    Differences in Arteriovenous Fistula Maturation between Diabetes Mellitus and Non-Diabetes Mellitus in Chronic Renal Failure Patients in Padang

    Background: Arteriovenous fistula (AVF) is the preferred vascular access for hemodialysis (HD). Several factors can affect AVF maturity, such as diabetes. This study aims to determine the differences in the maturation of arteriovenous fistulas between diabetes mellitus and non-diabetes mellitus in patients with chronic kidney failure in Padang. Methods: This was a comparative cross-sectional study of patients with chronic kidney failure who underwent arteriovenous fistula surgery in Padang. The sampling technique in this study was consecutive sampling. The analysis was carried out using statistical analysis according to the existing data scale and using SPSS software. Results: This study involved 46 patients with chronic kidney failure who underwent arteriovenous fistula surgery in Padang, of which 23 patients with diabetes mellitus and 23 patients with non-diabetes mellitus. The diameter of the arteriovenous fistula between diabetes mellitus and non-diabetes mellitus in patients with chronic renal failure had a significant difference (p<0.05). There was no significant difference in flow, depth, and maturation of arteriovenous fistulas in patients with diabetes mellitus and non-diabetes mellitus with chronic renal failure.     Analysis of Family Planning Services in Mohammad Hoesin Hospital Palembang on 2016

    Background. Family planning services in hospitals have been held since 1970 by the functional implementing unit led by obstetric and gynecologist specialists. Situational changes following decentralization in the 1990s caused policymakers and family planning practitioners' lack of attention to family planning services at province, district, and city levels. Data from 2002-2007 showed a decrease in family planning services from 6.2% to 4.9% in government hospitals and from 3.4% to 2.2% in private hospitals. The decrease in contraception use was affected by family planning users and providers. This study aims to analyze the factors contributing to family planning services in Mohammad Hoesin Hospital. Methods. This was a descriptive-analytic study with a cross-sectional design. The author used primary and secondary data with quantitative and qualitative approaches through the study of documents, observation, questionnaire, in-depth interview, and focused group discussion. Results. We identified various factors that play a role in the implementation of family planning services in almost all aspects, such as policy system, incomplete infrastructures; competence and the number of health workers; standard service procedure; sources and mechanism of the contraceptive tools; financial problems; recording and reporting system; quality control services; monitoring and evaluation; development services; also knowledge and behavior of the health workers. Conclusion. Family planning services need support and more serious attention from the directors to improve and overcome the problems and obstacles. In addition, the board of directors requires better coordination effort and cooperation on the internal level (between divisions or departments within the hospital) and with the relevant instance

    Relationship between Interleukin 6 Serum Levels and Degree of Depression in Systemic Lupus Erythematosus Patients Internal Disease in Dr. Moh Hoesin Palembang

    Introduction. Systemic Lupus Erythematosus (LES) is a chronic autoimmune inflammatory disease affecting many organ systems with varied clinical manifestations, disease course and prognosis. One of the clinical manifestations that can occur is depression as much as 25%. In LES patients, there is an increase in proinflammatory cytokines, one of which is IL-6, which is often associated with contributing to the pathophysiology of depression. The purpose of this study was to determine the relationship between serum IL-6 levels and the degree of depression in LES patients at RSUP DR. Mohammad Hoesin Palembang. Methods. Analytic observational study in the form of correlation test with cross sectional design which was conducted in the Internal Medicine room RSUP DR. Mohammad Hoesin Palembang from May 2019 to March 2020. The number of study subjects was 40 samples who were LES patients with depression who had been calculated using the BDI (Beck's Depression Inventory). All subjects were examined for serum IL-6 levels and evaluated for factors that influence it. Results. There were 40 study subjects aged 18-56 years. The median degree of depression was 16 with a range of values from 10 to 40. There were 17 people (42.5%) with mild depression degree, 14 people (35%) moderate depression, and 9 people (22.5%) major depression. In LES activities, there were 5 people (12.5%) with light LES activities, 20 people (50%) with moderate LES activities, and 15 people (37.5%) with heavy LES activities. The mean serum IL-6 level was 11.57 pg / mL with a value range of 1.1-50pg / mL. There was a significant correlation between the degree of depression and LES activity (p = 0.004; r = 0.444) and between serum IL-6 levels and the degree of depression (p = 0.025; r = 0.169). However, there was no significant correlation between serum IL-6 levels and LES activity (p = 0.057; r = 0.304). Conclusion. There is a relationship between serum IL-6 levels and the degree of depression in LES patients at Dr. Moh. Hoesin Palembang with a very weak correlation

