Comparison of single-incision laparoscopic percutaneous and cystoscope forceps assisted morgagni hernia repair techniques
Objective: Our goal was to compare single-incision laparoscopic percutaneous and cystoscope forceps-assisted Morgagni hernia repair techniques. Materials and methods: A total of 40 patients were allocated to two groups, each with 20 patients. Group 1: Single incision (port) laparoscopic surgical percutaneous Morgagni hernia repair (with a 5 mm Storz laparoscopic scope entered through the umbilicus). Group 2: Single incision (port) laparoscopic surgical percutaneous Morgagni hernia repair (with an 11 Fr [3.6 mm] cystoscope entered through the umbilicus + using forceps + sac plication, and sac cauterization). In Group 1; the sac was not removed. In group 2; we advanced the forceps through the cystoscope, caught the sac, pushed the needle through the sac, plicated the sac, and then cauterized the sac with Bugbee electrode. Results: Of the 40 patients, 70 % (n = 28) were male. The symptoms at admission included repeated chest infections (40 %), dyspnea (30 %), vomiting (22 %), and abdominal pain (22 %). No difference was found between groups in terms of age, gender symptomatology, or associated anomalies. The operation time was shorter in group 2 compared to group 1 (p 0.05). Conclusions: Cystoscope-assisted repair of Morgagni hernia was found to be superior in terms of safety and shorter operation time. Level of evidence: Type III. The effects of technological developments on the accounting profession: A research on professional members in Diyarbakır province
Bu çalışma, muhasebe mesleği üzerinde teknolojik gelişmelerin etkilerini incelemekte ve Diyarbakır ilindeki muhasebe meslek mensuplarına yönelik bir araştırmayı kapsamaktadır. Çalışmada kullanılan veriler, birincil kaynaklardan elde edilmiş olup, belirlenen örneklem üzerinden toplanan veriler anket yoluyla toplanmıştır. Anket verileri SPSS programı aracılığıyla Cronbach Alpha iç tutarlık testine tabi tutulmuş ve anketin güvenirlik katsayısı 0,890 olarak bulunmuştur. Bu yüksek güvenilirlik değeri, anketteki maddelerin tutarlılığını ve güvenilirliğini göstermektedir. Katılımcıların demografik özellikleri incelendiğinde, %49'unun kadın, %51'inin erkek olduğu, yaş aralığında ise en yoğun grubun %30 ile 26-35 yaş arası olduğu görülmüştür. Eğitim durumu açısından katılımcıların %24,8'i yüksek lisans, %21,5'i lisans mezunu iken, mesleki tecrübeleri 8-11 yıl arasında olanların %25,2 ile en yüksek orana sahip olduğu tespit edilmiştir. Çalışma ifadelerine verilen yanıtlar analiz edilmiştir ve teknolojinin muhasebe mesleğine olan etkileri değerlendirilmiştir. Katılımcıların büyük bir kısmı, teknolojinin evrak yükümlülüğünü azalttığını (%35,2), iş verimliliğini artırdığını (%26,8) ve iş yükünü azalttığını (%35,0) ifade etmiştir. Ayrıca, dijital teknolojilerin güvenlik ihlallerini tetiklediği görüşü de %33,2 oranında destek bulmuştur. Araştırma hipotezlerinin test edilmesi için T-testi ve ANOVA analizleri yapılmış, cinsiyet, yaş, eğitim durumu ve mesleki tecrübe değişkenleri ile muhasebe mesleğinde teknolojik gelişmelerin etkisi arasında anlamlı farklılıklar tespit edilmiştir. T-testi sonuçlarına göre, kadın katılımcıların erkeklere göre teknolojik gelişmelere daha fazla ilgi gösterdiği belirlenmiştir. Yaş değişkeni ile yapılan ANOVA analizinde, yaşlı katılımcıların genç katılımcılara göre teknolojik gelişmelerin etkisini daha yüksek düzeyde algıladığı tespit edilmiştir. Eğitim durumu değişkenine göre yapılan ANOVA analizinde ise, eğitim seviyesi yüksek olan katılımcıların teknolojik gelişmeleri daha olumlu değerlendirdiği belirlenmiştir. Mesleki tecrübe değişkeni ile yapılan analizlerde, uzun süreli tecrübeye sahip katılımcıların teknolojik gelişmelere daha olumlu baktığı tespit edilmiştir. Bu bulgular, muhasebe meslek mensuplarının teknolojik gelişmelerin mesleklerine olan etkilerini nasıl algıladığını ve bu algıların demografik özellikler ile nasıl değişiklik gösterdiğini ortaya koymaktadır. Çalışma, muhasebe mesleğinin teknoloji ile entegrasyonunun