    The Role of Percutaneous Ventricular Restoration Therapy in Heart Failure After Myocardial Infarction

    The process of ventricular remodeling has a major role in the pathogenesis of heart failure in patients with myocardial infarction. Angiotensin-converting enzyme (ACE) inhibitors, angiotensin receptor blockers (ARBs), antialdosterone diuretics, and beta receptor blockers are drugs that have been widely accepted as anti-remodeling agents. However, sometimes the efficacy of these drugs is inadequate, or their use is constrained by low blood pressure. Percutaneous ventricular restoration therapy is an action that aims to exclude segments of the myocardium that have akinetic or aneurysms by implanting a ventricular partitioning device percutaneously. This technique is expected to prevent the progression of ventricular remodeling through a procedure with a lower risk of intraprocedural mortality than the surgical approach

    Effect of Aloe vera Hydrogel Application on Increasing the Number of Fibroblasts in Socket Wounds Post-Tooth Extraction: An In Vivo Study

    Background. Aloe vera has been used as an anti-inflammatory, antimicrobial, and immune-boosting agent since ancient time. Hydrogel is a polymeric material that can retain a significant amount of water component and can last as long as possible in the gingival sulcus because the hydrogel is hydrophilic. This study is the first study that seeks to explore the potential of Aloe vera in healing socket wounds after tooth extraction. Methods. Twenty-five white rats of the Wistar strain were acclimatized for seven days before being included in the study. After acclimatization, the experimental animals were grouped into five groups. Evaluation of fibroblast response process with HE staining. Results. There were differences in the mean number of fibroblasts in all groups. From the results of this study, it can be stated that there is an effect of the application of hydrogel Aloe vera 1%, 2.5%, 5%, 10%, and xylazine on the increase in the number of fibroblasts in socket wounds after tooth extraction in Rattus novergicus. Conclusion. There is an effect of Aloe vera hydrogel application on increasing the number of fibroblasts in socket wounds after tooth extraction in Rattus novergicus

    The Association between Aromatase Gene Polymorphism Cyp19 Val 80 and Endometriosis Risk

    Background: Endometriosis implant has been known to express aromatase enzyme, Cytochrome p450 that catalyzes androgen into estrogen. It causes local estrogen production, leading to increased estrogen level, and subsequently triggers endometriosis lesion. CYP19 gene resided at chromosome 15q21.1 is the biggest component of aromatase coding cytochrome p450 enzyme. Objective: To identify relationship between aromatase gene polymorphism CYP19 Val 80 and the risk of endometriosis. Methods: This is an observational case-control study using frozen DNA sample from women with endometriosis and/ or adenomyosis who had undergone laparotomy/ laparoscopy at Obstetrics and Gynecology Department Dr. Mohammad Hoesin General Hospital Palembang January-November 2013. Samples were amplified and cut by PCR-FRLP using Rsa1 restriction enzyme. Results were divided into A/A genotype (homozygote mutant), G/A (heterozygote mutant), and G/G (homozygote wild type).  Data were analyzed by SPSS 21.0 version. Results: PCR-RFLP results for A/A genotype were 20 (21.3%) in endometriosis group and 8 (8.5%) in control group. G/A genotype were 18 (19.1%) in endometriosis group and 22 (23.4%) in control group. G/G genotype were 9 (9.6%) and 17 (18.1%) in endometriosis group and control group, respectively. There was significant increase risk of endometriosis in women carrying genotype A/A to those with genotype G/G with OR 4.722 (p<0.05). Conclusion: Polymorphism on aromatase gene CYP19 Val 80 A/A increases risk of endometriosis

    Introduction: Campylobacter rectus and Provetella intermedia bacteria as the main etiology causes of periodontal disease. One of the locally synthesized antimicrobials that is widely used for the treatment of periodontitis is Chlorhexidine, however this drug has side effects when used continuously. To overcome this, an alternative periodontitis therapy with medicinal plants is needed. This study aims to determine the effectiveness of hydrogel Aloe vera antibacterial concentrations of 2.5%, 5%, 10% and 20% in the bacteria Campylobacter rectus and Provetella intermedia. Methods: This study was a laboratory experimental study with posttest only control group design in vitro. The samples were Campylobacter rectus and Provetella intermedia. The number of groups in this study was six. The sample size was determined by Federer's formula and obtained four replications for each group. Data collection by measuring the inhibition diameter using a slide caliper. Data were analyzed using oneway ANOVA and posthoc LSD. Results: The results showed that there was a difference in the diameter of the hydrogel Aloe vera inhibition with a concentration of 2.5%, 5%, 10%, 20% and a positive control for Campylobacter rectus and Provetella intermedia bacteria. Conclusion: This study concluded that there is an antibacterial effectiveness of hydrogel Aloe vera in concentrations of 2.5%, 5%, 10% and 20% on Campylobacter rectus and Provetella intermedia bacteria. &nbsp

