Relationship between Febrile Seizures and the Incidence of Microcytic Hypochromic Anemia in Children at Sanjiwani General Hospital, Gianyar, Indonesia
Background: Febrile seizures are the most common seizures in children. As many as 2% to 5% of children aged less than 5 years have had seizures accompanied by fever, and most occurrences are at the age of 17-23 months. Microcytic hypochromic anemia causes hypoxic conditions and neuronal instability due to iron depletion. This study aimed to explore the relationship between febrile seizures and the incidence of microcytic hypochromic anemia in children at Sanjiwani General Hospital, Gianyar, Indonesia.
Methods: This study was an analytic observational study with a cross-sectional approach. A total of 59 subjects participated in this study. Data analysis was carried out with the help of SPSS software in univariate and bivariate to determine the relationship between febrile seizures and microcytic hypochromic anemia.
Results: There is a relationship between the incidence of febrile seizures and the incidence of microcytic hypochromic anemia, with p<0.05. This study also showed that the risk of febrile seizures increased 1.7 times higher in individuals with microcytic hypochromic anemia.
Conclusion: There is a relationship between the incidence of febrile seizures and the incidence of microcytic hypochromic anemia in pediatric patients at Sanjiwani General Hospital, Gianyar, Indonesia
Overview of Risk Factors for Toddlers with Congenital Heart Disease Suffering from Pneumonia at Dr. M. Djamil General Hospital, Padang, Indonesia
Background: Congenital heart disease (CHD) includes structural and non-structural abnormalities that arise from birth. Disorders of the circulatory system can also affect the respiratory system, causing symptoms and disturbances that similar to primary respiratory disease. This study aimed to describe the risk factors for toddlers with congenital heart disease who suffer from pneumonia at Dr. M. Djamil General Hospital, Padang, Indonesia.
Methods: Descriptive observational study using secondary data. A total of 58 research subjects participated in the study. The research subjects met the inclusion criteria in the form of patients who had been diagnosed with congenital heart disease, aged 29 days – 60 months, and were treated at Dr. M. Djamil General Hospital, Padang, Indonesia. Data analysis was performed using SPSS software univariately.
Results: The majority of patients with various age levels have a cured outcome. The majority of subjects with medium and high parental education had a greater recovery percentage than subjects with low parental education. Subjects with medium and high socioeconomic status had a greater percentage of recovery outcomes than subjects with low socioeconomic status. The majority of subjects with normal nutritional status and normal birth weight had a higher percentage of recovered outcomes than those who died. The history of DPT immunization and the type of congenital heart disease did not show a large percentage difference between patients who recovered and died. History of measles immunization and leukocytosis showed a greater percentage of patients with cured outcomes than death outcomes.
Conclusion: CHD patients with pneumonia are under 1 year old, male, come from families with low-medium socioeconomic status, low-medium educational level, and malnutrition status have worse patient outcomes
Differences in Clinical Characteristics of Patients with Valvular and Non-Valvular Atrial Fibrillation
Background: There is an increasing global prevalence of atrial fibrillation (AF), including in Indonesia. Based on the presence of mitral stenosis and/or prosthetic valve, AF is divided into two groups, namely valvular AF and non-valvular AF. The differences in clinical characteristics between valvular and non-valvular AF in Indonesia, especially in Semarang, have not been described. This study aimed to determine the differences in clinical characteristics between valvular and non-valvular AF in Semarang.
Methods: This study was an observational analytic study with a cross-sectional design and was performed in July-August 2020. The subjects were 54 AF patients, which were divided into two categories, namely valvular (n=28) and non-valvular (n=26) AF. The data were collected from medical records. The differences between the two groups were analyzed with bivariate analysis.
Results: The patients with valvular AF were predominantly female (82.1% vs 69.2%; p <0.001), of a younger age (46.54 ± 12.20 vs 61.04 ± 8.68; p <0.001), more likely to have rheumatic heart disease (46.4% vs 3.8%; p <0.001), and have had heart surgery (57.1% vs 0%; p <0.001). The patients with non-valvular AF were more likely to have hypertension (17.9% vs 80.8%; p <0.001), myocardial infarction (0% vs 19.2%; p = 0.021), dyslipidemia (7.1% vs. 42.3%; p = 0.003), and higher BMI (21.03 ± 4.16 vs 25.48 ± 4.20; p <0.001). The INR values in most of the AF patients have not reached the target. The valvular AF patients were more likely to be taking warfarin (100% vs 80,8%, p=0,021) and diuretic therapy (96,4% vs 57,7%, p=0,001).
