    The Prevalence of Peripheral Neuropathy in a Cohort of Egyptians with Prediabetes. Clinical and Electrophysiological Study

    Objective: We aimed to identify the prevalence of neuropathy in prediabetes (NPD) in a cohort of Egyptian subjects and to assess the risk factors associated with its development. Materials and methods: This cross-sectional study was conducted on 152 subjects with prediabetes diagnosed according to the American Diabetes Association. All participants were subjected to clinical assessment, laboratory investigations, and electrophysiological studies to assess the prevalence of NPD. Results: Peripheral neuropathy (PN) was diagnosed in 21.1% of subjects with prediabetes, all of whom had sensory neuropathy. Subjects with prediabetes diagnosed with PN were older, had increased body mass index (BMI), low-density lipoprotein (LDL), hemoglobin A1C (HbA1c), fasting plasma glucose (FPG), and 2-hour postprandial plasma glucose levels (2Hr-PG) and decreased high-density lipoprotein (HDL) levels compared to subjects with prediabetes without PN. However, in a multivariate linear regression analysis, none of the factors were found to be significant risk factors for the development of NPD. Conclusions: Approximately 21.1% of our subjects with prediabetes have been identified with sensory neuropathy. Advanced age, increased BMI, LDL, HbA1c, FPG, and 2Hr-PG levels as well as decreased HDL levels were risk factors of NPD. NPD is likely a multifactorial disease that is influenced by several risk factors

    Association Between Glycation Gap and Renal Complications in People with Type 2 Diabetes

    Objective: This study’s objective is to ascertain the degree of glycosylation gap in individuals with type 2 diabetes (T2D) and how it relates to renal failure. Material and methods: 210 participants, ages 25 to 75 years, were enlisted in this study and divided into three groups. Seventy people without diabetes made up Group 1, which acted as a control. Seventy patients with T2D who did not have chronic kidney disease (CKD) were in Group 2, while 70 patients with T2D who did have CKD were in Group 3. All groups had their levels of fructosamine (FA) and glycated hemoglobin (HbA1c) assessed to calculate the mean blood glucose (MBG), projected HbA1c, and glycosylation gap (GG). Results: In comparison to individuals with diabetes and controls (0.886 ± 0.14; 0.921 ± 0.1), patients with CKD had significantly higher serum creatinine levels (1.96 ± 0.88). In comparison to patients without CKD and controls, the study showed a substantial increase in serum FA, measured and projected HbA1c, and MBG in patients with diabetes with CKD. GG was detected in patients with T2D without CKD (0.631 ± 0.44), patients with diabetes with CKD (1 ± 0.96), and healthy controls (–0.52 ± 0.71). Conclusions: Glycation gap may be associated with renal complication in people with T2D

    Rheumatoid arthritis (RA) is an autoimmune disease that may lead to severe joint damage and mobility impairment. Disease modifying anti-rheumatic drugs (DMARDs) are used in the treatment of RA: conventional, biologic and targeted synthetic. The RA treatment may involve systemic and local adverse event s. The available medical literature describes cases of pathological lesions observed within the oral cavity. They may manifest the major systemic diseases such as: tuberculosis, lupus erythematosus, squamous cell carcinoma, sarcoidosis. The aim of this publication is to prese nt the clinical cases of RA pa tients receiving DMARDs, highlighting anti-TNF-alfa therapy, who were found to develop first symptoms of adverse events in oral cavity. The medical history of oral lesions conducted by a physician could advance the diagnosis and treatment of RA therapy complications.Reumatoidalne zapalenie stawów (RZS) jest chorobą autoimmunologi czną, która może prowadzić do poważnego uszkodzenia stawów i niepełnosprawności ruchowej. W terapii RZS stosuje się leki modyfikujące przebieg choroby (LMPCh): konwencjonalne, biologiczne oraz celowane syntetyczne. Terapia przeciwreumatyczna może wiązać się z występowaniem działań niepożądanych, zarówno ogólnoustrojowych, jak i miejscowych. W dostępnej literaturze medycznej opisywane są przypadki zmian patologicznych w obrębie jamy ustnej w trakcie terapii reumatoidalnego zapalenia stawów. Manifestują one poważne choroby ogólnoustrojowe, takie jak: gruźlica, toczeń rumieniowaty, rak kolczystokomórkowy, sarkoidoza. Celem niniejszej publikacji jest przedstawienie przypadków klinicznych pacjentów chorych na reumatoidalne zapalenie stawów przyjmujących LMPCh, ze szczególnym uwzględnieniem terapii anty-TNF-alfa, u których pierwszymi objawami działań niepożądanych były wykwity w jamie ustnej. Wywiad chorobowy prowadzony przez lekarza podstawowej opieki zdrowotnej w kierunku zmian w jamie ustnej może przyspieszyć proces wykrywania i leczenia powikłań w przebiegu leczenia RZS

