    Investigation of two new grafting techniques for dorsal augmentation rhinoplasty: An experimental study with New Zealand white rabbits

    Introduction Cartilage is an important source in supporting the structure of the nose for dorsal augmentation rhinoplasty. However, it is known that its viability is not always on the ideal level. Various wrapping materials are used to increase the strength of cartilage. Donor site morbidity, which develops following the harvesting of both cartilage and fascia as one such cover material, has attracted interest in recent years. Objective In this study, we aimed to investigate the potential of dermis and tendon autografts as alternatives to fascia and cartilage. Material and Method The sample of the study included 16 New Zealand white rabbits. The right auricular cartilage of all rabbits was amputated, and it was transformed into diced cartilage autografts. The dermis autografts from the right gluteal areas of the rabbits were deepithelialized, and lumbosacral fascia autografts were harvested from the same incision. Additionally, the Achilles tendon of each rabbit was harvested and transformed into diced tendon autografts. Four different autografts were embedded under the skin of each rabbit from 4 different pouches opened in the back of the rabbit. These autografts included diced cartilage alone (Intervention 1), fascia-wrapped cartilage (Intervention 2), dermis-wrapped cartilage (Intervention 3) and fascia-wrapped tendon (Intervention 4) autografts. Results Intervention 1 had the most irregular appearance, the outcomes in Intervention 4 were volumetrically smaller and softer. Connective tissue formed between the diced pieces in all interventions, and it was observed that the dermis and fascia had a capsule-like appearance, and their viability was preserved. The differences between the initial and final measurements of the volumes of interventions 1, 2 and 3 were statistically significant (p 0.05). More peripheral proliferation was observed in the interventions of fascia-wrapped and dermis-wrapped diced cartilage compared to the other interventions. The intervention including fascia-wrapped diced tendon grafts had displayed more fibrosis, fragmentation and collagen fibers, while it showed a lower amount of elastic fiber. There were no significant differences among the intervention in terms of other histological parameters. Conclusion Tendon autografts may be a good option for dorsal augmentation rhinoplasty as they are easily harvested and have minimal donor site morbidity.     Five-year follow-up of 96 weeks peginterferon plus tenofovir disoproxil fumarate in hepatitis D

    Background & Aims: Until recently, pegylated interferon-alfa-2a (PEG-IFNa) therapy was the only treatment option for patients infected with hepatitis D virus (HDV). Treatment with PEG-IFNa with or without tenofovir disoproxil fumarate (TDF) for 96 weeks resulted in HDV RNA suppression in 44% of patients at the end of therapy but did not prevent short-term relapses within 24 weeks. The virological and clinical long-term effects after prolonged PEG-IFNa-based treatment of hepatitis D are unknown. Methods: In the HIDIT-II study patients (including 40% with liver cirrhosis) received 180 μg PEG-IFNa weekly plus 300 mg TDF once daily (n = 59) or 180 μg PEG-IFNa weekly plus placebo (n = 61) for 96 weeks. Patients were followed until week 356 (5 years after end of therapy). Results: Until the end of follow-up, 16 (13%) patients developed liver-related complications (PEG-IFNa + TDF, n = 5 vs PEG-IFNa + placebo, n = 11; p =.179). Achieving HDV suppression at week 96 was associated with decreased long-term risk for the development of hepatocellular carcinoma (p =.04) and hepatic decompensation (p =.009). Including complications irrespective of PEG-IFNa retreatment status, the number of patients developing serious complications was similar with (3/18) and without retreatment with PEG-IFNa (16/102, p >.999) but was associated with a higher chance of HDV-RNA suppression (p =.024, odds ratio 3.9 [1.3–12]). Conclusions: Liver-related clinical events were infrequent and occurred less frequently in patients with virological responses to PEG-IFNa treatment. PEG-IFNa treatment should be recommended to HDV-infected patients until alternative therapies become available. Retreatment with PEG-IFNa should be considered for patients with inadequate response to the first course of treatment. Clinical Trial registration: NCT00932971

