‚Wide awake‘ - Anwendung in der Handchirurgie: Eine retrospektive Untersuchung der subjektiven Zufriedenheit von handchirurgisch operierten Patienten in wide awake im Vergleich zur Allgemein- und Regionalanästhesie
WALANT ist eine revolutionäre Technik für Operationen an der Hand. Gerade in der Zeit, in
den Ressourcen, Zeit- und Kosteneffizienz eine größere Rolle spielen, gewinnt WALANT
zunehmend an Bedeutung. Hierbei handelt es sich um ein blutungsarmes
Lokalanästhesieverfahren, bei dem zu einem Lokalanästhetikum zusätzlich hochverdünntes
Adrenalin subkutan in das Operationsgebiet injiziert wird. Dabei bewirkt das Adrenalin eine
Vasokonstriktion und die Herbeiführung einer optimalen Sicht im Operationsgebiet. Das
Operieren am wachen Patienten ohne den Einsatz einer Blutsperre oder Narkose wird dadurch
ermöglicht. Der kanadische Handchirurg Donald H. Lalonde gilt als einer der Pioniere von
WALANT. Er widerlegte den Mythos von Fingernekrosen bei Applikation von Adrenalin in die
Akren. Seit dem Sicherheitsnachweis von Adrenalin durch Lalonde und die Optimierung der
Technik wird WALANT weltweit erfolgreich eingesetzt. WALANT hat bedeutende Vorteile: Es
reduziert Operationen auf die Einfachheit eines Zahnarztbesuchs, dadurch kann in jeglicher
Hinsicht ressourcenschonend und kostenoptimiert gearbeitet werden.
Die Sicherheit und die Wirksamkeit von WALANT sind in der Literatur ausreichend belegt,
hingegen wurde die Sicht der Patienten zu WALANT nicht ausreichend aufgearbeitet. Diese
Studie hatte daher zum Ziel, die subjektive Patientenzufriedenheit, den Komfort, die Angst und
die Schmerzwahrnehmung von WALANT mit den konventionellen Anästhesiemethoden zu
vergleichen.
Im Zeitraum von Juli 2020 und Juli 2022 wurden alle Patienten erfasst, die im MVZ am St.
Vinzenz-Krankenhaus, Sektion Handchirurgie, in Hanau sowie in der Universitätsmedizin
Mainz aufgrund eines Karpaltunnels oder einer Tendovaginitis stenosans handchirurgisch
behandelt wurden. Nach Anwendung der Ausschlusskriterien wurden sämtliche relevante
Items aus den Krankenakten retrospektiv erfasst. Die Erhebung des Fragebogens erfolgte
mittels telefonischer Interviews. Hierbei kam ein selbst erstellter, nicht validierter Fragebogen
zur Anwendung. Insgesamt nahmen 348 Personen an der Studie teil, wovon 226 den
Fragebogen beantworteten. Anschließend wurde das Patientenkollektiv in die jeweiligen vier
Narkoseformen unterteilt und miteinander verglichen. Die statistische Analyse erfolgte mithilfe
von IBM SPSS. Die Prüfung von Korrelationen wurde nach Spearman und Pearson und mittels
des t-Tests durchgeführt. Der Fragebogen wurde mit dem Mann-Whitney-U-Test ausgewertet.
Für alle quantitativen Merkmale wurden statistische Kennzahlen berechnet.
Die Studie kam zu dem Ergebnis, dass die WALANT-Technik mit einer höheren
Patientenzufriedenheit verbunden ist. Diese Verbesserung der Patientenzufriedenheit geht
nicht zulasten der Qualität der OP, ebenso gibt es kein erhöhtes Risiko für das Auftreten von
Komplikationen.
Insgesamt empfanden die Patienten den Eingriff in allen Anästhesieformen als wenig
schmerzhaft bzw. unangenehm. Hinsichtlich des intra- und postoperativen Schmerzes gibt es
keine Unterschiede zwischen den jeweiligen Anästhesieformen. Der Schmerz des Tourniquets
verursachte den Patienten der Regional- und Allgemeinanästhesie das meiste Unbehagen,
jedoch empfanden die Patienten den Einstichschmerz der Betäubung bei WALANT als stärker.
