Testing for cattle allergy: modified diagnostic cutoff levels improve sensitivity in symptomatic claw trimmers
The diagnosis of cattle-related sensitization is complicated by the variability and complexity of cattle allergen extracts. To evaluate a modified diagnostic procedure leading to more accurate results especially in the early phase of sensitization. We tested 27 claw trimmers with and 65 without cattle-related symptoms using two commercially available cattle allergen extracts. We also used a self-prepared cattle allergen mix designed to represent the full spectrum of cattle allergens from a typical agricultural workplace. More than 50% of symptomatic claw trimmers showed negative test results with commercial extracts and a sensitization cutoff point of 0.35 kU/l. In contrast, with the self-prepared cattle allergen mix, positive results were observed for almost all of them. Evaluating the results of the commercial test kits at different cutoff levels, we found an ideal cutoff point to improve the sensitivity at 0.2 kU/l. Additional tests with self-made cattle hair extracts can help to bridge the diagnostic gap seen in patients showing cattle-related symptoms, but negative results in commercially available tests. For early-stage sensitization screening, we propose to lower the cutoff level indicating sensitization to 0.2 kU/l
Classification of urticaria
Classification is based on GA 2 LEN/EAACI/WAO/EDF guidelines (2009). These guidelines classify urticaria according to clinical manifestations. Urticaria is mediated by mast cells. According to level of mast cell degranulation clinical signs are superficial (Urticaria) or deep swelling (Angioedema)
IgE allergy diagnostics and other relevant tests in allergy, a World Allergy Organization position paper
Zuberbier, Torsten/0000-0002-1466-8875Currently, testing for immunoglobulin E (IgE) sensitization is the cornerstone of diagnostic evaluation in suspected allergic conditions. This review provides a thorough and updated critical appraisal of the most frequently used diagnostic tests, both in vivo and in vitro. It discusses skin tests, challenges, and serological and cellular in vitro tests, and provides an overview of indications, advantages and disadvantages of each in conditions such as respiratory, food, venom, drug, and occupational allergy. Skin prick testing remains the first line approach in most instances; the added value of serum specific IgE to whole allergen extracts or components, as well as the role of basophil activation tests, is evaluated. Unproven, non-validated, diagnostic tests are also discussed. Throughout the review, the reader must bear in mind the relevance of differentiating between sensitization and allergy; the latter entails not only allergic sensitization, but also clinically relevant symptoms triggered by the culprit allergen.WAO Board of DirectorsThe authors wish to thank the allergy societies from the membership of the World Allergy Organization who provided critical review of the draft and the WAO Board of Directors for supporting this work
Systematic review of monotherapy with biologicals for children and adults with IgE‐mediated food allergy
BACKGROUND: Biological therapies relieve symptoms in allergic inflammatory diseases so we systematically reviewed the evidence about whether biological monotherapy could benefit people with IgE‐mediated food allergy. METHODS: We searched six bibliographic databases from 1946 to 30 September 2021 for randomised and non‐randomised controlled trials about biological monotherapy in people with IgE‐mediated food allergy confirmed by oral food challenge. We used the Grading of Recommendations, Assessment, Development and Evaluation approach to narratively summarise findings from three trials with 118 participants. The studies were too heterogeneous and sparse to conduct meta‐analysis. RESULTS: We included one randomised trial about etokimab, one about omalizumab and one about the discontinued TNX‐901. All were in people with peanut allergy in the USA, mostly aged 13+ years. There was a trend towards improved tolerance of peanut during treatment, with few side effects. However, we have very low certainty about the evidence due to the small number of trials and participants. No included trial reported on quality of life or cost‐effectiveness. CONCLUSIONS: There is not yet enough certainty to support offering etokimab or omalizumab widely for food allergy. Clinicians may consider the merits for individuals, but large randomised trials with standardised measures are needed to confirm the safety, efficacy and most suitable candidates, doses and durations of treatment before more universal use
Concurrent validity, cut-offs and ability to change of patient-reported outcome measures for rhinitis and asthma in MASK-air®
Patient-reported outcome measures (PROMs) are used to assess a patient's health status at a particular point in time. They are essential in the development of person-centred care. This paper reviews studies performed on PROMs for assessing AR and asthma control, in particular VAS scales that are included in the app MASK-air® (Mobile Airways Sentinel networK) for asthma and rhinitis. VASs were initially developed on paper and pencil and tested for their criterion validity, cut-offs and responsiveness. Then, a multicentric, multinational, double-blind, placebo-controlled, randomised control trial (DB-PC-RCT) using an electronic VAS form was carried out. Finally, with the development of MASK-air® in 2015, previously validated VAS questions were adapted to the digital format and further methodologic evaluations were performed. VAS for asthma, rhinitis, conjunctivitis, work and EQ-5D are included in the app. Additionally, two control-medication scores for allergic symptoms of asthma (e-DASTHMA) were validated for their criterion validity, cut-offs and responsiveness.This work has received funding from ARIA and the European Union's Horizon Europe research and innovation programme under Grant Agreement No 101057131, Climate Action To Advance HeaLthY Societies in Europe (CATALYSE). To commemorate the fiftieth anniversary of the discovery at The Rockefeller University that genes are made of DNA - considered by many to be the single most important biological discovery of the twentieth century - the university has kicked off a year-long series of events that were running through May 1994. The celebration was formally inaugurated in November 1993 with a lecture by Nobel laureate James D. Watson, best known for discovering the double-helical structure of DNA.

In 1934 Torsten Fogelqvist, a prominent member of the Swedish Academy and a well-known journalist and intellectual, visits Nazi Germany. He writes about his visit to the Third Reich in 17 articles published in the Stockholm daily newspaper Dagens Nyheter. The author, highly critical of the Hitler regime, scrutinizes several aspects of the nazified German society such as the attempts to re-educate the German citizen in accordance with the ideology of the new regime, the hero cult in the Nazi movement, and the relationship between the German state and the churches. In order to further an understanding of political and social developments in Germany Fogelqvist uses a specific strategy. He “translates” them into an imaginary Swedish context.
