ИНТРАОПЕРАЦИОННАЯ ФОТОДИНАМИЧЕСКАЯ ТЕРАПИЯ ПРИ МЕЗОТЕЛИОМЕ БРЮШИНЫ
Abstract Results of application of a new technology of intraoperative photodynamic therapy (IOFDT) in patients with peritoneal mesothelioma developed at P. Herzen Moscow Oncology Research Institute are presented. The study included 8 patients. 3 patients underwent surgery in various amount: 1 – limited peritonectomy in the volume of tumor foci resection and resection of a large omentum, 1 – limited peritonectomy in the volume of tumor foci resection and atypical resection of the right lobe of the liver, 1 – only resection of the large omentum due to the fact that the tumor was located only in a large omentum and no signs of lesions of the parietal peritoneum was revealed by intraoperative revision. Surgical intervention in these patients was concluded by IOPDT. The remaining 5 patients underwent only IOPDT. After the treatment, two patients underwent additional courses of laparoscopic IOPDT. Of the 8 patients enrolled in the study, 4 died from the underlying disease, 1 from cardiovascular disease with recurrence of the disease, 1 from cardiovascular disease without signs of recurrence, 2 were monitored for 6 months and 146 months (12 years). Thus, in the group of patients with peritoneal mesothelioma, the maximum observation period was 146.44 months, the median survival was 48.4 months, the total specific 1-year survival was 85.7±13.2%, the three-year survival was 68.5±18.6%, the 5-year survival was 45.7 ± 22.4 %. The average life expectancy after treatment of patients with repeated courses of laparoscopic IOPDT was 87 months, without repeated courses – 35.8 months. Thus, life expectancy was higher in patients with repeated courses of laparoscopic IOPDT. Small sample size caused to the rarity of this pathology does not allow for statistically significant conclusions. However, the results of the study indicate the prospects of multi-course intraoperative photodynamic therapy in patients with peritoneal mesothelioma
ИСПОЛЬЗОВАНИЕ АУТОФЛУОРЕСЦЕНТНОЙ ДЕРМАТОСКОПИИ У БОЛЬНЫХ ПСОРИАЗОМ
Results of autofluorescence dermatoscopy using videodermatoscope «EcoSkin» in patients with different dermatologic diseases are represented. The study included 64 patients, 27 of them had different types of psoriasis, 11 – acne, 3 – rosacea, 5 – eczema, 3 – mite-induced dermatitis, 4 – superficial mycosis of the glabrous skin, 7 – atopic dermatitis, 4 – seborrheic dermatitis. The major changes of autofluorescence were detected in pathological foci in patients with psoriasis. Authors suppose that increased autofluorescence of porphyrins in psoriatic plaques in the study is associated with enhanced prolyferation in pathological tissues and with liver dysfunction. The analysis of autofluorescence images in patients with rosacea, eczema, atopic and mite-induced dermatitis did not show significant difference in autofluorescence images of affected skin comparing with intact skin in same patients. In patients with acne a pattern of spot red-orange autofluorescence of skin in seborrheic areas corresponding with location of openings of pilosebaceous apparatus was detected that was associated by authors with Propionibacterium acnes infection
ФЛУОРЕСЦЕНТНАЯ ДИАГНОСТИКА ПРИ РЕЦИДИВЕ РАКА МОЧЕВОГО ПУЗЫРЯ
The clinical case of successful use of local fluorescence spectroscopy combined with fluorescence imaging during cystoscopy for diagnosis of recurrent bladder cancer is represented in the article. Histological study of fluorescent foci confirmed tumor growth (urothelial carcinoma) in all areas with high levels of diagnostic parameter. In the fluorescent focus with low diagnostic parameter inflammation was detected.В статье описано клиническое наблюдение успешного применения локальной флуоресцентной спектроскопии в комбинации с методикой флуоресцентной визуализации с препаратом на основе 5-аминолевулиновой кислоты при цистоскопии в диагностике рецидива рака мочевого пузыря. Результаты гистологического исследования флуоресцирующих очагов подтвердили наличие опухолевого процесса (уротелиальная карцинома G1) во всех очагах с высокими значениями диагностического параметра. Во флуоресцирующем очаге с низкими значениями диагностического параметра выявлен очаг воспаления
ИЗУЧЕНИЕ АПОПТОЗА В ЛИМФАТИЧЕСКИХ УЗЛАХ ЗОНЫ АБДОМИНАЛЬНОЙ ЛИМФОДИССЕКЦИИ ПОСЛЕ ИНТРАОПЕРАЦИОННОЙ ФОТОДИНАМИЧЕСКОЙ ТЕРАПИИ ПРИ ЗЛОКАЧЕСТВЕННЫХ НОВООБРАЗОВАНИЯХ ЖЕЛУДОЧНО-КИШЕЧНОГО ТРАКТА
