    Predictors for long-term incomplete nidus obliteration following stereotactic radiosurgery for brain arteriovenous malformations: a systematic review and meta-analysis

    Introduction. This study aimed to identify predictive factors for long-term incomplete nidus obliteration following stereotactic radiosurgery (SRS) for brain arteriovenous malformations (AVMs).Material and methods. A systematic search across the PubMed, Web of Science, and Scopus databases identified observational studies reporting such factors. Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed. The study protocol was registered through PROSPERO. Each eligible study’s quality was assessed using the modified Newcastle-Ottawa Scale. Odds ratios (ORs) were calculated for dichotomous parameters.Results. Two high-quality prospective cohort and three high-quality retrospective cohort studies were included, covering patients with complete (n = 638) and incomplete (n = 297) nidus obliteration. The mean age of the patients was 25.54 ± 12.81 years and the mean follow-up time was 95.98 ± 27.64 months. Predictors for incomplete obliteration of nidus included: AVM classified as Spetzler-Martin (SM) grade ≥ IV (odds ratio (OR) 10.57, 95% confidence interval (CI) 2.00–55.96, p = 0.006), the presence of multiple (> 1) feeding arteries (OR 6.47, 95% CI 2.20–19.10, p = 0.0007), nidus volume > 10 mL (OR 5.08, 95% CI 1.68–15.33, p = 0.004), and the occurrence of intranidal aneurysm (OR 3.33, 95% CI 1.10–10.08, p = 0.03). No statistically significant difference in proportions of patients with incomplete nidus obliteration was found between paediatric (≤ 18 years) and adult (> 18) patient cohorts (p = 0.95).Conclusions. The following factors were found to be predictive for long-term incomplete nidus obliteration post-SRS for brain AVMs: SM grade equal to or higher than IV; the presence of multiple feeding arteries; AVM nidus volume exceeding 10 mL; and the occurrence of intranidal aneurysm. These findings will be beneficial in refining patient selection for radiosurgical treatment

    Background: This review examines the role of rectal spacers in gynecological cancer radiation therapy, highlighting their potential to mitigate radiation-induced toxicities, improve treatment precision, and enhance patient quality of life. The analysis explores different spacer materials, insertion techniques, and clinical applications while addressing current challenges and limitations. Materials and methods: A systematic review of studies evaluating rectal spacers in external beam radiation therapy (EBRT), brachytherapy, and stereotactic body radiation therapy (SBRT) for gynecological malignancies was conducted using PubMed, Scopus, and Embase. The review includes prospective and retrospective studies, clinical trials, and case reports from 2005-2024. Keywords used were “rectal spacers”, “gynecological cancer radiotherapy”, and “hydrogel spacers”. Results: Rectal spacers demonstrate significant dosimetric advantages in reducing rectal dose exposure in gynecological cancers, particularly in brachytherapy. However, the spacer placement area overlaps with the clinical target volume (CTV), necessitating careful consideration in treatment planning. The integration of rectal spacers in SBRT is still emerging, with limited clinical data available. Limitations include cost, risk of migration, and patient discomfort. Additionally, cost-effectiveness and routine implementation in clinical practice require further investigation. Conclusions: While rectal spacers provide significant benefits in reducing toxicity, their role in treatment precision remains debatable. Further research is needed to refine patient selection and cost-effectiveness. Future research should focus on refining patient selection criteria, improving spacer stability, and evaluating long-term clinical and economic impacts

    ERRATUM: Investigation of the beam width and profile of kilovoltage CBCT using different measurement techniques and analysis of the dosimetric effects of beam parameters

    Background: The aim of this study is to investigate the beam width and beam profile of kilovoltage cone beam computed tomography (kV CBCT) using different measurement techniques on an O-ring linear accelerator. The effect of the imaging beam on the dosimetric parameters was analysed. Materials and methods: The uncertainty of field size adjustment, the dependence of beam width on field size, and the effect of deflection from the isocenter on the beam profile were investigated by ionization chamber detector matrices. The 2D beam profile of the CBCT was analysed by relative ionization chamber measurements. Results: The average setup uncertainties of the field sizes were 0.3 mm ± 0.02 mm. The dependence of beam width on field size investigation revealed that the largest discrepancies occurred for small field sizes, which are important in determining computed tomography dose index (CTDI) values of the kV CBCT. The beam width deviation between measured and vendor-based data was larger than 1 mm below 40 mm field of view. The pelvis protocol demonstrated the smallest CTDI value difference of 2.3%, yet presented the largest effective dose deviation of 0.12 mSv. Conclusions: The measured CTDI coefficients were higher than predicted by the manufacturer for all cases. The currently internationally recommended CTDI measurement protocols for CBCT contain no reference to the determination of the beam width as a basic element of the calculations. Based on our measurement results, the beam width parameters affect CTDI: therefore, it would be advisable to apply this type of correction.

