    The Value Of Tissue Retinoblastoma Gene (RB1) Expression Levels In Colorectal Cancer And Inflammatory Bowel Disease

    Background: Colorectal cancer (crc) is one of the most important and prevalent digestive tract malignant tumors with poor prognosis. Cancer biomarkers play a crucial role in outlining the prognosis of a disease independently of any treatment (known as prognostic biomarkers) or in predicting how a cancer will respond to a specific treatment, which helps anticipate treatment outcomes (referred to as predictive biomarkers). Retinoblastoma (Rb1) represents a critical tumor suppressor gene in different tumor types, governing diverse cellular processes implicated in cancer biology Subjects and Methods: 49 paired colorectal tumors and non-tumor marginal tissues, as well as 21 tissue biopsies taken from patients with inflammatory bowel disease. Total RNA was extracted from the samples and cDNAs were synthesized. Their expression was quantified by qRT-PCR. Results: We observed significant upregulation of RB1 in the colorectal tumors compared with non-tumor as well as inflammatory tissues sample from IBD patients. RB1 expression was significantly elevated with higher tumor marker (CA19-9 and CA125). ROC curve analysis showed that RB1 expression levels could discriminate tumor from non-tumor and IBD. Conclusion: RB1 expression levels could discriminate colorectal tumors from non-tumor tissues, as well as from IBD, highlighting their potential as biomarkers for colorectal cancer developmen

    Nonarteritic anterior ischemic optic neuropathy associated with interferon and ribavirin in a patient with hepatitis C

    Purpose: To report a case of a temporal artery biopsy negative anterior ischemic optic neuropathy associated with a recently completed course of pegylated interferon 2 α with ribavirin for chronic hepatitis C. Observations: Despite the early presentation with symptoms and prompt treatment with systemic intravenous steroids the patient experienced deterioration of their optic neuropathy over the following few days. Although nonarteritic anterior ischemic optic neuropathy is a common disorder with known risk factors, the timing of onset of symptoms in our patient was suggestive of a possible etiology related to treatment with ribavirin and interferon 2 α, as found in the previously reported cases. Conclusions and importance: There have been a few reported cases of the association between the use of interferon/ribavirin for treatment of chronic hepatitis with nonarteritic anterior ischemic optic neuropathy. In these cases stopping the drug caused some improvement of symptoms or halting the progression of optic neuropathy. Having reviewed the literature on previous cases, we postulate that there may be a dose related reaction to explain the delay and deterioration of vision in some cases despite stopping the drugs. We also advise that any person who is started on this treatment for chronic hepatitis are appropriately counselled as to the potential optic nerve side effect of the drug, based on the evidence reported in the literature

    Design, construction and field testing of a manually feeding semiautomatic sugarcane dud chipper

    Sugarcane is the main sugar crop, and sugar is an important agricultural product in Egypt. There are many problems with the technology used in the current planting method of sugarcane, which has a great impact on the planting quality of sugarcane, which have a series of problems, such as low cutting efficiency and poor quality. Therefore, the aim of the current study was to design, construct, and field testing of a semiautomatic sugarcane bud chipper assisted with pivot knives for cutting sugarcane buds and germinating them in plastic trays inside a greenhouse until they reached an average length of 35 cm, and then planting them in the field. In the field tests five cutting speeds (35, 40, 45, 50, and 56 rpm. (Revolution Per minute), three cutting knives (1.5, 2.0, and 2.5 mm) were used for cutting sugarcane stalks with four different diameters (1.32, 1.82, 2.43, and 2.68 cm). The obtained results showed that the values of the damage index and invisible losses were within acceptable limits (ranging between - 1.0 and 0.0) for all the variables under the test. Still, the lowest damage index and invisible losses were recorded with the buds that were cut with a knife of 1.5 mm thickness and cutting speeds less than 50 rpm. The skipping rate increases with the increase in cutting speed and stalk diameter, ranging between 0.0 to 13%. The maximum machine productivity was 110 Buds per minute at a cutting speed of 35 rpm and stalk diameter of 1.32 cm. The paper's findings have important application values for promoting the designing and development of sugarcane bud chipper and sugarcane planting technology in the future

