    Increasing Public Knowledge About Sedentary Activities Through Counseling at SMPIT Zain Al-Muttaqin, Palembang, Indonesia

    Background: Sedentary activities are activities that require very low energy expenditure, such as sitting or lying down while watching television, playing electronic games, and reading which can have negative health effects. This study aimed to describe the comparison of participants' knowledge before and after counseling about sedentary activities. Methods: An experimental study with one group pre-test and post-test design was conducted on 22 students of SMPIT Zain Al-Muttaqin Palembang. Data were analyzed using the Wilcoxon test. Results: There were 63.6% of participants' knowledge was in the poor category before counseling, and after counseling, the participant's knowledge increased by 59.1% in the good category. From the Wilcoxon test analysis, the result is the value of Sig. (2-tailed) of 0.000 < 0.05. Conclusion: Student knowledge is increased about activities sedentary after counseling, and there is a significant difference in the results of sedentary lifestyle counseling based on data pre-test and post-test (P<0,05)

    The Effect of Unripe Papaya Consumption on the Smooth Production of Breast Milk in Ciandam Village, Mande District, Cianjur, Indonesia

    Background. Consumption of unripe papaya is one of the local wisdom which is believed to be useful in increasing the breast milk production of breastfeeding mothers. Hereditary unripe papaya has been consumed in public areas to increase milk production. This study is one of the first studies to provide scientific evidence of the effects of the unripe papaya on breast milk production in Ciandam Village, Mande District, Cianjur, Indonesia. Methods. This study was an experimental study, where as many as 30 subjects participated in this study. Data analysis was carried out using SPSS 25 in a bivariate manner to determine the effect of unripe papaya consumption on breast milk production. Results. After consumption of fruit papaya on the first day, there was increased production of Breast milk in the intervention group compared to before consumption of unripe papaya p=0.02 (p<0.05). Likewise, after the consumption of unripe papaya on the second to the seventh day, an increase in milk production was found in the intervention group compared to before the consumption of unripe papaya, p < 0.05. Conclusion. There is an influence of unripe papaya consumption on the smooth breast milk of breastfeeding mothers in Ciandam village, Mande District, Cianjur, Indonesia

    Effect of Extraction Method and Solvent Type on Total Phenolics Content, Total Flavonoid and Antioxidant Activity of Pegagan Extract (Centella asiatica (Linn.) Urban)

    Background: Centella asiatica (CA) or pegagan or horse foot leaves is one of the traditional plants widely used to treat various diseases. This study aims to evaluated the content of total phenolics and total flavonoids, as well as the antioxidant activity of Centella asiatica extracts from different methods and solvents. Methods: The sample was divided into three groups, (70% ethanol maceration, methanol maceration and water infundation). Total phenolic compounds in the extract were determined by the Follin-Ciocalteu method, total flavonoid content was determined by the aluminum chloride colorimetric assay method, and antioxidant activity was assessed by the immersion DPPH free radicals method. Determination data of IC50 was assessed by regression test and differences between the three extract groups were tested by ANOVA or Kruskal Wallis test. Results: The highest total phenolics and total flavonoid were obtained in Centella asiatica extract with water solvent, 251.88 ± 0.96 mg/g GAE and 7.26 ± 0.03 mg/g QE, followed by 70% ethanol and methanol. In addition, the best antioxidant activity was also shown in the extract with water, namely 24.09 ± 0.01. The difference test results between all groups showed significant results (p<0.05) in all parameters. Conclusion: The best content of total phenolic, total flavonoid, and antioxidant activity was obtained by infundation with water solvent. In the maceration method, the best content of the three parameters measured was obtained in the group with ethanol solvent

    Cochlear Implantation with Fontan Circulation: A Rare Case of Non-Cardiac Pediatric Surgery