    Pierwotna małopłytkowość immunologiczna (pITP) występuje w dwóch szczytach zachorowań: u młodych kobiet oraz starszych mężczyzn. W chwili obecnej nie ma wytycznych dotyczących leczenia grupy starszych pacjentów. W niniejszym opisie przypadku przedstawiono nawrotowy przebieg pierwotnej małopłytkowości immunologicznej u starszego 80-letniego pacjenta. U chorego zastosowano w pierwszej linii leczenia sterydy z dobrym efektem terapeutycznym. Po czterech latach nastąpił nawrót małopłytkowości. Ponowne włączenie steroidoterapii było nieefektywne. Zastosowanie analogu receptora trombopoetyny pozwoliło na ustabilizowanie liczby płytek krwi. Leczenie to było dobrze tolerowane przez pacjenta

    Rozpoznanie osiowej spondyloartropatii zapalnej — w zależności od źródła — dotyczy od 0,1% do 1% populacji ogólnej. Niestety, mimo nowych kryteriów diagnostycznych, dostosowanych także dla postaci nieradiograficznej spondyloartropatii osiowej, oraz postępu technik obrazowania (powszechne stosowanie rezonansu magnetycznego), nadal istnieje poważny problem zbyt późnego rozpoznawania choroby i tym samym opóźnienia w rozpoczęciu terapii. Czas między wystąpieniem pierwszych objawów choroby a jej rozpoznaniem to średnio kilka lat. Nadal opóźniony jest również dostęp pacjentów do nowoczesnych metod leczenia. W niniejszej publikacji przedstawiono praktyczny poradnik dotyczący zarówno diagnozowania, jak i leczenia spondyloartropatii osiowych w warunkach polskich

    Piodermia zgorzelinowa jest rzadką chorobą zapalną skóry, która należy do dermatoz neutrofilowych i charakteryzuje się bolesnymi, szybko powiększającymi się owrzodzeniami. Zwraca uwagę jej częste współwystępowanie z takimi chorobami, jak zapalne choroby jelit czy też zaburzenia reumatologiczne, w tym łuszczycowe zapalenie stawów. Na przykładzie opisanej pacjentki, a także dostępnej literatury, zaprezentowano, że zarówno rozpoznanie, jak i leczenie piodermii zgorzelinowej jest trudne i wymaga wielospecjalistycznego podejścia z uwagi rzadkość występowania oraz współwystępujące z nią choroby układowe. Nowe kryteria diagnostyczne i opracowane algorytmy leczenia mogą ułatwić postawienie diagnozy, a także skrócić czas do wprowadzenia skutecznej terapii, w tym biologicznej, a obecnie prowadzone badania nad PG wydają się obiecujące

    Aggressiveness of papillary thyroid carcinoma: a comprehensive analysis from molecular mechanisms to clinical applications

    Papillary thyroid carcinoma (PTC) constitutes the predominant subtype among thyroid malignancies. Despite its generally favorable prognosis, certain aggressive subtypes, along with recurrent and metastatic manifestations, substantially affect patient survival outcomes. Recent advancements in the diagnostic and therapeutic strategies for PTC have ushered in a new era, characterized by the integration of molecular mechanisms and imaging-based evaluations. This review offers an integrated perspective of the clinicopathological features, molecular genetic characteristics, epigenetic regulation, and the contribution of the immune microenvironment to the aggressiveness of PTC. Primary inve stigation targets include BRAF/RAS/RET-related molecular mechanisms and the functional significance of non-coding RNAs [especially long non-coding RNAs (lncRNAs) and microRNAs (miRNAs)] in molecular regulation. Additionally, the impact of clinical factors such as age, sex, obesity, and comorbidity with Hashimoto’s thyroiditis on the aggressiveness of PTC is thoroughly examined. Furthermore, this review systematically synthesizes the clinical advances in the early detection and risk assessment of aggressive PTC by emerging imaging modalities such as conventional ultrasound, interventional ultrasound, ultrasound elastography, contrast-enhanced ultrasound, and artificial intelligence-assisted analysis. Looking ahead, multidisciplinary collaborations integrating pathology, genomics, and imaging are anticipated to enhance the precise evaluation of PTC aggressiveness and facilitate the development of individualized treatment strategies. This review serves as a comprehensive reference for mechanistic exploration and clinical translation in the study of PTC aggressiveness, and provides guidance for the progression of precision medicine and management models for PTC patients

    [18F]FDG PET/CT revealing extensive cutaneous involvement in Sézary syndrome

    Sézary syndrome is a rare cutaneous T-cell lymphoma characterized by erythroderma and circulating malignant T cells. This study presents the case of a 31-year-old man with extensive skin involvement detected by 2-deoxy-2-[18F]fluoro-D-glucose positron emission tomography/computed tomography ([18F]FDG PET/CT), demonstrating widespread hypermetabolic lesions across the body. In contrast to computed tomography (CT) or magnetic resonance imaging (MRI), which may overlook subtle or millimetric cutaneous lesions, [18F]FDG PET/CT accurately delineated the extent of disease and provided critical information for staging, therapeutic decision-making, and response assessment. This case underscores the clinical value of [18F]FDG PET/CT in evaluating extensive cutaneous involvement in Sézary syndrome
