MIGLIORAMENTO DELLA CLASSIFICAZIONE DEL MELANOMA BASATA SULLA MICROSCOPIA CONFOCALE E APPLICAZIONE DI DOPPIA VALUTAZIONE IN TELEMEDICINA
Il melanoma cutaneo è una malattia complessa, la cui patogenesi è influenzata da fattori genetici e ambientali. Lo scopo di questa ricerca è definire una classificazione di malattia che sia basata sui descrittori di imaging, sulla presenza di biomarcatori cutanei e sulle caratteristiche genetiche della neoplasia; al fine di poter predire i diversi comportamenti clinici del melanoma, in modo da identificare con precisione la prognosi del paziente e il suo rischio di recidiva. Abbiamo analizzato retrospettivamente un ampio numero di melanomi di nevi melanocitici e di lesion in diagnosi differenziale con dermoscopia, microscopia a riflettanza confocale, immunoistochimica delle principali vie di segnale, e analisi delle mutazioni genetiche più comunemente associate al melanoma. Una valutazione retrospettiva di nevi e melanoma è stata condotta al fine di identificare i differenti sottotipi morfologici correlati alle diverse caratteristiche cliniche e al fenotipo del paziente. L’analisi dermoscopica, di microscopia confocale, immunoistochimica, e delle mutazioni somatiche è stata la base morfologica e biomolecolare utilizzata per costruire una nuova classificazione e per corrrelare le alterazioni biologiche con l’aggressività tumorale, come stimata sulla base dei parametri istopatologici, del pattern di crescita e della stadiazione della malattia. In aggiunta, l’accuratezza diagnostica della telemedicina, di tipo store-and-forward, è stata testata attraverso 1000 casi analizzati da due valutatori per definire l’applicabilità e chiarire i potenziali della telemedicina per la futura gestione clinica dei pazienti, così come per verificare il costo-beneficio della metodica nella realtà clinica.Cutaneous melanoma is a complex disorder with genetic, environmental and host factors contributing to its pathogenesis. The aim of our research proposal was to define a better classification of this disease based on imaging characteristics, tissue biomarkers and genetic signature that can explain different melanoma behavior, help in melanoma diagnosis and predict clinical outcome and disease relapse. We retrospectively investigated a large series of melanomas and cutaneous lesions in differential diagnosis with dermoscopy, reflectance confocal microscopy, immunohistochemistical analysis of melanoma signaling pathways and analyzed the most common melanoma-associated somatic/germline mutations in a selected high risk population in order to identify different morphologic subtypes related to clinical characteristics and patient phenotype. Dermoscopic and confocal microscopy aspects, immunohistochemistry and somatic mutation analyses were used to characterize the different melanoma subtypes to correlate biological alterations with tumor aggressiveness, as estimated according to histopathologic parameters, tumor growth pattern and staging outcome. Additionally accuracy of melanoma identification through store-and-forward telemedicine reading was tested in a double reader evaluation of 1000 cases to determine the potential for its application in future clinical care as well as potential cost-benefit of applying this technology

The Author of the Alexander Romance
This paper, which is based on a portion of the introduction of the author’s edition of Il Romanzo di Alessandro (Mondadori: Fondazione Valla 2007), surveys the generic components of the Alexander Romance in an attempt to arrive at a definition of the work. The argument builds on Merkelbach’s categorisation of elements and uses Fusillo’s insight into the novel as an ‘encyclopaedic genre’ to propose that ‘historical novel’ is not, as Hägg contended, a misnomer for the work. The main components I discuss are: ‘life’; praxeis; chreiai; Cynic elements, including choliambic poetry and utopian perspectives; and the Egyptian aspects of the narrative. A concluding jeu d’esprit offers a characterisation of the putative author, his antecedents and his process of composition.Richard Stoneman was for 25 years editor for classics at Croom Helm and then Routledge. In 1997 he was appointed an Honorary Fellow in the department of classics, University of Exeter. After retiring from publishing in 2006 he has been pursuing his researches on the Alexander legends and teaching a course on the subject at Exeter. His Penguin translation of the Alexander Romance was published in 1991, and a volume of translated Legends of Alexander the Great appeared from Everyman in 1994. Also in 1994 he co-edited Greek Fiction with John Morgan. His edition of the Greek recensions of the Alexander Romance was published (volume I) by the Fondazione Valla in 2007 – volumes II and III will follow over the next few years – and his Alexander the Great: A Life in Legend appeared from Yale University Press in spring 2008. He is the author of a number of other books on Greek history and travel, and is writing a book on oracles
