Recent Research Trends in NDBIs for toddlers with Autism Spectrum Disorder ─ By using Scoping review ─
Get PDF本スコーピングレビューは，自閉スペクトラム症（ASD）の乳幼児に対するNaturalistic Developmental Behavioral Interventions（NDBIs）の研究動向を整理し，介入法，強度，保護者の関与，アウトカム指標，研究デザイン，研究地域の観点から分析した。PRISMA-ScRに準拠し，データベース５種で検索し，ASDのNDBIsによる早期介入のRCTを抽出した結果，17件が適格となった。内訳は介入法別でみるとESDM８件，JASPER６件，PRT１件，Pathways１件，MAYAC１件であった。介入強度や保護者関与の程度は研究毎で多様であった。アウトカム指標はESDM・PRT・Pathwaysで標準化された検査，JASPERでは行動指標を使用していた。研究地域は米国が14件，中国２件，欧州１件であった。介入条件や評価方法は研究ごとに異なり，エビデンスの解釈には慎重を要すると考えられた。特にアウトカム指標を何にするかは効果測定に直接的に影響を及ぼすと考えられるため，今後は標準化された検査と臨床的変化を鋭敏に捉えると考えられる行動指標の併用が必要になると考えられた。This scoping review examined recent research trends in Naturalistic Developmental Behavioral Interventions (NDBIs) for toddlers with Autism Spectrum Disorder (ASD), focusing on intervention types, intensity, parental involvement, outcome measures, study design, and study regions. Following PRISMA-ScR guidelines, five databases were searched, and randomized controlled trials (RCTs) on early interventions using NDBIs for ASD were extracted, yielding 17 eligible studies. By intervention type, 8 studies evaluated the Early Start Denver Model (ESDM), 6 examined JASPER, 1 assessed PRT, 1 investigated Pathways, and 1 evaluated MAYAC. Intervention intensity and the degree of parental involvement varied widely across studies. Outcome measures differed: standardized assessments were employed in studies of ESDM, PRT, and Pathways, while behavioral indicators were primarily used in JASPER trials. Geographically, 14 studies were conducted in the United States, 2 in China, and 1 in Europe. Given the diversity of intervention conditions and evaluation methods, interpretation of the evidence requires caution. In particular, the choice of outcome measures directly influences the assessment of intervention effects. Future research should consider combining standardized tests with sensitive behavioral indicators that can capture clinically meaningful changes.Bulletin資料Research Notedepartmental bulletin pape
