    Objective: A significant impact of the patient’s mental health on the management of diabetes and its course has been reported in many studies. Therefore, psychological screening is recommended in individuals with diabetes. The Polish Diabetes Association (PDA) recommends three screening tools — the World Health Organization-Five Well-Being Index (WHO-5), Patient Health Questionnaire-9 (PHQ-9), and Problem Areas in Diabetes 1 (PAID-1) — which have not been based on reliable standards so far and have been guided by the English-language versions. Material and methods: In this article, we briefly discuss results from three studies that analyzed the psychometric properties of the Polish versions of these questionnaires in a large representative group of patients with type 2 diabetes (T2D) who were treated in two Polish clinics. Results and conclusions: We showed that WHO-5 score ≤ 12 and PHQ-9 score ≥ 5 are optimal cutoff points to identify patients requiring clinical assessment of mental health. Moreover, the PAID questionnaire has been shown to have a one-factor structure, and the correlation between the PAID-1 question and the total PAID-20 score confirms the usefulness of the tool in the diagnosis of diabetes distress

    Maternal and Fetal Implications of Gestational Diabetes Mellitus: A Retrospective, Longitudinal Study

    Objective: This study followed Diabetes in Pregnancy Study Group India (DIPSI) guidelines and compared the clinical profiles, and maternal and fetal outcomes of pregnant women with gestational glucose intolerance (GGI) and gestational diabetes mellitus (GDM). Material and methods: This retrospective, single-center study analyzed data from 958 pregnant women between April 2019 and March 2022. All women underwent an oral glucose tolerance test (OGTT) and, based on OGTT values, were categorized as normal, GGI, GDM, or diabetes mellitus (DM). Results: The mean ages for GGI, GDM, and DM groups were 31.14, 31.39, and 33.00 years, respectively. Significant differences were observed in body mass index (BMI), which was highest in the DM group (p = 0.001). Increased conversion rates to GDM were noted in the third trimester. Conversion from GGI to GDM (p = 0.1142) was lower than from normal to GGI (p = 0.005) or GDM (p = 0.006). Women with a family history of diabetes, particularly with both parents affected, had a higher risk of GDM/DM. GDM and DM were associated with adverse outcomes like history of abortion, stillbirth, intrauterine growth restriction, low birth weight, and large-for-gestational-age infants. Preterm birth and oligohydramnios were highest in the DM group. Conclusions: GGI and GDM are associated with maternal and fetal health, increasing the risk of complications. Identification, and appropriate monitoring and management of GGI may help reduce the conversion into GDM and help prevent future adverse outcomes

    Nowotwory stanowią jedną z głównych przyczyn zgonów na świecie, a prognozy wskazują na dalszy wzrost liczby zachorowań. Kluczowym wyzwaniem w onkologii jest personalizacja leczenia, uwzględniająca indywidualne cechy pacjentów, w tym skład ciała. Otyłość, będąca globalnym problemem, zwiększa r yzyko rozwoju wielu nowotworów, takich jak rak endometrium, jajnika, piersi czy jelita grubego. Badania potwierdzają, że wyższy wskaźnik masy ciała (BMI) wiąże się ze większym ryzykiem zachorowania i śmier telnością w pr zypadku wybranych nowotworów. Otyłość wpływa na skuteczność leczenia, zwiększając ryzyko powikłań i zmniejszając efektywność chemioterapii oraz terapii hormonalnej. Z kolei sarkopenia jest powiązana z gorszym rokowaniem u pacjentów onkologicznych. Wpływa negatywnie na przeżycie całkowite (OS) i przeżycie wolne od progresji choroby (PFS), a także zwiększa ryzyko toksyczności leczenia. Miosteatoza czy też otyłość sarkopeniczna stanowią dodatkowe czynniki ryzyka, wpływające negatywnie na tolerancję terapii i jej skuteczność. Precyzyjne dostosowanie dawek leków i właściwa strategia leczenia mogą pop rawić wyniki leczenia i jakość życia chorych

    Primary healthcare in island regions: A comprehensive analysis of trends, insights and future directions

    Background: Primary healthcare in island regions is a critical component of global health systems, yetit faces unique challenges due to geographical isolation, resource constraints and sociocultural factors.Aims: This study aimed to systematically analyze the global research landscape of primary healthcarein islands through a bibliometric analysis. Material and methods: A comprehensive search was conducted to identify relevant publications usingScopus as the primary database. Keyword analysis, co-authorship networks and citation analysis wereemployed to explore research trends, collaboration patterns and influential authors and institutions. Results: The results revealed a significant increase in research output over the past few decades, withthe United States, Spain and Australia emerging as leading contributors. Key research themes includedaccess to care, health workforce, chronic disease management and health promotion. Collaborativeresearch networks were identified, particularly among researchers from island nations and developed countries.The analysis also highlighted the growing importance of digital health technologies in addressingthe challenges of island healthcare. Conclusions: Research on island primary healthcare has grown steadily since 1972, with key contributionsfrom the United States, Spain and Australia. Major themes include clinical care, health systems and policy.Trends evolved from clinical to systemic and demographic focuses, with future directions emphasizingdigital technologies and participatory methods.     Implantacja reducera do zatoki wieńcowej — opis przypadku