Trotz dessen waren Patienten nach einer OP mit WALANT insgesamt zufriedener.
Patienten der WALANT-Methode waren über mehrere Dimensionen hinweg zufrieden bis sehr
zufrieden. Sie waren in jeder Hinsicht im Gesamten zufriedener als Patienten der alternativen
Anästhesiemethoden, jedoch ist der Unterschied gering oder statistisch nicht signifikant. Die
Ergebnisse zeigen, dass Patienten bei WALANT mindestens genauso zufrieden sind wie
Patienten in konventioneller Anästhesie (60, 88).
Bei jeder Anästhesieform berichteten Patienten gleichermaßen über eine ehe geringe Angst.
Im Mittelwertvergleich konnte in keiner Gruppe ein statistisch signifikanter Unterschied
festgestellt werden, daher verursacht der Gedanke, bei der OP wach zu sein, in der Regel
keine zusätzliche Angst. Das wurde bereits in der Literatur erwähnt (8, 84). Im Unterschied
dazu konnte diese Studie darlegen, dass präoperativ ängstlichere Patienten nach einer OP mit
WALANT postoperativ zufriedener sind als nach einer OP mit einer konventionellen
Anästhesie. Demnach profitieren ängstlichere Patienten von einer Operation mit WALANT.
Erklärt werden könnte dieses Phänomen durch die Möglichkeit des intraoperativen interaktiven
Gespräches, das beruhigend für den Patienten wirkt und nur in WALANT möglich ist.
Diese Studie hebt den Nutzen und die Sinnhaftigkeit der Patientenedukation hervor. Patienten,
die mit WALANT operiert wurden, können durch ihr eigenes Mitwirken selbst etwas zum Erfolg
der Behandlung beitragen. Das resultiert in einem verbesserten klinischen Outcome und einer
optimierten postoperativen Zufriedenheit. Anderseits zeigt die Studie auf, dass sich nicht alle
Patienten Edukation wünschen. Die Mehrheit der Patienten, die in Vollnarkose operiert
wurden, haben es bevorzugt, bei ausgeschaltetem Bewusstsein behandelt zu werden, und
wünschen es sich wieder. Dies zeigt auf, dass WALANT nicht für jede Person die geeignetere
Alternative ist, daher sollte der Operateur mitbestimmen, welche Narkose zu welchem
Patienten passt.
Es ist in der Literatur belegt, dass WALANT komplikationsarm ist und ein gutes funktionelles
Ergebnis bietet (59, 94). Im Vergleich ist der Anteil der Komplikationen bei den verschiedenen
Anästhesieformen weitestgehend ähnlich. Es wurde kein signifikanter Unterschied in der
Komplikationsrate zwischen den Anästhesieformen festgestellt. Zudem gibt es keinen
signifikanten Unterschied bei der Fingerstreckung und dem Faustschluss am ersten
Nachuntersuchungstag zwischen den jeweiligen Anästhesieformen. Aus diesem Grund kann
angenommen werden, dass alle Methoden in gleichem Maße sicher und wirksam sind und
dem Patienten bei der Wahl der Anästhesietechnik kein Nachteil entsteht. Insgesamt stützen
die Ergebnisse der hier durchgeführten Studie die Behauptung, dass WALANT eine sichere,
schnelle, wirksame und gut akzeptierte Methode für die Handchirurgie ist.
Zuletzt bekräftigen die Studienergebnisse die Entbehrlichkeit von Opiaten nach Karpaldach-
oder Ringbandspaltung.
Limitiert wurde diese Studie dadurch, dass etwa ein bis drei Jahre zwischen der Operation und
der Befragung lagen, was zu einem recall bias geführt haben könnte. Zudem wurden nicht alle
Anästhesieformen an beiden Standorten durchgeführt, weswegen es zu einer Verzerrung in
der Zufriedenheit aufgrund innerbetrieblicher Abläufe und zu einem lost-to-follow-up

Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