    Background: Fontan circulation is a cardiac circulation following the Fontan procedure in patients with a single ventricle congenital heart defect. This circulation depends on the difference in pressure between pulmonary capillaries and central venous pressure to achieve optimal cardiac output. This case report aims to discuss intraoperative anesthesia management in a patient with Fontan circulation undergoing a non-cardiac surgical operation. Case presentation: The patient is a 7-year-old male with a history of congenital heart disease who has undergone the Fontan circulation procedure and is now undergoing Cochlear implantation surgery. The patient was managed with general anesthesia - endotracheal intubation and controlled ventilation during the operation, following the principle of minimizing intrathoracic positive pressure, low PEEP, and low tidal volume with a target peripheral saturation > 95%. Cardiac output monitoring during the intraoperative period is done using intra-arterial blood pressure. After 5 hours of surgery, the patient was extubated with stable hemodynamics.   Conclusion: The primary goal of intraoperative anesthesia management in patients with Fontan circulation is to maintain an optimal transpulmonary gradient by ensuring optimal transpulmonary blood flow, thus preserving cardiac output

    Tuberculosis with Drug-Induced Hepatitis: A Narrative Literature Review

    Tuberculosis (TB) is an infectious disease that is a global health problem. The problem is that 5-28% of patients receiving tuberculosis treatment suffer from drug-induced hepatitis on antituberculosis drugs (OAT). The clinical picture should be differentiated from other liver diseases. The level of symptoms varies from asymptomatic to symptomatic, such as nausea, vomiting, abdominal pain, jaundice, hepatomegaly, and increased liver function. OAT use is stopped if clinical symptoms are found and ALT/AST increases ≥3 times, or if there are no symptoms but there is an increase in bilirubin ≥2 mg/dl or ALT/AST values ≥5 times without clinical symptoms. The use of OAT can be continued, but with supervision, if there are no clinical symptoms and the increase in ALT/AST is <2 times and the bilirubin value is <2 mg/dl. Treatment can be carried out again by reintroducing OATs one by one according to ATS recommendations

    Antibiotic Management in Bacterial Pneumonia: A Narrative Literature Review

    Pneumonia is an acute inflammation of the lung parenchyma caused by various types of microorganisms, such as bacteria, viruses, fungi, and parasites. The use of antibiotics is often faced with many types and inappropriate doses, and using antibiotics for too long can increase the risk of bacteria multidrug resistance (MDR). Therefore, the choice and dosage of antibiotics must be appropriate to reduce the rate of bacterial resistance. The increasing incidence of bacterial resistance is an indicator of failure in treating pneumonia because, apart from clinical improvement, optimal eradication of bacteria should also be the goal in every antibiotic administration. The aim of this literature review is to explain the basis for administering and selecting empiric antibiotics for pneumonia infections as well as appropriate antibiotic management and adequate especially in bacterial pneumonia, to provide a better prognosis

    Comparison of the Effectiveness of Lyophilized Amniotic Membranes in the Partial Thickness Wound Healing Process in Mus musculus

    Background: Currently, tulle or gentamicin ointment is generally used to prevent wound infections in wound care. Human amniotic membrane has been used in medical fields such as wound care. So far, there is no comparative evidence of the effectiveness of each ingredient. This study aimed to compare the effectiveness of lyophilized amniotic membrane, tulle, and 0.1% gentamicin ointment in the wound healing process. partial thickness on Mus musculus clinically and histologically. Methods: This type of research is experimental with a post-test-only control group design. There were 24 rats divided into each group. This research was conducted at the anatomical pathology laboratory, Faculty of Medicine, Universitas Andalas, and INA lab in July - September 2023 using rats (Mus musculus), which were divided into 3 treatment groups and one control group. Results: Clinically, lyophilized amniotic membrane, 0.1% gentamicin ointment, and tulle provided better results than controls regarding erythema variables. Histologically, the results showed that the lyophilized amniotic membrane group had the thinnest epidermis and dermis, as well as better histological scores for granulation, fibroblasts, and collagen than the 0.1% gentamicin ointment, tulle, and control groups. In this study, significant differences in effectiveness were found in terms of erythema, skin thickness, granulation, fibroblast, and collagen histological scores, with better results in the lyophilized amniotic membrane group. Conclusion: Clinically, lyophilized amniotic membrane, tulle, and 0.1% gentamicin ointment were more effective than controls for wound treatment due to faster reduction in erythema. However, histologically, the lyophilized amniotic membrane was proven to be more effective than tulle, 0.1% gentamicin ointment, and control in terms of epidermis and dermis thickness, granulation level, fibroblasts, and collagen