    Laparoscopic Davydov Procedure for Creation of Neovagina in MRKH Syndrome

    Introduction: The Mayer-Rokitansky-Kuster-Hauser (MRKH) syndrome is a condition in which there is an absence of the uterus and the upper part (2/3) of the vagina. Women with MRKH syndrome show normal development of secondary sexual characteristics and a normal 46, XX karyotype. MRKH syndrome is a common cause of primary amenorrhea. Ultrasonography examination confirms kidneys, presence of the ovaries, and absence of uterus. Treatment should include a multidisciplinary approach to create a functional vagina. One of the procedures is laparoscopic Davydov vaginoplasty. Aim: To report MRKH syndrome case treated with laparoscopic Davydov vaginoplasty. Method: Ms. A, 21 years old, P0A0, came to FER clinic Moh. Hoesin Hospital Palembang with complaint, never had menstruation, the patient admitted her breast, and her pubic had growth. She wants to get married in the next six months. From the US result, there was non visualized uterus, genital band (+), and both ovaries within normal limit ~ MRKH Syndrome. Karyotype Examination: 46, XX. Patient was diagnosed with primary amenorrhea caused by MRKH syndrome and was planned for neovagina (Davydov Procedure). Result: A woman 21-year-old, P0A0, was diagnosed with primary amenorrhea caused by MRKH syndrome. The Davydov procedure was done to create a neovagina. Intraoperative, after protrusion of the wood mold as a marker shown, an incision is made to the left and right side so that the mold penetrates the abdominal cavity, then the mold is pulled into the vaginal canal slowly. The anterior and posterior peritoneal layers are drawn into the vaginal canal. The anterior and posterior peritoneal layers were withdrawn with an ovum clamp and then interrupted suture. The anterior and posterior peritoneal layers were sutured in a circular method to form a vaginal cuff. The vaginal wall is sutured (interrupted) to hold mold fixed. Conclusion: MRKH syndrome is a disorder that occurs in females and mainly affects the reproductive system. This condition causes the vagina and the uterus to be underdeveloped or even absent at birth. Patients with diagnosed MRKH syndrome require interdisciplinary care. Davydov procedure is a laparoscopically-assisted technique, which has an advantage over traditional approaches. It has shorter operating time, lower intraoperative complications, shorter hospital stay, and no external scars. Postoperatively, sexual function is similar to women without gynecological disorders

    Correlation between Lgr5 Expression and 5-FU based Chemotherapy Response in Stage IV Colorectal Cancer Patients

    Background. Colorectal cancer is the third commonest malignancy and the second leading cause of cancer death in the world. 5-Fluorouracyl-based chemotherapy is the primary treatment modality for colorectal cancer. Cancer stem cells are known to be responsible for chemotherapy resistance. Lgr5 is a colorectal cancer stem cell marker that is the target gene for Wnt signaling. Lgr5 potentiates the Wnt signaling pathway through inhibition of a regulator that inhibits Wnt signaling. Lgr5 overexpression is associated with a worse prognosis and chemotherapy resistance. This study was aimed to investigate the correlation between Lgr5 expression and 5-FU-based chemotherapy response in stage IV colorectal cancer patients at Dr. Mohammad Hoesin Hospital Palembang.Methods. This study used a correlative analysis study with a retrospective design using secondary data from medical records and paraffin blocks of stage IV colorectal cancer patients who received 5-FU-based chemotherapy from September 2018 to September 2020. The number of samples was 30 subjects consisting of 22 cases of negative responses and eight positive responses. All samples were stained with Lgr5 immunohistochemistry. Data analysis used the contingency coefficient correlation test.Results. Of the 30 research subjects, 20 subjects (66.7%) had high Lgr5 expression and ten subjects (33.3%) with low Lgr5 expression. Correlation analysis using the contingency coefficient test showed a weak correlation between Lgr5 expression and 5-FU based chemotherapy response with a positive direction, which means the higher the Lgr5 expression, the less response to chemotherapy.Conclusion. There is a weak correlation between Lgr5 expression and 5-FU based chemotherapy response in stage IV colorectal cancer patients at dr.Mohammad Hoesin Hospital Palembang

