    Studi Kasus di Indonesia: Kepatuhan Pengisian Clinical Pathway Stroke Iskemik dan STEMI di Beberapa Rumah Sakit Tahun 2019

    A clinical pathway (CP) is a flow of clinical service provided to patients from entering to leaving the hospital. Clinical pathways are needed as an effort to improve the quality and control the cost of services in hospitals. Some hospitals that have developed CP have not fully implemented it to patients. On the other hand, hospitals that have implemented CP, still encounter general problems, such is low compliance with CP. The purpose of this study was to obtain an overview of compliance with filling out the clinical pathway (CP) form for ischemic stroke and ST-Elevation Myocardial Infarction (STEMI) cases in several hospitals in Indonesia. The research was conducted in public and private hospitals class A and B which have implemented CP for ischemic stroke and STEMI in 2019. Data collection was carried out in 20 hospitals spread across 11 provinces in 5 regions. The research is a case study using a mix method design. Quantitative data collection was carried out by observing/checking the CP form document for ischemic stroke and STEMI, to determine compliance in filling out of ischemic stroke and STEMI CP forms. Compliance is measured in five care services, namely medical care, nursing, support, pharmacy and nutrition. It is stated as compliant if the percentage of filling in the CP is ≥80%. The total number of 1094 CPs consisted of 681 ischemic ischemic stroke CPs and 413 STEMI CPs. The results showed that: 1). Compliance with implementing ischemic stroke CP is very low, only 32 out of 681 (5%) CP filled ≥80% in five care services, 2). Compliance with implementing STEMI CP is also very low, only 81 out of 413 (20%) CP filled ≥80% in five care services.The conclusion of the study showed that compliance with filling CP ischemic stroke and CP STEMI was very low, as measured by 5 types of care services (medical care, nursing care, supporting care, pharmaceutical care, and nutritional care).  Abstrak Clinical pathway (CP) merupakan suatu alur pelayanan klinik yang diberikan pada pasien sejak masuk hingga keluar rumah sakit. Clinical pathway diperlukan dalam upaya meningkatkan mutu dan pengendalian biaya pelayanan di rumah sakit. Beberapa rumah sakit yang telah mengembangkan CP belum seluruhnya mengimplementasikan pada pasien. Di sisi lain rumah sakit yang telah mengimplementasikan CP, masih menemui kendala umum yaitu rendahnya kepatuhan terhadap CP. Tujuan penelitian untuk memperoleh gambaran kepatuhan pengisian formulir clinical pathway (CP) kasus stroke iskemik dan ST-Elevation Myocardial Infarction (STEMI) di beberapa rumah sakit di Indonesia. Penelitian dilakukan di rumah sakit pemerintah dan swasta kelas A dan B yang telah mengimplementasikan CP kasus stroke iskemik dan STEMI pada tahun 2019. Pengumpulan data dilakukan di 20 rumah sakit yang tersebar di 11 provinsi dalam 5 regional. Penelitian berupa studi kasus menggunakan desain mix method. Pengumpulan data kuantitatif dengan cara melakukan observasi/ cek dokumen formulir CP stroke iskemik dan STEMI, untuk mengetahui kepatuhan dalam mengisi formulir CP stroke iskemik dan STEMI. Kepatuhan diukur pada lima asuhan pelayanan yaitu asuhan medis, keperawatan, penunjang, farmasi dan nutrisi.. Dikatakan patuh jika persentase pengisian CPterisi ≥80%. Jumlah total 1094 CP terdiri dari 681 CP stroke iskemik dan 413 CP STEMI. Diketahui dari hasil penelitian bahwa: 1). Kepatuhan melaksanakan CP stroke iskemik sangat rendah, hanya 32 dari 681 (5%) CP terisi≥80% pada lima asuhan pelayanan 2). Kepatuhan melaksanakan CP STEMI juga sangat rendah, yaitu 81 dari 413 (20%) CP terisi ≥80% pada lima asuhan pelayanan. Kesimpulan penelitian menunjukkan bahwa kepatuhan pengisian CP stroke iskemik dan CP STEMI sangat rendah, yang diukur dari 5 jenis asuhan pelayanan (asuhan medis, asuhan keperawatan, asuhan penunjang, asuhan farmasi, dan asuhan nutrisi)

    Tropical countries are the largest contributor to the incidence of Dengue HemorrhagicFever (DHF), but research on risk factors is still independent in various countries, it cannot beconcluded holistically. Through the research design, a systematic review is able to summarize andanswer the causes of DHF in this tropical country. This research method is a systematic review withguidelines following the 2009 PRISMA Checklist. In the initial search, 1,680 articles were foundusing the keyword “risk factors for Dengue Hemorrhagic Fever”, reduced to 274 article titles afteradding the keyword “tropical country”. Furthermore, the relevant abstracts were fi ltered and found37 selected article items. Through critical appraisal of the full text of the article, it was found that 17articles met the selection criteria for further review in this study. The results showed that there were5 major groups of risk factors that were widely studied, namely sociodemography, climatology, placeof dwelling, environment, and behavior. The sociodemographic factor associated with the incidenceof DHF in tropical countries is age. In terms of climatology, temperature and rainfall are importantfactors in the vector breeding process. Rural areas (rural areas) are the place of dwelling with the mostcases of DHF found. The environmental aspect that has been widely studied is mosquito breeding. Themost signifi cant risk behavior factor in transmission was the behavior of hanging clothes. Of the 17articles, it was found that 77.8% of the articles examined environmental variables

