    Abstract. The increasing cases of Dengue Hemorrhagic Fever (DHF) in various places leads to increased efforts to control the Aedes aegypti mosquito populations using anti-mosquito insecticide. The use of insecticides continuously, the absence of insecticide rotation and errors in the application has been lead toinsecticide resistance of dengue vector. The purpose of this study is (1) to identify active ingredients of insecticide; (2) used of anti-mosquito insecticide that has been used by households and programs, (3) as well as knowing the susceptibility status of Aedes aegypti to organophosphate insecticides (Malathion 0,8% and Temephos 0.02 ppm). Descriptive research with cross sectional approach conducted in three highest endemic cities in Banten province: Cilegon City, Serang City, and South Tangerang City. Identification of antimosquito insecticide has been done by interview, and identifying health centers and Health Service reports. The results showed that most respondents have been using anti-mosquito insecticide applied daily at night. Respondents prefer to use repellent which can be applied by swab. Based on the active ingredient, D-alethrin is a type of active ingredients which mentioned most often, followed by Pralethrin and Diethyltoluamide (DEET). Insecticides used by the program are Malathion and Pirimiphos-methyl, rotated by Cypermethrin. Susceptibility test results showed that Aedes aegypti is not susceptible to Malathion 0,8 % and Temephos 0.02 ppm.Keywords: insecticide, anti-mosquitoes, Aedes aegypti, susceptibility, organophosphate.Abstrak. Meningkatnya kasus Demam Berdarah Dengue (DBD) di berbagai tempat memacu peningkatan upaya pengendalian populasi nyamuk Aedes aegypti menggunakan insektisida antinyamuk. Penggunaan insektisida secara terus menerus tanpa adanya rotasi dan kesalahan dalam aplikasi akan menyebabkan resistensi terhadap nyamuk vektor DBD. Tujuan dari penelitian ini adalah (1) mengidentifikasi jenis-jenis bahan aktif (2) pola penggunaaan insektisida anti nyamuk yang digunakan oleh rumah tangga dan program serta (3) mengetahui status kerentanan nyamuk Ae. aegypti terhadap insektisida organofosfat (Malation 0,8% dan Temefos 0,02 ppm). Penelitian ini merupakan penelitian deskriptif dengan pendekatan potong lintang yang dilakukan di 3 kota endemis tertinggi di Provinsi Banten yaitu Kota Cilegon, Kota Serang, dan Kota Tangerang Selatan. Identifikasi insektisida antinyamuk telah dilakukan dengan cara wawancara terhadap masyarakat, Puskesmas maupun Dinas Kesehatan. Uji kerentanan terhadap nyamuk Ae. aegypti dewasa dan pra dewasa telah dilakukan untuk mengetahui status kerentanan vektor DBD. Hasil penelitian menunjukkan bahwa sebagian besar masyarakat menggunakan insektisida antinyamuk yang diaplikasikan setiap hari pada waktu malam hari. Masyarakat lebih banyak menggunakan repellent yang diaplikasikan dengan cara diusapkan atau dioleskan. Berdasarkan bahan aktifnya D-aletrin merupakan jenis bahan aktif yang paling sering di paparkan, disusul Praletrin dan Diethyltoluamide (DEET). Insektisida yang digunakan program adalah Malation dan Metil pirimifos yang dirotasi dengan penggunaan Sipermetrin. Hasil uji kerentanan nyamuk Ae. aegypti terhadap Malation 0,8 % dan Temefos 0.02 ppm menunjukkan bahwa nyamuk tersebut sudah resisten terhadap kedua jenis bahan aktif tersebut.Kata Kunci: insektisida, antinyamuk, Aedes aegypti, kerentanan, organofosfat

    Pengembangan Model Surveilans Aktif Demam Berdarah Dengue Melalui Metode Pelaporan Kewaspadaan Dini Rumah Sakit (KDRS) di Kota Tasikmalaya

