    Preliminary Result in Endoscopic Video Assisted Breast Surgery

    Background : The development of technology has grown rapidly in surgical science, especially the use of endoscopy to develop operations in producing minimal scar tissue. Endoscopic VABS is a surgical technique and began to develop since 1995. The technique has several approaches, namely through the chest wall, mammary and axilla. The descriptive study aims to report our first experience in the management of benign breast tumors with Endoscopic Video Assisted Breast Surgery. Methods : Ten patients with benign breast tumor have performed Endoscopic VABS in the period March 2017 to September 2017 at the Hospital in Surakarta. The largest diameter at the nodule, duration of operation, length of postoperative care, postoperative pain, cosmetic outcomes and complications were observed retrospectively. Result : All patients are women with median age 28 years. The largest diameter that can be taken is 3.5 cm with median 2 cm. One patient performed the conversion into a lumpectomy with conventional techniques due to difficult dissection and uncontrolled bleeding. The median duration of surgery was 60 minute with an average length of treatment for 2 days postoperatively. Average VAS Score was 4. No postoperative complications were found. But cosmetically, all patients are satisfied with the results of surgical wounds. Conclusion : The VABS endoscopic procedure is safe and effective procedure and has excellent cosmetic benefits because it does not show scar tissue in the breast area. This VABS Endoscopic technique has a weakness in terms of longer duration of operation compared to conventional techniques, but this can be overcome with the expertise and experience of the operator

    Background: Law of the Republic of Indonesia No. 36 of 2009 concerning traditional health services (Yankestrad), namely articles 1, 48, 59, 60 and 61. Acupressure is included in Yankestrad skills that are easy, cheap, simple, effective, and without any side effects. harmful. Acupressure at the Large Intestine 4 Point (LI4) based on the literature is able to reduce labor pain in the first stage of the active phase. Methods: This type of research is quasi-experimental with a pre-test and post-test research design. The samples were divided into two groups, the treatment group and the control group. Results: Based on the results of the research that has been carried out, it can be concluded as follows: Active phase I labour pain before being given Acupressure Therapy for primigravida mothers in PMB Aceh Besar District averaged 7.35. Active phase I labour pain after being given Acupressure Therapy to primigravida mothers in PMB Aceh Besar District decreased by an average of 5.47. Conclusions: There are differences in labour pain in the stage-I of active phase in the Acupressure therapy group with the Routine care group for the stage-I of labour for primigravida mothers in PMB Aceh Besar District

    The Incidence Rate of Cranii Base Fracture in Dr. Kariadi Hospital Semarang Period 2019

    Introduction: Head injury is a major cause of death, especially in young adults and a major cause of disability. The incidence of head injuries is increasing due to the increasing use of motorized vehicles. As many as 4% of cases of head fracture occur in the base of the cranii. Method: This research used a descriptive observational method. The data used is secondary data taken from medical records. In this study, we wanted to know the incidence of cranii base fracture cases due to head trauma in patients treated at Dr. Kariadi period 2019. Result: From 29 samples of medical records obtained, 48.3% had mild head trauma, 27.6% had moderate head trauma, and 24.1% had severe head trauma. Symptoms of patients with the most clinical symptoms of cranii base fracture were raccoon eyes 51.7%, with the most fracture locations in the anterior fossa as much as 51.7%. Conclusion: The incidence of cranii fracture due to head trauma at RSUP Dr. Kariadi in 2019 was found mostly over 40 years old and most of them were men. The type of injury is a minor head injury, with the largest location in the anterior fossa, and the most common clinical sign found in patients with cranii fracture is raccoon eyes

    The International Association for the Study of Pain defines pain as “unpleasant sensory and emotional experiences associated with actual or potential tissue damage.” Pain is common among patients with cancer, with the prevalence of pain being 66.4% in advanced metastatic disease or terminal, 55% during anticancer treatment, 39.3% after curative treatment, and moderate to severe pain were reported in 38.0% of all patients. Pain is one of the most common and dreaded symptoms in cancer sufferers, but the exact number of cancer patients who experience pain is difficult to determine. There are a wide variety of studies in defining pain and the tools they use to measure the complexity of this symptom. Good or complete pain control can be achieved in 80-90% of cancer patients. Nevertheless, despite major advances in pain control, cancer-related pain continues to be a major public health problem globally

    Ferning, Schimer I and Tear Break Up Time (TBUT) Accuracy Test in Post-Operative Cataract Patient with Extra Capsular Cataract Extraction (ECCE) Technique