önemini vurgulamakta ve bu entegrasyonun iş verimliliği, iş yükü, güvenlik ve mesleki etik gibi çeşitli alanlarda önemli etkileri olduğunu göstermektedir.This study examines the effects of technological developments on the accounting profession and includes a research on accounting professionals in Diyarbakır province. The data used in the study were obtained from primary sources and the data collected from the sample was collected through a questionnaire. The survey data were subjected to Cronbach Alpha internal consistency test through the SPSS program and the reliability coefficient of the questionnaire was found to be 0.890. This high reliability value indicates the consistency and reliability of the items in the questionnaire. When the demographic characteristics of the participants were analyzed, it was seen that 49% of the participants were female and 51% were male, and the most intense group in the age range was between 26-35 years old with 30%. In terms of educational status, 24.8% of the participants had a master's degree and 21.5% had a bachelor's degree, while those with 8-11 years of professional experience had the highest rate with 25.2%. The responses to the study statements were analyzed and the effects of technology on the accounting profession were evaluated. The majority of the respondents stated that technology reduces paperwork burden (35.2%), increases work efficiency (26.8%) and reduces workload (35.0%). There was also 33.2% support for the view that digital technologies trigger security breaches. T-test and ANOVA analyses were conducted to test the research hypotheses, and significant differences were found between gender, age, educational level and professional experience variables and the impact of technological developments in the accounting profession. According to the results of the T-test, it was determined that female participants showed more interest in technological developments than male participants. In the ANOVA analysis conducted with the age variable, it was determined that older participants perceived the impact of technological developments at a higher level than younger participants. In the ANOVA analysis conducted according to the education level variable, it was determined that the participants with higher education level evaluated technological developments more positively. In the analysis conducted with the professional experience variable, it was determined that the participants with long-term experience had a more positive view of technological developments. These findings reveal how accounting professionals perceive the effects of technological developments on their profession and how these perceptions vary with demographic characteristics. The study emphasizes the importance of the integration of the accounting profession with technology and shows that this integration has significant effects in various areas such as work efficiency, workload, security and professional ethics
Altered luteal expression patterns of genomic and non-genomic progesterone receptors in bitches at different reproductive states
The binding of steroid hormones to their specific receptors is necessary to exert their effects on target cells. Progesterone (P4), a steroid hormone, carries out its effects through both genomic and non-genomic (the cell membrane-associated) receptors. This study aimed to ascertain luteal expression patterns of genomic and non-genomic progesterone receptors in bitches in physiological (early dioestrus and early pregnant) and pathological (pyometra) reproductive states. Luteal tissue was collected from the bitches at early dioestrus (ED, n = 5), early pregnant (EP, n = 5), and pyometra (PY, n = 5). The expression profiles of Steroidogenic Acute Regulator Protein (STAR), Progesterone Receptor (PGR), Membrane Progestin Receptors (PAQR5, PAQR7 and PAQR8), and Progesterone Membrane Components (PGMRC1 and PGMRC2) were examined at the mRNA levels using Real-Time Polymerase Chain Reaction (RT-PCR). Protein levels of PGR, PGMRC1 and PGMRC2 were detected by western blotting (WB). The STAR expression was found in all groups, with a statistical difference observed between EP and PY groups (P < 0.05). The protein level of PGR was determined to be highest in the EP group and lowest in the PY group. The expression of PAQR8 increased in the EP group (P < 0.05). The PAQR5 exhibited high expression in the EP group and low expression in the PY group (P < 0.05). PGRMC1 was more elevated in the EP group and lower in the PY group (P < 0.05). Protein levels of PGMRC1 and PGMRC2 were also observed at the highest expression in EP group. According to the altered expression profiles for examined receptors, we suggest that those progesterone receptors have roles in early pregnancy or pyometra in bitches
Classification of Faults in Power System Transmission Lines Using Deep Learning Methods with Real, Synthetic, and Public Datasets
Every part of society relies on energy systems due to the growing population and the constant demand for energy. Because of the high energy demands of transportation, industry, and daily life, energy systems are crucial to every part of society. Electrical transmission lines are a crucial component of the electrical power system. Therefore, in order to determine the power system's protection plan and increase its reliability, it is critical to foresee and classify fault types. With this motivation, the main goal of this paper is to design a deep network model to classify faults in transmission lines based on real, generated, and publicly available datasets. A deep learning architecture that was based on a one-dimensional convolutional neural network (CNN) was utilized in this study. Accuracy, specificity, recall, precision, F1 score, ROC curves, and AUC were employed as performance criteria for the suggested model. Not only synthetic but also real data were used in this study. It has been seen that the created model can be used successfully for both real data and synthetic data. In order to measure the robustness of the network, it was tested with three different datasets consisting of real, generated, and publicly available datasets. In the paper, 1D CNN, one of the machine learning methods, was used on three different power systems, and it was observed that the machine learning method was successful in all three power systems
Post occupancy evaluation of traditional buildings: The case of Kırklareli yayla neighborhood
Geleneksel yapıların kullanım süreçlerinde meydana gelen değişiklikler dönemin fiziksel, sosyo-kültürel ve ekonomik özellikleri başta olmak üzere birçok etmenin etkisiyle oluşmaktadır. Bu süreçte yapının özgün hali ve mekânsal özellikleri de büyük değişikliklere maruz kalabilmektedir. Kullanıcılar, bu yapılarda kendi ihtiyaçlarına cevap verecek niteliklere göre bazı müdahalelerde bulunmaktadırlar. Fakat bu müdahaleler, yapının özgün mimari dokusuna ve kimliğine zarar verecek şekilde yapılırsa o geleneksel yapının özgünlüğünü kaybettirerek temel niteliklerine zarar verebilmektedir. Bu durum geleneksel yapıların gelecek kuşaklara aktaracağı değerlerin giderek azalmasına sebep olmaktadır. Bu nedenle geleneksel yapılarda kullanıcıların isteklerine yönelik yapılan işlevsel ve mekânsal değişiklikler yapının özgünlüğüne zarar vermeden kontrollü bir şekilde gerçekleştirilmelidir. Ülkemizde geleneksel yapılarda özellikle ekonomik etmenlerden ötürü yapılan değişiklerin başında işlev değişikliği gelmektedir. Mesela konut olarak kullanılan bir geleneksel yapı ticari getirisinden ötürü kafe, otel vs. birçok farklı mekana dönüştürülerek eski işlevinden neredeyse tamamen farklı bir işlev kazanabilmektedir. Bu değişiklik, yapıdaki mekânsal değişiklikleri de beraberinde getirmektedir. Oda olarak kullanılan mekanların birleştirilerek yemek salonu, eğlence mekanı gibi farklı mekanlara dönüştürüldüğü birçok örneğe rastlanmaktadır. Bu örneklerden biri de çalışmada kullanılan Kırklareli Yayla Mahallesi'nde bulunan geleneksel yapılardır. Bu çalışmanın amacı Kırklareli geleneksel yapılarında yapılan işlev değişikliklerinin kullanım sonrası yaşanan sorunlarını ve yapıların özgünlüğünde