    An assessment of the use of insecticides needs to be done because of the threat of insecticide resistance problems in the application of vector control for dengue. A descriptive observational study was conducted to determine the susceptibility status of Ae. aegypti against 0.05% cypermethrin insecticide at Tanjung Emas Port, Semarang. The study was conducted using the WHO standard bioassay, biochemical, and molecular (PCR) assays. Interviews were conducted with selected respondents to find out the history of insecticide use. The results showed that the susceptibility status of the Aedes aegypti mosquito in the perimeter and buffer area with the WHO standard bioassay test was still vulnerable, with the mortality of Aedes aegypti samples collected from the two study sites being 100%. Biochemical tests showed that 0.05% cypermethrin was still susceptible to Ae. aegypti, with a yield of 100% mortality in both study areas. However, the results of the PCR test showed that homozygous resistance and heterozygous resistance were 80% and 20% respectively in the perimeter region, while in the buffer region it was 40% and 60%, respectively. The results of interviews with program managers showed that 100% of the officers had complied with the standard operating procedure for fogging activities with the insecticides used,  i.e. malathion and cypermethrin. Both types of insecticides have been used on a rotational basis. However, the rotation of these insecticides has not been carried out routinely and periodically. Abstrak Evaluasi terhadap penggunaan insektisida perlu dilakukan karena adanya ancaman masalah resistensi insektisida dalam aplikasi program pengendalian vektor demam berdarah dengue. Studi observasional deskriptif dilakukan untuk melihat status kerentanan Aedes aegypti terhadap insektisida sipermetrin 0,05% di Pelabuhan Tanjung Emas Semarang. Studi menggunakan uji bioassay standar WHO, biokimia, dan molekuler (PCR). Kegiatan wawancara dilakukan pada responden terpilih untuk mengetahui riwayat penggunaan insektisida. Responden dalam penelitian ini adalah pengelola program dan masyarakat. Hasil penelitian menunjukkan Ae. aegypti di area perimeter dan buffer dengan uji bioassay standar WHO masih rentan, dengan kematian nyamuk uji yang dikoleksi dari dua lokasi studi  masing-masing sebesar 100%. Uji biokimia  menunjukkan bahwa sipermetrin 0,05% masih susceptible terhadap Ae. aegypti, dengan hasil sebesar 100% kematian di kedua area studi. Namun, uji PCR menunjukkan sudah terdapat proses mekanisme menuju resistensi dengan hasil resisten homozigot dan resisten heterozigot di area perimeter adalah 80% dan 20%, sedangkan area buffer adalah 40% dan 60%. Hasil wawancara dengan pengelola program menunjukkan 100% petugas sudah menerapkan standar, operasi, dan pelaksanaan kegiatan fogging dengan benar menggunakan insektisida bahan aktif malathion dan sipermetrin.     Lethal ovitrap is a new alternative in dengue vector control (Aedes spp.) that needs to be socialized to the public. A study of community knowledge and attitudes towards the use of lethal ovitrap as a vector control tool is required as initial data before field application. The purpose of the study was to describe the knowledge and attitudes of the community towards the use of lethal ovitrap before it was applied in the field. An analytical description study with a cross sectional design was conducted in Pulutan Village, Salatiga City. A total of 107 respondents were taken by proportional random sampling to be interviewed. The results showed that 54.30% of respondents had low knowledge and 57.05% of respondents had a less supportive attitude towards the application of lethal ovitrap. In general, the knowledge and attitude of the community towards lethal ovitrap as a vector control tool is still low. Knowledge and gender variables were shown to be significantly related to the supportive attitude given by the community to the application of lethal ovitrap. Education about lethal ovitrap needs to be done before installing lethal ovitrap in the community.   Abstrak Lethal ovitrap merupakan salah satu alternatif baru dalam pengendalian vektor dengue (nyamuk Aedes spp.) yang perlu disosialisasikan ke masyarakat. Studi tentang pengetahuan dan sikap masyarakat terhadap penggunaan lethal ovitrap sebagai alat pengendali vektor diperlukan sebagai data awal sebelum aplikasi lapangan. Tujuan penelitian adalah untuk mendeskripsikan pengetahuan dan sikap masyarakat terhadap penggunaan lethal ovitrap sebelum diaplikasikan di lapangan. Studi deskripsi analitik dengan rancangan cross sectional yang dilakukan di Kelurahan Pulutan, Kota Salatiga. Total 107 responden yang diambil secara proportional random sampling untuk di wawancarai. Hasil penelitian menunjukkan 54,30% responden memiliki pengetahuan rendah dan 57,05% responden memiliki sikap kurang mendukung terhadap aplikasi lethal ovitrap. Secara umum pengetahuan dan sikap masyarakat terhadap lethal ovitrap sebagai alat pengendalian vektor masih rendah. Variabel pengetahuan dan jenis kelamin terbukti secara signifikan berhubungan dengan sikap mendukung yang diberikan masyarakat terhadap aplikasi lethal ovitrap.     Gambaran Epidemiologi Peningkatan Kasus Chikungunya di Desa Kajongan Kecamatan Bojongsari Kabupaten Purbalingga