    Wyniki badań przeprowadzonych w ostatnich latach wskazują na znaczny wpływ zwyczajów żywieniowych na patogenezę trądziku pospolitego (acne vulgaris). Trądzik pospolity to choroba skóry, której częstość występowania ostatnio wzrosła, obejmując zarówno młodzież, jak i dorosłych. Do procesów odpowiadających za wystąpienie trądziku zalicza się: kolonizację bakterii Cutibacterium acne, zaburzenia rogowacenia jednostek włosowo-łojowych, łojotok i stan zapalny. Dodatkowo wyróżnia się rolę pracy gospodarki hormonalnej, glukozowo-insulinowej, mikrobioty jelitowej oraz stresu oksydacyjnego w patogenezie trądziku. Dieta odpowiada częściowo za modulację powyższych obszarów , dlatego jej jakość może być istotnym czynnikiem odpowiadającym za nasilenie lub łagodzenie zmian trądzikowych. W niniejszym artykule dokonano obiektywnego przeglądu aktualnych doniesień naukowych dotyczących patogenezy trądziku oraz wpływu wybranych czynników żywieniowych na stan skóry trądzikowej. Analiza źródeł wskazuje, że dieta śródziemnomorska jest modelem żywienia, którego charakter może przyczyniać się do polepszenia stanu skóry

    Retrospective 10-year study on Filipino seafarer medical repatriations: comparing two time periods

    Background: Our group has published two 5-year studies on the repatriation rates of Filipino seafarers from 2010 to 2014 and 2015 to 2019. The Maritime Labor Convention (MLC) 2006, which promotes seafarers’ rights, was ratified in the Philippines in 2012. The current study investigates whether any change occurred since then. Materials and methods: We analyzed the total repatriation rates and medical causes of repatriation throughout the 10-year period. We performed a t-test to compare the repatriation rates in both 5-year periods, and χ-square tests for the medical causes. Results: The repatriation rate decreased from 2015 to 2019 with a t-test score of 0.0453 (P < 0.05). Musculoskeletal and gastrointestinal diseases, as well as injuries/traumas were the most frequent causes of repatriation. There was a significant increase in the number of musculoskeletal and psychiatric/ /psychological causes for repatriation, and a decrease in injuries/traumas. Conclusions: A historical review of the 10-year period across the two studies revealed that these changes coincided with implementation of MLC 2006 in August 2013. This was designed as the seafarers’ Magna Carta, promoting and establishing various provisions to improve global seafarers’ welfare including areas such as minimum age, seafarers’ employment agreements, hours of work or rest, payment of wages, onboard medical care, accommodation, food and catering standards, health and safety protection, and accident prevention. Our study shows changes in repatriations that indicate an improvement in trauma rates, and while no method can definitively show statistical correlation or cause and effect, we can safely conclude that there is a positive correlation between the implementation of the 2006 MLC and the repatriation rates of Filipino seafarers

    Zespół Guillaina–Barrégo (GBS, Guillain-Barré syndrome) definiowany jest jako ostra poliradikuloneuropatia zapalna, która może przebiegać pod różnymi postaciami klinicznymi o zróżnicowanym nasileniu objawów. Niewielki odsetek zachorowań, często nieuwzględniany w opisach symptomatologii, stanowią chorzy, u których rozwija się stan naśladujący śmierć mózgu. Do cech śmierci mózgu zalicza się: brak odruchu źrenicznego i rogówkowego, nieobecne odruchy oczno-głowowy i oczno-przedsionkowy, brak odruchu kaszlowego oraz brak reakcji ruchowej na ból. W pracy przedstawiono 77-letniego mężczyznę, u którego po kilkudniowej infekcji górnych dróg oddechowych pojawił się gwałtownie postępujący wiotki niedowład czterokończynowy z arefleksją, dyzartrią, dysfagią i niewydolnością oddechową. Na podstawie wyników badania płynu mózgowo-rdzeniowego oraz badań elektrofizjologicznych ustalono rozpoznanie GBS. Od momentu przyjęcia na oddział do rozwoju objawów naśladujących śmierć mózgu upłynęły niecałe 24 godziny

    The actual approach of sentinel node biopsy in cutaneous melanoma

    Sentinel lymph node biopsy (SLNB) is a well-established standard of care in melanoma treatment, but its role is evolving. Before the advent of modern systemic therapies, SLNB was critical in identifying which patients would benefit from complementary lymphadenectomy and which would not require this procedure.  Currently, after a positive sentinel node biopsy, lymphadenectomy is no longer performed, altering the procedure’s significance. The SLNB identifies patients who will benefit from adjuvant treatment. Additionally, it enhances control over locoregional lymphatic flow. As for now, there is no alternative to sentinel node biopsy.  The gold standard for marking sentinel nodes in melanoma remains the dual radioisotope-dye method, using lymphoscintigraphy and SPECT-CT.  There is ongoing research for new tracers that might replace this method, likely ferromagnetic tracers, which have shown non-inferiority in breast cancer, have not yet proven to be of comparable value in melanoma, due to unpredictable lymphatic drainage, which required preoperative imaging
