    Experiencing complications after metabolic and bariatric surgeries is a risk factor for postoperative emergency department admissions: a retrospective cohort study

    BackgroundEmergency department admissions significantly burden hospital staff and countries' health system. Studies are encouraged for effective and correct utilization of emergency departments. Rational management of obesity-related medical problems and postoperative complications may reduce emergency department visits. This study aimed to determine the rates, characteristics, and antecedents of emergency room admissions after metabolic and bariatric surgeries (MBSs) performed in our hospital. According to our hypothesis, experiencing postoperative complications is the most common reason for emergency department admissions.MethodsThe study was designed as a single-center, retrospective, cohort study. Metabolic and bariatric surgeries performed in our hospital between June 2021 and June 2023 were evaluated. Clavien Dindo Classification was used to classify complications. The reasons for emergency department admissions, re-hospitalization and surgical requirements, time relationships, and possible antecedents were examined in stages.ResultsA total of 153 patients were evaluated in the study. The average follow-up period was found to be 609.63 +/- 222.89. The emergency department admission rate following MBSs was found to be 31%, and the admission rate within the first month was 7.8%. The major complication rate following MBSs was 4.6%. Experiencing complications was the most important antecedent for admissions 1 month postoperatively.ConclusionsIn patients with complications after MBS, the discharge decision should be provided with stricter controls, and outpatient clinic controls should be planned more frequently. Providing training to patients on managing complications should be considered as a strategy that may reduce the number of emergency department visits.Clinical Trial RegistrationACTRN1262400081051

    Vanishing of conserved charges in Cotton gravity

    Cotton gravity was recently introduced as a higher derivative extension of general relativity. The field equations of the theory involve the rank-3 Cotton tensor. Here we show that all solutions of the theory, including the black holes, have vanishing conserved charges, i.e. mass and angular momentum. This result implies that either the theory is unphysical since all the black holes carry the charges of the vacuum and can be created at no energy cost, or the theory has confinement of mass/energy and all other conserved quantities

    Optimization and prediction of colored pervious concrete properties: Enhancing performance through augmented grey wolf optimizer and artificial neural networks

    This research introduces an innovative method that integrates glass fiber reinforcement, pigment integration, and advanced artificial intelligence (AI) modeling to enhance the performance of colored pervious concrete. Glass fiber inclusion improved packing density and decreased porosity, with a reduction from 18.42 % to 11.24 %. The incorporation of pigment (up to 2.5 kg/m3) affected aesthetics while not significantly altering porosity, permeability, or mechanical properties. The addition of glass fibers significantly enhanced compressive and flexural strengths, achieving values of 24.08 MPa and 6.3 MPa, respectively, at a 1 % glass fiber content. Albedo results indicated that solar reflectance varies by color, with white concrete showing the highest reflectance values. The artificial neural network (ANN) and Grey Wolf Optimizer (GWO) models demonstrated high predictive accuracy for key properties, achieving R2 values greater than 0.9 for compressive strength (CS), flexural strength (FS), and Cantabro loss (CL). Data augmentation enhanced model reliability and optimization results. The research indicates that 1 % GF and 0 % pigment are optimal for achieving a balance between strength and durability, providing essential guidance for the high performance, aesthetically adaptable permeable concretes, especially for urban infrastructure applications where, effective stormwater management, reduced heat island effects, and improved durability are critical

    Environmental impact of thermal power plants: Heavy metal analysis in sediments and macroalgae from Karabiga (Çanakkale), Türkiye

    The accumulation of heavy metals in ecosystems and their transfer through the food chain pose significant risks to both human health and aquatic life. In the research, the presence of toxic metals in the marine environment because of thermal power plants and industrial facilities located on the Karabiga coast was evaluated, and the bioaccumulation levels of heavy metals in algae species were determined. Heavy metal (Co, Cr, Cu, Fe, Mn, Ni, Pb, Zn) analyses were performed using the ICP-OES device in sediment and macroalgae samples taken from 5 stations in the Karabiga coastal region in the Northwestern Anatolia of Turkey in September 2023. To assess the extent of heavy metal pollution, several indices were calculated, including the Enrichment Factor (EF), Pollution Factor (CF), Pollution Load Index (PLI), and Geoaccumulation Index (Igeo), which provide insights into the sources and levels of contamination. No significant pollution was detected based on the Pollution Indexes. All measured heavy metal concentrations in sediment samples were below the regulatory limits set by the WWQG (2021) and classified as Class A quality according to the SACS (2015) guidelines, indicating minimal pollution in the study area. Macroalgae species, particularly Cladophora sericea and Gongolaria barbata, showed elevated levels of Cu, Pb, and Zn, exceeding regulatory limits set by ANZFA (2005) and EC (2011). Potential risk calculation methods for human health were performed on 5 analyzed macroalgae samples. Considering the HI values for macroalgae, no metal was detected with a HI value greater than 1, therefore no potential health risks are expected