Conclusion: There are significant demographic and clinical characteristics differences between valvular and non-valvular AF
The Difference of Lens Thickness in Acute and Chronic Primary Angle Closure Glaucoma
Background: Lens thickness is one of the factors that influence the mechanism of acute primary angle closure glaucoma (PACG). The thicker lens usually results in a shallower anterior chamber angle (ACA), and the gradual and progressive increase in lens thickness with age results in exaggerated shallowing of the ACA. Because lens thickness has an important role in acute PACG mechanism, prophylactic treatment like lens extraction might prevent PACG in early stages. This study aimed to describe the difference in lens thickness in acute and chronic PACG.
Methods: The research was a cross-sectional study with samples conducted in Dr. M. Djamil General Hospital from September-December 2019. The number of eyes that fulfilled the inclusion criteria were 50 eyes from 50 patients consisting of 25 eyes with acute PACG and 25 eyes with chronic PACG. Subjects were examined with ocular biometry using A-scan biometry to evaluate lens thickness. Data were processed and analyzed using a computer program with a chi-square test.
Results: Mean of lens thickness in acute PACG is 5.02 ± 0.45, while in chronic PACG is 4.86 ± 0.40. The difference in lens thickness in acute and chronic PACG were statistically significant (p = 0,037).
Conclusion: Lens thickness is thicker in acute PACG than in chronic PACG
The Prevalence of Bimaxillary Protrusion in Ethnic Malay, Medan, Indonesia
Background: Malocclusion is a deviation from the normal position of the teeth when the jaw is closed. The bimaxillary protrusion is the most prominent or most common of Angle's class I malocclusions. This study aimed to determine and present the prevalence of bimaxillary protrusion disorder in ethnic Malay adolescents in Medan, Indonesia.
Methods: Descriptive observational study. A total of 35 research subjects were included in this study. An exploration of bimaxillary protrusion disorders was also carried out, and an exploration of demographic data was also carried out. Univariate analysis was carried out to determine the frequency distribution of each test variable with the help of SPSS version 25 software.
Results: The majority of study subjects had bimaxillary protrusion angle class I malocclusion, namely 22 people (62.9%), while only 13 respondents (37.1%) had no bimaxillary protrusion malocclusion. The results of this study indicate that the prevalence of bimaxillary protrusion abnormalities in the Malay ethnicity is quite high, namely 62.9%.
Conclusion: The prevalence of bimaxillary protrusion disorders in ethnic Malay is 62.9%
Management of Resistance to Targeted Therapy in EGFR Mutational Lung Adenocarcinoma: A Case Report
Advances in diagnosis and treatment have enabled a targeted therapy approach to inhibit tumor progression and provide a favorable prognosis. One of the targeted therapies in non-small cell lung cancer (NSCLC) is tyrosine kinase inhibitor (TKI) in mutations epidermal growth factor receptor (EGFR). This study aimed to present a case with the management of resistance to targeted therapy in EGFR-mutated lung adenocarcinoma. Data shows median progression-free survival patients on TKI therapy at 10 to 14 months. Progressive caused by resistance to TKI and assessed using response evaluation criteria in solid tumors (RECIST) 1.1. and confirmed by repeat biopsy. Reported a male patient aged 66 years with adenocarcinoma of the right lung EGFR deletion exon 19 mutation and received afatinib therapy for 16 months. Patients experience oligo progressive in the 13th month of therapy and systemic progression 16th month
Comparison of the Effectiveness between Povidone-Iodine and Bleomycin as Pleurodesis Agents in Patients with Malignant Pleural Effusion at Dr. Hasan Sadikin General Hospital, Bandung, Indonesia
Background: Malignant pleural effusion (MPE) is a clinical problem in which fluid accumulates in the pleural cavity. This condition is generally caused by malignancy. Pleurodesis is a medical procedure in the form of fusion of the visceral and parietal pleura in order to prevent the recurrence of pleural effusion. Pleurodesis has the risk of causing chronic inflammation, which will trigger excessive attachment between the visceral and parietal pleura. Of course, an intervention is needed to reduce the risk of inflammation, one of which is by using a pleurodesis agent, which has an anti-inflammatory effect. This study aimed to compare the efficacy of povidone-iodine compared to bleomycin as a pleurodesis agent in malignant pleural effusion patients at Dr. Hasan Sadikin General Hospital, Bandung, Indonesia.
Methods: Prospective cohort study, where as many as 46 research subjects participated in this study and were grouped into groups that received povidone-iodine and bleomycin. The success of pleurodesis agents was assessed by the production of WSD. Data analysis was carried out using SPSS univariate and bivariate.
