Angiomatosis of the Breast in a Male Child
Angiomatosis is a benign vascular lesion that has been described rarely in the breast. We describe a case in a seven-year-old boy of African descent who presented with progressively increasing, unilateral breast enlargement, the first such report in a male child. The patient underwent excisional biopsy of the breast mass followed by mastectomy. Pathologic examination revealed a diffuse proliferation of variably-sized, thin-walled vascular channels lined by flattened endothelium that showed negative immunohistochemical staining for von Willebrand factor, factor VIII-related antigen, CD34 and S-100 protein. There is no evidence of recurrence after 24 months of follow-up
IMAGE AND DIAGNOSIS
This 47-year-old female has mixed connective tissue disease
manifested mainly by the cutaneous changes of scleroderma
(systemic sclerosis) for nearly 30 years. She first presented
to dermatology with nodules on the forearms over 15 years
ago. These hard nodules have coalesced and are tender and
oozing at some sites along the forearm (Fig. 1). The legs
have been more problematic. In some areas, the skin has
ulcerated and one can see a yellow-white substance in the
base of the ulcers (Fig. 2). She is able to remove this hard
debris sometimes and this, on occasion, has enabled an ulcer
to heal.
At other sites, there have been sheeted hard masses in
the leg with associated large, painful, purulent ulcers (Fig.3).
Other features of systemic sclerosis such as telangiectasias
and atrophie blanche are seen in Figs. 1 and 3 respectively.
She has been on repeated courses of topical and
systemic antibiotics with only minimal benefits. Colchicine,
nifedipine and diltiazem have not been very helpful so far
