Comparison of Antibacterial Effectiveness of Arabica Coffee Bean Extract (Coffea arabica L.) Roasted and Non-Roasted Against Bacteria Enterococcus faecalis
Background: Dental caries that are not handled properly cause cavities and open the dental pulp so that it becomes a pathway for microorganisms to enter the root canals. The most common microorganisms found in root canal infections are Enterococcus faecalis. Arabica coffee beans have bioactive compounds such as alkaloids, saponins, and tannins, which inhibit the growth of bacteria. This study aimed to compare the antibacterial effectiveness of roasted and non-roasted Arabica coffee bean extracts against bacteria Enterococcus faecalis.
Methods: This study was an in vitro experimental study. This research uses culture Enterococcus faecalis, which were divided into 10 groups (2 control groups, 4 treatment groups of roasted coffee beans, and 4 groups of non-roasted coffee beans). Inhibition zone diameter analysis was carried out with the help of SPSS using univariate and bivariate.
Results: Non-roasted Arabica coffee bean extract showed better effectiveness in inhibiting bacterial growth Enterococcus faecalis, along with increasing the concentration of the extract.
Conclusion: Non-roasted Arabica coffee bean extract is more effective as an antibacterial against the bacteria Enterococcus faecalis compared to roasted Arabica coffee bean extract
Overview of Knowledge, Attitudes, and Behaviors Related to Halitosis in Online Motorcycle Taxi Drivers in Sei Putih Tengah Sub District, Petisah District, Medan, Indonesia
Background: Halitosis is a sign of dental and oral health problems. Halitosis is characterized by the presence of an unpleasant odor in the oral cavity. Halitosis is caused by many things, one of which is a result of impaired saliva production, which is caused by smoking. Online motorcycle taxi drivers are in a community that is very close to smoking. This study aimed to provide an overview of aspects of knowledge, attitudes, and behavior related to halitosis in online motorcycle taxi drivers in Sei Putih Tengah Sub District, Petisah District, Medan, Indonesia.
Methods: Descriptive observational study. A total of 36 research subjects, namely online motorcycle taxi drivers, participated in this study. Observation of knowledge, attitudes, and behavior refers to a questionnaire developed by the halitosis clinic at the University of Basel, Switzerland. Univariate analysis was carried out with the help of SPSS software.
Results: The majority of research subjects are independently aware of bad breath and experience similar complaints <1 year. The majority of research subjects felt bad breath after waking up in the morning. The majority of research subjects also stated that dentists were the choice for consultation regarding bad breath disorders. The majority of research subjects know the origin of bad breath, the source of bad breath, and know that smoking is the cause of bad breath. Most research subjects did not feel ashamed of other people even though they felt they had bad breath.
Conclusion: Knowledge and behavior related to halitosis are good, although attitudes related to halitosis are not good among online motorcycle taxi drivers in Sei Putih Tengah Sub District, Petisah District, Medan, Indonesia
Mixed Type Basal Cell Carcinoma Treated with Excision and Defect Closure Using Advancement Flap: A Case Report
Background: Basal cell carcinoma (BCC) is a localized malignant tumor in the basal layer of the epidermis thought to be the result of prolonged sun exposure and associated with gene mutations in most cases. Although rarely metastazises, BCC can cause high levels of morbidity due to its locally destructive nature. There are several modalities for managing BCC, and the defect caused by the lesion can reconstructed by local flap.
Case presentation: We present a 55-year-old woman with a chief complaint of a blackish lump on the left maxillary region, growing bigger, and frequent episodes of bleeding. The lesion was excised and followed by histopathological examination, which revealed mixed subtype BCC. Closure of the defect with an advancement flap was performed with favorable results.
Conclusion: Surgical excision is the best option for managing BCC, while a skin flap is preferred to close defects for lesions on the cheek.
 
Diagnosis and Treatment of Typical Cold-Induced Urticaria: A Case Reports
Background: Urticaria is a common disease in society. 15-25% of individuals must have experienced urticaria at least once in their life. Cold urticaria is included in the type of chronic inducible urticaria and is a type of urticaria that accounts for 3% of cases of all types of chronic urticaria that occur. The age group that most suffers from urticaria is the age 12-25 years. This study was aimed at presenting the diagnosis and treatment of cold urticaria.
