    Extracorporeal vs. intracorporeal urinary diversion at robot-assisted radical cystectomy: adverse in-hospital outcomes

    The advantage of intracorporeal (ICUD) over extracorporeal urinary diversion (ECUD) at robot-assisted radical cystectomy (RARC) regarding adverse in-hospital outcomes is still under debate. We addressed these knowledge gap in the National Inpatient Sample (2016–2019). RARC patients receiving either ileal conduit or neobladder were stratified according to UD technique (ECUD vs. ICUD). Propensity score matching (PSM), univariable and multivariable logistic and Poisson regression models were used to test differences in adverse in-hospital outcomes rates. Subgroups analyses focused on specific population: female patients, patients receiving neobladder, patients treated in low volume hospitals. Of 1747 RARC patients, 462 (26.4%) underwent ECUD vs. 1285 (73.6%) underwent ICUD. ECUD patients were more frequently treated in low volume hospitals (72.3 vs. 66.2%, p = 0.02) and more frequently received neobladder than their ICUD counterparts (10.2 vs. 4.7%, p < 0.001). The proportions of ECUD and ICUD significantly differed within United States regions (p = 0.001). After one-to-one PSM between ECUD and ICUD patient, 444 (96.1%) ECUD patients and 444 (34.6%) ICUD patients were compared. After separate univariable and multivariable logistic and Poisson regression models, ECUD was not an independent predictor of adverse in-hospital outcome in 13 examined-categories. Similarly, no differences in adverse in-hospital outcomes were identified between ECUD vs. ICUD within the three examined subgroups. The current data indicates no differences between ECUD and ICUD when adverse in-hospital outcomes represent the endpoint of interest. In consequence, surgical expertise with one or the other technique within individual Institutions might represent the most relevant factor to guide choice between ECUD and ICUD

    HER2-expressing cancers currently benefit from targeted therapies, including monoclonal antibodies and antibody-drug conjugates that specifically bind to the extracellular domain of the receptor. Peptides targeting HER2 represent promising candidates for the development of alternative molecular drugs. In this study, we report a dimeric version of the previously validated A9 peptide as a ligand specifically targeting HER2. The novel A9-PEG-A9 conjugate consists of two A9 peptides whose N-terminal amino groups are linked via a polyethylene glycol chain. It was synthesized using a solvent-free protocol and validated as an improved ligand, demonstrating enhanced water solubility and increased affinity for the model receptor HER2-DIVMP, as determined by the fluorescence spectroscopy titration method

    Questo articolo riporta i risultati di un'indagine qualitativa condotta su 30 studenti delle scuole superiori di Vercelli, in Italia, sulle percezioni degli adolescenti riguardo alle biblioteche e alla loro comunicazione digitale. I focus group hanno trattato argomenti quali il comportamento online, la frequenza delle biblioteche, le pratiche di marketing digitale e la promozione della lettura. Lo scopo della ricerca è indagare le ragioni della scarsa frequentazione delle biblioteche da parte degli adolescenti e della cattiva reputazione di cui soffrono, e scoprire linguaggi, abitudini ed esigenze non intercettati dalle attuali strategie di marketing delle biblioteche. I dati rivelano un maggiore bisogno di coinvolgimento ed empatia, un deficit nelle strategie di comunicazione digitale e pregiudizi più o meno giustificati nei confronti dell'istituzione

    Measurements of differential two-particle number and transverse momentum correlation functions in pp collisions at sqrt(s) = 13 TeV