    Performance evaluation of geopolymer mortars containing waste ferrochrome slag and fly ash for sustainable green building

    The aim of the present study, an attempt to shed light on the use of industrial-based wastes as alkali-activated binder (AAB) material is mainly. The present novel research work, the characterization of waste ferrochrome slag (FCS) and the performance of alkali-activated mortar consisting of fly ash (FA) were investigated. The characterization of used materials were carried out using advanced microstructural analysis techniques (XRF, XRD and SEM). A total of thirty two mortars are prepared using FCS (90–60%) and FA (10–40%) with 5 M, 10 M sodium hydroxide (NaOH), Na2SiO3/NaOH (SS/SH = 1 and 2) solution. All specimens were cured in an oven at 70 °C and 100 °C for 24 h. After oven curing, the geopolymer mortars were kept in the laboratory for 28 days and thermal and mechanical tests were applied to them. The A5 mixture (SS/SH = 1 with 10%FA, 90%FCS and 5 M NaOH) was found to be optimum in terms of thermal insulation properties, making it suitable for use in sustainable construction in terms of low energy cost through exterior insulation. The C8 mixture (SS/SH = 1 with 40%FA, 60% FCS and 10 M NaOH) was found to be optimum in terms of strength and durability, making it suitable for use in sustainable construction. As a result, in this study, an optimum mixture of waste FCS and FA was obtained and geopolymer building materials that provide thermal insulation and structural performance and are resistant to external influences were produced

    The Effect of Complementary and Alternative Treatment Methods Applied by Women in Postmenopausal Period on Their Menopausal Symptoms and Quality of Life

    Objective: This study was carried out to determine the complementary and alternative treatment methods used by women in the postmenopausal period and their effects on menopausal symptoms and quality of life. Material-Method: This study was designed as a cross-sectional and correlational study, and it was conducted at a Family Health Center in a province in the Southeast Anatolia Region of Türkiye between March and August 2018. The sample consisted of 259 postmenopausal women aged 45-60 registered at the Family Health Center. Data were collected using the Personal Information Form, the Menopause Rating Scale, and the Short-Form 36 (SF-36) Quality of Life Scale. The face-to-face interview technique was employed for data collection. Descriptive statistics, Pearson's correlation coefficient, and multiple linear regression (MLR) analyses were used to analyze the data. Results: It was determined that 77.6% of the women in the postmenopausal period used complementary and alternative treatment methods to reduce menopausal symptoms. The analysis revealed that as postmenopausal women increasingly utilized nutrition, vitamins, and minerals as complementary and alternative medicine methods, their physical role limitations, a subdimension of quality of life, decreased (β = -0.359; p < 0.05). Additionally, physical functioning, another subdimension of quality of life, improved with the increased use of mind-body techniques as a complementary and alternative medicine method (β = 0.273; p < 0.05). Conclusion: Nutritional, vitamin, and mineral treatments were significant predictors of physical role limitations in postmenopausal women, which is a subdimension of quality of life. Similarly, mind-body techniques were found to be significant predictors of physical functioning