    Refractory angina is a chronic condition characterized by chest pain resulting from insufficient myocardial perfusion that cannot be controlled using standard treatment methods such as pharmacotherapy, angioplasty, or coronary artery bypass grafting (CABG). Among modern therapeutic methods, coronary sinus narrowing with the use of a reducer (coronary sinus reducer, CSR) represents a new alternative. The concept of coronary sinus narrowing as an anti-anginal therapy has its origin in preclinical studies, and its effectiveness was confirmed in the ORBITA-COSMIC clinical trial, which showed a significant reduction of angina symptoms in patients with refractory angina. This article presents the case of a 71-year-old patient successfully treated with the implantation of a CSR. The patient, with a history of CABG and coronary angioplasty, had multiple comorbidities and had no further therapeutic options for treating coronary artery disease. Due to persistent angina symptoms and significant exercise intolerance with angina distance of approximately 100 meters, the decision was made to implant a CSR. The procedure was performed through the right jugular vein, and control angiography confirmed the correct positioning and expansion of the device. After a short hospitalization, the patient was discharged home and placed under further outpatient observation. 10 months later the patient reported a significant improvement in exercise tolerance and a reduction in angina symptoms during a follow-up visit. A scintigraphy examination confirmed the positive effects of the applied treatment. This case presents the efficacy of a CSR leading to improved patient quality of life not achievable with available methods of treatment.Oporna dławica piersiowa to przewlekłe schorzenie charakteryzujące się bólem wieńcowym wynikającym z deficytu ukrwienia mięśnia sercowego, którego nie można kontrolować za pomocą standardowych metod leczenia, takich jak farmakoterapia, angioplastyka czy pomostowanie aortalno-wieńcowe. Wśród nowoczesnych metod terapeutycznych zwężenie zatoki wieńcowej z użyciem reducera (CSR, coronary sinus reducer) stanowi nową alternatywę. Koncepcja zwężenia zatoki wieńcowej jako metody przeciwdławicowej ma swoje korzenie w badaniach przedklinicznych, a jej skuteczność została potwierdzona w badaniu klinicznym ORBITA-COSMIC, które wykazało istotne zmniejszenie objawów dławicy u pacjentów z oporną dławicą piersiową. Artykuł przedstawia przypadek 71-letniego pacjenta skutecznie leczonego implantacją reducera. Pacjent z wywiadem przebytego pomostowania aortalno-wieńcowego (CABG) i po angioplastyce wieńcowej był obciążony licznymi schorzeniami współistniejącymi, jak również nie miał dalszej opcji terapeutycznej w zakresie leczenia miażdżycy tętnic wieńcowych. Ze względu na utrzymujące się objawy dławicy oraz znaczące ograniczenie tolerancji wysiłku do około 100 metrów, zdecydowano o implantacji reducera do zatoki wieńcowej. Zabieg wykonano z dostępu poprzez prawą żyłę szyjną, a kontrolna angiografia potwierdziła prawidłowe położenie i rozprężenie urządzenia. Po krótkiej hospitalizacji pacjent został wypisany do domu i poddany dalszej obserwacji ambulatoryjnej. Dziesięć miesięcy po zabiegu pacjent w czasie kontroli kardiologicznej zgłosił znaczną poprawę tolerancji wysiłku i zmniejszenie dolegliwości dławicowych. Wykonane badanie scyntygraficzne potwierdziło pozytywne efekty leczenia. Przypadek opisany w artykule potwierdza skuteczność tej metody leczenia w zakresie poprawy komfortu życia pacjentów z oporną dławicą, nieosiągalnej dotychczasowymi sposobami leczenia

