"Practical Knowledge" and Perceptions of Antibiotics and Antibiotic Resistance Among Drugsellers in Tanzanian Private Drugstores.
Studies indicate that antibiotics are sold against regulation and without prescription in private drugstores in rural Tanzania. The objective of the study was to explore and describe antibiotics sale and dispensing practices and link it to drugseller knowledge and perceptions of antibiotics and antibiotic resistance. Exit customers of private drugstores in eight districts were interviewed about the drugstore encounter and drugs bought. Drugsellers filled in a questionnaire with closed- and open-ended questions about antibiotics and resistance. Data were analyzed using mixed quantitative and qualitative methods. Of 350 interviewed exit customers, 24% had bought antibiotics. Thirty percent had seen a health worker before coming and almost all of these had a prescription. Antibiotics were dispensed mainly for cough, stomachache, genital complaints and diarrhea but not for malaria or headache. Dispensed drugs were assessed as relevant for the symptoms or disease presented in 83% of all cases and 51% for antibiotics specifically. Non-prescribed drugs were assessed as more relevant than the prescribed. The knowledge level of the drugseller was ranked as high or very high by 75% of the respondents. Seventy-five drugsellers from three districts participated. Seventy-nine percent stated that diseases caused by bacteria can be treated with antibiotics but 24% of these also said that antibiotics can be used for treating viral disease. Most (85%) said that STI can be treated with antibiotics while 1% said the same about headache, 4% general weakness and 3% 'all diseases'. Seventy-two percent had heard of antibiotic resistance. When describing what an antibiotic is, the respondents used six different kinds of keywords. Descriptions of what antibiotic resistance is and how it occurs were quite rational from a biomedical point of view with some exceptions. They gave rise to five categories and one theme: Perceiving antibiotic resistance based on practical experience. The drugsellers have considerable "practical knowledge" of antibiotics and a perception of antibiotic resistance based on practical experience. In the process of upgrading private drugstores and formalizing the sale of antibiotics from these outlets in resource-constrained settings, their "practical knowledge" as well as their perceptions must be taken into account in order to attain rational dispensing practices
Baptism and Circumcision: The Relation to Judaism in the Belgic Confession
The relation to Judaism is not a theme in Guy de Bres’ Belgic Confession. Yet article 34 on baptism contains both repeated positive comparisons with circumcision and negative evaluations of the Jewish law. The tension here visible can be understood in light of the violent repression of religious dissent by the Spanish rulers in the 16th century. The Anabaptists, most persecuted of all, were also targeted by the Calvinists; their ‘errors’ are denounced three times in the Belgic Confession. It appears that the anti-Anabaptist stance motivated the comparison with circumcision since this is practised on infants. As compared with Calvin, de Bres shows a more sympathetic attitude to the Jews especially in La racine, source fondement et des Anabaptistes (1565). Interdialytic weight gain and 48-h blood pressure in haemodialysis patients
Background. Hypertension, which is often associated with hypervolaemia, is
common in haemodialysis patients and is a known determinant of target organ
damage. Interdialytic weight gain due to volume overload has also been
associated with mortality in haemodialysis patients. Methods. We therefore
studied 27 chronic haemodialysis patients who underwent 48-h ambulatory
blood pressure monitoring between two midweek dialysis sessions, and 2D and
M-mode echocardiography for determination of left ventricular mass index.
Results. Left ventricular hypertrophy (left ventricular mass index in men >
131 g/m(2), women > 100 g/m(2)) was present in 70% (19/27) patients despite
a mean 48-h blood pressure of 132 (plus or minus) 19/81 (plus or minus) 15
mmHg. Mean interdialytic weight gain was 1.6 (plus or minus) 0.8 kg and was
not related to left ventricular mass index. Two patterns of interdialytic
blood pressure change were apparent: in group 1 (16 patients) 48-h blood
pressure increased (+ 19 (plus or minus) 12/13 (plus or minus) 9 mmHg),
whereas in group 2 (11 patients) blood pressure fell (- 10 (plus or minus)
13/-8 (plus or minus) 10 mmHg P < 0.0001). In both groups the number of
hypertensive patients (group 1, 10/16; group 2, 6/11), the 48-h blood
pressure (132 (plus or minus) 20/80 (plus or minus) 15 vs 132 (plus or
minus) 18/82 (plus or minus) 15 mmHg) and interdialytic weight gain (+ 1.9
(plus or minus) 0.7 vs + 1.3 (plus or minus) 0.7 kg) were similar. There
was also no correlation between interdialytic blood pressure change and
weight gain in either group. Conclusions. We conclude that interdialytic
blood pressure changes cannot be directly related to interdialytic fluid
gain, even in apparent volume-dependent hypertension, emphasizing the
importance of additional factors in the control of blood pressure in
end-stage renal disease

Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more
sophisticated methods