Case presentation: A 29-year-old woman came with complaints of itching and redness on the hands, feet, and neck, starting with a small round reddish rash that widened and the center was paler than the edges, then disappeared within 1-2 hours, there is no a history of infectious or autoimmune diseases but has a family history of the same complaint. Examination of the ice cube test obtained positive results after 7 minutes of contact with ice cubes. Patients were given therapy in the form of cetirizine 10 mg per day for 2 weeks, which was then increased by a dose of 20 mg with a combination of cetirizine 10 mg in the morning and loratadine 10 mg at night.
Conclusion: Exploration of precipitating factors through detailed anamnesis and supporting examinations with a cold stimulation test can establish a diagnosis of typical cold-induced urticaria
Update Management of Chemotherapy-Induced Neutropenia: A Narrative Literature Review
One of the most frequent and significant complicated side effects of myelosuppressive drugs is neutropenia. Neutropenia is not only associated with high morbidity but also mortality. Generally, the onset of neutropenia occurs within 3-7 days after the administration of anticancer agents. Approximately 5-30% of patients will develop febrile neutropenia. The highest incidence occurs during the first cycle of chemotherapy. This literature review aimed to describe chemotherapy-induced neutropenia and its prevention and management. Administration of granulocyte colony-stimulating factor as prophylaxis can prevent neutropenia-induced anticancer drugs. The use of granulocyte colony-stimulating factor (G-CSF) as therapeutic is contradictive, and some guidelines recommend not to use the G-CSF for therapeutic purposes. Up to 70% of mortality among these populations was associated with gram-negative sepsis, therefor empirical antibiotics, and antifungals are the keys to the management of neutropenia. In conclusion, prevention and appropriate management of neutropenia are extremely important in the treatment of patients with cancer
Overview of Interleukin-6 Levels in COVID-19 (Coronavirus Disease 2019) Patients at Dr. M. Djamil General Hospital, Padang, Indonesia
Background: COVID-19 infection can cause an exaggerated immune response. This immune response is associated with an increase in proinflammatory cytokines, especially interleukin-6 (IL-6). High IL-6 levels are found in the acute stage of COVID-19, and IL-6 can induce an excessive humoral inflammatory response. This study aimed to provide an overview of IL-6 levels in coronavirus disease 2019 (COVID-19) patients at Dr. M. Djamil General Hospital, Padang, Indonesia.
Methods: Descriptive observational study of 102 research subjects. Observations on sociodemographic, clinical, and laboratory data were carried out in this study. Univariate analysis was carried out using SPSS version 25.
Results: Patients with symptom onset <7 days had higher IL-6 levels than those with an onset of more than 7 days. Patients with critical degrees have the highest IL-6 levels compared to moderate and severe degrees. Patients with more than 1 comorbid had higher IL-6 levels than patients who had no comorbid or only had 1 comorbid. Patients with <21 days of treatment had higher IL-6 levels than patients with more than 21 days of treatment.
Conclusion: COVID-19 patients at Dr. M. Djamil General Hospital, Padang, Indonesia, with an onset of less than 7 days, a critical degree, and more than 1 comorbidity have higher IL-6 levels
Graft Outcome of Corneal Patch for Perforated Corneal Ulcer
Background: Corneal patch graft (CPG) is a part of the tectonic penetrating keratoplasty procedure performed on the perforated corneal ulcer. This procedure involves filling affected areas with full or partial-thickness cornea donors. This study aimed to determine the clinical results of corneal patch grafts in perforated corneal ulcers.
Methods: A retrospective study assessing clinical results of CPG on the perforated corneal ulcer with iris prolapse. Clinical assessments include graft condition, anterior chamber depth (ACD), and visual acuity after surgery, with a follow-up duration of up to 8 weeks.
Results: Twenty-one cases of perforated corneal ulcer with iris prolapse in 15 males (71.4%) and 6 females (28.6%). Locations of ulcer are peripheral and paracentral corneal. Visual acuity showed around light perception (LP) to 0.4. The most common etiology for corneal ulcers was infection (88.9%), followed by non-infection (Mooren’s ulcer) (11.1%). The graft condition after surgery showed promising results in 18 cases (85.7%), infection in 1 case (4.8%), and melting in 2 cases (9.5%). Adequate ACD was obtained in 77.8% of cases. An increase in visual acuity was shown in 44.4% of cases, depending on the location and severity of the ulcer.