    Diagnostic Value of CT Angiography (CTA) in Carotid Cavernous Fistula (CCF) Patients Using the Digital Subtraction Angiography (DSA) as The Gold Standard: Observation in Dr. Soetomo General Hospital Surabaya from January 2016 to July 2020

    Background: CT angiography (CTA) is the initial modality for diagnosing the Carotid Cavernous Fistula (CCF), identifying the type of CCF, measuring the diameter of the fistula and the diameter of the internal carotid artery (ICA) pre and post fistula, and then confirmed by Digital Subtraction Angiography (DSA) as the gold standard. CTA is expected to provide more information to improve the accuracy of CCF diagnosis to benefit therapy and prevent complications. This study aimed to determine the diagnostic value of CTA in CCF patients in Dr. Soetomo General Hospital Surabaya, using the DSA as a gold standard. Methods: An observational retrospective study of CCF patients met the inclusion and exclusion criteria and underwent CTA and DSA examinations at the Radiology Installation of Dr. Soetomo General Hospital, Surabaya, from January 2016 to July 2020. Results: In the comparison of types of CCF CTA to DSA, the direct group was 53.8% and 57.7%, respectively, while the indirect group was 46.1% and 42.3% with a very strong correlation (κ = 0.922; p = 0.000), sensitivity was 93.3%, specificity was 100 %, PPV of 100%, NPV of 91.7% and accuracy of 96.15%. Direct type evaluation on the DSA showed a large picture of the fistula diameter; the ICA pre fistula's size was enlarged; therefore, the ICA post fistula diameter was reduced or absent. There was a strong correlation of fistula diameter measurement results between CTA and DSA (r = 0.695, p = 0.006). Conclusion: CTA has a similarity rate of 92.2% with DSA in identifying the type of CCF. The accuracy value of CTA is close to DSA in fistula diameter measurements

    Introduction: Cancer has become a major public health problem worldwide, with breast cancer is the most common cancer among women and one of the most important causes of death among them. Management of breast cancer have some adverse effects, with many factorx=s shiouln be considered to ordered the therapy. Use of chemotherapy will suppress cell proliferation and trigger apoptosis, but it cannot just focus on cancer cells only, but also would affected on other cells and cause. Objective: This study aim to analyze the inhibition effect of the Annonanceous sp. ethanolic extract to Vascular Endothelial Growth Factor (VEGF) on MCF-7 cells line Material and Methods: This study is an analytic experimental study used Randomized Complete Block Design (RCBD) with three repetitions. The extract was dissolved in 1 ml of DMSO with a concentration of 0.1%. Furthermore, dilution was made with a dose of 0 µg / ml (K); 25 µg / ml (P1); 50 µg / ml (P2); 100 µg / ml (P3); 200 µg / ml (P4). MCF-7 cells line were cultured used RPMI 1640 Medium with 80 – 90% confluent. The ethanolic extract of Annonaceous sp. were exposed to MCF-7 cells for 48 hours. Analyze of VEGF level use ELISA Methods with λ=405 nm. Results: The results of ELISA analyze shows that the ethanolic extract of Annonaceous sp. have potential effect to decreased of VEGF expression on sample with treatment on some concentration (p< 0,05).     The International Association for the Study of Pain (IASP) defines pain as an unpleasant sensory and emotional experience associated with potential or tissue damage or described in terms of such damage. Pain in patients with decreased consciousness is individual and needs to consider many aspects. Following the expansion pain definition according to the IASP in 2020, the inability to communicate does not exclude the possibility that humans experience pain. Therefore, a good pain assessment in patients with decreased consciousness can increase the value of disease management and improve the patient's quality of life both during the treatment period and after hospitalization. Because of the inability of patients with decreased consciousness to verbally convey their pain, this poses a challenge for the clinician.     Anatomical and Hemodynamic Evaluation of Mitral Stenosis Patients with Echocardiography

    Mitral stenosis (MS) is the most common valvular heart disease encountered in developing countries. The cause of MS is almost always chronic rheumatic heart disease. Echocardiography is the single most important diagnostic tool in the evaluation of MS. The objectives are to confirm the etiology, to assess the severity of stenosis, to recommend the type and timing of intervention, to assess other valvular lesions, presence of thrombus, and vegetation. According to current guidelines and recommendations for clinical practice, the severity of MS should not be defined by a single value but rather be assessed by a multimodality approach that determines valve areas, mean Doppler gradients, and pulmonary arterial pressures. The European Society of Echocardiography/American Society of Echocardiography (EAE/ASE) recommendations of measurement method for clinical practice were categorized into three level of recommendations. Mitral valve area (MVA) can be assessed by planimetry using either 2D or 3D imaging, pressure half-time (PTH), the continuity equation, and the proximal isovelocity surface area (PISA) method. These result echocardiographic examinations can increase the accuracy and appropriate management with a good prognosis

