Juvenile Glaucoma Related to Pineal Tumor Post Ventriculoperineal Shunt Procedure: A Case Report
Background: Glaucoma is a cause of irreversible blindness worldwide. This study aims to describe a case of juvenile open-angle glaucoma with a history of post-ventriculoperitoneal shunt perineal tumor.
Case presentation: A 27 years old man came with complaints of blurred vision in both eyes about 2 years ago. The vision in both eyes is felt to be progressively decreasing, almost the same in both eyes. Blurred vision felt suddenly while driving 1 year ago. The patient had been diagnosed with a pineal tumor and underwent surgery to install a ventriculoperitoneal (VP) shunt for hydrocephalus indications 3 years ago. The patient also underwent radiotherapy 26 times for the treatment of the pineal tumor. Ophthalmological examination revealed 20/80 OD and OS vision; not advanced with pinhole, no eyelid edema, intraocular pressure OD 21 mmHg, OS 23 mmHg. Visual fields index on OD 85% and OS 81%. Central corneal thickness (CCT) examination showed CCT of the right eye at 541 mm and left eye at 547 mm. The results of a brain CT scan showed that there was a heterogeneous solid mass with calcification and cystic (necrotic) in the pineal region. This patient was diagnosed with juvenile glaucoma with bilateral papillary atrophy et causa pineal tumor post VP shunt. The therapy given was latanoprost 1 drop/day ODS and Cendo Lyteers 1 drop/6 hours ODS, and Thymol 0.5% 1 drop/12 hours ODS.
Conclusion: Monitoring and evaluation of staging and clinical progression of glaucoma patients with increased intracranial pressure is a condition that needs attention. The correlation between the two is the involvement of the lamina cribrosa in the form of a translaminar pressure gradient
Intestinal Malrotation with Midgut Volvulus Comorbidity with Bronchopneumonia: A Case Report
Background: Intestinal malrotation is a congenital anomaly resulting from abnormal rotation and fixation of an imperfect midgut during embryonic development. This study aims to describe the diagnosis and management of intestinal malrotation and its comorbid diseases in children.
Case presentation: A boy, aged 9 months, was referred from a regional hospital with complaints of repeated vomiting accompanied by a distended stomach and no bowel movements for 4 days. Based on the birth history, the patient was born with vacuum assistance for the indication of prolonged phase II. The results of the physical examination showed that the patient was seriously ill, with compos mentis, pulse rate 130 x/minute, respiratory rate 34x/minute, blood pressure 90/60 mmHg, temperature 37.6ºC, 99% saturation with the help of O2 nasal canule 2L/minute. There are increased bowel sounds on abdominal examination. Blood examination showed an increase in leukocytes (10.850/mm3). The chest X-ray showed lateral bronchopneumonia, and the abdominal X-ray showed low-lying obstructive ileus. The patient was diagnosed with obstructive ileus due to volvulus, moderate dehydration, comorbid with bronchopneumonia, and global developmental delay.
Conclusion: Management of intestinal malrotation with volvulus in this patient includes general improvement and rehydration, Ladd's procedure for definitive correction of the cause. Management of comorbid global developmental delay is carried out after the emergency is resolved
Extensive Actinomycetoma Clinical Manifestations and Histopathology: A Case Report
Background: Actinomycetoma is a chronic infection of cutis, subcutaneous, bone, and viscera tissue. It is characterized by sinuses containing masses of causative organisms often referred to as "grains." Actinomycetoma is caused by aerobic bacteria, and they have been misclassified as a fungal species. Diagnosis of actinomycetoma was based on clinical features and confirmation by direct examination. This study aimed to describe the clinical and histopathological features of actinomycetoma.
Case presentation: A 34-year-old woman complained of a mass on her left leg that had increased in number and pain 3 months before admission to the hospital with a history of trauma. Regio pedis sinistra has trias mycetoma, such as edema (swelling), multiple sinuses, and grain with hypertrophic scars. Histopathological with fibro collagenous connective tissue, moderate to dense diffuse PMN inflammatory cells, plasma cells, lymphocytes, hemosiderophages, multinucleated giant cells, epithelioid cells, and microorganisms. In periodic acid Schiff (PAS) there is the Splendore-Hoeppli phenomenon.
Conclusion: Diagnosis of actynomycetoma was confirmed by the mycetoma triad and the Splendore-Hoeppli phenomenon. Actinomycetoma is more invasive and can cause bone destruction more quickly than eumycetoma
Factors Related to Violence Against Children in Families in Kebayakan District, Central Aceh Regency in 2022
Background: There have been 3,087 cases of violence against children, including 852 physical violence, 768 psychological, and 1,848 cases of sexual violence. This figure is quite high. In Central Aceh, in 2019, there were 20 cases of violence against children, while in 2020, there were 22 cases of violence against children consisting of physical, sexual, and psychological violence to child neglect. This study aims to determine the factors associated with violence against children in the family of Kebayakan District, Central Aceh Regency.