    Abstract. Dengue hemorrhagic fever (DHF) is still a health problem that difficult to handle in Indonesia including in Tasikmalaya City as one of DHF endemic city in West Java Province. However, the case reports from Hospital are usually delayed and incomplete.  This research aims is to develop an active DHF surveillance model to increase the quality of Hospital’s early warning reports in Tasikmalaya City. This is a quantitative research using quasi experiment design and applies a design of pretest-intervention-posttest in a specific group. Qualitative approach is added to gain deeper information. Respondents of this research are 11 persons consist of 8 surveillance officers from 7 hospitals in Tasikmalaya City and 3 DHF program officers from Tasikmalaya City Heatlh Office. Interview and observation are conducted to measure knowledge and attitude of support facility’s officer also to measure the quality of hospital’s DHF early warning report.  Intervention done as Workshop and Brief Training to determine which report model is most suitable to apply.  Post-intervention monitoring was conducted in 3 months and all the research aspects are re-measured. The result shows that there are enhancements in all of the research aspects. The respondent’s knowledge level is increase from “less category” in pre-intervention to “enough-category” in post-intervention, attitude level is also increase from “enough category” to “well category”.The support facility are increasing as well from “less category” to “enough category” and the application of report system model is able to increase the quality of hospital’s early warning report from “less category” to “enough category”. The application of the Active Surveillance Model which developed by this research is able to increase the quality of Hospital’s early warning report, therefore if the model can be applied continuously, it expected to support more effective and efficient DHF handling effort in community. Demam Berdarah Dengue masih menjadi masalah kesehatan yang sulit ditangani di Indonesia, tidak terkecuali di Provinsi Jawa Barat terutama di Kota Tasikmalaya sebagai salah satu daerah endemis. Pelaporan kasus di rumah sakit seringkali terlambat dan kurang lengkap. Penelitian ini bertujuan untuk mengembangkan model surveilans aktif DBD untuk meningkatkan kualitas pelaporan Kewaspadaan Dini Rumah Sakit di Kota Tasikmalaya. Penelitian ini merupakan penelitian kuantitatif dengan design eksperimen semu dan rancangan pretest, intervensi dan postest dalam satu kelompok. Pendekatan kualitatif ditambahkan untuk menggali informasi secara lebih mendalam. Responden berjumlah 11 orang yaitu 8 orang petugas surveilans dari 7 RSU di Kota Tasikmalaya dan 3 orang pengelola program DBD Dinas Kesehatan Kota Tasikmalaya. Wawancara dan observasi dilakukan untuk mengukur pengetahuan dan sikap petugas, sarana penunjang serta kualitas laporan KDRS DBD. Intervensi berupa Lokakarya dan Pelatihan Singkat untuk menentukan model pelaporan yang paling tepat untuk diterapkan. Monitoring pasca intervensi dilakukan selama 3 bulan dan diukur kembali seluruh aspek yang diteliti. Hasilnya terjadi peningkatan pada seluruh aspek yang diukur. Pengetahuan responden meningkat dari “kategori kurang” saat pra intervensi menjadi “kategori baik” saat post intervensi dan aspek sikap dari “kategori cukup” menjadi “baik”. Sarana penunjang mengalami peningkatan dari “kategori kurang” menjadi “cukup”, dan model yang telah diterapkan mampu meningkatkan kualitas KDRS dari “kategori kurang” menjadi “kategori cukup”. Penerapan Model Surveilans Aktif yang dikembangkan dalam penelitian ini telah mampu meningkatkan Kualitas Laporan KDRS sehingga apabila model ini dapat digunakan secara berkesinambungan, diharapkan dapat mendukung upaya penanganan penyakit DBD di masyarakat secara lebih efektif dan efisien.     