    Demir Eksikliği Olan Erişkin Yaş Grubu ve Geriatrik Yaş Grubu Bireylerde Üst Gastrointestinal Sistem Endoskopi ve Kolonoskopi Bulgularının Karşılaştırılması Giriş ve Amaç:Demir eksikliği (DE), demir eksikliği anemisinden (DEA) çok daha sık görülen bir klinik durumdur.Buna karşın DEA hastalarında endoskopik incelemelerin yapılması gerektiği net bir şekilde ortaya koyulmuşken, DE hastaları için bu konuda bir fikir birliği yoktur. Çalışmamızın amacı anemisi olmayan DE hastalarını, DEA hastaları ile karşılaştırarak endoskopi ile tanımlanan ciddi gastrointestinal sistem (GİS) lezyonlarının ve GİS malignitelerinin prevalansını belirlemektir. Hastalar ve Yöntem: AÜTF Gastroenteroloji Bilim Dalı endoskopi ünitesinde 01/2014-01/2018 arasında endoskopi yapılan demir eksikliği hastalarının verileri retrospektif olarak incelendi. Çalışmamızda erişkin ve geriatrik yaş grubunda toplam 571 demir eksikliği hastası yer aldı. Hastalar anemi varlığına göre iki gruba ayrıldı.292 DEA hastası ve 279 DE hastası özofagogastroduodenoskopi (ÖGD) ve kolonoskopi bulguları, demografik özellikleri, kronik hastalıkları ve ilaç kullanımları açısından karşılaştırıldı. Bulgular: DE ve DEA hastaları arasında ÖGD veya kolonoskopide endoskopik lezyon sıklığı açısından anlamlı fark bulunmamaktadır. Malignite sıklığı ise DEA hastalarında, DE hastalarına kıyasla anlamlı düzeyde daha yüksektir (p=0,004). Kronik hastalıklardan hipertansiyon (HT), diabetes mellitus (DM), aterosklerotik kalp hastalığı (ASKH), atriyal fibrilasyon (AF) ve kronik böbrek hastalığı (KBH) DEA hastalarına DE hastalarına göre anlamlı düzeyde daha fazla eşlik etmektedir. DEA hastalarında DE hastalarına göre CRP değeri anlamlı düzeyde yüksektir (p<0,001). Malignite eşlik eden DEA hastalarında CRP ortancaları, malignite eşlik etmeyen DEA hastalarına kıyasla anlamlı düzeyde daha yüksektir (p<0,001). Proton pompa inhibitörü (PPİ), ASA, klopidogrel, varfarin, yeni oral antikoagülan ilaçlar ve PPİ'lerin ASA ve varfarin ile kombinasyonlarının kullanımı DEA hastalarında anlamlı düzeyde daha yüksekti. DEA hastalarında geriatrik yaş grubunda kolonoskopide lezyon saptanma sıklığı erişkin yaş grubuna kıyasla anlamlı düzeyde yüksek saptandı (p=0,012). Sonuç: DE ve DEA hastaları arasında endoskopik lezyon sıklığı açısından fark bulunmamaktadır. Ancak malignite riski DEA hastalarında istatistiksel olarak anlamlı düzeyde daha yüksektir. 50 yaş ve üzerinde hem DE hem de DEA hastalarında ÖGD ve kolonoskopi birlikte yapılmalıdır. On the other hand, it has been clearly demonstrated that endoscopic examinations should be performed in patients with iron deficiency anemia (IDA), while there is no consensus on this issue for iron deficiency (ID) patients. The aim of this study was to determine the prevalence of severe gastrointestinal lesions and gastrointestinal malignancies, identified by endoscopy, by comparing IDA patients with ID patients without anaemia. Patients and Methods: The data of patients with iron deficiency who underwent endoscopy between 01 / 2014 - 01 / 2018 in the endoscopy unit of the Gastroenterology Department of Ankara University Medical Faculty were retrospectively reviewed. In our study, 571 iron deficiency patients in adult and geriatric age groups were included. Patients were divided into two groups according to the presence of anemia. 292 patients with IDA and 279 patients with ID were compared in terms of esophagogastroduodenoscopy (EGD) and colonoscopy findings, demographic characteristics, chronic diseases and drug use. Results: There was no significant difference between patients with ID and IDA in terms of the incidence of endoscopic lesion in EGD or colonoscopy. The prevalence of malignancy was significantly higher in patients with IDA compared to patients with ID (p = 0.004). Among chronic diseases, hypertension (HT), diabetes mellitus (DM), atherosclerotic heart disease (AHD), atrial fibrillation (AF) and chronic kidney disease (CKD) are associated with IDA patients significantly more than ID patients. CRP levels were significantly higher in patients with IDA than patients with ID (p <0.001). CRP median in IDA patients with malignancy was significantly higher than in IDA patients without malignancy (p <0.001). The use of proton pump inhibitor (PPI), ASA, clopidogrel, warfarin, new oral anticoagulant drugs, and combinations of PPIs with ASA and warfarin were significantly higher in patients with IDA. The incidence of colonoscopy lesion in the geriatric age group was significantly higher in patients with IDA than in the adult age group (p = 0.012). Conclusion: There was no difference in the incidence of endoscopic lesions between patients with ID and IDA. However, the risk of malignancy is significantly higher in IDA patients. EGD and colonoscopy should be performed together in both IDA and ID patients aged 50 years and older. In geriatric patients, if anemia is present, all GIS should be evaluated endoscopically, even if a lesion to explain anemia is detected in the EGD or colonoscopy.     Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