    Effect of different designs of minimally invasive cantilever resin-bonded fixed dental prostheses replacing mandibular premolar: Long-term fracture load and 3D finite element analysis

    Purpose: To evaluate the fracture load and stress magnitude of different retainer designs of minimally invasive cantilever resin-bonded fixed dental prostheses (RBFDPs) after artificial aging. Materials and methods: Fifty caries-free human mandibular molars were prepared as abutments for cantilever fixed dental prostheses using different retainer designs: one wing (OW), two wings (TW), inlay ring (IR), lingual coverage (LC), and occlusal coverage (OC). Computer-aided design and computer-aided manufacturing were used for milling the RBFDPs using fiber-reinforced composite (FRC), and the restorations were adhesively bonded. The specimens were then subjected to thermomechanical aging and loaded until failure. The 3D finite element analysis (FEA) was performed with five models of retainer designs similar to the in vitro test. Modified von Mises stress values on enamel, dentine, luting resin, and restorations were examined. Data were analyzed with Kruskal–Wallis and Mann–Whitney U tests (p < 0.001). Results: A statistically significant difference (p < 0.001) was found between all groups except between IR and LC and between OW and TW designs, with the highest mean failure load detected for OC (534.70 N) and the lowest detected for OW (129.80 N). With regard to failure mode, OW, TW, and LC showed more incidences of favorable failure patterns than IR and OC designs. FEA showed that FRC transmitted low stresses in tooth structure and high stresses to the luting resin. Conclusions: LC and OC designs can be used to design cantilever RBFDPs in premolar area. IR design transmitted more stresses to the tooth structure and resulted in 30% catastrophic failure. OW and TW were below the normal occlusal force and should be carefully used

    Biomechanical behavior of posterior metal-free cantilever fixed dental prostheses:effect of material and retainer design

    Objective: To study the fracture resistance and stress distribution pattern of translucent zirconia and fiber-reinforced composite cantilever resin-bonded fixed dental prostheses (RPFDPs) with two retainer designs. Materials and methods: Forty human mandibular molars were divided into two groups according to the retainer design. The restorations included a premolar pontic and 2 retainer designs: (D1) inlay ring retainer and (D2) lingual coverage retainer. Each main group was then divided according to the material used (n = 10): zirconia (Z) or fiber-reinforced composite (FRC) (F). Restorations were cemented using dual polymerizing adhesive luting resin. All specimens were thermo-cycled (5–55 °C for 10,000 cycles), then subjected to dynamic loading (50 N, 240,000, and 1.6 Hz) and fracture resistance test. The finite element analysis includes the two models of retainer designs used in the in vitro test. Modified von Mises stress values on enamel, dentin, luting resin, and restorations were examined when the restorations failed. Results: A significantly higher failure load was recorded for zirconia groups (505.00 ± 61.50 and 548.00 ± 75.63 N for D1Z and D2Z, respectively) than for FRC groups (345.00 ± 42.33 and 375.10 ± 53.62 N for D1F and D2F, respectively) (P = 0.001). With regard to failure mode, D2 showed a more favorable failure pattern than D1. Model D2 resulted in lower stresses in tooth structure than model D1, and zirconia transmitted more stresses to the tooth structure than FRC. Conclusions: The lingual coverage retainer (D2) enhanced the biomechanical performance of the restoration/tooth complex. Considering the failure mode and tooth stress, FRC is a promising treatment option when constructing a cantilever RPFDP. Clinical relevance: Dentists should be aware of the biomechanical behavior during the selection of the material and for the replacement of a single missing mandibular premolar tooth with minimally invasive RBFDP.</p

    Can Endothelin-1 Levels in Patients with Esophageal Variceal Bleeding at Admission Predict Rebleeding Within 5 Days?