Tikus merupakan hewan yang mudah beradaptasi dengan lingkungan hidup manusia dan sering bersifat parasit. Kepadatan tikus di area permukiman di Kabupaten Banyumas cukup tinggi (14,14% dan 17,2%). Pemeriksaan agen penyakit zoonosis yang berpeluang ditularkan melalui tikus seperti cacing perlu dilakukan. Tujuan penelitian untuk mengidentifikasi telur cacing zoonosis dalam organ sekum tikus pada area pemukiman di Kabupaten Banyumas. Lokasi penelitian di Desa Beji Kecamatan Kedung Banteng dan Desa Kedung PringKecamatan Kemranjen, Kabupaten Banyumas. Jenis penelitian deskriptif dengan desain cross sectional. Sampel berupa sekum tikus dari penangkapan tikus yang diperiksa dengan metode pengapungan.Telur cacing yang ditemukan pada sekum tikus di areal permukiman Kabupaten Banyumas adalah Toxocara spp. (2,44%), Capillaria hepatica (2,44%), Echinostoma spp. (2,44%) dan Ancylostoma spp. (12,19%) di Desa Beji serta Echinostoma spp. (17,39 %) dan Ancylostoma spp. (13,04%) di Desa Kedung Pring. Infeksi cacing tersebut perlu diwaspadai mengingat kehidupan tikus dekat dengan manusia sehingga mudah menginfeksi manusia

    Kecacingan masih merupakan masalah di Indonesia khususnya pada anak usia balita dan sekolah dasar (SD) meskipun tidak menyebabkan kematian, kecacingan mengakibatkan penurunan kondisi gizi, anemia, gangguan saluran pencernaan, penurunan kecerdasan hingga penurunan kualitas sumber daya manusia (SDM). Tulisan ini membahas tentang perhitungan kehilangan nutrisi dan darah serta kerugian finansial akibat kecacingan. Penelitian observasional dengan desain cross sectional. Populasi dan sampel adalah 98 anak sekolah dasar kelas 1-6 yang bersekolah di SDN Manurung 1 Pagatan pada bulan Oktober 2014 (total sampling). Pemeriksaan sampel feses dilakukan secara langsung dan Kato Katz di laboratorium parasitologi Balai Litbang P2B2 Tanah Bumbu. Hasil ditemukan 31 sampel positif (31,63%) terdiri atas 22 Trichuris trichiura (22,45%), 1 Ascaris lumbricoides (1,02%), 4 mix A. lumbricoides dan T. trichiura (4,08%), 1 mix T. trichiura dan Hookworm(1,02%), 1 mix T. trichiura dan Hymenolepis sp (1,02%) dan 2 Enterobius vermicularis (2,04%). A. lumbricoides mengambil karbohidrat 0,14 gram/ekor/hari dan protein 0,035 gram/ekor/hari. T. trichiura menghisap darah 0,005 cc/ekor/hari dan Hookworm menghisap darah 0,2 cc/ekor/hari. Perhitungan rupiah, karbohidrat diasumsikan seharga beras Rp. 10.000/kg, protein seharga daging ayam Rp. 20.000/kg dan darah seharga Rp. 300.000/pak. Hasil perhitungan didapatkan kehilangan karbohidrat sebanyak 15,96 gram/hari senilai Rp. 159.6,- dan 5.745,6 gram/tahun senilai Rp. 57.456,-. Kehilangan protein sebanyak 3,99 gram/hari senilai Rp. 79,8,- dan 1.436,4 gram/tahun senilai Rp.28.728,-. Kehilangan darah sebanyak 11,54 cc/hari atau senilai Rp. 13.848,- dan 4.154 cc/tahun senilai Rp. 4.985.280,-.Total kerugian biaya akibat kecacingan adalah Rp. 14.087,-/hari, dan Rp. 5.071.464,-/tahun

    Penggunaan insektisida di masyarakat mengalami peningkatan yang signifikan dalam beberapa tahun terakhir. Peningkatan ini tidak diimbangi oleh pengetahuan masyarakat dan kebijakan pemerintah untuk mengatur penggunaannya secara baik dan bijak, yang akhirnya muncul permasalahan baru seperti meningkatnya residu insektisida di lingkungan, keracunan, dan risiko serangga resisten. Penelitian ini bertujuan memberikan gambaran penggunaan insektisida dalam rumah tangga meliputi bahan aktif yang digunakan, dan lamanya pemakaian insektisida. Penelitian dilakukan di wilayah endemis DBD di Kab. Bone, Kota Palopo dan Kota Makassar Provinsi Sulawesi Selatan pada Bulan Mei-Juni 2015. Penelitian dilakukan dengan desain potong lintang dan metode sistematik sampling. Data dianalisis secara deskriptif dan disajikan dengan frekuensi berdasarkan bahan aktif dan lamanya penggunaan insektisida. Hasil penelitian menunjukan penggunaan bahan aktif insektisida di ketiga kabupaten terbesar pada bahan aktif transflutrin, praletrin, d-aletrin, dan imiprotrin baik dalam formula tunggal maupun formula campuran diantaranya. Selain insektisida, responden dari Kota Makassar juga banyak menggunakan bahan aktif repelen Diethyl Toluamide (DEET) berupa krim oles (lotion). Lamanya waktu penggunaan, sebagian besar responden dari ketiga kab/kota menggunakan satu produk insektisida selama 2-5 tahun, bahkan ada sebagian lebih dari 5 tahun. Hasil penelitian ini jelas menunjukkan penggunaan insektisida berbahan aktif Synthetic Pyrethroid (SP) telah luas digunakan dalamjangka waktu yang cukup lama.     