    片顎光学印象における上下顎歯列モデルの位置合わせ法が小臼歯の咬合接触に及ぼす影響

    大阪歯科大学Osaka Dental University博士(歯学)We investigated the effect of the alignment method on the occlusal registration of the mandibular quadrant arch model and maxillary quadrant arch model using two intraoral scanners on a jaw model based on the occlusal contact area of the premolar. Intraoral scanning was performed once for the left quadrant arch and three times for the buccal scan. The left mandibular molars were then reduced and scanned in the same manner. The alignment methods were as follows: IOS software was used to scan the dentition and the buccal scan before reduction (INN); IOS software was used to scan the dentition and buccal scan after reduction (IRR); dental CAD software was used to scan the dentition and buccal scan after reduction (CRR); and, using dental CAD software, the dentition after reduction and the buccal scan before reduction were aligned (CRN). Although the occlusal contact area was significantly larger in the IRR group and CRR group compared with the INN group, no significant difference was observed between the CRN group. The results of this study suggest that the use of buccal scan data acquired before tooth preparation is more accurate in reproducing the maxillomandibular relationship in quadrant arch scans.doctoral thesi

    大阪歯科大学Osaka Dental University博士(歯学)Abstract Purpose: The purpose of this study was to investigate the influence of substrates under the three-dimensional periodontal ligament (3D PDL) tissue on its biological functions after compressive stimulation. Methods: A 3D PDL tissue was created using a poly(L-lactic acid) (PLLA) porous scaffold impregnated with human periodontal ligament fibroblasts (hPDLFs). It was then placed on a polyetheretherketone (PEEK) substrate, which has a comparable elastic modulus to bone and was compressed (25 g/cm2) for 1, 3, and 7 days. The morphology and biological functions of the hPDLFs in the 3D PDL tissue on the PEEK substrate were evaluated and compared with those on the polystyrene (PS) substrate. Results: Compressive forces for the PLLA porous scaffold on the PEEK substrate were higher than those on the PS substrate. hPDLFs were present throughout the PLLA porous scaffold and there was no morphological change upon compressive stimulation. Increased expression of osteoclastogenic genes was observed after 3 days of compressive stimulation, while the level of these genes was increased by changing the substrate under the PDL tissue from PS to PEEK. Conclusion: The substrate placed under the PDL tissue during compressive stimulation affects the biological functions of hPDLFs.doctoral thesi

    大阪歯科大学Osaka Dental University博士(歯学)Objective: Receptor activator of nuclear factor-κB ligand (RANKL) is intimately involved in regulating bone remodeling during osteoclast differentiation and promotion of osteoclast function. Upon binding to its receptor, RANK, RANKL activates various signaling cascades that induce osteoclast differentiation of osteoclast precursor cells into osteoclasts. In the innate immune system, host pattern recognition receptors, such as Toll-like receptors and nucleotide-binding oligomerization domain-like receptors (NLRs), detect pathogen-associated molecular patterns and elicit an immune response. The NLR, nucleotide-binding oligomerization domain 2 (NOD2), is known to bind muramyl dipeptide (MDP) and regulate inflammatory responses via nuclear factor-κB (NF-κB). The objective of this study was to investigate the effect of MDP on RANKL stimulation of osteoclast differentiation to elucidate the mechanism of bone resorption in a bacterial infection-induced inflammation model. Methods: The extent of osteoclast formation in MDP-stimulated RAW 264.7 cells was assessed using a tartrate-resistant acid phosphatase activity assay. The protein levels of intracellular signaling molecules were assessed by western blotting.Results: In RAW 264.7 cells, MDP stimulation did not affect the expression of RANK. MDP enhanced the expression of osteoclast-specific proteins, such as nuclear factor of activated T cells 1 (NFATc1) and cathepsin K, which are osteoclast differentiation markers, in RANKL-stimulated RAW 267.4 cells. Furthermore, JSH23, an NF-κB inhibitor, suppressed the expression of NFATc1 after co-stimulation with MDP and RANKL. Conclusion: MDP promoted osteoclast differentiation in RAW 267.4 cells by upregulating the activators, NF-κB and NFATc1, which are important for osteoclast differentiation, through enhancement of the RANKL signaling pathway.doctoral thesi

    ベトナムにおける成人期・高齢期の口腔の状況および口腔機能の実態調査

    大阪歯科大学Osaka Dental University博士(口腔科学)This study aimed to clarify the oral health status and function of adults, including elderly individuals, in Vietnam. We included 147 Vietnamese residents between 20 and 90 years of age in this study. A selfadministered questionnaire was used to assess sex, age, marital status, number of household members, occupation, annual income, regular dental visits, subjective health situation, and food diversity. The items evaluated included height, weight, grip strength, lower leg circumference, the oral health assessment tool, Japanese version, an oral examination, tongue coating index, oral dryness, tongue pres sure, oral diadochokinesis (ODK), and masticatory function evaluation using glucose dis solved in the gummy jelly. The participants were divided into three groups for compari son: 20‐49, 50‐74, and over 75 years of age. The total number of teeth and number of functional teeth were significantly lower in the over 75 year group. Significant differences were found among the three groups with regard to food intake diversity scores. Further more, the tongue pressure, ODK, and glucose elution were significantly lower in the over 75 year group indicating that their oral function was significantly reduced. We concluded that it is necessary to maintain oral function and provide appropriate intervention to the aging population in Vietnam.doctoral thesi

    歯科保健指導における口腔内スキャナーの応用 ー患者の体位と記録時間による影響ー

    大阪歯科大学Osaka Dental University博士(口腔科学)The aim of this study was to improve the accuracy of treatment effect examinations usingan intraoral scanner (IOS) for dental health guidance and basic periodontal treatment,and to facilitate patient understanding by quantifying and visualizing changes in periodontaltissue. The intraoral shape was measured using the IOS with the patient seatedin a dental chair, and blood vessels of the marginal gingiva were observed using a bloodflow scope. The dental chair was then moved to a horizontal position and the patient waskept in a resting state for 1 hour. Afterwards, the same measurements were retaken. Thedeviation of threedimensional data from the two IOS measurements was calculated forthe proximal, central, and distal marginal gingiva of each tooth, and blood flow velocityand blood vessel diameter were analyzed. The marginal gingiva after 1 hour in the restingstate compared to the sitting position often showed positive deviation, representing swelling.In addition, deviation was greater for the posterior teeth and blood flow velocity wassignificantly faster in the horizontal position (1,598 μm/s) than in the sitting position(1,282 μm/s), although no significant difference was evident in the thickness of blood vessels. Recording in a fixed position therefore to be appears important for accurately evaluating treatment effects.doctoral thesi