    Factors Affecting Intratympanic Corticosteroids Injection Therapy Results in Sudden Deafness Patients at Dr Mohammad Hoesin Hospital Palembang

    Background. Sudden deafness is an emergency in the Ear Nose Throat Department, a subset of sensorineural hearing loss within a 72-hour window period and consists of a decrease in hearing of 30 decibels affecting at least three consecutive frequencies. The therapy given is corticosteroids, systemic corticosteroids or intratympanic corticosteroids injection, or both. The prognosis of sudden deafness depends on several factors. Patient with sudden hearing loss does not always have the same result in hearing improvement. This study was aimed to determine the factors that affect the results of intratympanic corticosteroid injection therapy in patients with sudden deafness.Methods. This study is an observational study with a cross-sectional design. The data were collected using medical records on 96 ears given intratympanic corticosteroid injection therapy at Dr. Mohammad Hoesin Hospital Palembang from July 2018 to February 2021.Results. From 96 ears conducted in the study, the improvement of therapeutic results on intratympanic corticosteroid injection in patients with sudden deafness was 72.9%. The study found factors that influenced the outcome of therapy, namely age (p=0.002), the onset of therapy (p = 0.003), and the use of a combination of systemic steroids (p=0.010). From the logistic regression test, it was found that the factors that most influenced the results of intratympanic corticosteroid injection therapy were young age 18-40 years (p=0.016) with (OR) 6.654 (CI95% 1.418-31.222) and therapy onset less than two weeks (p=0.027) with (OR) 3.108 (95% CI 1.134-8.515).Conclusions. The factors associated with the improvement in the outcome of intratympanic corticosteroid injection therapy in patients with sudden hearing loss were patient age and early onset of therapy

    Correlation of Prolonged Total Ischemic Time with Body Mass Index in Kidney Transplant: A Single Center Report

    Background: Chronic kidney disease (CKD) is a rising health concern worldwide, especially in Indonesia. The treatment of choice for end-stage renal disease is Kidney Transplantation.1 Numerous studies showed that prolonged total ischemic ischemic time may cause hypoxia of the graft tissue and increased risk of ischemia and reperfusion injury (IRI) and delayed graft function (DGF).2 Body mass index of kidney transplant recipients may cause prolonged duration of the procedure, as well as prolonged total ischemic time. This study aimed to determine the correlation between prolonged total ischemic time with body mass index. Method: This was an observational and cross-sectional analysis at Kariadi General Hospital Semarang involving patients who underwent kidney transplantation from January 2016 to December 2019. The total ischemic time was recorded intraoperatively. The Body Mass Index data were provided by medical records. The program used to statistically analyze the data was SPSS 23.0, and Spearman was used for hypothesis testing. Result: This study included 25 kidney transplant recipients. The mean total ischemic time was 43,27 ± 6,63 minutes. There was a significant positive correlation between prolonged ischemic time and body mass index (r= 0,506 ; p= 0,010). Conclusion: Prolonged total ischemic time was positively correlated with increased body mass index and these results are statistically significant

    Correlation Between Candidiasis And Mortality In Gastric Ulcer Perforation Patients

    Background: Gastric ulcer disease remains the most common gastrointestinal disease. This affects about 4 million people every year worldwide with an incidence of 1.5%-3% and 2-14% of these will progress to perforation. Mortality is reported in 30% of cases and caused morbidity in 50% of patients. Some studies have suggested that mortality in patients with peritonitis accompanied by candidiasis ranges from 20%-75%, whereas others have argued that the presence of fungal isolates in patients with perforated peptic ulcer does not affect the outcome. A recent study reported an incidence of 45% of patients peptic ulcer perforation cases accompanied by candidiasis in Dr. Soetomo General Hospital Surabaya in 2019.Methods: This was a cross sectional study based on medical record data of patients with gastric ulcer perforation accompanied by candidiasis who underwent laparotomy and gastric repair surgery at Dr. Soetomo General Hospital Surabaya from January 2019 to December 2020.Results: There was no significant correlation between candidiasis and mortality in gastric ulcer perforation patients (p=0.989).Conclusion: Candidiasis does not increase mortality in patients with gastric ulcer perforation
