target for the 2007 Contemporary Education Needs Initiative Support Program (Contemporary GP) sponsored by the Ministry of Education, Culture, Sports, Science and Technology. This initiative was designed in view of the Government's promoting of the introduction of electronic medical charts to hospitals. Thus, at a hospital employing an electronic chart system, we identified what nursing students had found beneficial or troublesome on using electronic medical charts during the basic nursing practice, and discussed the effectiveness and future tasks of electronic chart-based training systems in the College. A study of the assessment of the "Nursing care for patients undergoing cardiac surgery" class in the "Adult Nursing B" course
Get PDF本研究は,成人看護学Bの「心臓手術を受ける患者の看護」における学生の授業評価を実施することを通して,本授業の改善を要する点を明らかにし,今後の教授活動や教材の改善の手がかりを得ることを目的としている。A短期大学看護学科2年生を対象に「授業過程評価スケール-看護学講義用-」を用いて調査を実施した。その結果,授業全体の評価として平均点は3.6点であった。「具体と抽象の連関と教員意見の織り込みの程度」に関する項目が高く評価され,「具体例・事例・経験談などで抽象的な内容を具体的に説明した授業」と評価された。さらに,「看護師や看護を重要なものと価値づけていることが伝わった」という項目も高かった。平均点の低い項目は「学生への対応」で,3.2点と平均点より低かった。特に「教員は学生への一方的な講義ではなく学生も参加できた」や「教員は学生の発言を取上げて講義を進めていた」が低く評価された。以上のことから,具体例・事例・経験談などで具体的に説明しながら,学生への一方的な講義ではなく学生の発言を取上げ,学生も参加できるような授業を展開する必要性が示唆された。This study was conducted to identify ways to make improvements, based on an assessment by students, to the "Nursing care for patients undergoing cardiac surgery" class in the "Adult Nursing B" course as well as its teaching methods and materials. We conducted a survey of second-year nursing students of Junior College A, using a "scale to assess the teaching process in nursing lectures". Regarding the assessment of the class, the average score was 3.6 points. A high score was achieved for assessment items: "Explanation of the relationship between concrete and abstract matters and an appropriate level of opinions from instructors" and "Nurses and nursing care were recognized as important in the class". The most common comment from students was "Specific examples and experiences were used to explain abstract matters". The score for "response to students" was 3.2 points, slightly lower than the average. Their comments included: "Lessons were not interactive, and we just listened to the instructor" and "We had no opportunity to express our opinions in the class". Therefore, it is important to explain things using specific examples and experiences in class, and provide more interactive lessons to encourage students to express their opinions.4KJ00005819022原