    A B S T R A C TObjective: To determine the accuracy of the Ferning, Schimer I and TBUT test incataract patients after cataract surgery with the ECCE technique at the EyePolyclinic at Mohammad Hoesin Hospital Palembang and the Special Eye HospitalPalembang Methods: Diagnostic test was undertaken at the Eye Polyclinic atMohammad Hoesin Hospital Palembang and the Special Eye Hospital Palembangfrom May 2019 to January 2020. There were 42 postoperative cataract patients whomet the inclusion criteria. Ferning, Schimer I and TBUT tests were performed beforesurgery and one month after surgery. Data analysis using SPSS version 22.0 (SPSS,Inc Chicago, Illinois). Results: Fourty two samples of postoperative cataract surgerywith ECCE technique obtained by female sex (57,1%) with a mean age of cataractpatients of approximately 58.09 years. The accuracy of Schimer I and TBUT is 0.952with 100% sensitivity, 50% specificity, 95% positive predictive value (PPV) and 100%negative predictive value (NPV), the accuracy of TBUT and Ferning is 0.881 with90.2% sensitivity, 0% specificity , positive predictive value (PPV) 97.4% and negativepredictive value (NPV) 0% while the accuracy of Ferning and Schimer I was 0.928with a sensitivity of 97.5%, specificity of 0%, positive predictive value (PPV) of 95.1%and negative predictive value (NPV) 0%. Conclusion: Schimer I test is more accurateto use as a predictor of dry eye in postoperative cataract patients than Ferning andTBU

    Comparison of the Effectiveness 3% Tranexamic Acid Cream Versus 4% Hydroquinone Cream for Treatment of Epidermal Type Melasma

    Background: Melasma is an acquired hypermelanosis in areas that are exposed to sun. especially in face. Severity of melasma is measured using modified Melasma Area Severity Index (mMASI) and Melanin Index (MI). Hydroquinone (HQ) is an effective drug and also a gold standard in terms of melasma treatment. However. thedrawback of hydroquinone is the treatment duration which is quite long. the occurence of side effects. and also recurrence potential. Several recent studies shows that one of the anti-pigmentation agent options. the tranexamic acid (TA). is able to prevent melanocyte activation. The results of various studies found that the use of TA showed a significant improvement in MASI scores. Objective: To compare the effectivity of 3% TA cream with 4% HQ cream as an alternative therapy in epidermal type melasma treatment. Methods: A double-blind. matched. randomized clinical trial was conducted during July 2019 until August 2019 at Dr. Mohammad Hoesin General Hospital Palembang. Subjects were divided into two groups. The first group. consists of 30 participants. get 3% TA cream; and the second group. consists of 30 participants. get 4% HQ cream. Subjects who fulfilled the inclusion criteria was included through a simple randomization. Study subjects wer given random cream pot (with label A or B on it) according to the result of simple randomization. Finally. every subjects’ melasma severity was measured using mMASI and mexameter ® . Results: There were differences in terms of baseline. 4 th weeks and 8 th weeks after treatment mMASI scores in 3% TA and 4% HQ group. At 8 th week. the mMASI score the 3% TA group was lower than 4% HQ group. There was no differences in baseline. 4 th weeks and 8 th after treatment MI between the two groups. Melanin index for 3% TA group was lower than 4% HQ group. Conclusion: Three-percent TA cream and 4% HQ cream is effective in decreasing mMASI score and MI in epidermal type melasma. There were differences in terms of the mMASI score during the 8th week after treatment between 3% TA and 4% HQ group

    Recognition of pain depends upon intact pathways from pain receptors to the thalamus and cerebral cortex, as well as functional cerebral cortex and subcortical structures. Thus any means that renders the cerebral cortex nonfunctional, such as hypoxia or drug depression, prevents pain. When this happens, stimuli that evoke motor nerve reflexes that may be painful to the conscious animal are not painful in the unconscious animal. Equally painful stimuli administered to animals chemically paralyzed by curare or succinylcholine will not evoke a motor reflex simply because of paralysis, but will cause pain because of the conscious state. Hence, it is possible that unconscious animals may feel no pain but respond to certain stimuli, and paralyzed animals may feel pain but cannot respond

    Introduction: Pulmonary malignancies may easily be overlooked and valuable time may be lost. Lung cancer is sometimes diagnosed as a tuberculous cavity or an abscess. Abscess formation can appear in several different ways. Carcinoma that occurs in medium-sized bronchi causes partial bronchial obstruction, atelectasis, and infection due to retention. Inflammation can progress to damage to lung tissue, resulting the formation of multiple suppurative foci or more localized lung abscess. The link between lung abscess and lung cancer has been known, but the presence of malignancy in lung abscesses often undiagnosed. Obstruction from lung cancer can predispose to the development of a lung abscess. Case of a 54 year old man with increased pain at the right chest when breathing in since two months. On physical examination, it was found decreased of fremitus at the right hemithorax, deafness and decreased breath sounds as high as the II - V thoracic right hemithorax. Thorax CT-scan showed a round, homogeneous (HU: 17-30), with a cavity-like image with air fluid level, size: 7,88 cm x 8,2 cm x 9,29 cm and honey comb appearance around it. On the examination of TTNA (Transthoracic Needle Aspiration) results obtained Squamous Cell Carcinoma. A lobectomy is planned for the patient. Conclusion: We reported a rare case, a 54 year old male patient, with the diagnosis of Carcinomatous Lung Abscess. This case report was prepared with the aim of increasing awareness of malignancy in patients with a clinical presentation of abscesses, especially in old age