medyana gelen değişimleri irdeleyerek bunları en aza indirmek için çözüm önerileri oluşturmaktır. Bu kapsamda konutlardaki eski ve yeni mekansal özellikler değerlendirilerek kullanım sonrası oluşan değişimler ve bunların yapıya kattığı olumlu ve olumsuz durumların tespit edilmesi amaçlamaktadır.The changes that occur in the processes of using traditional structures are caused by the influence of many factors, especially the physical, socio-cultural and economic characteristics of the period. In this process, the original state and spatial characteristics of the structure may also be subject to major changes. Users make some interventions in these structures according to the qualities that will meet their needs. However, if these interventions are made in a way that damages the original architectural texture and identity of the building, they may damage the basic qualities of that traditional structure by making it lose its originality. This situation causes the values that traditional structures will transmit to future generations to decrease gradually. For this reason, functional and spatial changes made in traditional structures for the wishes of users should be carried out in a controlled manner without harming the originality of the structure. At the beginning of the changes made in traditional structures in our country, especially due to economic factors, there is a change in function. For example, a traditional building used as a residence is a cafe, hotel, etc. due to its commercial return. by being converted into many different places, it can gain an almost completely different function from its former function. This change also brings about spatial changes in the structure. There are many examples where the spaces used as rooms are combined and converted into different spaces such as a dining hall, an entertainment venue. One of these examples is the traditional buildings located in the Kırklareli Highland Neighborhood used in the study. The aim of this study is to create solution suggestions to minimize the problems experienced after the use of the functional changes made in the traditional structures of Kırklareli and the changes in the originality of the structures to the median by examining them. In this context, it aims to evaluate the old and new spatial features in the houses and to determine the changes that occur after use and the positive and negative situations that they add to the structure
Comparative outcomes of early, elective, and delayed treatment for lateral condyle fracture of the humerus in children: A retrospective study from a single center in Turkey (2013-2021)
Background: Lateral condyle fracture of the humerus in children should be diagnosed and treated quickly to avoid the complications of malunion and varus deformity of the elbow. Worldwide, pediatric orthopedic departments experienced delays in patient diagnosis and treatment during the COVID-19 pandemic. This retrospective study from
a single center in Turkey aimed to compare outcomes from early treatment, elective treatment, and delayed
treatment in 140 children with lateral condyle fracture of the humerus between 2013 and 2021.
Material/Methods: In the study, 140 patients with Milch type 2 fractures were included. Patients underwent fixation with K-wires
or screws after open or closed reduction. Data collected included age, sex, trauma details, surgery timing, operating conditions, perioperative issues, and rehabilitation outcomes. Fracture union and complications were
monitored through clinical examinations and X-rays. Clinical outcomes were evaluated using the Mayo Elbow
Performance Score (MEPS).
Results: There were 58 patients in the early group, 52 in the elective group, and 30 in the delayed group. Surgery durations varied among the groups (P=0.000). The early and delayed groups as well as the early and elective
groups had significantly different incision sizes (P=0.000 for both). The early and delayed groups and the early and elective groups had significantly different MEPS scores (P=0.002 and P=0.011, respectively).