Methods: The research design was analytically observational with a cross-sectional approach. A total of 62 subjects participated in this study, where the subjects were mothers with children aged 6-15 years. Data analysis was carried out univariate and bivariate in order to determine the factors that play a role in child abuse.
Results: There is no relationship between family income (p-value = 0.272 (p> 0.05), there is a relationship between the mother's education (p-value = 0.033 (p <0.05), and there is no relationship between mother's psychological state (p-value = 0.465 (p>0.05) with violence against children in the family
Conclusion: A mother’s education is a factor associated with violence against children in the family
Relationship between Primary Tumors of Nasopharyngeal Carcinoma with the Degree of Conductive Hearing Loss in Dr. Mohammad Hoesin Hospital Palembang
Background. Nasopharyngeal carcinoma is a malignant tumor that grows in the nasopharyngeal area with predilection in the fossa Rossenmuller and the nasopharyngeal roof adjacent to the Eustachian tube, so one of NPC’s early symptoms is ear symptoms. Hearing loss is a common symptom found in people with NPC due to dysfunction of the Eustachian tube, a continuing middle ear disorder that can result in conductive hearing loss.This study aims to find out the relationship between primary tumor of NPC and the degree of conductive hearing loss at dr. Mohammad Hoesin Hospital Palembang.
Methods. This is a cross sectional study that obtained 42 samples from the medical records at Dr. Mohammad Hoesin Hospital Palembang that met the inclusion and exclusion criteria. The study subjects collected in total sampling have been conducted audiometry examinations at the ORLHNS clinic of Dr. Mohammad Hoesin Hospital Palembang during the period January 2019 - April 2021.
Results. The proportion of hearing loss in NPC patients in this study was 30 subjects (71.4%) with the highest proportion of hearing loss complaints being 33.3%. The proportion of conductive hearing loss of nasopharyngeal carcinoma patients in the study was 33 subjects (78.5%) right ear and 28 subjects (66.7%) left ear. There was a significant association between the degree of the NPC primary tumor and the incidence of conductive deafness of the left ear, but there was no significant association in the right ear. There is a significant correlation between NPC primary tumors and left ear hearing thresholds at frequencies of 500 Hz and 4000 Hz, but there is no significant association between the degree of NPC primary tumor and right ear hearing loss.
Conclusions. There is significant correlation between the primary tumor of NPC and the hearing threshold of the left ear but there was no significant association in the right ear
Knowledge and Attitude of Pregnant Women Related to Hyperemesis Gravidarum at Jagong Health Center, Jagong Jeget District, Central Aceh Regency in 2021
Background. Continuous hyperemesis gravidarum can cause a lack of food and fluids that can affect fetal development. Lack of food and fluids or dehydration has a bad effect on the unborn child as well as on the mother herself. This study aims to determine the extent of knowledge and attitudes of pregnant women to the incidence of hyperemesis gravidarum in the working area of the Jagong Health Center, Jagong Jeget District, Central Aceh Regency in 2021.
Methods. This type of research is descriptive, the sample of this research is pregnant women in the working area of the Jagong Health Center totaling 30 people. Data were collected and measured using a questionnaire.
Results. Mother's knowledge about hyperemesis gravidarum has a good level of knowledge as many as 15 respondents (50%), attitudes of pregnant women with hyperemesis gravidarum have a level of a positive attitude as much as 28 (80%), while knowledge of pregnant women and attitudes of pregnant women with hyperemesis gravidarum as much as 15 ( 50%).
Conclusion. It is expected that pregnant women should be able to increase their knowledge and seek more information from health workers and media mass to increase the knowledge of pregnant women about health, especially regarding the incidence of hyperemesis gravidarum
Correlation of Voice Handicap Index 10 with Reflux Symptom Index and Reflux Finding Score in Patients with Dysphonia
Background. Voice disturbance occurs when the quality, pitch, loudness, or flexibility of other people's voices differ according to age, gender, and similar cultural groups. The Voice Handicap Index (VHI) is one of the most widely used tools worldwide to measure the physical, functional, and emotional aspects of voice disorders. This study aims to determine the correlation of voice handicap index-10 with reflux symptom index and reflux finding scores in patients with dysphonia complaints at Dr. Mohammad Hoesin General Hospital Palembang.
Methods. This study is a cross-sectional study using a correlation test design that aims to determine the correlation between VHI-10 and RSI and RFS in patients with dysphonia complaints.
