Primary thyroid lymphoma: a tertiary-center experience
Introduction: Primary thyroid lymphoma (PTL) is a rare thyroid malignancy, usually presenting as a rapidly enlarging neck mass. We aimed to describe its clinical, biochemical, imaging, and pathological features.
Material and methods: Retrospective single-center study, including 20 patients diagnosed with PTL between 2000 and 2023 (median age 76 years, range 47–84; 85% female). Clinical presentation, thyroid function, imaging, histopathology, treatment, and outcomes were reviewed.
Results: Nineteen patients (95%) presented with a rapidly growing neck mass with a median duration of one month. Compressive symptoms occurred in 70%, and B symptoms in 20%. Hypothyroidism was present in 45%, and thyroid autoimmunity in 67% of patients with available data. Ultrasound (performed in 16 patients) showed hypoechoic nodules in most cases, with a mean size of 49.7 mm; cervical lymphadenopathy was observed in 20%. Fine-needle aspiration (FNA) suggested lymphoma in 78%. Histological confirmation was obtained by core-needle biopsy in 70%, incisional biopsy in 10%, and surgery in 15%. Nineteen patients (95%) had diffuse large B-cell lymphoma, and one had mucosa-associated lymphoid tissue (MALT) lymphoma. At presentation, 59% had localized disease, 12% regional, and 29% disseminated. Two patients died before treatment. Most received R-CHOP (rituximab, cyclophosphamide, adriamycin, vincristine, and prednisolone) or reduced-dose R-CHOP (R-miniCHOP); three also underwent radiotherapy. Complete remission was achieved in 61% of patients. Median follow-up was 2.5 years.
Conclusions: PTL should be suspected in patients with a rapidly enlarging thyroid mass. While FNA is often informative, biopsy is usually required for diagnosis. Most cases are diffuse large B-cell lymphoma, with chemotherapy as the mainstay of treatment. Larger studies are needed to refine diagnostic pathways and prognostic markers

Selected aspects of epidemiology of multiple sclerosis in Poland: a multicenter pilot study
Introduction. Multiple sclerosis (MS) is a progressive disease that, if untreated, leads to significant disability. In recent years, several effective disease-modifying therapies (DMTs) have been approved in the European Union and are now available in Poland. This study aimed to evaluate the selected epidemiological and clinical aspects of Polish individuals with MS and to compare the findings with data collected 15 years ago. Material and methods. Sociodemographic and clinical data were collected from patients with MS between February and April 2024 using a standardized questionnaire across 19 treatment centers in Poland. Results. The study included 3,165 MS patients (mean age, 42.03 years; female-to-male ratio, 2.2:1). The mean disease duration was 10.6 ± 7.85 years, with symptom onset at 30.66 ± 9.84 years and diagnosis at 32.74 ± 10.22 years. The mean Expanded Disability Status Scale (EDSS) score was 2.58 ± 1.6, with mild disability (EDSS 0–3.5) in 78.35% of patients and moderate-to-severe disability (EDSS ≥ 4.0) in 21.65%. Multiple sclerosis subtypes included relapsing-remitting (RRMS) (86.87%), secondary progressive (SPMS) (7.04%), and primary progressive (PRMS) (6.03%). Relapses occurred in 20.43% of patients within the last year, with an overall annual relapse rate of 1.2 in this group. Monofocal onset of the disease was observed in 87.05% of patients, and multifocal onset in 12.95%. Comorbidities were present in 59.73% of patients. Diagnostic tests included magnetic resonance imaging (MRI) (99.84% of patients), cerebrospinal fluid (CSF) analysis (90.91% of patients), visual evoked potentials (VEP) (32.41% of patients), and optical coherence tomography (OCT) (16.11% of patients). At the time of data collection, 97.09% of patients were receiving DMTs, with the following distribution: dimethyl fumarate (25.91%), ofatumumab (16.44%), ocrelizumab (8.62%), teriflunomide (7.44%), natalizumab (5.80%), interferon beta-1a (5.58%), interferon beta-1b (4.97%), ozanimod (4.69%), siponimod (4.15%), glatiramer acetate (4.05%), fingolimod (3.29%), cladribine (3.20%), ponesimod (1.96%), and other therapies (0.88%). Conclusions. This study highlights substantial progress in the diagnostic and therapeutic management of MS in Poland over the past 15 years. The widespread implementation of MRI and CSF analysis, alongside significantly improved access to DMTs, has contributed to notably better clinical outcomes. These improvements are reflected in reduced relapse rates, slower disability progression, and a decreased prevalence of secondary progressive MS

Przeprowadzenie diagnostyki raka płuca w sposób spersonalizowany ma decydujące znaczenie w zaplanowaniu właściwej stra-tegii postępowania diagnostyczno-terapeutycznego. W artykule przedstawiono przypadek 76-letniego pacjenta z nawracającym płynem w jamie opłucnej, u którego obraz kliniczny i niejednoznaczne wyniki badań obrazowych, cytologicznych i histopatologicz-nych powodowały wydłużenie procesu diagnostycznego.