    LSTM-ESA HİBRİT MODELİ İLE MR GÖRÜNTÜLERİNDEN BEYİN TÜMÖRÜNÜN SINIFLANDIRILMASI

    Beyin tümörlerinin erken teşhisi, etkili bir tedavi için hayati öneme sahiptir. Manyetik rezonans (MR) görüntüleme, beyin tümörlerini tespit etmede temel bir araç olarak öne çıkmaktadır. Glioma, meningioma, pituitary gibi birçok tümör türü bulunmaktadır. Tümör türünü doğru bir şekilde belirlemek ve bu tespiti yapmak, beyin tümörlerini sınıflandırmanın en zorlu yönlerinden biridir. Geleneksel yöntemlerle hastalık tespiti yerine, yapay zekâ temelli bilgisayar uygulamalarının kullanılması, beyin tümörlerinin tespitinde uzmanlara önemli katkılar sağlayabilir. Özellikle derin öğrenme yöntemleri, medikal görüntülerin işlenmesine dayalı hastalık tespitinde etkili olmaktadır. Literatürde, beyin tümörlerini kategorize etmek için birçok derin öğrenme tabanlı yaklaşım bulunmaktadır. Bu çalışmada, MR görüntüleri ile beyin tümörlerini tespit etmek için bir ESA (Evrişimli Sinir Ağı) ve bir LSTM (Uzun Kısa Süreli Bellek) derin öğrenme katmanının birleştirilmiş olduğu bir model sunulmaktadır. LSTM'nin, ESA'nın özellik çıkarma yeteneklerini destekleyebileceği öne sürülmektedir. Yapılan deneylerde, önerilen LSTM-ESA modelinin standart ESA modelinden daha iyi performans gösterdiği belirlenmiştir. Bu modelin kullanılmasıyla, beyin tümörlerinin tespitinde %98,1 doğruluk skoru elde edilmiştir. Bu sonuç, literatürdeki benzer çalışmalarla karşılaştırıldığında daha yüksek bir başarı elde ettiğini göstermektedir

    Investigating the biology of microRNA links to ALDH1A1 reveals candidates for preclinical testing in acute myeloid leukemia

    Aldehyde dehydrogenase 1 family member A1 (ALDH1A1) is a member of the aldehyde dehydrogenase gene subfamily that encode enzymes with the ability to oxidize retinaldehyde. It was recently shown that high ALDH1A1 RNA abundance correlates with a poor prognosis in acute myeloid leukemia (AML). AML is a hematopoietic malignancy associated with high morbidity and mortality rates. Although there are a number of agents that inhibit ALDH activity, it would be crucial to develop methodologies for adjustable genetic interference, which would permit interventions on several oncogenic pathways in parallel. Intervention in multiple oncogenic pathways is theoretically possible with microRNAs (miRNAs or miRs), a class of small non‑coding RNAs that have emerged as key regulators of gene expression in AML. A number of miRNAs have shown the ability to interfere with ALDH1A1 gene expression directly in solid tumor cells, and these miRNAs can be evaluated in AML model systems. There are indications that a few of these miRNAs actually do have an association with AML disease course, rendering them a promising target for genetic intervention in AML cells

    The Effect of Kahramanmaras Earthquakes on Financial Performance: An Application in Companies Located in the Earthquake Area

    The aim of this study is to examine the impact of the earthquakes that occurred on February 6, 2023, in Kahramanmaras on the financial performance of companies located in the earthquake area and whose stocks are traded on the BIST using Lopcow-based Cobra method. Within the scope of the study Firstly, the financial data of these companies were weighted using the Lopcow method. Subsequently, the Cobra method was employed to rank the financial performance of the relevant companies. According to the ranking results of the Cobra method, it was observed that ARSAN had the best relative financial performance in the 2022Q1 period, SANKO in the periods 2022Q2-2022Q4, and ISDMR in the periods 2023Q1-2023Q3

    Analysis of complications of a neglected disease: 13 Years of experience with liver hydatid cysts in a high-volume hospital

    Background and Objectives: The aim of this study was to evaluate the clinical presentation, treatment outcomes, and complications associated with hepatic hydatid cysts in a pediatric population. Materials and Methods: This retrospective study analyzed 214 pediatric patients with liver hydatid cysts, focusing on clinical presentation, treatment outcomes, and associated complications. Patients were classified based on treatment modality, including non-operative management with albendazole, PAIR, and surgical intervention. This study compared cyst characteristics, recurrence rates, and complications such as cysto-biliary fistulas. Results: Among the patients, 68% (n = 145) had a single cyst and 86% (n = 184) were found to have isolated liver cysts. No significant statistical difference was observed between Group 1 and Group 2 in terms of age, gender, and basic laboratory values and general characteristics of the cysts, such as the lobe where the cyst was located, involvement of multiple organs, number of cysts, the state of cyst rupture, and recurrence; no statistically significant difference was found between the groups (p > 0.05 for each comparison). Cyst rupture incidence was 6%, and the average incidence of recurrence was 2%, with a surgical recurrence incidence of 3%. A total of 37 patients had a laparotomy, while 7 had laparoscopic surgery. In total, capitonnage was performed in 68 patients, omentopexy in 4, and cystostomy in 6. Consequently, among the treated patients (PAIR + surgery), the incidence of cysto-biliary fistula was 11%, anaphylaxis was 2%, surgical recurrence was 3%, and the incidence of reoperation (Clavien-Dindo >= 3) was 6%. The average follow-up period was 72 months, during which no mortality was observed. Conclusions: We identified key clinical outcomes related to both non-surgical treatments (cyst rupture and recurrence) and surgical groups (cysto-biliary fistulas, anaphylaxis, the need for reoperation, rupture, and recurrence)

