    Redifferentiation Therapies in Thyroid Oncology: Molecular and Clinical Aspects

    Since the 1940s, 131-I radioiodine therapy (RIT) has been the primary treatment for metastatic differentiated thyroid cancer (DTC). Approximately half of these patients respond favorably to RIT, achieving partial or complete remission or maintaining long-term stable disease, while the other half develop radioiodine-refractory DTC (RAI-R DTC). The main genomic alteration involved in radioiodine resistance is the activated mitogen-activated protein kinase (MAPK) pathway, which results in the loss of sodium iodide symporters (NIS). Therefore, RAI-R DTC requires alternative treatment options such as tyrosine kinase inhibitors. Over the past decade, several studies have investigated pharmacological induction or enhancement of NIS expression through “redifferentiation” therapies, mainly targeting the MAPK pathway. These novel approaches can restore radioiodine sensitivity in previously refractory patients and, therefore, potentially reestablish the efficacy of RIT.     Theranostics of thyroid cancer

    Molecular imaging is pivotal in evaluating and managing patients with different thyroid cancer histotypes. The existing, pathology-based, risk stratification systems can be usefully refined, by incorporating tumor-specific molecular and molecular imaging biomarkers with theranostic value, allowing patient-specific treatment decisions. Molecular imaging with different radioactive iodine isotopes (ie, I131, I123, I124) is a central component of differentiated carcinoma (DTC)'s risk stratification while [18F]F-fluorodeoxyglucose ([18F]FDG) PET/CT is interrogated about disease aggressiveness and presence of distant metastases. Moreover, it is particularly useful to assess and risk-stratify patients with radioiodine-refractory DTC, poorly differentiated, and anaplastic thyroid cancers. [18F]F-dihydroxyphenylalanine (6-[18F]FDOPA) PET/CT is the most specific and accurate molecular imaging procedure for patients with medullary thyroid cancer (MTC), a neuroendocrine tumor derived from thyroid C-cells. In addition, [18F]FDG PET/CT can be used in patients with more aggressive clinical or biochemical (ie, serum markers levels and kinetics) MTC phenotypes. In addition to conventional radioiodine therapy for DTC, new redifferentiation strategies are now available to restore uptake in radioiodine-refractory DTC. Moreover, peptide receptor theranostics showed promising results in patients with advanced and metastatic radioiodine-refractory DTC and MTC, respectively. The current appropriate role and future perspectives of molecular imaging and theranostics in thyroid cancer are discussed in our present review

    The social context of residential burglary: exploring neighborhood level risk factors in the city of Bursa, Turkey

    This doctoral dissertation evaluated neighborhood level social environmental characteristics and their effects on creating residential burglary risk in Bursa, Turkey. In doing so, it follows the social disorganization and environmental criminology frameworks. The dissertation describes a multi-method study. Quantitative analysis focused on three separate issues. First, Census and police crime data were used in a bivariate and multivariate neighborhood level analysis. Neighborhood level social disorganization measures correlate strongly with the burglary rate. Second, feature environmental geocoded data were obtained and the key variables were treated as risk factors for the Risk Terrain Modeling (RTM) and traditional crime hot spot analysis. RTM is a better predictor of future burglaries when compared to hot spot analysis. Third, 165 neighborhoods of Bursa were coded into 4 categories according to physical structure: shantytowns, single family housing, adjacent type buildings, and multi-family apartment complexes. The neighborhood level burglary risk was regressed on the neighborhood type. Results indicate that type of neighborhood is significantly related to neighborhood level criminogenic risk. This dissertation also explored the role of neighborhood’s social characteristics by adding field observations of these differing social contexts. The researcher focused on indicators of risk and the social environmental context of four neighborhood types. The study found that single family housing neighborhoods have the lowest risk of residential burglary. These findings have important implications for interventions in urban settings, providing insights into the role that neighborhood characteristics can have on these encouraging crime and social disorderPh.     Comparison of clinical and PET-derived prognostic factors in patients with non-Hodgkin lymphoma: a special emphasis on bone marrow involvement

    tuncel, murat/0000-0003-2352-3587Objective in patients with non-Hodgkin lymphoma (NHL), we investigated F-18 FDG PET/computed tomography (CT) parameters, clinical findings, laboratory parameters, and bone marrow involvement (BMI) status for predictive methods in progression-free survival (PFS) and overall survival (OS), and whether F-18 FDG PET/CT could take the place of bone marrow biopsy (BMB). Methods the performance of F-18 FDG PET/CT (BMPET) was evaluated. the prognostic value of maximum standardized uptake value (SUVmax), metabolic tumor volume (MTV), stage, international prognostic index (IPI) score, IPI risk, lactate dehydrogenase (LDH), B2 microglobulin, Ki67 proliferation index, and the presence of BMI was evaluated for OS and PFS. Kaplan-Meier curves were drawn for each designated cutoff value, and 5-year PFS and 7-year OS were evaluated using log-rank analysis. Results the sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy of BMPET and BMB to identify BMI were 69, 100, 86.1, 80, 100%, and 81.6, 100, 92.5, 89, 100%, respectively. the sensitivity, specificity, PPV, NPV, and accuracy of BMPET in patients with Ki67- proliferation index >25% were all 100%. BMPET, IPI risk, MTV, and LDH were found to be independent prognostic predictors for PFS, whereas BMPET, SUVmax, and MTV for OS. Five-year PFS analysis estimated as follows: BMPET (+) = 22%, BMPET (-) = 80%, LDH 437 (U/L) = 51%, MTV 56 (cm(3)) = 49%, low IPI risk = 87%, intermediate IPI risk = 69%, high IPI risk = 25%. Seven-year OS analysis was found as: SUVmax 17.6 = 48%, MTV 56 (cm(3)) = 45.8%, BMPET (-) = 72.5%, BMPET (+) = 42%. Conclusion in the Ki-67 proliferation index > 25% group, F-18 FDG PET/CT was able to differentiate BMI independently from NHL subgroups. We recommend using this method with large patient groups. MTV and BMPET were independent prognostic indicators for OS and PFS and may help to determine high-risk patients
