Abstract: Minimally invasive video-assisted thyroidectomy (MINAT) was described in 1998. In this study we collected the experience of four third-level referral centers that adopted this technique. A total of 336 patients (279 females, 57 males) were selected for MINAT. Selection criteria were thyroid volume < 15 ml, nodules not exceeding 3.5 cm of diameter, and an absence of thyroiditis, previous neck surgery, or previous irradiation. The procedure, totally gasless, is carried out through a 15 rum central incision above the sternal notch. Dissection is performed under endoscopic vision using conventional and endoscopic instruments. The mean operating time was 69.4 +/- 30.6 minutes for lobectomy (range 20-150 minutes) and 87.4 +/- 43.5 minutes for total thyroidectomy (range 30-220 minutes). The mean postoperative stay was 1.9 +/- 0.8 days. Postoperative complications were 7 transient and I definitive recurrent nerve palsies and 11 cases of hypoparathyroidism (9 transient, 2 definitive). Conversion to open surgery was necessary in 15 patients (4.5%). This study confirms in a large number of cases the safety and feasibility of MINAT, even in different surgical settings where similar results were achieved. The complication rate was not different from that of standard thyroidectomy. Although the operating time appears longer than with conventional procedures, the learning curve demonstrates a sharp decrease with increasing experience and the introduction of new technologies. The number of patients eligible for this approach remains low, thereby limiting its use, but it should be considered a valid option in selected surgical centers, offering some advantages to patients in terms of cosmetic results and postoperative distress

He is the author of The Trade between Egypt and Bilad As-Sudan (IFAO, 1978) as well as co-editor, with Kenneth Cuno, of Race and Slavery in the Middle East: Histories of Trans-Saharan Africans in Ninteenth Century Egypt, Sudan, and the Ottoman Mediterranean (AUC Press, 2010) as well as the author of numerous articles on the history of slavery. In this Qahwa and Kalam talk he will discuss several letters purportedly written by slaves and found in a private family archive in Egypt, which he is currently studying
Variáveis associadas ao desempenho cognitivo tardio de pacientes com traumatismo crânio-encefálico grave
Tese (doutorado) - Universidade Federal de Santa Catarina, Centro de Ciências da Saúde. Programa de Pós-Graduação em Ciências MédicasObjetivos: O trauma cranioencefalico (TCE) e uma das principais causas de mortalidade e morbidade. Ha raros estudos prospectivos que investigam a associacao de variaveis clinicas e laboratoriais da fase aguda do TCE e o prognostico cognitivo tardio dos pacientes vitimas de TCE. Este estudo tem como objetivo identificar variaveis clinicas, laboratoriais e biomarcadores de lesao tecidual associados ao prognostico cognitivo em pacientes vitimas de TCE. Metodos: Foram coletadas prospectivamente as variaveis da internacao hospitalar de 234 pacientes consecutivos com TCE grave (GCS admissao . 8). Dos 172 sobreviventes, uma amostra representada de 46 pacientes realizaram avaliacao cognitiva (composta de 15 testes neuropsicologicos) em media 3 (+ - 1,8) anos apos a hospitalizacao. Um sub-grupo de 22 pacientes que foram avaliados cognitivamente realizaram analise dos niveis plasmaticos de TBARS (indicativo de dano por estresse oxidativo a lipideos) e Carbonil (indicador de dano por estresse oxidativo a proteinas) em amostras de sangue coletadas na fase aguda de TCE (mediana de 10, 30 e 70 horas apos o impacto do TCE). Um grupo controle (n=23) pareado por sexo, idade e nivel socio-educacional foi avaliado cognitivamente para comparacao com os pacientes. Resultados: A media de idade dos pacientes foi 34 (+ - 13) anos sendo 85% do sexo masculino, com escolaridade media de 9 (+ - 4,7) anos. Os pacientes apresentaram um desempenho inferior em todos os testes neuropsicologicos. A analise por regressao linear evidenciou uma forte associacao independente (R coeficiente = 0,6 a 0,8) entre maior escolaridade e menor idade e o desempenho cognitivo em 14 dos 15 testes neuropsicologicos avaliados. O desempenho nos testes cognitivos nao esteve associado ao genero, escore de admissao na Escala de Coma de Glasgow (ECG), exame das pupilas, presenca de trauma em outros orgaos, e classificacao da escala de Marshall na tomografia computadorizada na admissao (TC). Niveis elevados de glicose e presenca de hemorragia sub-aracnoide na TC mostraram-se independentemente associados a um menor desempenho no teste de Retencao de Aprendizagem de Rey e de Memoria Logica respectivamente. Embora os niveis plasmaticos de TBARS e Carbonil tenham sido significativamente elevados na fase aguda do TCE, estes biomarcadores nao se mostraram associados ao desempenho cognitivo dos pacientes. Conclusoes: Baixa escolaridade e idade mais avancada sao preditores independentes de pior desempenho cognitivo tardio apos o TCE grave. O exame de TC e glicemia mostraram limitada capacidade de predicao do desempenho cognitivo enquanto que o exame das pupilas, ECG na admissao, presenca de trauma associado nao foram preditores do desempenho em nenhum dos testes neuropsicologicos avaliados. A medida dos niveis plasmaticos de TBARS e Carbonil tambem nao se mostrou associada com o desempenho cognitivo dos pacientes. A identificacao de variaveis clinicas e laboratoriais associadas ao prognostico cognitivo apos o TCE grave permanece um desafio para a area de neuropsicologia clinica