    Three-dimensional modeling and quantitative assessment of mandibular volume in ectodermal dysplasia: A Case series

    Background and Objectives: Ectodermal dysplasia (ED)-a genetic disorder-is characterized by severe tooth deficiency. We compared the mandibular volume and the sagittal and horizontal mandibular widths between patients with ED (ED group) and individuals without tooth deficiency (control group) using three-dimensional modeling. We hypothesized that the mandibular volume differs in ED cases owing to congenital tooth deficiency. Materials and Methods: We used previously obtained cone-beam computed tomography (CBCT) images of 13 patients with ED. The control group data comprised retrospective CBCT images of patients of similar age and sex with a skeletal relationship of class 1. Further, using the three-dimensional image analysis software, the tooth crowns were separated from the mandible, the mandible was reconstructed and the gonion-to-gonion distance in the mandible was marked, the distance to the menton point was measured, and the distance between the two condyles was measured and compared with the control group. Results: Overall, 46.2% and 53.8% of the participants were men and women, respectively. In the ED group, the mean age of the participants was 15.46 (range, 6-24) years, and the mean number of mandibular teeth was 4.62. Notably, the edentulous mandible volume of the ED group (27.020 mm(3)) was statistically significantly smaller than that of the control group (49.213 mm(3)) (p < 0.001). There was no difference between the two groups in terms of the marked points. For data analysis, the Shapiro-Wilk test, independent samples t-test, and Mann-Whitney U test were used. Conclusions: It has been considered that mandible volume does not develop in ED cases because of missing teeth. Modern practices, such as the CBCT technique and three-dimensional software, may be effective in identifying the true morphologic features, especially in patients with genetic syndromes affecting the maxillofacial structure

    Investigation of the effect of meningich (pistacia terebinthus l.) extract on ischemia/reperfusion injury induced by experimental torsion/detorsion in testis

