    Calabash Pregnancy: A Malingering Response to Infertility Complicated by Domestic Violence

    This is a case report of a 20-year old para 0+0 who presented with an 11-month pregnancy. On evaluation, the pregnancy was found to be a fake made-up ‘calabash pregnancy’. There were no pregnancy symptoms and she had just menstruated three weeks prior to presentation. This was a deliberate event in response to delayed pregnancy attainment complicated by domestic violence. Domestic violence was in the form of verbal and physical abuse and later was on a monthly basis precipitated by onset of her menstrual flow. The patient’s age, monogamous union and the fact that she is an orphan made her vulnerable to domestic violence. En la evaluación, se encontró que se trataba de un embarazo de calabaza, es decir, inventado, fingido. En realidad, no había ningún síntoma de embarazo, y había tenido la menstruación tres semanas antes de presentarse. Se trataba de un acontecimiento deliberadamente construido, en respuesta a una largamente demorada expectativa de lograr un embarazo, complicada con violencia doméstica. La violencia doméstica se producía en forma de abuso verbal y físico, precipitándose luego mensualmente con la aparición de cada flujo menstrual.     Acute Hepatitis C Infection with Unclear Route of Transmission

    A 43-year old man acquired acute hepatitis C virus (HCV) infection with unclear route of transmission. There were no known sexual or other risk factors for HCV acquisition. Phylogenetic analysis confirmed that the case was infected with identical genotype 1b strain. After symptomatic treatment for three weeks, the HCV was spontaneously cleared and liver function recovered.     Evaluating a Theory-based Health Education Intervention to Improve Awareness of Prostate Cancer among Men in Western Jamaica

    Objective: To evaluate the impact of a theory-based health education intervention on awareness of prostate cancer and intention to screen among men in Western Jamaica. Methods: One hundred and eighty-eight men attending outpatient clinics in a hospital in Western Jamaica completed an interviewer-administered pretest survey. Following the pretest, participants received a health education intervention related to prostate cancer and an immediate post-test survey. Results: There were statistically significant increases in the percentage of correct responses between the pre and post-test (p < 0.05). The greatest improvement was among items measuring knowledge of prostate cancer screening tests. Participant’s moved across the Stages of Change theoretical constructs indicating intention to screen. Conclusion: The sample was receptive to information about prostate cancer and the use of a theory-based educational intervention positively influenced knowledge of prostate cancer risk factors, symptoms, and types of screenings.     Huge Mucinous Cystadenoma Presenting as an Acute Abdomen in the Puerperium

    Adnexa masses occur in one out of two hundred pregnancies (1). Many are benign ovarian cysts diagnosed in women during antenatal care and managed conservatively with success. Occasionally, ovarian accidents occur that necessitate emergency surgeries. However, in women, unbooked during pregnancy, presentation to hospital may occur for the first time in the puerperium and it may pose a diagnostic challenge. We present the case of a 33-year old multipara who presented with acute abdomen in the puerperium.     Aorto-cavitary Fistula: A Complication of Infective Endocarditis

    Aorto-cavitary fistulae are rare complications of infective endocarditis. The diagnosis, in the absence of concomitant aortic valve disease, replacement of aortic valve with homograft or prosthetic valve, periannular abscess and negative blood culture, requires a high index of suspicion and has important prognostic and management significance. The sensitivity of the Modified Duke Criteria is challenged in this case report with a documented right sinus of valsalva fistula to the right ventricle seen on transthoracic echocardiography.