    Rola aspiryny w prewencji pierwotnej schorzeń układu sercowo-naczyniowego

    Aspirin (ASA, acetylsalicylic acid) plays a crucial role in secondary prevention of cardiovascular diseases since several decades, however, its use in primary prevention remains controversial. The benefits of ASA, such as reducing the risk of ischemic events are counterbalanced by an increased risk of bleeding complications. Recent studies and meta-analyses, including Aspirin to Reduce Risk of Initial Vascular Events (ARRIVE), A Study of Cardiovascular Events in Diabetes (ASCEND) and Aspirin in Reducing Events in the Elderly (ASPREE), highlight the limited efficacy of ASA in reducing overall mortalityand emphasize the need for individualized risk assessment. Current guidelines recommend use of ASA only in high-risk cardiovascular patients without an elevated risk of bleeding. Alternative strategies, such as lifestyle modification and statintherapy are gaining prominence in primary prevention. This paper discusses the evolution of recommendations for use of ASA in primary prevention, considering benefit/risk balance, and exploring future research directions.Kwas acetylosalicylowy (ASA, acetylsalicylic acid) od wielu dekad odgrywa kluczową rolę w prewencji wtórnej chorób sercowo-naczyniowych (CVD, cardiovascular disease), jednakże jego zastosowanie w prewencji pierwotnej pozostaje tematem dyskusji. Korzyści wynikające z działania przeciwzakrzepowego ASA, takie jak zmniejszenie ryzyka incydentów niedokrwiennych, są równoważone przez zwiększone ryzyko powikłań krwotocznych. Ostatnie badania i metaanalizy, w tym: Aspirin to Reduce Risk of Initial Vascular Events (ARRIVE), A Study of Cardiovascular Events in Diabetes (ASCEND) i Aspirin in Reducing Events in the Elderly (ASPREE), wskazują na ograniczoną skuteczność ASA w redukcji śmiertelności oraz podkreślają konieczność indywidualnej oceny ryzyka. Aktualne wytyczne zalecają stosowanie ASA jedynie u pacjentów z wysokim ryzykiem sercowo-naczyniowym, bez zwiększonego ryzyka krwawień. Alternatywne strategie, takie jak modyfikacja stylu życia oraz terapia statynami, zyskują na znaczeniu w prewencji pierwotnej.     Obecnie w leczeniu rzutowo-remisyjnej postaci stwardnienia rozsianego (RRMS, relapsing-remitting multiple sclerosis) preferowane są terapie wysoko aktywne (HET, highly effective treatment). Szybkie ich wdrożenie pozwala na wyhamowanie postępu niepełnosprawności oraz wyciszenie aktywności rzutowej na wczesnym etapie choroby. Jedną z terapii HET stanowi alemtuzumab (ALEM) — humanizowane przeciwciało monoklonalne anty CD-52, zatwierdzone do leczenia RRMS. Jego długookresową skuteczność wykazano w licznych badaniach klinicznych, a także podczas obserwacji w codziennej praktyce lekarskiej (RWE, real-world evidence). Jego stosowanie potencjalnie wiąże się z ryzykiem częstszych poważnych działań niepożądanych, dlatego też terapia ALEM zarezerwowana jest dla pacjentów o wysokiej aktywności choroby lub w przypadku nieskuteczności poprzednich terapii. W artykule przedstawiono przypadek leczenia indukcyjnego ALEM w przebiegu RRSM po nieskuteczności terapii I linii oraz autologicznym przeszczepieniu krwiotwórczych komórek macierzystych (autoHSCT) u 33-letniej chorej znajdującej się pod opieką Kliniki Neurologicznej Wojskowego Instytutu Medycznego — Państwowego Instytutu Badawczego (WIM-PIB)

    Biochemical assessment of neuronal damage in patients undergoing endovascular management of ruptured brain aneurysms: a preliminary observation

    Introduction: Due to the characteristics of brain-injured patients, early complications of aneurysmalsubarachnoid hemorrhage (aSAH) can easily go undetected for a period of time, resulting in the delay ofaccurate treatment. Serum biomarkers of neuronal damage, such as glial fibrillary acidic protein (GFAP),neuron-specific enolase (NSE), and S100B protein, may serve as diagnostic tools to improve care inneurocritical illness. This preliminary observation aimed to evaluate the absolute values of those markersof neuronal damage in aSAH or a planned embolization. Material and methods: The study group consisted of 12 patients in need of emergency (i.e. due to aneurysmalaSAH) (n = 4) or planned (n = 8) embolization. GFAP, NSE, and S100B were assessed before and24 hours after the procedure completion. Descriptive statistics were applied to elucidate the dynamics ofmarkers in the peri-procedural period. Results: Brain aneurysm embolization was associated with an increase in NSE and S100B, but not GFAP.A greater increase in the concentrations of these proteins is observed in emergency procedures. Conclusions: Brain aneurysm embolization is likely to be associated with an increase in NSE and S100B,but not GFAP. There are differences in the concentrations of these proteins between patients with aSAH andcontrols. More research is needed to assess the impact of their dynamics on the clinical consequencesof neuronal injury
