Get PDF相補鎖間の結合による二本鎖がDNAの基本的な構造であるが、片方のDNAがRNAと塩基の対をなし二本鎖を作ると、相補鎖側のDNAが塩基対を作ることなく外れて一本鎖になる. この状態をR-ループ構造という. 近年、Rループが細胞機能やゲノムDNAの安定性に重要な機能を持っていることが次々と報告されている. 遺伝子発現の際にDNA配列からRNAが転写されるとRループが生じる. Rループは遺伝子発現やDNA複製、転写終始において制御されており、Rループ制御の乱れが原因となる疾患の分子機構が明らかにされている. 本総説ではゲノムにおけるRループの機能とRループ制御に関わる因子について述べ、それら因子の機能不全が原因となる神経疾患の例を紹介する.The basic structure of DNA is double strand formed by bonds between complementary strands, but when one DNA pairs bases with RNA to form a hybrid double strand, the DNA on the complementary strand separates without forming base pairs. This is called R-loop structure. In recent years, it has been reported that R-loops have important cellular function and genomic DNA stability. R-loops occur when RNA is transcribed from DNA during gene expression. Formation of R-loops is regulated in gene expression, DNA replication, and transcription termination, and the molecular mechanisms of diseases caused by disturbances in R-loop regulation have been reported. Here, we review the functions of R-loops in the genome and the factors involved in R-loop regulation and introduce examples of neurological diseases caused by dysfunction of R-loop formation and factors involved in it.departmental bulletin pape
Total arch replacement with open stent graft placement for the proximal aortic neck enlargement of 45-mm stent graft: a case report
Get PDF症例は74歳男性．3年前に施行した左鎖骨下動脈直下 (Zone III)の胸部大動脈ステントグラフト内挿術後の中枢側大動脈の拡大で手術の方針となった．45mm径と大口径のステントグラフトが留置されており, 人工血管の末梢吻合をステントグラフトに直接行うことは, 口径差やステント拡張力の観点から困難と考えられた．胸骨正中切開でアプローチし人工心肺確立の後, 左鎖骨下動脈を閉鎖し, 左総頚動脈直下 (Zone II)の弓部大動脈に末梢吻合口を作成した．そこからopen stent graftを末梢に向けて挿入し, 弓部大動脈全置換術を施行した．術後3日目造影CTで, open stent graft末梢端でのエンドリークは無かった．経過良好で, 術後11日目に独歩退院となった．A 74-year-old man underwent surgery for proximal aortic neck dilation following Zone III TEVAR. Direct distal anastomosis of the prosthetic graft to the 45-mm stent graft was challenging because of size mismatch and the radial force of the stent. The operation was performed via median sternotomy under cardiopulmonary bypass. The left subclavian artery was transected at its origin, and the proximal stump was oversewn. The distal anastomotic site was created on the aortic arch at Zone II. An open stent graft was then introduced in an antegrade fashion, deployed, and followed by total arch replacement. Postoperative contrast-enhanced CT on day 3 demonstrated no endoleak at the distal end of the open stent graft. Comfortable Seatbelts for Pregnant Women with Twins in Japan: A Cross-Sectional Observational Study on Seatbelt Usage
No full text滋賀医科大学博士（医学）Optimal seatbelt practices for pregnant women with twins at different gestational ages remain uncertain. To offer recommendations for a comfortable seatbelt system, this cross-sectional observational study explored seatbelt usage and driving habits among women with twins across various pregnancy stages through an online survey that explored driving conditions and comfortable seatbelts at different stages of pregnancy. Women who drove daily before their pregnancy with twins decreased their driving frequency as the pregnancy progressed. Correct seatbelt usage was lower and no seatbelt usage was higher among pregnant women with twins than those with singleton pregnancies. They adapted their seatbelt-wearing techniques to minimise pressure on the chest in the first and third trimesters and the abdomen from the second trimester onwards. The comfortable seatbelts were those that could alleviate belt pressure, featuring waist belts to reduce pressure, wider belts to avoid localised pressure, and shoulder belts resembling a backpack type. When wearing a seatbelt, avoiding pressure on the thorax and abdomen is key for pregnant women with twins. This study suggests that the suitability of driving for pregnant women with twins in their last trimester and the reliability of seatbelts designed for such women should be further examined and validated.令和6年度doctoral thesi