Results: The results of the study showed that there was no statistical difference between the 10% povidone-iodine agent and the bleomycin agent, where p>0.05. However, in the 10% povidone-iodine group, the success rate was 100%, while in the bleomycin group, the success rate was 82.6%.
Conclusion: There was no significant difference in the efficacy of povidone-iodine as a pleurodesis agent compared to bleomycin in patients with malignant pleural effusion at Dr. Hasan Sadikin General Hospital, Bandung, Indonesia
The Effect of Birth Ball Exercise on the Progress of Labor During the First Active Phase at PONED (Basic Emergence Neonatal Obstetric Services) UPTD DTP Pedes Health Center, Karawang Regency, Indonesia
Background.Prolonged parturition or labor that lasts longer than usual can increase the risk of complications and the potential for maternal death. The birth ball exercise, also known as the birth ball exercise or the pregnancy ball exercise, can help harness the influence of gravity to induce labor progress. This study aimed to assess the effect of birth ball exercises on the progress of labor during the first active phase at the PONED UPTD DTP Pedes Health Center, Karawang Regency, Indonesia. Methods.This study is an experimental study, where as many as 30 research subjects participated in this study. Analysis of the progress of labor was carried out using SPSS software in a univariate and bivariate manner. Results. The results of the study showed that the birth ball exercise had an effect on the progress of labor with a p-value = 0.002 (p <0.05). Conclusion, there is an influence of birth ball exercises on the progress of labor at PONED UPTD DTP Pedes Health Center, Karawang Regency, Indonesia
Factors Affecting the Outcome of Breast Reconstruction Surgery in Modified Radical Mastectomy Procedure
Background: Breast cancer is the leading cause of death in women caused by cancer. The primary treatment for breast cancer is surgery, where several recommended surgical methods are breast-conserving surgery or mastectomy. Postoperative mastectomy reconstruction has provided an opportunity to overcome the emotional and aesthetic burden of malignancy.
Methods: This research is a descriptive observational study with a retrospective approach. The sample was 45 breast cancer patients who had undergone a modified radical mastectomy operation at Dr. Kariadi General Hospital for January 2019 - April 2023. The data were processed using SPSS for Windows software, and a descriptive analysis was carried out.
Results: Most of the patients who underwent post-MRM breast reconstruction were in the age group >40 years (71.1%), had stage 2 mammary carcinoma (44.4%) and stage 3 (40%), featured invasive ductal carcinoma (84 .4%), breast on the right side (64.4%). Some suffer from hypertension (13.3%) or diabetes mellitus (22.2%). The most used type of reconstruction was STSG (82.2%). A small proportion of patients experienced dehiscence (17.8%). In the Karnofsky score assessment, the highest score was 70 (64.4%), followed by 60 (17.8%), 80 (11.1%), 50 (4.4%), and 40 (2.2%).
Conclusion: The patients undergoing MRM are dominated by the age group > 40 years, not suffering from hypertension or diabetes mellitus, stage 2, histopathology of invasive ductal carcinoma, right-sided breast. STSG dominates the choice of reconstruction with postoperative complications in the form of dehiscence in a small proportion of patients. The Karnofsky score is dominated by a score of 70
Analysis of Factors Associated with Adherence to Taking Medication in Pediatric Patients with Pulmonary Tuberculosis in Mesuji Regency, Lampung, Indonesia
Background: One disease that is still a health problem in the world, including Indonesia, is tuberculosis (TB) which is caused by germs Mycobacterium tuberculosis. The success of TB treatment requires patient compliance in taking antituberculosis drugs (OAT), family support such as supervision, and motivating TB sufferers are factors that play a role in patient compliance. This study aims to determine the factors associated with medication adherence in pediatric patients with pulmonary tuberculosis in Mesuji Regency, Lampung, Indonesia.
Methods: This study was an observational study with a cross-sectional approach, where as many as 52 respondents were included in this study. Data collection through interviews using a structured questionnaire. The subject of the study consisted of tuberculosis patients who were reported to have attended treatment for at least two months in Mesuji Regency. Data analysis was carried out using SPSS using univariate and bivariate methods.
Results: The results showed that 76.9% of respondents adhered to the treatment of pulmonary tuberculosis. Patient medication adherence to tuberculosis was influenced by the presence of drug side effects (p-value = 0.007) and the supervisor's support for taking medication (p-value = 0.021).
Conclusion: Compliance with taking medication in tuberculosis patients is influenced by the presence of medication supervisors and the uncomfortable side effects of antituberculosis drugs