Clinical implementation of a hybrid solitary dynamic portal radiotherapy for left-sided post-mastectomy chest-wall and regional nodal irradiation using Monaco TPS
Background: A hybrid solitary dynamic portal radiotherapy (h-SDPRT) technique for complex chest-wall and regional nodal irradiation was clinically implemented on the Monaco treatment planning system (TPS) by effectively adapting the Eclipse-based h-SDPRT method with minimal amendments to align with Monaco-specific technical constraints while maintaining consistent and reproducible dosimetric outcomes.
Materials and methods: This technique was evaluated on ten left-sided post-mastectomy radiotherapy (PMRT) patients, delivering 80–85% of the prescribed dose through asymmetric static open field apertures with the Elekta Agility multileaf collimator (MLC), and the remaining 15–20% via a unidirectional solitary dynamic field with differential blocks, composed of 10–15 MLC control points (4–5% of the dose per segment).
Results: The h-SDPRT plans demonstrated excellent dose coverage to the chest-wall target (D99 ≥ 96.3 ± 0.2%) and regional nodes (D99 ≥ 96.4 ± 0.8%), optimal conformity (1.5 ± 0.1), and superior homogeneity (0.104 ± 0.01), while effectively sparing critical organs-at-risk (OAR) viz., ipsilateral lung (V20 ≤ 26.7 ± 2.0% and Dmean: 14.3 ± 5.7 Gy) and heart (V25 ≤ 6.4 ± 3.8% and Dmean: 6.1 ± 1.6 Gy), with complete sparing of contralateral lung (V5 ≤ 0.1 ± 0.2% and Dmean: 1.3 ± 0.3 Gy) and breast (V5 ≤ 1.5 ± 1.0% and Dmean: 1.6 ± 0.2 Gy). Gamma evaluation (γ) showed > 95% pixels passing the standard 3% dose difference and 3 mm distance-to-agreement γ criteria, with results closely aligning with Eclipse TPS data.
Conclusions: The h-SDPRT technique minimizes the risk of “geometrical miss” and reduces delivery uncertainties associated with irregular or thin chest-wall with comprehensive nodal irradiation. By combining dominant static portals with simplified unidirectional dynamic field sequencing strategy, this approach provides a feasible and effective solution for PMRT
Miejsce luspaterceptu w leczeniu pacjentów z nowotworami mielodysplastycznymi niskiego ryzyka w 2025 roku
This article presents a review of key Phase III clinical trials — MEDALIST and COMMANDS — which confirmed the superior efficacy of luspatercept compared to placebo and epoetin alfa in achieving transfusion independence and improving hematologic parameters. The analysis highlights patient subgroups who benefit most from treatment (e.g., those with SF3B1 mutations, low serum erythropoietin levels, or moderate transfusion burden) and outlines the role of luspatercept in current National Comprehensive Cancer Network guidelines (ver. 2.2025). The safety profile of the drug is also discussed.
Luspatercept is currently considered the preferred first-line treatment option for anemia in patients with lower-risk myelodysplastic neoplasms with ring sideroblasts [LR-MDS and RS(+)] and a viable alternative to erythropoiesis-stimulating agents (ESA) in non-RS MDS, particularly in cases of ESA failure. The integration of luspatercept into routine clinical practice enables a more personalized approach to anemia management in MDS.W niniejszym artykule przedstawiono przegląd wyników kluczowych badań III fazy, MEDALIST i COMMANDS, które potwierdziły wyższą skuteczność luspaterceptu w zakresie uzyskania niezależności od przetoczeń oraz poprawy parametrów hematologicznych w porównaniu z placebo i epoetyną alfa. Omówiono także populacje szczególnie korzystające z terapii (m.in. z mutacją SF3B1, niskim poziomem erytropoetyny w surowicy, umiarkowanym obciążeniem transfuzjami) oraz miejsca luspaterceptu w aktualnych rekomendacjach National Comprehensive Cancer Network (wer. 2.2025). Uwzględniono również analizę bezpieczeństwa leczenia.