    Differential two-particle normalized cumulants (R2) and transverse momentum correlations (P2) are measured as a function of the relative pseudorapidity and azimuthal angle difference (Δη,Δφ) of charged particle pairs in minimum bias pp collisions at s = 13 TeV. The measurements use charged hadrons in the pseudorapidity region of |η|<0.8 and the transverse momentum range 0.2 < 2.0 GeV/c in order to focus on soft multiparticle interactions and to complement prior measurements of these correlation functions in p–Pb and Pb–Pb collisions. The correlation functions are reported for both unlike-sign and like-sign pairs and their charge-independent and charge-dependent combinations. Both the R2 and P2 measured in pp collisions exhibit features qualitatively similar to those observed in p–Pb and Pb–Pb collisions. The Δη and Δφ root mean square widths of the near-side peak of the correlation functions are evaluated and compared with those observed in p–Pb and Pb–Pb collisions and show smooth evolution with the multiplicity of charged particles produced in the collision. The comparison of the measured correlation functions with predictions from PYTHIA8 shows that this model qualitatively captures their basic structure and characteristics but feature important differences. In addition, the R2CD is used to determine the charge balance function of hadrons produced within the detector acceptance of the measurements. The integral of the balance function is found to be compatible with those reported by a previous measurement in Pb–Pb collisions

    The ecology of the microbiota in children with obesity is associated with the intake of carbohydrates and insulin resistance

    Background & aims: Among diet, microbiota, and obesity exists a close correlation that remains insufficiently explored, particularly within the pediatric age. We aimed to deeply investigate the relationship between dietary composition and microbiota in pediatric subjects with obesity before an educational training in a Mediterranean-style diet. Methods: 55 subjects (10–18 years) with overweight or obesity and visceral adiposity, diet naïve, or failure to a previous weight loss program were phenotypically described through clinical and metabolic parameters, including circulating LPS levels. 16S DNA sequencing was used to profile the microbiota. Lifestyle habits (KIDMED score, 24-h recall, International Physical Activity Questionnaire for Adolescents) and diet composition were assessed. Results: Five different main enterotypes were found. Females at the end of puberty were well separated from the other subjects in the principal component analysis. Microbiota composition changed with the progression of puberty, and LPS levels negatively correlated with Tanner stages. Gut microbiota composition, considering all the clinical parameters, was significantly modulated mostly by the percentage of carbohydrate intake and insulin resistance. Carbohydrate intake ranging from 45 to 55 % determined a high number of species, but with a reduction of biodiversity (Shannon and Simpson indexes). Bacteroides dorei, Turicibacter sanguinis, and B. vulgatus were among the main species involved in signatures of carbohydrate intake, whereas an enrichment in Bacteroides stercoris was found in subjects with a low fiber intake. Adherence to the Mediterranean diet was associated with a great presence of dietary fiber metabolizers, including Faecalibacterium prausnitzii and Lachnospira sp. Alistipes finegoldii and Akkermansia muciniphila were more represented in low insulin resistance, while Bifidobacterium pseudocatenulatum and Clostridium clostridioforme were enriched in patients with high insulin resistance. Conclusion: Carbohydrate content of the diet, insulin resistance, and microbiota are strictly related in children with obesity. A certain degree of dysbiosis could link obesity to a severe phenotype.     Variation of mandibular radiomorphometric indices in metastatic bone cancer patients undergoing antiresorptive treatment: a prospective study

    Introduction The aim of this study was to identify and evaluate the changes of oral health and panoramic radiograph indexes in cancer patients that have been administered antiresorptive drugs. Materials and methods Subjects who had a diagnosis of cancer with bone metastasis that were about to start antiresorptive therapy with denosumab or zoledronic acid were included. A preliminary dental clinical assessment, together with a panoramic radiograph, was performed at T0 to prevent the onset of Medication related osteonecrosis of the jaw (MRONJ) and T1 (1 year follow up). In order to calculate at both T0 and T1 the mandibular cortical width (MCW), the panoramic mandibular index (PMI), and the mandibular cortical index (MCI), panoramic radiographs were analyzed by a single operator. Results Thirty-three subjects with a mean age of 59.9 years were included in the study. A significant reduction in the PMI value from T0 to T1 (p = 0.00236) and a significant variation from T0 to T1 for MCI (p < 0.05) were observed. Conclusions Radiomorphometric indices could represent a promising field of research that can take advantage from the easy detectability in dental panoramic radiographs. Further research is needed to explore the possible role of PMI and MCI in bone cancer patients