Filariasis Preventive Mass Drug Administration (MDA) program supposed to covered at least 65% of the target. According to the Indonesia’s program annual reports, the coverage from year of 2005-2009 are 28%-29%. Those coverage are still far below the expected coverage. Bandung Regency is one of 11 filariasis endemic areas in West Java that treatment coverage for four years (2009-2012) in a row is 70%, 62%, 64% and 68%. This study was an observational study with cross sectional study design studies. The study was conducted at the health center Cikaro Regency Bandung in 2013 to determine the variable leverage increase treatment coverage. The data collected in this study include primary data and secondary data. The primary data obtained through interviews of 200 respondents to the questionnaire and also through Focus Group Discussion (FGD). Secondary data include population and filariasis treatment coverage of data obtained from the agency terkait.Variabel levers determined through the analysis of dynamic system modeling software powersim. The results showed that the variable lever to increase the coverage of treatment is to reduce the negative impact of drug side effects, increasing the number of cadres and knowledge as well as increased monitoring activities of treatment.     HUBUNGAN KONSUMSI GOITROGENIK SIANIDA DENGAN KADAR TIOSIANAT URIN DI DAERAH ENDEMIK GAKI KABUPATEN JEMBER   Association of Goitrogenic Potential Vegetables Contained Cyanide Consumption and Urinary Thiocyanate Level in Jember District IDD Endemic Area ABSTRACT Background. Iodine Deficiency Disorder (IDD) is still a problem in Jember district. Goitrogenic potential food cyanide contained consumption pattern at least 3-5 times per week would be a risk factor of IDD. Cyanide, thiocyanate precursors contained in many commonly vegetables which consumed by society. Thiocyanate distrupt thyroid hormone hormogenesis through two pathways, inhibiting the active transport and interfere with the activity of thyroid peroxidase. Objective. This study aimed to determine the association between goitrogenic potential food cyanide contained consumption and urinary thiocyanate levels. Method. A cross-sectional observational study was conducted in the District Arjasa in March - May 2013. The data collected was goitrogenic potential food cyanide contained consumption and urinary thiocyanate levels from 203 housewives. The research instrument was a semi-quantitative food frequency form. Measurements of thiocyanate urine levels carried out in IDD laboratory, medical faculty, Diponegoro University at Semarang. Results. The results showed an average consumption of goitrogenic potential food cyanide contained in its raw state of 818.42 g/day, and after the cooking process 18.30 mg/day, while the average level of urine thiocyanate was 1.2 mg/dl. There were no correlation between cyanide consumption in a raw state and after cooking with urinary thiocyanate levels. Conclusion. There were no correlation between consumption of goitrogenic potential food cyanide contained in a raw state and after cooking to urinary thiocyanate levels. Keywords: cyanide, food consumption, urinary thiocyanate level.   