    Facial nerve paralysis in children

    Facial nerve palsy is a condition with several implications, particularly when occurring in childhood. It represents a serious clinical problem as it causes significant concerns in doctors because of its etiology, its treatment options and its outcome, as well as in little patients and their parents, because of functional and aesthetic outcomes. There are several described causes of facial nerve paralysis in children, as it can be congenital (due to delivery traumas and genetic or malformative diseases) or acquired (due to infective,inflammatory, neoplastic, traumatic or iatrogenic causes). Nonetheless, in approximately 40%-75% of the cases, the cause of unilateral facial paralysis still remains idiopathic. A careful diagnostic workout and differential diagnosis are particularly recommended in case of pediatric facial nerve palsy, in order to establish the most appropriate treatment, as the therapeutic approach differs in relation to the etiology

Successively, other family members have been asked to undergo clinical evaluation, which reported that all the descendants, both male and female, were affected by WSN

    "Wisdom does not live in one house": compiling environmental knowledge in Lesotho, Southern Africa, c. 1880-1965

    This dissertation reconstructs a history of the greater Qacha’s Nek district of Lesotho, southern Africa from 1880 when farmers first settled the area, until 1965 on the eve of independence from Great Britain. This place-based study speaks to broader questions. How have people incorporated new and often foreign ideas into existing beliefs and practices? How did a person’s social position affect how they interacted with new ideas? How have people applied knowledge to make and remake environments such as in gardens and fields? This study is based on field research in Lesotho, South Africa, and the United Kingdom. The author examined archival materials including colonial records, agricultural reports and surveys, national council proceedings, and vernacular newspapers. During four months of rural fieldwork in Lesotho the author collected oral histories, took photographs, and participated in village life. The approach focuses on colonial government interventions into agriculture and pastoralism. These interventions serve as sites for examining historical changes in how Basotho people engaged with the non-human world. In so doing, the study makes three main interventions. First, the claims are situated within scholarly conversations about local knowledge, science, and environment under colonialism. Second, the stories of chiefs, farmers, and government employees told here extend the literature on Lesotho’s political and economic history by highlighting the nuance of local politics, ecology, and agency. Finally, to contribute to the environmental historiography on Africa and rural places in general, the study probes the interplay of culture and nature. To do this, it narrates how people deployed eclectic knowledge to build, rebuild, and redefine environments. The dissertation argues that the compilation of environmental knowledge must be understood as a historical process that encapsulates the meanings that people have imbued the landscape with, for example, by building homesteads, along with how people have understood the landscape as a system of resources to be used economically for subsistence and market purposes. These aspects of knowing are part of a single process that has unfolded, and continues to unfold, along a temporal trajectory that has varied across different social groups, such as men and women and chiefs and commoners

Este trabalho se desdobrou na articulação entre teoria e prática, dois aspectos fundamentais para o artista na universidade. A criação de um solo em dança contemporânea tornou-se um meio e também um dos objetivos desta pesquisa. O reconhecimento das minhas memórias possibilitou a reflexão sobre o ensino e prática da técnica do ballet clássico, técnica esta frequentemente utilizada nos processos de formação em dança. O método somático da Ideokinesis foi escolhido para instrumentalizar a observação e, quando necessário, a mudança dos padrões de movimento que são parte das memórias de procedimento. Clarice Lispector constituiu o referencial poético para a criação e a escrita da dissertação. Busquei a leveza, o humor e a intensidade com que esta importante escritora conta suas lembranças no livro de crônicas "A descoberta do mundo" (1999). Os procedimentos técnicos e criativos utilizados e desenvolvidos, bem como os encontros, descobertas, dificuldades, inspirações e transpirações deste processo fazem parte da discussão apresentada nesta dissertação, cuja escrita buscou ser, como no solo de dança, uma obra artísticaAbstract: This research observed how memories determine choices; create patterns, shapes and unique ways of interaction with the creative process. This work was developed among theory and practice, two fundamental aspects for the artist at the university. The creation of a contemporary dance solo has become the means and also one of the objectives of this research. The recognition of my own memories enabled a reflection on teaching and practice of classical ballet technique, which is often used in educational processes in dance. The somatic method of Ideokinesis was chosen as an observation tool that allowed me, whenever necessary, to change movement patterns that is part of procedural memories. The author Clarice Lispector constituted my poetic reference. I sought for the delicacy, humor and intensity with which the writer told her memories in the book of chronicles "A descoberta do mundo" (1999).     A 20 µg/day levonorgestrel-releasing intrauterine system (LNG-IUS) and risk for breast cancer : a systematic review

    Orientador: Sarian, Luis Otávio ZanattaDissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências MédicasResumo: avaliar o risco de desenvolvimento de câncer de mama em mulheres usuárias do Sistema de liberação Intrauterina de levonorgestel (SLIU-LNG). Métodos: revisão sistemática da literatura segundo os critérios do PRISMA (Preferred Reporting Items for Systematic Reviews and Metanalysis). Estudos de caso-controle, coortes e ensaios clínicos, quando disponíveis na literatura, foram incluídos a partir de pesquisa nos bancos de dados do Medline (via PubMed), Embase (via OVID), LILACS (via BVS) e Scielo. Resultados: quatro estudos foram incluídos na revisão, sendo dois caso-controle e duas coortes, todos com análise de dados retrospectivos. Dois dos quatro estudos apontaram aumento do risco para câncer de mama em usuárias do SLIU-LNG, e nos outros dois a associação não demonstrou aumento de risco. Entre as coortes incluídas, apresentaram-se dados de 1.032.767 mulheres-anos, com seguimento de onze anos, evidenciando razão de incidência (RI) de carcinoma lobular invasivo RI (CLI)= 1,33, (IC 95%= 1,20-1,46) e de carcinoma ductal invasivo RI (CDI)=1,20 (IC 95%=1,14-1,25), mostrando aumento do risco em usuárias de SLIU-LNG. Coorte com 17.360 questionários enviados para usuárias finlandesas de SLIU-LNG, avaliados de acordo com faixas etárias de 30-34 anos, 35-39 anos, 40-44 anos, 45-49 anos e 50-54 anos, não demonstraram aumento de risco. Neste trabalho, a incidência global de câncer de mama na população finlandesa foi maior do que entre as usuárias de SLIU-LNG, sendo que em dois grupos (30-34 anos e 35-39 anos), usuárias de SLIU-LNG tiveram incidência mais elevada do que na população média finlandesa, porém, sem diferença estatística. Em relação aos estudos caso-controle, o risco para câncer de mama em usuárias de SLIU-LNG versus DIU-Cobre, ambos para contracepção, em mulheres com até 50 anos na Alemanha e Finlândia, não foram estatisticamente diferentes. Todas as mulheres deste trabalho foram analisadas quanto às características pessoais e fatores de risco para câncer de mama, como IMC, história ginecológica, uso de contracepção hormonal e terapia de reposição hormonal, história familiar de cânceres ginecológicos, presença de comorbidades, uso de fármacos, indicadores socioeconômicos e estilo de vida. Neste estudo, 5113 casos e 20.452 controles foram incluídos e o risco associado a câncer de mama (in situ e invasivo) não foi diferente entre os grupos, com OR SLIU-LNG=1.04 (IC 0,93-1.17) e OR DIU-Cobre=0.99 (IC=0.88-1.12). Por último, em estudo caso-controle (n= 5154), autores avaliaram o risco para câncer de mama em mulheres entre 50-62 anos usuárias de SLIU-LNG para terapia de reposição hormonal e mostraram aumento do risco para câncer de mama nos casos (OR= 1,45; IC=1,97-1,77), em relação aos controles. Conclusão: novos estudos são necessários para que a associação SLIU-LNG seja esclarecida, uma vez que há poucos estudos na literatura, todos fundamentados em dados retrospectivos, com população heterôgenea. Dada a relevância do assunto, conclusão sobre esta associação é fundamentalAbstract: the association between levonorgestrel-releasing intrauterine system (LNG-IUS) use and breast cancer risk is a theoretical possibility not well addressed in the medical literature. OBJECTIVES: to evaluate the risk of developing breast cancer in women using the 20 µg/day LNG-IUS. SEARCH STRATEGY: the systematic search was conducted using the search terms "levonorgestrel", "breast cancer", "intrauterine system" and "Mirena®" in Medline, Embase, LILACS and Scielo databases and if there were no clinical trials, the non-randomized comparative studies were included. SELECTION CRITERIA: systematic review of the literature was performed according to PRISMA criteria (Preferred Reporting Items for Systematic Reviews and Meta-Analysis). Two authors performed data extraction and a third author resolved disagreements. DATA COLLECTION AND ANALYSIS: descriptive analyzes were performed. The quality of the evidence was evaluated using GRADE criteria. All searches were conducted in 2017 with articles published after 1999. MAIN RESULTS: two of the four studies reported an increased risk of breast cancer in LNG-IUS users, and in the other two papers, no risk association was reported. Increased risk was also detected for women using the device for heavy menstrual bleeding or postmenopausal hormonal therapy; however, not for those using the LNG-IUS for contraception. Case-control and cohort studies reached similar results. CONCLUSION: no firm conclusion could be reached, since studies are scarce, retrospective and evaluated populations with dissimilar features. Given these preliminary findings and the widespread use of LNG-IUS for both pre- and post-menopausal conditions, prospective studies are warranted.     abstract: Adam Simons is an Interdisciplinary Studies major at Arizona State University with concentrations in business and music. He is also a member of the alternative rock group RadioDriveBy. RadioDriveBy is a five-piece band based in Arizona that writes, professionally records, and performs its own original songs. This thesis contains a narrative of the planning, execution, and analysis of two albums released by RadioDriveBy. Through his experience and research in managing multiple releases, Simons covers the critical factors for artists to consider when marketing music in today's changing industry. The author divides an album release into five critical components: original music, release structure, artist image, accessibility and availability, and online promotion. This method of analysis develops manageable steps for artists to succeed in the modern music industry. Overall, this thesis is a model and creative project to help understand not only the behind-the-scenes work of RadioDriveBy, but also to provide a framework for other musicians and artists navigating the same path

    The love theme in Gottfried's Tristan und Isolde and its treatment in German literature from the romantics to Wagner

    Although there are several versions of the story of Tristan and Isolde before Gottfried von Strassburg none has survived in more than fragmentary form since they lacked appeal to following generations. The version par excellence is that by Gottfried. His method of writing in allegories and symbols, which he uses to convey to his select audience of edele herzen the deeper meaning of his words, has given rise to many controversial interpretations. The contradicting interpretations of the most noted scholars are briefly touched upon and an attempt is made to offer a more balanced approach to Gottfried's ideal of love. The later medieval continuations by Ulrich von Türheim and Heinrich von Freiberg are considered in relation to each other as well as to Gottfried. Only the Eilhart version, entirely based on effective narration of outer action, remained popular. This was put into prose by an unknown author in the second half of the fifteenth century and became known as the Prose Romance. In it the old courtly epic was reduced to a story for entertainment and its style consequently altered to suit the level of the readers. The Prose Romance of the sixteenth century became the source for Hans Sachs who retold the Tristan legend in six Meisterlieder and one drama (1553). Hereafter the legend seemed forgotten for two centuries until the German Romantic Movement revived interest in it in the course of its general emphasis on the German cultural and literary past. Several attempts were made by the Romantics to create independent versions inspired by Gottfried's Tristan and under the influence of the critical writings of Schlegel and others. The attempts at literary versions of the Tristan-theme by August Wilhelm von Schlegel, Karl Phllipp Conz, Wilhelm Wackernagel and Friedrich Rückert are briefly discussed and analyzed in the light of Romantic theories of love. The results of Romantic interest in the theme of Tristan are surprisingly meagre and since no specific evidence can be adduced as to the reason for this, it is only possible to put forward a tentative theory regarding some of the causes. Only Immermann produced a work of any consequence and this is therefore discussed at some length. Only after the Romantics are there any serious efforts to produce linguistically and scholastically acceptable translations (Hermann Kurtz, Karl Simrock, Wilhelm Hertz). It was left to Richard Wagner, on the basis of modern translations, to "rehabilitate" the Tristan legend in his music drama and although it has very little in common with Gottfried, it is nevertheless the only work since Gottfried that has succeeded in provoking further interest in Tristan.