Conclusions: In patients with late-presenting lateral condyle fractures, although complications increase, surgical treatment
does not yield worse outcomes. Standardization of fracture management should be maintained during periods such as COVID-19
Comparison of serum USP9x and TGF-β levels in children with autism spectrum disorders with healthy controls
Introduction: USP9X has been associated with neurodevelopmental disorders due to its role in synaptic development and neural function. This study aimed to compare USP9X and TGF-(3 levels in children with autism and healthy controls, and explore their relationship with autism severity. Methods: Serum USP9X and TGF-(3 levels were measured in 41 healthy control children (aged 3-12 years) and 41 children with autism. Results: Our study revealed a significant increase in USP9X levels (p=0.001) among children with autism compared to controls. However, TGF-(3 levels showed no significant difference between the two groups. Furthermore, we observed a positive correlation between difficulty in making eye contact subscale and blood levels of both USP9X and TGF-(3. Conclusions: This study is the first to compare serum USP9X levels in children with autism to healthy controls. Our findings suggest USP9X's potential role in autism development, emphasizing the need for further research on its involvement in neurodevelopmental processes
Blood transfusion in pediatric intracranial tumor surgery
Background Pediatric central nervous system tumors are the most common solid tumors in children and leading cause of cancer-related morbidity and mortality. Various factors may influence the practice of blood transfusion during this tumor diagnosis. The primary aim of this study was to determine the factors that may influence intraoperative blood transfusion in pediatric patients undergoing surgery for intracranial tumors and to predict patients who may require blood transfusion. Methods A retrospective study was performed in all pediatric patients younger than 15 years who underwent craniotomy for brain tumor removal from January 2018 to December 2023 in our institution. Preoperative, intraoperative and postoperative data were collected from medical and store anesthesia records. The predictors of intraoperative blood transfusion were determined using multivariate logistic regression. Results A total of 138 patients were enrolled in the study, of whom 62 (44.9%) required intraoperative blood transfusion. In multivariate regression analysis age 490 min were determined as independent variables in terms of need for intraoperative blood transfusion. It was determined that the need for transfusion was higher in patient who were operated on urgently and patients with comorbidities (p = 0.023, p = 0.005). Conclusion In conclusion, the findings obtained in this study suggest that age and surgical duration are independent risk factors for intraoperative blood transfusion in pediatric patients undergoing surgery for intracranial tumors. Particularly, in younger patients and prolonged surgeries, closer monitoring and awareness may enhance early detection, leading to the prevention of complications
Comparison of refugee patients with cystic fibrosis and their counterpart children from Turkey during the war
Since the outbreak of the Syrian civil war in 2011, the population of Arab refugees in Turkey has rapidly increased. While cystic fibrosis (CF) is believed to be rare among Arabs, recent studies suggest it is underdiagnosed. This study aims to present the demographic, clinical, and genetic characteristics of CF patients among Arab refugees in Turkey. Additionally, a comparison is made between the findings in the National CF Registry 2021 in Turkey (NCFRT) and the refugee CF patient group. The study included refugee patients between the ages of 0 and 18 years who were diagnosed with CF and received ongoing care at pediatric pulmonology centers from March 2011 to March 2021. The study examined demographic information, age at diagnosis, age of diagnosis of patients through CF newborn screening (NBS), presenting symptoms, CF transmembrane conductance regulator (CFTR) mutation test results, sputum culture results, weight, height, and body mass index (BMI) z score. Their results were compared with the NCFRT results. The study included 14 pediatric pulmonology centers and 87 patients, consisting of 46 (52.9%) boys and 41 (47.1%) girls. All of the patients were Arab refugees, with 80 (92%) being Syrian. All the patients were diagnosed in Turkey. The median age at diagnosis of patients was 22.33 (interquartile range, 1-258) months. The median age of diagnosis of patients through NBS was 4.2 (interquartile range, 1-12) months. The median age of older patients, who were unable to be included in the NBS program, was 32.3 (interquartile range, 3-258) months. Parental consanguinity was observed in 52 (59.7%) patients. The mutation that was most frequently found was F508del, which accounted for 22.2% of the cases. It was present in 20 patients, constituting 32 out of the total 144 alleles. There was a large number of genetic variations. CFTR genotyping could not be conducted for 12 patients. These patients had high sweat tests, and their genetic mutations could not be determined due to a lack of data. Compared to NCFRT, refugee patients were diagnosed later, and long-term follow-up of refugee CF patients had significantly worse nutritional status and pseudomonas colonization. Conclusion: Although refugee CF patients have equal access to NBS programs and CF medications as well as Turkish patients, the median age at diagnosis of patients, the median age of diagnosis of patients through NBS, their nutritional status, and Pseudomonas colonization were significantly worse than Turkish patients, which may be related to the difficulties of living in another country and poor living conditions. The high genetic heterogeneity and rare mutations detected in the refugee patient group compared to Turkish patients.