    Purbalingga is one of the chikungunya endemic districts which have cases increased until April in 2021 as many as 512 cases from the previous 116 cases in 2020. One of the areas of the increasing case occurred in Kajongan Village, Bojongsari Subdistrict. This study aims were to describe chikungunya cases, identify sources of transmission, and risk factors for increased chikungunya cases. A case-control design was performed in Kajongan Village in March 2021. The sample consisted of 36 cases and 36 controls. Cases are patients with suspected chikungunya, controls are people who are not chikungunya suspects who live close to the suspected case. Data was collected by blood specimen collection, larvae survey, interviews and examination using Reverse Transcriptase-Polymerase Chain Reaction. The results showed that the peak of case transmission occurred in the fourth week of February 2021. The main symptoms are fever and joint pain. Of the 21 blood samples examined, 19 (90.5%) tested positive for Chikungunya virus. Of the 9 pools Aedes sp. analyzed, 1 pool tested also positive Chikungunya virus. Chi-square analysis showed that the presence of larvae in container (p-value=0.03; OR=3.5; 95% CI=0.953-24.746) and mosquito nest eradication has not carried out routinely (p-value=0.04; OR=4.8; 95% CI=0.953-24.746) have been identified as a potential risk factor.     CORRELATION BETWEEN URIC ACID LEVEL WITH BODY MASS INDEX AND HEMATOLOGY PARAMETERS AMONG HYPERURICEMIA PATIENTS

    Hyperuricemia has became the public health issue worldwide. Hematology parameters abnormality is considered to be one of the factors relating the disease such as hyperuricemia. Current study suggest that body mass index (BMI) related to uric acid (UA) levels. The aim of the study was to examine the association between uric acid levels and hematological parameters and BMI among hyperuricemia patients. This was cross sectional study with 50 hyperuricemia patients in Hortus Medicus clinic. Results demonstrate that of the participants, 54% were women but the mean of UA levels was higher in the men (8,41 + 0,80 mg/dL) than in women (7,38 + 0,97 mg/dL). In BMI classified, the mean BMI for men was normoweight, and the women was overweight. The mean of hematology parameters in both of sex were normal range. No significant association were testified between uric acid levels and hemoglobin, hematocrit, erythrocyte, trombocyt, leukocyte and BMI.     PERAN KADER SEBAGAI TENAGA PELAKSANA ELIMINASI PROGRAM PEMBERIAN OBAT PENCEGAHAN MASSAL LIMFATIK FILARIASIS TAHAP III DI KABUPATEN BANYUASIN

    Kabupaten Banyuasin merupakan daerah endemis filariasis di Provinsi Sumatera Selatan dengan jumlah kasus kronis yang dilaporkan pada tahun 2014 sebanyak 89 kasus dan pada tahun 2018 mengalami penurunan menjadi 46 kasus. Koordinasi strategi serta kerjasama yang baik dari berbagai pihak diperlukan untuk mencapai tujuan eliminasi filariasis, salah satunya dengan melibatkan kader sebagai Tenaga Pelaksana Eliminasi (TPE) dalam pengobatan massal filariasis. Tujuan penelitian ini untuk mengetahui peran TPE dalam pelaksanaan Pemberian Obat Pencegahan Massal (POPM) filariasis Tahap III di Kabupaten Banyuasin. Pengumpulan data dilakukan melalui wawancara mendalam dengan informan terpilih yaitu: Pengelola Pogram Filariasis Dinas Kesehatan Kabupaten Banyuasin, Pengelola Program Filariasis Puskesmas di Kabupaten Banyuasin, bidan desa, dan kader. Jenis penelitian ini adalah penelitian kualitatif maka jumlah informan disesuaikan dengan kebutuhan data. Data yang diperoleh dianalisis menggunakan analisis konten. Hasil penelitian menunjukkan pelaksanaan POPM filariasis tahap III di Kabupaten Banyuasin belum berjalan sesuai dengan prosedur. Pada pelaksanaannya, TPE berperan membantu petugas kesehatan melakukan pendataan penduduk sasaran dan penduduk yang mendapatkan obat, mensosialisasikan kegiatan POPM ke masyarakat serta membagikan obat massal filariasis di pos pengobatan maupun rumah ke rumah. Kendala di lapangan yaitu penduduk yang tidak minum obat filariasis tidak tercatat, pengawasan terhadap efek samping obat tidak dilakukan karena minimnya jumlah TPE, anggaran untuk TPE masih minim baik untuk mobilisasi dan pelatihan khusus untuk TPE