    Amaç: Bu tezin amacı testiste deneysel torsiyon/detorsiyon sonucu oluşturulmuş iskemi/reperfüzyon hasarında menengiç (Pistacia terebinthus L.) ekstraktının koruyucu ve tedavi edici etkisini araştırmaktır. Gereç ve Yöntem: 48 adet Sprague Dawley türü erkek sıçan 6 gruba ayrıldı. Torsiyon-detorsiyon işlemi için hayvanların sağ ve sol testisleri 720 derece döndürülerek 3 saat torsiyon, ardından 3 saat detorsiyon işlemi uygulandı. Sham grubundaki hayvanların skrotumları cerrahi protokol ile açıldı, başka müdahale yapılmadan kapatıldı. Menengiç grubundaki hayvanlara sham grubundan farklı olarak 15 gün boyunca menengiç ekstresi verildi. Torsiyon-detorsiyon grubundaki hayvanlara 3 saat torsiyon, 3 saat detorsiyon işlemi uygulandı, hayvanlar sakrifiye edildi. Menengiç+torsiyon-detorsiyon grubundaki hayvanlara torsiyon-detorsiyon işleminden önceki 15 gün 2 ml/kg, torsiyon-detorsiyon+menengiç grubundaki hayvanlara torsiyon-detorsiyondan sonraki 15 gün 2 ml/kg, menengiç+torsiyon-detorsiyon+menengiç grubundaki hayvanlaraysa torsiyon-detorsiyondan önce ve sonra 15 gün boyunca 2 ml/kg menengiç ekstresi verildi. Deney sonunda hayvanların testisleri alınarak histopatolojik ve immünohistokimyasal değerlendirmeler yapıldı. Bulgular: Sham ve menengiç gruplarındaki hayvanların testis dokularında düzenli seminifer tübüller, bazal lamina üzerinde 5-6 sıralı germinal epitel izlendi. İntertstisyel alandaki kapillerler ile leydig hücrelerinin görünümlerinin normal olduğu görüldü. Torsiyon-detorsiyon grubundaki hayvanlarda seminifer tübüllerde harabiyet, spermatogenik hücrelerde bozulmalar, İnterstisyel alanda hemoraji, ödem ve damarlarda konjesyon izlendi. Menengiç+Torsiyon-detorsiyon grubundaki hayvanların seminifer tübüllerinde kısmen düzelmeler, damarlarda dilatasyon, konjesyon görüldü. Torsiyon-detorsiyon+menengiç ve menengiç+torsiyon-detorsiyon+menengiç gruplarında bazal laminanın sham grubuna benzer özellikler gösterdiği görüldü, damarlarda konjesyon izlendi. Sonuç: Menengiç ekstraktının testiste torsiyon-detorsiyon sonucu gelişen iskemi-reperfüzyon hasarına karşı testis dokusunun rejenerasyonuna katkıda bulunduğu tespit edildi. Tedavi gruplarında Apaf-1 ekspresyonunu baskılayarak hücrelerin yaşamlarına devam etmelerini desteklediği görüldü. Tedavi gruplarında Sox-9 ekspresyonunu arttırarak spermatogenezis sürecinin devam etmesine katkı sağladığı gözlendi.Aim: The aim of this thesis was to investigate the protective and therapeutic effects of meningic (Pistacia terebinthus L.) extract on ischemia/reperfusion injury induced by experimental torsion/detorsion in the testis. Materials and Methods: 48 male Sprague Dawley rats were divided into 6 groups. For the torsion-detorsion procedure, the right and left testes of the animals were rotated 720 degrees and torsion was performed for 3 hours, followed by detorsion for 3 hours. The scrotums of the animals in the sham group were opened with a surgical protocol and closed without any further intervention. The animals in the meningic group were given meningic extract for 15 days different from the sham group. The animals in the torsion-detorsion group were subjected to torsion for 3 hours and detorsion for 3 hours, and the animals were sacrificed. The animals in the meningic+torsion-detorsion group received 2 ml/kg of meningic extract for 15 days before torsion-detorsion, the animals in the torsion-detorsion+meningic group received 2 ml/kg of meningic extract for 15 days after torsion-detorsion, and the animals in the meningic+torsion-detorsion+meningic group received 2 ml/kg of meningic extract for 15 days before and after torsion-detorsion. At the end of the experiment, testes of the animals were removed and histopathologic and immunohistochemical evaluations were performed. Results: In the testicular tissues of animals in sham and meningic groups, regular seminiferous tubules and 5-6 rows of germinal epithelium were observed on the basal lamina. Capillaries and leydig cells in the interstitial area were normal. In the torsion-detorsion group, destruction of seminiferous tubules, deterioration in spermatogenic cells, hemorrhage, edema and congestion in the interstitial area were observed. Partial recovery in seminiferous tubules, dilatation and congestion were observed in the seminiferous tubules of animals in Menengiç+Torsion-detorsion group. In torsion-detorsion+menengic and menengic+torsion-detorsion+menengic groups, basal laminae showed similar characteristics to the sham group and congestion was observed in the vessels. Conclusion: Menengiç extract was found to contribute to the regeneration of testicular tissue against ischemia-reperfusion injury caused by torsion-detorsion in the testis. It was observed that it supported the survival of cells by suppressing Apaf-1 expression in the treatment groups. It was observed that it contributed to the continuation of spermatogenesis process by increasing Sox-9 expression in the treatment groups