    Background/Aims: Portal hypertension complicating liver cirrhosis is associated with vascular resistance, possibly due to overexpression of humoral vasoconstrictors, including endothelin. The study aimed to evaluate the efficacy of serum endothelin-1 levels as a noninvasive predictor of early esophageal rebleeding (within 5 days) following endoscopic treatment. Materials and Methods: Of the patients presented to the endoscopy unit at Mansoura University Hospital, 50 patients were chosen for this study on the basis of endoscopically proven acute esophageal variceal bleeding consequent to hepatitis C viral infection complicated by liver cirrhosis and portal hypertension. Routine laboratory parameters and serum endothelin-1 levels were assessed prior to endoscopic treatment. Patients were divided into 2 groups depending on the development of early postendoscopic rebleeding. Group A consisted of 16 patients who developed rebleeding, while group B included 34 patients who did not. Statistical analysis was performed to determine the predictors of rebleeding. Results: Multivariate logistic regression demonstrated that endothelin-1 level (P < .001) and serum albumin level (P = .04) were independent risk factors for early rebleeding. The most efficient cutoff value for endothelin-1 levels in predicting variceal rebleeding within the 5 days after endoscopic intervention was 65.29, which had an 88.2% specificity, 87.5% sensitivity, 88% accuracy, and area under the curve value of 0.89. In addition, hemoglobin, albumin, and creatinine levels were significantly different between bleeding and nonrebleeding groups (P = .03, P = .014, and P <.001, respectively), as was the duration of hospital stay (P < .001). Conclusion: Serum endothelin-1 levels appear to be a reliable, practical, noninvasive predictor of early variceal rebleeding and related comorbidities such as the severity of kidney affection and duration of hospital sta

The multifrequency force spectroscopy formalism with higher modes, including trimodal AFM, can outperform bimodal AFM for material property quantification. Bimodal AFM with the second mode is valid when the drive amplitude of the first mode is approximately an order of magnitude larger than that of the second mode. The error increases in the second mode but decreases in the third mode with a decreasing drive amplitude ratio. Externally driving with higher modes provides a means to extract information from higher force derivatives while enhancing the range of parameter space where the multifrequency formalism holds. PhD dissertation, Durham University, January 2013. Introduction: In sub-Saharan Africa, HIV treatment is initiated with combination of antiretroviral medications comprising of a backbone of either stavudine or zidovudine plus lamivudine with a non-nucleoside reverse transcriptase inhibitor of either efavirenz or nevirapine. Efavirenz is highly efficacious, durable and well tolerated. The risk for toxicity of efavirenz is determined by several factors including single nucleotide polymorphisms in the hepatic enzymes responsible for its metabolism and concurrently administered medications such as antimalarials, which share common metabolic pathways. The aims of this dissertation are to assess the long-term effectiveness of efavirenz-based antiretroviral therapy and the impact of pharmogenomics and pharmacokinetic interactions of artemisinin-based antimalarial therapy on efavirenz exposure among Ghanaian HIV-infected patients. Methods: The effectiveness of efavirenz- compared with nevirapine-based antiretroviral therapy was assessed retrospectively in nearly 4000 patients starting treatment between 2004 and 2010. The main outcome measure was a composite of toxicity, disease progression and attrition, and CD4 count changes. A prospective pharmacokinetic study of artesunate and efavirenz was conducted among 22 HIV-infected and 21 controls. Plasma efavirenz and artesunate/ dihydroartemisinin concentrations were measured using validated and standardised methods. Genotyping for single nucleotide polymorphisms in CYP2B6 G516T, T983C; CYP2A6*9B, UGT2B7*735 and *802 as well as CAR rs2307424 were performed for 800 patients with real-time polymerase chain reaction with allelic discrimination. Results: Antiretroviral therapy was associated with robust CD4 increases. Efavirenz was comparable with nevirapine in composite outcomes but better tolerated. Artesunate was well tolerated when administered to HIV-infected patients on efavirenz. Single nucleotide polymorphisms in the CYP2B6 G516T and T983C were associated with increased plasma efavirenz concentrations. Conclusions/Recommendation: Among this Ghanaian cohort, both efavirenz and nevirapine-based antiretroviral therapy were effective. The better tolerability of efavirenz compared with nevirapine means it can be safely used as the preferred first line non-nucleoside reverse transcriptase inhibitor in sub-Saharan Africa