The method of DNA-electrophoresis was used to study apoptosis in the lymph nodes of the abdominal lymphodissection zone after local intraoperative photodynamic therapy (PDT) for the gastrointestinal cancers. Apoptosis (apoptotic ladder) was determined in the four cell populations of lymph nodes:– in the lymph nodes with metastases that have not been irradiated;– in the irradiated during PDT lymph nodes with metastases;– in the lymph nodes without metastases that have not been irradiated;– in the irradiated during PDT lymph nodes without metastases.The implementation of this study was possible due to intraoperative division of lymph nodes into two parts, one of which was removed while another one intentionally wasn’t removed and was irradiated (in human). It was also removed 60-90 minutes after PDT treatment and examined for apoptosis. As a result, it was found that apoptosis is detected in the irradiated during PDT lymph nodes with metastases. The obtained results show the fact that in the lymph nodes of the abdominal lymphodissection zone tumor cells selectively die by apoptosis as a result of the local intraoperative photodynamic therapy (PDT) for the gastrointestinal cancers while in non-malignant cells apoptosis doesn’t occur after PDT (p <0.01). Thus, the obtained clinical and experimental data made it possible to evaluate the target PDT effect and confirm the safety of the photodynamic effect on healthy tissues of the lymphoid system located in the irradiation zone
ФОТОДИНАМИЧЕСКАЯ ТЕРАПИЯ СТОЙКОЙ КЕРАТОАКАНТОМЫ ЛБА
The results of the follow-up of the patient with persistent forehead keratoacanthoma are represented. The patient underwent one course of PDT with intralesional administration of photosensitizer radachlorin at a dose of 0.75 ml/cm2 of tumor. The characteristics of the irradiation were as follows: the laser power density 0.39 W/cm2, the light dose – 300 J/cm2. Two weeks after treatment there was a cosmetically satisfying soft normotrophic scar with no attachment to underlying tissues in the site of the treatment. According to cytological study tumor cells were not detected. For one-year follow-up there was no recurrence.Приведены результаты клинического наблюдения за пациенткой со стойкой кератоакантомой лба. Пациентке проведен 1 курс фотодинамической терапии с внутриочаговым введением фотосенсибилизатора радахлорин в дозе 0,75 мл/см2 опухоли. Параметры облучения: плотность мощности лазерного излучения – 0,39 Вт/см2, плотность энергии лазерного излучения – 300 Дж/см2. Через 2 нед на месте лечения остался удовлетворительный в косметическом отношении мягкий нормотрофический рубец, не спаянный с окружающими тканями. При цитологическом исследовании опухолевые клетки не обнаружены. При наблюдении в течение 1 года – без рецидива
ФОТОДИНАМИЧЕСКАЯ ТЕРАПИЯ ОСТАТОЧНОЙ ОПУХОЛИ ГОРТАНИ ПОСЛЕ ДЛТ (КЛИНИЧЕСКОЕ НАБЛЮДЕНИЕ)
A case of successful photodynamic therapy of residual laryngeal tumor after external beam radiotherapy (EBRT) is reported. The female patient with diagnosis of laryngeal cancer T1N0M0 underwent a course of EBRT on larynx, fraction dose = 2 Gy, 5 fractions per week, total radiation dose = 60 Gy. One month after EBRT the continued growth of residual tumor in the left vocal fold was diagnosed. The patient had 4 courses of photodynamic therapy with photosensitizer fotoditazin at a dose of 100 mg combined with electroresection of residual laryngeal tumor. The control video-assisted laryngoscopy 1 month after the 4th course of photodynamic therapy showed that larynx was flexible, vocal function was fully restored, tumor of the left vocal fold wasn’t detected. Non-recurrence follow-up period is 10 month. The patient is under follow-up.Приведено клиническое наблюдение успешной фотодинамической терапии остаточной опухоли гортани после ДЛТ. Пациентке с диагнозом рак гортани, T1N0M0, было проведен курс ДЛТ на область гортани, РОД = 2 Гр, 5 фракций в неделю, СОД = 60 Гр. Через 1 мес после завершения курса ДЛТ диагностирован продолженный рост остаточной опухоли левой голосовой складки. Пациентке проведено 4 курса фотодинамической терапии с фотосенсибилизатором фотодитазин в дозе 100 мг в сочетании с электрорезекцией остаточной опухоли гортани. При контрольной видеоларингоскопии через 1 мес после 4-го курса фотодинамической терапии гортань подвижна, голосовая функция полностью восстановлена, опухоль левой голосовой складки не определяется. Безрецидивный период наблюдения составляет 10 мес. Пациентка находится под динамическим наблюдением
ИССЛЕДОВАНИЕ ФОТОСЕНСИБИЛИЗАТОРА ДЛЯ АНТИБАКТЕРИАЛЬНОЙ ФОТОДИНАМИЧЕСКОЙ ТЕРАПИИ НА ОСНОВЕ ЦИКЛОДЕКСТРИНОВОЙ КОМПОЗИЦИИ МЕТИЛОВОГО ЭФИРА 133-N-(N-МЕТИЛНИКОТИНИЛ) БАКТЕРИОПУРПУРИНИМИДА
ФОТОДИНАМИЧЕСКАЯ ТЕРАПИЯ БАЗАЛЬНОКЛЕТОЧНОГО РАКА КОЖИ С ФОТОСЕНСИБИЛИЗАТОРОМ ФОТОЛОН
The short-term and long-term outcomes of treatment in 130 patients with basal cell carcinoma (T1N0M0, I stage) using photodynamic therapy with photosensitizer photolon based on chlorine e6 are represented in the article. The session of photodynamic therapy was performed 2.5-3 h after intravenous injection of photolon at dose of 2-2.5 mg/kg using semiconductor laser (λ=660±5 nm, laser power density — from 0.1 to 0.52 W/cm2, light dose — from 50 to 300 J/cm2). Complete regression of primary and recurrent carcinomas was observed in 90.9% and 88.9% of patients, respectively. For follow-up period of 3 to 76 months the local recurrence of the tumor was in detected in 6.9% of cases. Patients, who followed the light regimen for 2–3 days after photolon administration avoiding direct sun light exposure, had no manifestation of phototoxicity. Ten patients who failed to follow the light regimen had mild hyperemia, itching and burning in the exposed skin area selflimiting in several hours. Cosmetic results of photodynamic therapy with photolon are superior to those for traditional treatment methods for this disease