    The Effectivity of Supplementation Artemisia vulgaris for Adenocarcinoma Mammae Chemotherapy to Reduce CD 34 and Tumor Massa Diameter (Study in C3H Mice Given Adriamycin - Cyclophosphamide Chemotherapy)

    Introduction: Breast cancer is still a major health problem in the world. In the case of breast cancer, surgery is the main treatment option besides chemotherapy, radiation, and immunotherapy such as Artemisia vulgaris (AV). AV is cytotoxic selectively acts as a supplement to breast adenocarcinoma chemotherapy given the Adriamycin-Cyclophosphamide regimen, to improve chemotherapy response.The study was aimed to proving AV extract enhances the chemotherapy response in C3H mice with adenocarcinoma mammae given Adriamycin-Cyclophosphamide Chemotherapy. Method: This study used Post test only control group design on 24 females C3H mice that were randomly selected and divided into four groups: group K (control), P1 (chemotherapy), P2 (extract), and P3 (combination). Adenocarcinoma mammae comes from the inoculation of donor mice. Chemotherapy of Adriamycin 60 mg / m 2 and Cyclophosphamide 600 mg / m 2 were given in two cycles. AV 13 mg (0.2 ml) was given once daily orally. CD34 were evaluated by imunohistochemical staining and tumor mass diameter were counted by calipers. Result: The microvascular density CD34 and tumor mass diameter were obtained in groups of K, P1, P2, P3 respectively 60.76 ± 1.5; 39.70 ± 2.00; 57.10 ± 1.29; 35.26 ± 2.06 and 12.52 ± 1.49; 6.20 + 1.04; 9,94 + 1.21; 3.94 + 0.76. Statistical analysis showed significant differences in CD34 between groups K vs P1, P2, P3 (p = 0.001, p = 0.014, p = 0.001), P1 vs P2 and P3 (p = 0.001, p = 0.033) and P2 (P = 0.001). Tumor mass diameter between groups K vs P1, P2, P3 (p=0.001; p=0.014; p=0,001), P1 with P2 (p= 0.001) P1 with P3 (p = 0.033) and P2 with P3 (p = 0.001). Correlation analysis between CD34 with tumor mass diameter was found to have significant correlation (p = 0.001 and r = 0.932). Conclusion: Artemisia vulgaris is a potential to reduce angiogenesis in terms of decreasing the microvascular density CD34 and tumor mass diameter of adenocarcinoma mammae of C3H mice treated with Adriamycin-Cyclophosphamide chemotherapy and can improve the effectivity

    Factors Associated with The Incidence of Infection in Tracheostomy Stoma at Mohammad Hoesin Hospital

    Introduction: Tracheostomy is a surgery to open the trachea to allow air to enter the airway. Tracheostomy cannot be separated from possible complications. Tracheostomy stoma infection is one of the complications of post-tracheostomy that occurs due to the activity of several bacteria. Many factros can cause the incidence of tracheostomy stoma infection. Objective: The aim of this study is to determine the factors associated with the incidence of tracheostomy stoma infection. Methods: Observational study with cross-sectional design. Data collection was carried out using medical record data on 71 subjects who underwent tracheostomy tube replacement from july 2019 to November 2020. Results: From 71 subjects, it was found that the subjects who had tracheostomy stoma infection were 54.9%. The results of bivariate analysis showed that age (p=0.031), comorbidities (p=0.009), the type of tracheostomy tube (p=0.015), the use of antiseptic in tracheostomy dressing (p=0.020), tracheostomy tube care (p=0.013), and the interval of changing the tracheostomy tube (p=0.041) are the significant factors. The results of multivariate analysis showed that tracheostomy tube care and the interval of changing the tracheostomy tube had a significant effect on tracheostomy stoma infection. Tracheostomy tube care has a risk of 9.362 times to experience infection with CI95%(1.535-57.086) and the interval of changing the tracheostomy tube has a risk of 10.669 with CI95% (1.074-105.985). Conclusion: There is a significant relationship between age, comorbidities, type of tracheostomy tube, the use of antiseptic in tracheosotmy dressing, tracheosotmy tube care, and interval of changing the tracheostomy tube with the incidence of tracheostomy stoma infection
