Self-care agency for continued employment among breast cancer patients undergoing chemotherapy
Get PDF本研究では、化学療法を受ける乳がん患者の就労継続に関わるセルフケア能力を明らかにすることを目的とした。化学療法期間中に就労を継続していた乳がん患者12名に半構造化面接を実施し、質的帰納的に分析した。その結果、セルフケア能力として、【自分なりの働き方を見出す力】、【働き方の検討に活用できる情報を得る力】、【乳がん治療が及ぼす影響を予測する力】、【副作用症状に応じて自分と家族の日常生活を整える力】、【副作用症状に応じて仕事を調整する力】、【仕事を続けるための支援を得る力】が明らかとなった。化学療法期間中に就労を継続する乳がん患者は、情報獲得と自己の状況に応じた働き方の選択、乳がん治療の影響の予測を踏まえ、副作用症状に応じた日常生活と仕事の調整、支援の獲得という自らの意思と行動を含む力を備えていた。就労継続支援において、これらの能力に着目した援助の必要性が示唆された。This study aimed to elucidate the constructs of self-care agency required for continued employment in breast cancer patients receiving chemotherapy. Semistructured interviews were conducted with 12 patients who had continued employment while undergoing breast cancer chemotherapy, and the interview data were qualitatively and inductively analyzed.
The analysis identified the following six factors as constructs: the ‘ability to discover own ways of working’, the ‘ability to obtain information useful in considering own ways of working’, the ‘ability to estimate the effects of the breast cancer treatment’, ‘the ability to adjust one's and family's daily lives depending on the symptoms of the side effects’, the ‘ability to adjust work depending on the symptoms of the side effects’, and the ‘ability to obtain support to continue employment’. Patients who had continued employment during breast cancer chemotherapy possessed abilities including their own will and behaviors, such as choosing ways of working depending on their conditions by collecting information, adjusting their daily lives and work depending on the symptoms of side effects based on the estimated influence of breast cancer treatment, and obtaining support from others independently. The findings highlight the importance of providing assistance that strengthens these six abilities to support sustaine employment among breast cancer patients receiving chemotherapy

Get PDF本論文では宮城県石巻地域において“ 障がいで町興し” というキャッチフレーズを掲げて活動する就労継続支援B 型事業所である「A 事業所」に注目する。「A 事業所」は「浦河べてるの家」が生み出した当事者研究やミーティングを実践する集まりとしてスタートし、現在水産加工品を仕入れて販売する事業を展開しているが、複数の会社と取引していることを活かし、異なる会社の商品を組み合わせて販売するという工夫をしている。
新型コロナ禍の現在、売上が激減するという課題が浮上しているが、「A 事業所」では新たな販路の開拓や新たな商品の開発を検討している。その際、各地に点在する様々な災害の被災地の商品と連携することが念頭に置かれている。「A 事業所」の理事長は、障がい当事者自身が取り組むこうした試みが、被災地の復興と障がい当事者の心の復興（回復）を同時に達成していくものと考えている。その思いが“ 障がいで町興し” というキャッチフレーズに込められている
Holistic Reorganization of Policy Studies
Get PDFContemporary policy studies require a trans-disciplinary reorganization of the inter-disciplinary assembly of individual sciences. Policies cannot be effective without harmonized dimensions for comprehensiveness, diversity, efficiency and soundness. It is necessary to recognize environmental issues, informatization, aging, globalization, innovated education in a holistic perspective with a trans-disciplinary framework beyond conventional disciplines. The diversity of policy issues further increases the complexity. It is difficult to deal with the present policy themes in a fragmented manner based on conventional individual sciences. Attempts are made to integrate public and private or macro and micro aspects in the emergence of diverse values through transpersonal networks based on progress hi informatics. In the same manner, it cannot be any more feasible to divide the total system into internality and externality to elucidate the whole mechanism. This article stresses breaking occlusion of traditional individual sciences, in order to challenge the complexity of policies, to realize pragmatic policy processes and to enhance holistic policy studies with focus on their actualization involving wide spread agents, organizations and spheres. An appeal is made for the necessity of serious introspection concerning conventional policy studies which have introduced various inconsistencies up to the present centered on tendency to neglect common sense, a negative attitude towards the inevitable perspectives and an exaggeration of the market mechanism driving commons away to externality misunderstanding Adam Smith's context. In particular, this article brings into focus the emergence of co-products through cooperation or collaboration as symbolized by the shareware development. This relates to the arising of civil society based on partnership. Progress in development of the integrated package software, combined with the total simulation model, will support public decision making and cost engineering. In order to meet the coming resource and environmental constraints without possible conflicts, not only an invisible hand but to shake hands in trust and cooperation must be valued with an egalitarian concept. The public consensus to innovate social systems emerging from civil ethics should be secured through opened public information systems and transpersonal communication