ABSTRAK Latar belakang. Kabupaten Jember masih menghadapi masalah Gangguan Akibat Kekurangan Iodium (GAKI). Pola konsumsi pangan sumber goitrogenik minimal 3-5 kali per minggu menjadi faktor risiko GAKI. Sianida, prekursor tiosianat banyak terkandung dalam sayuran yang biasa dikonsumsi masyarakat. Tiosianat mengganggu hormogenesis hormon tiroid melalui 2 jalur, menghambat transpor aktif dan mengganggu aktivitas thyroid peroxidase. Tujuan. Penelitian ini bertujuan untuk mengetahui hubungan konsumsi pangan sumber goitrogenik sianida dengan kadar tiosianat urin. Metode. Penelitian ini adalah penelitian observasional menggunakan disain cross sectional yang dilakukan di Kecamatan Arjasa pada bulan Maret-Mei 2013. Data berupa tingkat konsumsi bahan pangan sumber goitrogenik sianida dan kadar tiosianat urin dari urin sewaktu. Responden adalah 203 orang ibu rumah tangga. Instrumen penelitian adalah formulir semi quantitative food frequency dan urin sewaktu. Pengukuran kadar tiosianat dilakukan di Laboratorium GAKI FK Universitas Diponegoro Semarang. Hasil. Hasil penelitian menunjukkan rata-rata 102 konsumsi bahan pangan sumber goitrogenik sianida dalam keadaan mentah sebesar 818.42 gram/haridengan konsumsi sianida setelah proses pengolahan sebesar 18.30 mg/hari, sedangkan rata-rata kadar tiosianat urin responden adalah 1.2 μg/dl. Uji spearman menunjukkan tidak ada hubungan konsumsi bahan pangan sumber sianida dalam keadaan mentah dan sianida setelah proses pengolahan dengan kadar tiosianat urin. Kesimpulan.Tidak ada hubungan antara konsumsi bahan pangan sumber goitrogenik sianida dalam keadaan mentah dan sianida setelah pengolahan dengan kadar tiosianat urin.   Kata kunci: sianida, tingkat konsumsi, tiosianat urin

    The Use Capitation Funds in the First Level Health Facility (FKTP) the Implementation JKN

    Background: The fi rst level of health facilities (FKTP) as gatekeeper must function properly. A capitation system payment, demanded FKTP not only give treatment but also provide promotive and preventive services. This study aimed to analyze the use of capitation funds by FKTP in the implementation of National Health Insurance (JKN). Methods: The study was conducted in two provinces with the very high Fiscal Capacity Index (FCI) which is East Kalimantan and low FCI which is Central Java. From each province selected 2 districts and 1 municipality. From each district/municipality chosen two primary health centers, two primary health care clinics, two phycisian and two dentists. Collecting data through interviews, secondary data and questionaires. The data were analyzed discriptively and logical thinking through expert judgmant. Results: The results showed that the capitation grant biggest acquisition in a row is primary health centers, primary healh clinics and physician as well as the dentist. A large part of the funds in primary health center capitation is topay for services and remainder of operational (drugs). Primary health clinic, doctor and dentists are relatively similar to the use of funds. Revenue compared to the use of capitation funds for health centers are relatively suffi cient, but for primary health care clinics, doctors and dentist are still lacking, especially for the purchase of drugs and laboratory. Conclusion: The biggest acquisition of the capitation grant is a primary health center, while the smallest is a dentist. Most of capitation funds paid for the service, but operating mostly for drugs. The amount of capitation for primary health care is suffi cient, but not for primary health clinics, physician and dentists. Recommendation: BPJS which organizes JKN should expand cooperation with primary health clinics, physician and dentists in order to increase participants JKN

    KERAGAMAN INSTRASPESIFIK AKSESI EKINASE (Echinacea purpurea (L.) Moench) HASIL SELEKSI MASSA TAHAP I BERDASARKAN ANALISIS ISSR

