The Efficacy and Patient Satisfaction with a Mini-Scleral Lens after penetrating Keratoplasty
Background: To analyse the efficacy and patient satisfaction of fitting a mini-scleral lens (SL) after keratoplasty surgery in patients unsatisfied with their visual outcomes.
Material and Methods: A retrospective chart review of 22 consecutive patients was performed between 2018 and 2019. Demographic data, indications for keratoplasty, ocular parameters, and visual results were evaluated. In addition, subjective visual quality (SVQ) and comfort, complications, daily wear time, and the reason for discontinuing SL wear during the 6-month follow-up were analysed.
Results: Twenty-two eyes of 22 patients (6 females and 16 males; mean age 48.82 ± 17.19 years) were assessed. Keratoconus was the main indication for keratoplasty (9 eyes, 40.9%), followed by corneal opacities (5 eyes), and Fuchs endothelial dystrophy (4 eyes). High refractive anisometropia was found in 14 eyes (63.6%) and 6 eyes presented significant irregular astigmatism. LogMAR visual acuity improved significantly with SL when compared with the best spectacle-correction (mean, 0.12 ± 0.12 vs 0.52 ± 0.33, respectively; p < 0.01). Six patients discontinued SL wear (27.3%). Among the other 16 patients, the mean daily wear time was 11.47 ± 1.52 h; of these, 14 (87.5%) were successfully fitted and enjoyed favourable SVQ and comfort. No significant complications were found during the follow-up period.
Conclusions: Mini-scleral lenses may be a safe and effective alternative for treating complicated eyes after keratoplasty; they may provide optimal visual rehabilitation and/or restore binocular vision with significant efficacy and patient satisfaction.Medicin
Evaluation of the effect of etamsylate on thromboelastographic traces of canine blood with and without the addition of heparin
The objective of this study was to investigate the effect of etamsylate on canine blood and
heparinised canine blood from healthy dogs using thromboelastography (TEG). Citrated blood
was obtained from twenty healthy client-owned dogs, and 3 experiments were performed.
Experiment 1 compared TEG in blood versus blood with etamsylate (250 mM). Experiment 2
evaluated TEG in heparinised blood (1 U/mL) with and without the addition of etamsylate
(250 mM). Experiment 3 evaluated dose escalation of etamsylate (control, 250 μM, 500 μM and
1000 μM) in heparinised blood (1 U/mL). The addition of etamsylate to canine blood in
experiment 1 increased the percentage of clot lysis at 30 min (z = −2.103, p = .035) and 60 min
(z = −1.988, p = .047), suggesting that etamsylate could have a fibrinolytic effect. When
etamsylate was added to heparinised canine blood (1 U/mL), etamsylate produced a dosedependent
inhibition of the effect of heparin when higher concentrations of etamsylate were
used (500 μM and 1000 μM). The linear mixed effects model showed significant increases in α
angle and maximal amplitude when high dose etamsylate was added compared to the
control. In conclusion, etamsylate could be used at higher doses to inhibit the effect of heparin
in dogs when protamine might not be available. However, etamsylate might have a fibrinolytic
effect when used in healthy dogs.Ecuphar Pharmaceuticals donated vials containing etamsylate 125 mg/mL (Hemo®) used in this study.Veterinari
Although it may seem to be an encyclical for Catholics, since they are the first addressees, it is an encyclical to remind us Catholics what accents we should emphasise at this time and what tone of language we should use to communicate the Christian message to the people of our time, so that it may be perceived as something that is a source of life and joy by the people of today. For this reason, we must affirm that it is not an encyclical “only for Catholics”, but also an encyclical for non-Catholics, so that through our mediation, the Christian message may be accepted by them as a word of life.Religió
Efficacy and safety of arthroscopy in femoroacetabular impingement syndrome: a systematic review and meta-analysis of randomized clinical trials
This study aimed to compare the efficacy and safety of arthroscopy with physiotherapy or joint lavage in patients with femoroacetabular impingement (FAI). A meta-analysis using PubMed, Embase, Scopus, and the Cochrane Collaboration Library databases was performed in September 2022. We included studies focusing on patients with FAI who underwent arthroscopic surgery versus those who underwent physiotherapy or arthroscopic lavage. The outcomes were functional scores (iHOT-33 and HOS ADL) and adverse events. Randomized clinical trials were included in the study. The risk of bias in each study was assessed according to Cochrane guidelines for clinical trials. The data were combined using Review Manager version 5.4. (PROSPERO CRD42022375273). Six RCTs were included, from a pool of 839 patients (407 females). The iHOT-33 and HOS ADL scales showed significant differences at 12 months in favor of the arthroscopy group (MD, 10.65; 95% CI 6.54–4.76) and (MD, 8.09; 95% CI 3.11–13.07). MCID was not achieved through arthroscopy in functional variables. The rates of osteoarthritis (OR, 6.18; 95% CI 1.06–36.00) and numbness (OR, 73.73; 95% CI 10.00–43.92) were significantly higher in the arthroscopy group. Arthroscopic surgery showed statistical superiority over the control group without exceeding the MCID in most studies; however, the results might have been influenced by secondary variables. Finally, arthroscopic surgery results in a high rate of conversion to osteoarthritis.Medicin

Differences in playing style and technical performance according to the team ranking in the Spanish football LaLiga. A thirteen seasons study
This study aimed to explore the differences in playing style and technical performance
according to the ranking level in Spanish football teams. The sample comprised 38 professional teams that competed in LaLiga from 2008/09 to 2020/21, with a total of 4940
matches. The teams were grouped by their final ranking position: Champion (1st); Champions League (2nd–4th); Europa League (5th–6th); middle teams (7th–17th); and relegated
teams (18th–20th). Linear mixed models were used to examine the effects of the team ranking on variables related to playing style and technical performance. The F2 statistic was calculated as effect size (ES). Regarding the style of play, the Champion teams initiated
offensive sequences from a more advanced field position than the remaining ranking groups
with a descending effect as the ranking position decreased (p < 0.001; ES = medium). The
sequence duration and passes per sequence showed a decreasing effect across ranking
groups (both p < 0.001; ES = small). In contrast, the direct speed showed an increasing
effect as the ranking position decreased (p < 0.001; ES = small). A decreasing effect was
observed in the number of sequences ending in the final third as the ranking position
decreased (p < 0.001; ES = large) while no effect was found for the sequences ending in a
shot. There was a reduction effect in ball possession, passing accuracy, through balls and
crosses as the teams’ ranking decreased (all p < 0.001; ES = small-to-large). In summary,
higher-ranked teams had an advanced starting position, longer offensive sequences, slower
progression speed, more ball possession, higher passing accuracy, and more crosses and
through balls than lower-ranked teams. Football coaches should consider that adopting a
playing style focused on regaining the ball possession in advanced field zones and using
long passing sequences can be an effective tactical strategy to improve the teams’ ranking
during the season.