    Oral Vitamin D Supplementation as Add-On Therapy in Adult Patients with Atopic Dermatitis

    Background: Recent studies have highlighted the possible role of vitamin D in atopic dermatitis (AD) so that it can be used as therapeutical of AD. The aim of study to evaluate the effect of vitamin D supplementation as add-on therapy in adult patients with AD. Methods: Twenty-four adult patients with AD were included in a randomized, double-blind, placebo-controlled trial study. This study was conducted in Dr. Moewardi General Hospital Surakarta, Indonesia, from February to March 2023. Subjects were randomly assigned to oral cholecalciferol 5,000 IU/day versus placebo for 4 weeks and all subjects were given emollient. The severity of AD was evaluated by using scoring of atopic dermatitis (SCORAD) before and after the trial. Results: Compared to placebo, vitamin D supplementation for 1 month obtained clinically and statistically improvement in SCORAD score compare to control (-4.508 : 4.500, p= 0.000). Moreover, vitamin D supplementation had strong negative correlation to SCORAD score after the trial (r= -0.780, p= 0,000). There were no adverse effects in either group. and Conclusion: Clinical improvement was achieved after vitamin D supplementation as add-on therapy in adult patients with AD

    Supraclavicular Brachial Plexus Block Guided by Ultrasonography in Pediatric Patient: A Case Report

    Background: Supraclavicular brachial plexus block has proven to be very useful in upper limb surgeries in pediatric patients. Supraclavicular brachial plexus block is preferred over general anesthesia for pediatric patients undergoing upper limb surgeries. This study aimed to report a procedure of supraclavicular brachial plexus block guided by ultrasonography in a pediatric patient undergoing orthopedic surgery on the upper extremity, which yielded good results, a high success rate, reduced opioid usage, and aided in faster recovery while minimizing complications. Case presentation: The patient was a 14-year-old child with a diagnosis of malunion in the left distal radius after open reduction internal fixation (ORIF) planned for osteoclasis surgery and ORIF with regional anesthesia through supraclavicular brachial plexus block. The patient was monitored during the surgery, and after the surgery, the patient returned to the ward; the patient was given oral analgesics without opioids for pain management. Conclusion: The efficient regional anesthesia procedure option for upper limb surgeries in pediatric patients is the supraclavicular brachial plexus block. Due to lower risks and fewer side effects, the supraclavicular brachial plexus block procedure is preferred over general anesthesia in pediatric patients

    The Relationship between Menstrual Patterns and Pain Scale in Endometriosis Patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia

    Background: Endometriosis is a disease characterized by the presence of tissue resembling the endometrium (uterine lining) outside the uterus. This causes a chronic inflammatory reaction that can result in the formation of scar tissue in the pelvis and other parts of the body. Pain is the most common clinical complaint in cases of endometriosis. Pain during menstruation is often confused with pain due to endometriosis, so treatment delays often occur. This study aimed to determine the relationship between menstrual patterns and pain in endometriosis patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia. Methods: This was cross-sectional analytic observational study. A total of 55 research subjects participated in this study. Data analysis was carried out with the help of SPSS version 25 software in univariate and bivariate. Results: Menstrual cycle, duration, color, and volume of menstrual blood are not related to pain intensity in endometriosis patients. Conclusion: There is no relationship between menstrual patterns and pain in endometriosis patients at Dr. Mohammad Hoesin General Hospital, Palembang, Indonesia