    Role of Fibroblast Growth Factor-23 in Coronary Slow Flow Phenomenon Pathogenesis

    The phenomenon of angina chest pain without significant epicardial coronary artery stenosis, but accompanied by a slowdown in coronary blood flow is often found in patients with symptoms of acute coronary syndrome who undergoing invasive coronary angiography. This phenomenon of slow coronary blood flow is then called the coronary slow flow phenomenon (CSFP). The pathogenesis mechanism of CSFP remains unclear. The pathogenesis of CSFP is thought to be multifactorial. Endothelial dysfunction, small vessel disease, inflammation, renin system angiotensin aldosterone, atherosclerosis are thought to be involved in the pathogenesis of CSFP. Cardiovascular disease incidence and death were associated with elevated levels of Fibroblast growth factor-23 (FGF-23). High levels of FGF-23 can lead to formation of blood vessel calcification, left ventricular hypertrophy, arterial stiffness, endothelial dysfunction, increased inflammatory markers and elevated levels of angiotensin II. It is suspected that FGF-23 has a role in this event other than as a regulator of bone and mineral metabolism. This literature review aims to determine the relationship between fibroblast growth factor-23 and the pathophysiology of CSFP. Based on the broad role of FGF-23, it is possible that FGF-23 is involved in the pathogenesis of CSFP

    Comparison of Interleukin-1β Levels In Open and Closed Fracture Patients of The Long Bone In Padang

    Background and Purpose. Trauma is a significant burden on causes of death, disability, and financing in least developed or "third world" countries. Fracture healing, especially in wounds and tissues, begins with a hemostatic and inflammatory phase triggered by inflammatory mediators such as cytokines, particularly IL-1β and TNF-. The purpose of this study was to determine the difference in levels of interleukin-1β in open and closed fractures of long bones in PadangMethod. This type of research is analytic observational with cross sectional design with primary data. The data were taken from the examination of IL-1β levels in long bone fracture patients at RSUP Dr. M. Djamil, RSKB Ropanasuro and RST Reksodiwiryo Padang for 4 months starting from March 2021 to July 2021. The sampling technique in this study used non-probability sampling with the consecutive sampling method. The analysis was carried out using the Independent sample T-Test to see the difference in mean IL-1 which would be calculated for open fractures and closed fractures of long bones.Results. The results showed that most (92.9%) of the patients were male, 50% had open fractures, and 50% closed fractures. The mean value of the patient's age was 34 years and the mean level of IL-1β was 555,951 pq/L. Most of the fracture sites in patients were tibia and fibula (35.7%) and femur (35.7%) and 28.6% were radius and ulna. The results showed that there was a significant difference in the mean levels of IL-1β in patients with open and closed fractures of long bones (p-value = 0.007)Conclusion. There are differences in the levels of interleukin-1β in open and closed fractures of long bones in Padang. This research is expected to provide consideration to support examinations in health services, in this case in fracture patient

    Condylomata Acuminata in Adolescent Girl 19th Years Old Successful with Multimodality Therapy and Combination with HPV Vaccination: A Case Report

    Background: Condylomata acuminata (KA) or better known as genital warts disease is a genital area infectious disease caused by Human papilloma virus (HPV). The highest prevalence infection of the condylomata acuminata occurs during active sexual periods, namely age 17-33 years, with the peak occurring at the age of 20-24 years. Currently HPV 16 and 18 are known to cause malignancy in the genital and anogenital region including cervical cancer and anal cancer, while HPV 6 and 11 are the cause of 90% of condylomata acuminata. HPV is linked to the findings of 500,000 new cases of cervical cancer and 250,000 deaths from cervical cancer each year worldwide.1 Case report: One case of condylomata acuminata in a 19-year-old adolescent girl with a major complaint of warts around her genitals that felt itchy and gradually getting bigger since 2 weeks ago. Patient was a student and unmarried. History of sexual contact with men with genito-genital without using condoms since 5 months ago. Venereological state : on the vulva and perineal there were multiple vegetations with verucose surfaces with the largest size 0.8 cm x 0.5 cm x 0.1 cm and the smallest size 0.2 cm x 0.2 cm x 0.1 cm. Acetowhite examination result was positive and PCR examination results found positive HPV types 6,11,16 and 18. Discussion: Patients are diagnosed to condylomata acuminata with a history of free sex at an early age. The condylomata acuminata experienced by patients was type 6,11,16 and 18. These types of 16 and 18 HPV are know to cause malignancy in the genital and anogenital region including cervical cancer and anal cancer