visual impairment and inclusion. Sometimes, there is difficulty in implementing inclusive
strategies in ordinary schools, despite being an obligation at the curricular level. To address this
problem, an intervention called Awareness Days about visual impairment is proposed. This
initiative provides, both teachers, students and families, several practical experiences and
theoretical knowledge to sensitize them to the needs of this students group.
In this sense, this FDP includes a theoretical framework with solid contributions from
experts and specialized agencies in the field of visual impairment. It also presents a detailed
methodology with the resources and research methods used and, finally, it includes the
conclusions drawn at the end of it. The latter address issues such as the results obtained and the
justifications offered in this regard, to analyse the fulfilment of the proposed objectives.Educació
Pandemic, poverty and education: A challenge for the sustainable development goals
COVID-19 has changed the socio-economic and educational landscape, increasing poverty rates
and generating psychosocial problems. It has also conditioned the achievement of the Sustainable
Development Goals (SDGs). Thus, this article pursues three-fold objectives: to describe the
situation generated worldwide by the increase in poverty due to COVID-19; to analyse the degree
of compliance with some post-pandemic SDGs and to establish educational measures to alleviate
the psycho-pedagogical consequences. Interpretative hermeneutics is applied as it makes it possible
to interpret and understand the socio-economic-educational reality and establish intervention
measures adapted to the educational context. Amongst the macro measures designed, we can
mention core actions such as establishing legal and motivational mechanisms to re-engage students
at risk of dropping out of school; generating policies of economic and cultural compensation for
possible educational inequalities; implementing school support programmes for families and students
in vulnerable situations; carrying out socio-emotional learning programmes to combat
anxiety, stress and self-esteem among students; establishing policies to raise awareness and appreciation
of the teaching profession and stimulate collaboration between family and school;
developing teacher training programmes in ICTs. At the micro level, the core actions applied are
target vulnerable groups of students to overcome learning losses; promote emotional education;
establish initiatives to reduce stigmatisation and discrimination; improve educational environments;
and ensure equal opportunities at school.
Chicken eggs (Gallus gallus) are considered a staple food, widely
consumed throughout the world, due to their nutritional content and the fact that there are no religious restrictions on their consumption. When buying eggs, the
Spanish population values factors such as freshness, size and price, followed by the production system. There is a trend, mainly in European countries, towards an increase in the rearing of hens in cage-free systems, although the vast majority
of eggs produced in the world are produced in cage systems, due to their productive efficiency and economic profitability.
Based on the hypothesis that hens housed in cages are capable of
achieving high levels of production, without these being negatively altered by the
housing system and management, this study was conducted to evaluate the
production, mortality rate, feed consumption, weight and external shell quality of
eggs produced by hens housed in three group sizes (20, 26 and 60 hens), all
three groups at the same stocking density.
Twenty-two flocks of Hy-Line hens (2,849,869 hens) housed in 22
enriched cage battery houses complying with the requirements of Directive 99/74/EC were studied. The data collection period covered from June 2018 to
November 2020. The hens were housed in batteries in three types of enriched
cages, with a capacity of 20 (S20), 26 (S26) and 60 hens (S60). Each hen had
more than 750 cm2 of floor space in all three cage types. The laying cycle was
divided into three periods: Initial from 19 to 26 weeks of age, Middle from 27 to
52 weeks of age and Final from 53 to 74 weeks of age. The variables studied
were the number of total eggs (HT), the number of dirty eggs (SUC), the number
of broken eggs (ROT), the number of cracked eggs (FIS), the number of eggs classified S, M, L and XL, as well as feed consumption (CONSUM), expressed in
grams per bird/day) and mortality (MORT).
.
Cluster size analysis showed no significant differences in the data
obtained for laying percentage, mortality, L grade eggs and percentage of dirty eggs. The percentage of XL eggs was higher in hens housed in the smallest group size (S20), while M eggs were more numerous in the S26 group. For the
shell integrity variables, the percentage of broken eggs was lower in S20, and for
cracked eggs the S60 group had the highest incidence. Feed consumption was highest in the group with the lowest number of hens, S20.
The production phase influenced all the variables studied. Laying
percentage was significantly higher during the Middle period, while mortality was higher in the Final period. Egg weight data showed a higher percentage of XL
eggs in the Final period, when a higher number of broken and cracked eggs were
counted. Feed consumption per hen was lower in the Initial period. The group of
20 hens had the best data for XL egg weight and the lowest number of cracked and broken eggs and is therefore proposed as the recommended group size to
optimise cage profitability.