    Ekinase (Echinacea purpurea (L.) Moench) is one of a medicinal plant that has immunostimulatory activity. This plant has been cultivating in Tawangmangu region by Medicinal Plant and Traditional Medicine Research and Development Office since 2002. Ten accessions of E. purpurea were found based on their morphological variation, three were selected as promising accessions namely BH2, BHU3 dan BHU5. The objective of this research was to observe the intraspecific variation of  those  accessions and 8 variants accession from mass selection year I using ISSR analysis. Those accessions were amplified using 10 ISSR primers. A total of 108 scorable fragments were generated from 10 ISSR primers, among which 88 fragments (81,5 %) were polymorphic. The Dice coefficient was used to calculated the genetic similarity and UPGMA was used to generate the dendogram. The genetic similarity index among accessions ranged from 68,96-81,25% thus indicating that low level of genetic diversity. ISSR analysis proved to be efficient for intraspecific diversity of ekinase accessions from mass selection year I.AbstrakEkinase (Echinacea purpurea (L.) Moench) merupakan salah satu tumbuhan obat yang mempunyai aktivitas sebagai imunostimulan. Tanaman ini telah ditanam dan dibudidayakan di Tawangmangu oleh Balai Besar Penelitian dan Pengembangan Tanaman Obat dan Obat Tradisional sejak tahun 2002. Sebanyak sepuluh aksesi ekinase telah dikarakterisasi berdasarkan keragaman morfologisnya, tiga aksesi diantaranya terpilih sebagai aksesi untuk pengembangan lebih lanjut yaitu BH2, BHU3 dan BHU5. Tujuan dari penelitian ini adalah mengetahui keragaman intraspesifik 3 aksesi terpilih ekinase dan variannya hasil seleksi massa tahap I berdasarkan ISSR. Amplifikasi aksesi ekinase mnggunakan 10 primer ISSR. Total 108 fragmen dihasilkan dari amplifikasi menggunakan 10 primer, 88 fragmen (81,5%) diantaranya merupakan fragmen polimorfik. Perhitungan indeks similaritas menggunakan koefisien Dice dan konstruksi dendrogram berdasarkan UPGMA. Keragaman intraspesifik antar aksesi ekinase sebesar 68,96-81,25%, nilai tersebut mengindikasikan keragaman yang rendah. Analisis ISSR dapat digunakan sebagai penanda untuk mengetahui keragaman intraspesifik aksesi ekinase hasil seleksi massa tahap I

    Torch ginger (Etlingera elatior (Jack) R.M.Sm.) has long been known by Indonesian people as an ornamental, vegetable, and traditional medicine. Rhizomes, stems, leaves, fruits, flowers and essential oils are part that is often used in daily life. The purpose of this review is to present a comprehensive information of traditionally using, chemical constituents, biological and pharmacological research as the scientific evidence of the torch ginger efficacy for further development of traditional medicine. Traditionally used of torch ginger indicates that it is widely used as a flavor enhancer in food, food preservatives, wounds and earaches medicine and body odor remover. Torch ginger is rich of phenols, polyphenols, flavonoids and terpenoids. The other research indicate that torch ginger has a wide range of pharmacological activity as an antioxidant, antibacterial, anticancer, larviciding and repellent.AbstrakKecombrang (Etlingera elatior (Jack) R.M.Sm.) telah dikenal sejak lama oleh masyarakat Indonesia sebagai tanaman hias, sayur, dan obat tradisional. Rimpang, batang, daun, buah, bunga dan minyak atsiri merupakan bagian yang sering digunakan sehari-hari. Tujuan dari tinjauan ini adalah untuk menyajikan informasi yang komprehensif mengenai penggunaan secara tradisional, konstituen kimia, penelitian biologi dan farmakologi sebagai bukti ilmiah khasiat kecombrang guna pengembangan lebih lanjut dalam aspek pengobatan tradisional. Penggunaan secara tradisional menunjukkan kecombrang banyak digunakan sebagai penambah rasa pada masakan, pengawet makanan, mengobati luka dan sakit telinga serta sebagai penghilang bau badan.  Kecombrang  kaya akan golongan fenol, polifenol, flavonoid, dan terpenoid.