Luspatercept stanowi obecnie preferowaną opcję terapeutyczną w pierwszej linii leczenia niedokrwistości u pacjentów z nowotworami mielodysplastycznymi niskiego ryzyka z obecnością syderoblastów pierścieniowatych [LR-MDS RS(+)], a także alternatywę wobec leków stymulujących erytropoezę u chorych z MDS bez obecności RS, szczególnie w przypadku nieskuteczności wcześniejszej terapii. Włączenie luspaterceptu do codziennej praktyki klinicznej umożliwia bardziej spersonalizowane podejście do leczenia niedokrwistości w MDS
Przydatność trichoskopii w rozpoznawaniu androgenopodobnego łysienia bliznowaciejącego
Androgenopodobne łysienie bliznowaciejące (FAPD, fibrosing alopecia in pattern distribution) jest rzadką jednostką chorobową charakteryzującą się utratą włosów w okolicy czołowej i okolicach ciemieniowych. W obrazie klinicznym i trichoskopowym obserwuje się połączenie cech zarówno liszaja płaskiego, jak i łysienia androgenowego. Niniejszy raport podkreśla przydatność trichoskopii w rozpoznaniu FAPD. U 49-letniej pacjentki w okresie postmenopauzalnym stwierdzono postępujące stopniowe przerzedzanie się włosów w okolicy czołowo-ciemieniowej. Pacjentka zgłaszała subiektywne objawy, takie jak swędzenie i pieczenie skóry głowy. W badaniu trichoskopowym ujawniono liczne małe obszary pozbawione ujść mieszków włosowych, dyskretne złuszczanie okołomieszkowe, obecność włosów skręconych (pili torti) oraz heterogeniczność grubości łodyg włosów. Na podstawie wyników badania trichoskopowego ustalono rozpoznanie FAPD i zalecono leczenie hydroksychlorochiną w dawce 200 mg/dobę i finasterydem w dawce 5 mg/dobę. Osiągnięto zahamowanie progresji choroby i ustąpienie objawów subiektywnych (roczna obserwacja). Odróżnienie FAPD od FPHL jest istotnym aspektem diagnostycznym. Pomimo podobnego obrazu klinicznego oba schorzenia wykazują różne cechy w badaniu trichoskopowym, a właściwe rozpoznanie ma istotne implikacje dla przebiegu leczenia i rokowania

Acute liver failure (ALF) is a rare condition with an incidence rate of 1 to 8 cases per 1,000,000 population and a mortality rate of up to 80%. Replacing liver functions and treating the underlying causes play a major role in the management of patients with ALF. Nutritional therapy is crucial for maintaining proper physical functioning required for transplant. A 22-year-old female patient was admitted to our tertiary intensive care unit (ICU) because of ALF. The origin of the liver failure was primarily unclear. Extracorporeal therapies were started. Thorough nutritional intervention of oral and parenteral nutrition was introduced from the beginning of ICU stay. Nutritional therapy required modifications throughout the patient’s stay and helped prevent deterioration of their overall condition. After 35 days, 18 plasma exchanges, 2 CytoSorb (CS, CytoSorbentsTM Europe GmbH, Berlin, Germany) cycles, 2 single-pass albumin dialysis, and 29 days of continuous renal replacement therapy (CRRT), the patient was transferred to receive liver transplant. A post-transplant liver biopsy confirmed Wilson’s disease. Acute liver failure due to Wilson’s disease is rare and requires extracorporeal support as a bridging therapy to liver transplantation, which remains the only method of treatment, and good overall condition is necessary to survive the procedure.
Ostra niewydolność wątroby (ALF, acute liver failure) to rzadka choroba, której częstość występowania wynosi od 1 do 8 przypadków na 1 000 000 osób, a śmiertelność sięga 80%. W leczeniu pacjentów z ALF istotną rolę odgrywa zastępowanie funkcji wątroby i leczenie przyczyn leżących u jej podłoża. Terapia żywieniowa ma kluczowe znaczenie w utrzymaniu wydolności organizmu niezbędnej do procedury przeszczepu. Dwudziestodwuletnia pacjentka została przyjęta na oddział intensywnej (ICU, intensive care unit) z powodu ALF. Przyczyna niewydolności wątroby była początkowo niejasna. Rozpoczęto terapie pozaustrojowe. Od początku pobytu na oddziale wprowadzono kompleksową interwencję żywieniową w postaci żywienia doustnego i pozajelitowego. Terapia żywieniowa wymagała modyfikacji przez cały okres pobytu pacjentki i pomogła zapobiec pogorszeniu stanu ogólnego. Po 35 dniach, 18 wymianach osocza, 2 cyklach leczenia hemoadsorpcyjnego z użyciem CytoSorb (CS, CytoSorbentsTM Europe GmbH, Berlin, Niemcy), 2 jednoprzebiegowych dializach albuminowych i 29 dniach ciągłej terapii nerkozastępczej (CRRT, continuous renal replacement therapy), pacjentkę podano procedurze przeszczepu wątroby. Na podstawie biopsji wątroby po przeszczepie potwierdzono chorobę Wilsona. Ostra niewydolność wątroby spowodowana chorobą Wilsona jest rzadka i wymaga pozaustrojowego wspomagania krążenia jako terapii pomostowej do przeszczepu wątroby, który pozostaje jedyną metodą leczenia. Zachowanie dobrego stanu ogólnego znacząco poprawia rokowanie pacjenta