Conclusion: A corneal patch graft is an appropriate choice in treating perforated corneal ulcers to maintain ocular integrity (tectonic keratoplasty). However, in non-infectious ulcers, this procedure has not shown a good result, which may be related to disease progressivity
Atlas of Human Oocyte: A Review in the Daily In Vitro Fertilization (IVF) Practices
Background: Oocytes are gamete cells that play an important role in the daily practice of in vitro fertilization (IVF) laboratories. Oocyte morphology is important to study and assess in order to increase the success of IVF pregnancy.
Methods: A computerized database search process in Pub med was performed to obtain data. Then, the most updated and relevant articles were selected.
Results: Normal oocyte morphology should have normal-appearing cytoplasm, single polar body, appropriate zona pellucida (ZP) thickness, and appropriate perivitelline space (PVS). Abnormal oocyte morphology can be in the form of immature oocytes, large in size, ovoid or amorphous in shape, having fragment/double/multiple polar body I (PBI) and granular cytoplasm or smooth endoplasmic reticulum (SER).
Conclusion: Abnormal oocyte morphology in the IVF laboratory can be seen by its maturity level, size, shape, condition of PBI, and cytoplasmic granularity. A better selection of oocytes can increase the performance of the IVF laboratory
Analysis of Patient Safety Culture at Semen Padang Hospital Indonesia
Background: Hospitals have a role and function to provide integrated health services for patients as consumers of health services. When providing services to patients, hospitals must pay attention to quality and safety. This study aimed to find out the patient safety culture that has been built at Semen Padang Hospital in 2022.
Methods: This study uses an approach mixed methods research with a design sequential explanatory, namely research that uses a combination of quantitative and qualitative methods simultaneously (quantitative-qualitative). The subjects of this study were employees of Semen Padang Hospital, with as many as 81 respondents for quantitative data and 11 respondents for qualitative data. Measurement of patient safety culture using the MaPSaF instrument (Manchester patient safety framework) in the form of filling out the MaPSCAT questionnaire (Manchester patient safety culture assessment tool). This questionnaire consists of 10 dimensions with a total of 24 questions and is equipped with in-depth interview data.
Results: Data collection was carried out using the MaPSCAT instrument on 10 dimensions of patient safety culture, 6 dominant dimensions were at the proactive level, namely the dimension of overall commitment to continuous improvement (1st dimension), the dimension of system error and individual responsibility (3rd dimension), the dimension of incident evaluation and best practices (5th dimension), effective learning and change (6th dimension), communication about patient safety issues (7th dimension) and staff education and training dimension (9th dimension).
Conclusion: The safety culture at Semen Padang Hospital is generally at a proactive level. This indicates that the hospital places priority on improving patient safety and is carried out on an ongoing basis. A system that is integrated and comprehensive has a wide scale, involved stakeholders, and an evidence-based approach
Risk Factors for Uterine Atony in Postpartum Hemorrhage Patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia
Background: Uterine atony can be life-threatening for a pregnant woman in labor and bleeding after delivery. It is important to explore the risk factors that can cause uterine atony in order to detect it early and take preventive measures. This study aimed to explore the risk factors for causing uterine atony in postpartum hemorrhage patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia.
Methods: This study was an analytic observational study with a case-control approach. A total of 52 subjects (13 case group subjects and 39 control group subjects) participated in this study. Risk factor analysis was carried out with the help of SPSS version 25 in univariate and bivariate.
Results: Maternal age at delivery is associated with the risk of uterine atony in postpartum hemorrhage patients. Mothers aged less than 20 years or more than 35 years are at risk of experiencing uterine atony by 5.8 times more at risk than mothers aged 20-35 years. The risk factors for parity, prolonged labor, macrosomia, gemelli, hydramnios, induction of labor, history of postpartum hemorrhage, and type of delivery were not associated with uterine atony in postpartum hemorrhage patients, p>0.05.
Conclusion: The age of delivery of mothers who are less than 20 years or more than 35 years is a risk factor for uterine atony in postpartum hemorrhage patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia
