Evaluation of the Health Impact of the National Diabetes Program in Paraguay
The clinical evolution of a diabetic patient is linked to its compliance with nonpharmacological measures such as diet, physical activity, acquiring habits, medical controls and also with its pharmacological treatment. In different Latin American countries such as the Republic of Paraguay, there are National Programs to provide free support and treatment to the diabetic population that mostly have vulnerable social conditions. However, the impact of these government policies is not routinely evaluated. In order to establish the achievements of a state diabetes program, the present study was carried out. Methodology: This is a descriptive, cross-sectional study with an analytical stage. Results: low adherence to non-pharmacological measures such as diet and physical activity was detected. A high percentage of patients were overweight (38%) or obese (46.5%). 75% of the population enrolled in this study did not comply with the program recommendations regarding the time and frequency of physical activities, nor they complied with the minimum 6 daily meals required. Only 37% of the patients had normal levels of glycated hemoglobin and 51.3% of them had pharmacological treatment adherence. Low compliance was observed in relation to regular medical examinations (which were under free coverage by the program) that are transposed in 22% of retinal studies and 60% in 24-hour urine analysis for the early detection of complications such as retinopathy and nephropathy. Conclusion: Despite the economic and logistical efforts carried out by State programs in order to care of people with diabetes mellitus, data showed a low adherence of patients to the proposed pharmacological and non-pharmacological measures. A personalized follow-up strategy is necessary to improve treatment adherence and to obtain better therapeutic results.Fil: Rodríguez, Patricia Vera. Advancing fast relaxation imaging to determine protein stability and folding kinetics on poly(n-isopropyl acrylamide) films
Several studies have reported the use of the thermo-responsive polymer PNIPAM to capture and release cells or proteins for medical purposes; however, this assumes that PNIPAM does not perturb the stability of biomolecules. Previous analytical and spectroscopic techniques have not been able to confirm this assumption due to limitations in detection of proteins on surfaces. Additionally, protein absorption above the lower critical solution temperature of PNIPAM (32℃) is dependent on the grafting density and molecular weight of the end grafted PNIPAM. The work presented here applies a novel technique to demonstrate how protein adsorption on PNIPAM affects protein thermodynamic stability and kinetics. Fast Relaxation Imaging (FReI) was applied to quantify the folding stability and dynamics of proteins in contact with PNIPAM at varying grafting conditions categorized as brush, weakly overlapping, and mushroom regime. FReI detects protein unfolding in situ by imaging changes in fluorescence resonance energy transfer (FRET) after fast temperature perturbations. The protein used in experiments is phosphoglycerate kinase (ePGK1), which undergoes cooperative unfolding. Contrary to previously accepted notions, the melting temperature and kinetic parameters of ePGK1-FRET reveal that the weakly overlapping and mushroom regimes do in fact perturb the thermodynamic stability and kinetics of ePGK1. Similar to the protein adsorption trend, the perturbations exhibit a bell-shape dependence on the PNIPAM regimes. Additionally, this work shows that FReI can be used to confirm how biomaterials perturb proteins on polymer surfaces.Submission published under a 24 month embargo labeled 'U of I Access', the embargo will last until 2022-12-01The student, Zully Mora Sierra, accepted the attached license on 2020-12-10 at 22:41.The student, Zully Mora Sierra, submitted this Thesis for approval on 2020-12-10 at 22:54.This Thesis was approved for publication on 2020-12-11 at 14:07.DSpace SAF Submission Ingestion Package generated from Vireo submission #16120 on 2021-03-04 at 16:20:59Made available in DSpace on 2021-03-05T21:42:55Z (GMT). Factors Associated with the Lack of Adherence to Drug Therapy in Diabetic Patients
According to the World Health Organization, there are more than 347 million people living with diabetes, being one of theleading causes of death in our days. Diabetes mellitus is a chronic metabolic disorder characterized by an increase in glycemiavalues above normal, which is accompanied by late complications derived from progressive damage in kidney, retina, vessels, heart, and nervous system. The best way to avoid these complications is adherence to treatment. However, the lack of treatmentcontinuity is very frequent. This paper demonstrated that in 76% of the cases, patients with diabetes do not accomplish theprescribed treatment. The main causes of this situation are related to the patient?s habits (since diabetes has no symptoms, the patients skip doses) and to the health system (due to the lack of free drug provision, patients do not have access to thetreatment). From this data, health country authorities initiated a program to aware patients of the importance to accomplish thetreatment and provided free drug for those patientsFil: Rivas, Alba. Cost-Effectiveness of drug compliance in type 2 diabetes mellitus patients
Type 2 diabetes mellitus affects 15% of the population around the world, causing health complications and increasing the treatment’s costs. Being a chronic outpatient treatment disease, patient compliance is higher importance to achieve therapeutic success. The present study measure the cost effectiveness of the compliance of this disease. An observational, cross-sectional, pharmaco-economic study was performed. A National program to stimulate treatment adherence was taken as intervention stage. Compliance was measure by Morisky-Green test and details of the events in each patient's health (medication, consultations, hospitalization and concomitant treatments) were used to determinate the direct costs for Ministry of Public Health. Effectiveness was measured by percentage of glycated hemoglobin (HbA1c). The sample obtained was 55 patients, which were grouped into 31 compliant and 24 non-compliant. The costeffectiveness of compliance was 466 US / non-compliant effective patient. The cost-effectiveness of hypoglycemic drug therapy compliance is significantly lower (almost 4 times) than non-compliance. Type 2 Diabetes Mellitus patients that have no therapy adherence are not able to maintain protective levels of HbA1c. In addition, for the Ministry of Public Health, the lack of adherence requires more budget to accomplished the health goals.Fil: Maciel, Olga.
Tesi

Evaluation of the partial or total substitution of PVC and Phthalates by atoxic raw materials in the production of a conventional rubber eraser at laboratory scale
Currently the rubber erasers of the company I.D.E.A.S are made from PVC and plasticizers, these two products have harmful effects for the environment and human health, however, the performance of the product is good and the costs are low. In European markets, they implemented regulations that eliminate products that contain PVC in their composition. When wanting to reach this market, it is necessary to replace these components with others that don't have implications for the environment or represent a risk to health but keep up the same quality as PVC erasers.
