    La vaccinazione antinfluenzale: analisi e monitoraggio delle campagne vaccinali nell’ASL 3 di Nuoro

    L’influenza è responsabile di epidemie a cadenza stagionale con possibilità di complicanze, anche gravi, per alcune categorie a rischio. Attualmente la vaccinazione rappresenta lo strumento più efficace per la prevenzione del rischio correlato all’infezione virale. In questo lavoro viene analizzata l’efficacia delle campagne vaccinali di prevenzione attuate nei quattro distretti della Asl n. 3 di Nuoro per verificare il raggiungimento degli obiettivi di copertura per le categorie a rischio. È stata esaminata, in particolare, la variazione del numero di soggetti vaccinati precedentemente e successivamente all’inclusione, nel 2012, della fascia d’età degli ultrasessantaquattrenni nel calendario vaccinale. Sono stati considerati i dati di pertinenza relativi agli anni compresi tra il 2010 ed il 2014 e confrontato l’impatto economico per il Servizio Sanitario Regionale (SSR) nel periodo in esame. L’analisi mette in luce un incremento apprezzabile degli ultrasessanquattrenni vaccinati (+12%) a partire dal 2012. Per contro, nel complesso si registra una flessione dei soggetti vaccinati, probabilmente legata ad effetti dissuadenti della disinformazione da parte dei mass media su presunti danni conseguenti alle vaccinazioni. Il mancato raggiungimento degli obiettivi ottimali di copertura vaccinale richiede maggiore impegno da parte del Ministero della Salute e degli operatori sanitari e un miglioramento della incisività delle campagne di informazione istituzionale.Influenza is responsible for seasonal epidemic outbreaks with the possibility of complications for certain risk groups, often severe. Currently, vaccination is the most effective instrument for risk prevention related to viral infection. In the present paper, we analyzed the effectiveness of preventive vaccination campaigns delivered in the four districts of ASL n. 3 of Nuoro (Local Health Organization in Nuoro, Sardinia, Italy) in order to verify the achievement of coverage targets for risk groups. In particular, we examined changes in the number of vaccinated subjects before and after the inclusion of the over65yearold group in the Vaccination Schedule in 2012. We considered the period 2010 2014 and compared the economic impact for the SSR (Regional Health Service) in the reviewed term. The present study highlights a notable increase of over65 vaccinated subjects from 2012 (+12%). However, we noticed a recourse of vaccination’s decrease among the total vaccinated population in the overall period, probably due to the dissuasive effects of disinformation about presumed damage subsequent to vaccination given by the media. Failure in achieving the optimized vaccination coverage objectives requires more active commitment from the Italian Health Minister and health professionals, and furthermore improvement of incisiveness in the information campaigns

    Reproducibility of liver iron concentration measured on a biopsy sample: A validation studyin vivo

    Determination of liver iron concentration is essential to predict iron related tissue damage and to guide chelation therapy. To assess the reliability of a single biopsy iron concentration determination in representing the whole liver iron concentration, we conducted a prospective study performing two immediately successive liver biopsies from 61 noncirrhotic, iron overloaded thalassemia patients, directing the needle to different direction from the same skin cut. The correlation among sample biopsies was determined by both regression analysis and the Bland-Altman method. The results showed that overall correlation between the two samples was high (Pearson's coefficient of correlation r = 0.970, P < 0.0001; 95% CI 0.951-0.981; R(2) = 0.941). To evaluate if sample dimension had an impact on the analysis we analyzed separately biopsy couples were both sample gross weight were ≥1 mg dry weight (n = 16) from the others [one or both had a specimen gross weight <1 mg dry weight (n = 45)]. In the first case, correlation coefficient r was equal to 0.998 (P < 0.0001; 95% CI: 0.995-0.999; R(2) = 0.996) while in the latter was 0.960 (P < 0.0001; 95% CI: 0.928-0.977; R(2) = 0.921). In no instance second specimen prediction interval was outside the interval implying different prognostic and therapeutic decision if both liver samples gross weight were ≥1 mg dry weight. The Bland-Altman plot analysis showed the same trend observed using Pearson's correlation coefficient in the analyzed sample categories. Hepatic iron concentration determined in "good quality" biopsy specimen (i.e. sample gross weight ≥1 mg dry weight) is a reliable indicator of whole liver iron concentration