Results. The sample of this study includes the demographic factors of the research subjects, namely age, gender, and occupation. This study shows that patients with RFS ≥ 7 affect the VHI-10 score ≥ 11 with a proportion of 72%.
Conclusion. There is a moderate positive correlation between the RFS score and the VHI-10 score, where the higher the RFS value, the higher the VHI-10 score in patients with dysphonic complaints
Clinical Profile of Proptosis in Dr. M. Djamil General Hospital Padang
Background. Proptosis is a condition that describes a protrusion or forward shift of the eyeball and is commonly used to describe the condition of protrusion of the eyeball. Proptosis generally occurs due to a mismatch between the orbital capacity and the volume of the tissue structure in the orbital cavity. It is a common manifestation of a wide variety of diseases of the structures in and around the orbit and in some systemic diseases. This study aimed to describe the clinical profile of proptosis patients in Dr. M. Djamil General Hospital Padang.
Methods. We reviewed the medical record of the oncology subdivision of ophthalmology patients associated with proptosis retrospectively, such as inflammatory, infectious, tumors, and vascular, admitted to the M.Djamil General Hospital in Padang from January 2019 to August 2020. The ocular and systemic history and examination details were recorded.
Results. We found 48 proptosis patients from January 2019 to August 2020, consisting of 30(62.5%) males and 18(37,5%) females. The majority of subjects were in the Adult age group (79,17%). Unilateral and bilateral proptosis were found in 37(77%) and 11(23%), with mostly axial proptosis (83,33%). Inflammatory tumors were found in 17(35,41) and 29(60%) patients.
Conclusion. Proptosis is a common clinical feature of various local and systemic diseases. Even though proptosis have diverse etiology and pathogenesis, they usually present with similar clinical manifestation. Proptosis may be one of the most important keys to disease guidance from a close structure that requires a multidisciplinary approach
Profile of p53 Protein Expression in Basal Cell Carcinoma Low-Risk and High-Risk Subtype
Background: Basal cell carcinoma is one of the most common cancers in the world, and its incidence is increasing over time. It is estimated that it accounts for three-quarters of all non-melanoma skin cancers, and the incidence increases exponentially in predominantly white areas. The p53 gene is the gene that most undergoes mutations due to sun exposure and is the cause of skin cancer, especially basal cell carcinoma. This finding explains that p53 expression can be used as marker analysis in cases of basal cell carcinoma.
Methods: Observational analytic design was carried out with a cross-sectional. The population and samples were paraffin blocks which histopathologically diagnosed Basal Cell Carcinoma in the anatomical pathology laboratory, Dr. Kariadi General Hospital Semarang. Samples were processed by the p53 immunohistochemical staining method. The p53 expression was given a quantitative score to be 0 = negative, 1-2 = low, 3-4 = high. The expression of p53 was considered positive when it was colored in the cell nucleus. The correlation between variables was analyzed by Spearman and Mann Whitney correlation test.
Results: There is no correlation between p53 expression and the age of p53 patients with p = 0.390 (p > 0.05). There is no correlation between p53 expression and lesion location with p = 0.817 (p > 0.05) There is no difference significant, between p53 expression and gender with p = 0.576 (p > 0.05).
Conclusion: There is no correlation between high and low-risk Basal Cell Carcinoma subtype associated with age, gender, and lesion location
Lactating Adenoma in Pregnancy: A Case Report
Background: Lactating adenoma is a rare palpable breast lesion that occurs late in pregnancy or the lactation period and is commonly found in young primiparous women in the second or third decade of life. This case report aims to describe lactating adenoma and its management.
Case presentation: A woman, 24 years old, 26 weeks pregnant, came to the Surgical Oncology Polyclinic of Dr. Moewardi Hospital Surakarta with a complaint of a lump in the right breast from the last 4 months. The lump was felt to be getting bigger, painful to the point that sometimes fluid came out of the nipple of her right breast. The patient had a history of previous lumps with a diagnosis of fibroadenoma that had been operated on. On palpation multiple nodules are palpable. Mammography examination showed a mass with partial calcification (BIRADS 3). The patient was diagnosed with a tumur of mammae dextra suspected of malignant cT2N1Mx with G1P0A0 26 weeks gestation. Histopathological examination results showed a lactating adenoma.
Conclusion: There are several recommendations for the management of lactating adenoma during pregnancy. An antepartum biopsy is recommended for women with a breast mass in the first or second trimester, postpartum for masses that appear in the late third trimester, and fine-needle aspiration biopsy as an alternative for masses that appear in the early third trimester. Treatment with bromocriptine, a dopamine agonist, may be given to reduce the size of the lactating adenoma