    Ağrı, eski zamanlardan bu yana çözülmesi zor bir sağlık sorunudur ve yenidoğanların ağrıyı hissettikleri yapılan çalışmalarla ortaya konulmuştur. Yenidoğan ve çocukluk döneminde hissedilen ağrı kişinin ileriki hayatında ağrıya vereceği tepkiyi etkilemektedir. Yenidoğanda ağrının giderilmesinde farmakolojik ve nonfarmakolojik yöntemlerden yararlanılmaktadır. Bu tez çalışması; yenidoğanda topuk kanı alımında buzzy®'nin hissedilen ağrı düzeyine ve yaşam bulgularına etkisini incelemek amacıyla planlandı. Randomize kontrollü deneysel tasarımlı bu çalışmanın verileri, Aralık 2023- Mart 2024 tarihleri arasında bir devlet hastanesinde miadında doğan ve postnatal 0-28 günde olan 100 yenidoğandan elde edildi. Yenidoğanlar 50 girişim, 50 kontrol olacak şekilde randomize olarak iki gruba ayrıldı. Girişim grubuna topuk kanı alma işlemi öncesi Buzzy® cihazı ile titreşim uygulaması, kontrol grubuna ise rutin topuk kanı alma işlemi uygulandı. Bebeklerin araştırma gruplarına göre demografik özelliklerinde; gestasyon yaş ortalaması, doğum kilosu ve boy ortalamaları, cinsiyet, beslenme şekli, anne yanında kalma durumu, anne eğitim düzeyi, mesleği, kronik hastalık varlığı, çocuk sayısı ile girişim ve kontrol grupları arasında anlamlı farklılık bulunmadığı (p>0,05) belirlendi. Girişim grubundaki bebeklerin NIPS (t=-2,095, p=,039) değerlerinin, topuk kanı alma sırasındaki kalp atımının (t=-2,746, p=,007) ve solunum (t=--3,344, p=,001) düzeylerinin kontrol grubuna göre daha düşük, SPO2 (t=3,024, p=,003) değerinin ise daha yüksek ve istatiksel olarak anlamlı olduğu (p0.05). However, the NIPS scores (t=-2.095, p=0.039), heart rate during heel blood sampling (t=-2.746, p=0.007), and respiratory rate (t=-3.344, p=0.001) were significantly lower in the intervention group than in the control group, whereas the SPO₂ levels (t=3.024, p=0.003) were significantly higher (p<0.05). Due to its positive effects on pain relief and vital signs during heel blood sampling, the use of the Buzzy® device is recommended

    A three-dimensional memristor-based hyperchaotic map for pseudorandom number generation and multi-image encryption

    The resistance state of a memristor can be influenced by external stimuli, and these variations can be converted into a pseudorandom sequence through appropriate circuitry and control mechanisms. By leveraging this property, a reliable and complex pseudorandom number generator suitable for encryption can be designed. To enhance the chaotic complexity of memristor-based discrete systems, this paper introduces a three-dimensional hyperchaotic map based on a memristor (3D-HMBM), which integrates a sine-function nonlinearity with a discrete memristor model. Analyzing its dynamical properties via Lyapunov exponents, the 3D-HMBM exhibits evolution from periodicity to chaos and hyperchaos. The complexity of its iterated sequences is verified through metrics such as Spectral Entropy and C0 complexity. Furthermore, the 3D-HMBM displays a unique phenomenon of infinite coexisting attractors. As initial values vary, the system generates attractors at different positions, suggesting that—in theory—an infinite number of attractors exist. Finally, the simulation results are validated via digital-circuit implementation. Building on this foundation, we propose a multi-image encryption algorithm based on the 3D-HMBM, offering a more secure solution for encrypting large volumes of data. Through statistical testing and cryptographic analysis, we confirm the significant potential of the keystream generated by the 3D-HMBM for cryptographic applications

    Methods for transition to owned dogs: A suggestion for Türkiye

    Bu çalışmanın amacı en az on beş bin yıl öncesinde evcilleştirilen, o zamandan bu zamana pek çok konuda insana yardımcı olan ve hatta insanın sadık dostu olarak adlandırılan köpeklerin, son yüzyılda sahipsiz köpeklere dönüşmesiyle ortaya çıkan çeşitli sorunların çözümüne yönelik seçenekleri ortaya çıkarmak, kısaca sahipsiz köpeklerden sahipli köpeklere geçiş aşamasında etkili yöntemlerin neler olabileceğini ifade etmektir. Çalışmada halihazırda sorunun çözümü adına ileri sürülen geçiş yöntemleri önce gruplandırılmış, ardından karşılaştırmalı olarak her bir yöntemin avantajları ve dezavantajları belirtilmiş, sonrasında 2004 yılında yürürlüğe girmiş ve 2021 yılında güncellenmiş olan 5199 sayılı Hayvanları Koruma Kanunu temelinde oluşturulan geçiş yöntemi üzerinden karşılaştırmalar yapılmıştır. Sonuç kısmında ise Hayvanları Koruma Kanunu kapsamında seçilen yöntemin sorunun çözümü adına ne kadar gerçekçi ve sürdürülebilir olduğu analiz edilmiş, nihayetinde ise alternatif çözüm yolları adına öneride bulunulmuştur.The aim of this study is to reveal the ways of solving various problems arising from the transformation of dogs, which were domesticated at least fifteen thousand years ago, which have helped humans in many matters since then and have even been called man's loyal friend, into stray dogs in the last century, briefly to express what effective methods can be used in the transition from stray dogs to owned dogs. In the study, the transition methods currently suggested to solve the problem are firstly grouped, then the advantages and disadvantages of each method are comparatively stated, and then comparisons are made based on the transition method created basis of the Animal Protection Law No. 5199, which entered into force in 2004 and was updated in 2021. In the conclusion part, it is analyzed how realistic and sustainable the method chosen within the scope of the Animal Protection Law is for the solution of the problem and ultimately, suggestions are made for alternative solutions

    Salivary levels of supar, hıf-1α and tnf-α in different grades of stage ııı periodontitis

    The aim of the study is to evaluate saliva levels of Soluble urokinase plasminogen activator receptor (suPAR), Hypoxia-inducible factor-1 alpha (HIF-1α) and tumor necrosis factor-alpha (TNF-α) in stage III grade A, grade B, grade C periodontitis and periodontal health and to understand the roles of these molecules in periodontal inflammation process and also to compare the three biomarkers' discriminative efficacy in periodontal disease. Methods: A total of 80 individuals, 20 with stage III grade A periodontitis (group A), 20 with stage III grade B periodontitis (group B), 20 with stage III grade C periodontitis (group C) and 20 with healthy periodontium (group H) were recruited for this study. Full-mouth clinical periodontal measurements were recorded in periodontal charts. Whole saliva samples were collected to determine the levels of suPAR, HIF-1α and TNF-α in study groups using enzyme-linked immunosorbent assay (ELISA) method. Results: The saliva concentration of suPAR, HIF-1α, and TNF α was significantly higher in group A, group B, and group C compared with group H (p <.05). Additionally, salivary suPAR concentration was significantly higher in group C than in groups A and B (p <.05). Positive statistically significant correlations were observed between three biomarkers and all clinical parameters (p <.05). Conclusions: Increased levels of saliva suPAR, HIF-1α, and TNF α suggest that these molecules may play a role in periodontitis. In addition, the higher salivary suPAR levels in grade C periodontitis compared to other grades suggest that suPAR may be one of the potential molecules that can be used to predict disease progression and periodontal disease classification.     Effect of MIND diet on oxidative stress markers in multiple sclerosis

    The aim of this dietary intervention study is to evaluate the effect of the MIND diet on oxidative stress markers and nutritional status in patients with Multiple sclerosis (MS). Methods: The study was conducted on 30 participants aged of 20–45 who were diagnosed with Relapsing-remitting MS. After the initial evaluation, healthy nutrition recommendations were given to the participants through a MIND diet sample menu list for 8 weeks. At the end of the study, all measurements were repeated. Results: The mean score of MIND diet adherence questionnaire and serum total antioxidant status level increased significantly at the end of the eighth week (p < 0.001 and p < 0.05). Although the results were not significant, serum superoxide dismutase, catalase and glutathione peroxidase levels tended to increase. In females, there was a significant decrease in body weight, body mass index, waist circumference and total body bone-free lean mass at the end of the study (p < 0.05). In addition, mean daily protein intake and protein intake per kg increased significantly in males (p = 0.028 for both variables) and dietary fiber intake increased significantly (p = 0.047) in all participants. Conclusion: This study showed that the MIND diet may has positive effects on increasing the defense system against inflammation and oxidative stress