This session features live demos by expert users or creators of a selection of no-cost (some freemium) platforms and/or collaborative communities, including: VTechWorks, Merlot, Open Textbook Library, OER Commons, VT’s Odyssey learning object repository, Overleaf, Pressbooks, and Rebus Community for Open Textbook Creation. This event was offered during Open Education Week 2017.

A Library Viewpoint: Exploring Open Educational Practices
There are a wide range of faculty responses to open; everything from curiosity to resistance. Some of these reactions are due to professional tradition, guild thinking, and lack of awareness about open. Academic libraries offer a unique context for exploring open. They do not fall prey to disciplinary concerns and, as such, are hotbeds of collaboration and innovation. It is no wonder that libraries are a natural home for open educational philosophy. In this chapter, author Anita Walz offers her personal insights into the current state of open education. She candidly shares her thoughts on faculty adoption of OERs, the current cost of learning resources, and the promise of open pedagogy

Kampf um die rechte Ordnung: Sozialistische Strafverteidiger zwischen spätem Kaiserreich und "zweiter Revolution" 1919
Anhand einer Anwaltsgruppe um Kurt Rosenfeld und Theodor Liebknecht wird gezeigt, wie sozialistische Anwälte Gerichtsverfahren sowie deren öffentliche Wahrnehmung mitprägten und im Sinne der eigenen politischen Bewegung nutzbar machen konnten. Wie setzten sie juristische Verfahrensmöglichkeiten für weitergehende Ziele ein? Nach Hinweisen zu den Ursprüngen dieses Anwaltstypus und den biographischen Hintergründen der Anwälte fokussiert der Beitrag das Fallbeispiel der Berliner "Spartakusprozesse". In diesen Verfahren gegen (angebliche) Beteiligte am Januaraufstand 1919 klagten die untersuchten Anwälte die Rechtsprechung, die neue Regierung und die bewaffnete Macht an. Schon hier stellten sie die für die Weimarer Republik zentralen Fragen nach der Legitimität und Legalität der Regierung sowie nach einer alternativen Ordnung. Deutlich wird ferner der besondere Resonanzraum für politische Strafverteidigung in Umbruchszeiten und für Anwälte, die zugleich als Politiker öffentlichkeitswirksam arbeiteten. Sozialistische Anwälte können folglich nicht auf die Rolle von Kronzeugen gegen die einseitige Justiz der Weimarer Republik reduziert werden, sondern sie waren selbstbewusste Akteure, die zur Debatte um den rechtlichen und politischen Charakter der neuen Ordnung wesentlich beitrugen.With particular reference to a group of defence lawyers around Kurt Rosenfeld and Theodor Liebknecht, the article demonstrates how socialist lawyers shaped court proceedings as well as their public perception and were able to harness them for the purposes of their own political movement. How did they utilise legal procedures for broader objectives? After providing some information on the origin of this type of lawyer and the biographical backgrounds of those involved, the article focuses on the trials following the 'Spartacist Revolt'. During these trials of (alleged) participants in the January uprising of 1919, the above-mentioned group of defence lawyers accused the judiciary, the new government and the armed forces. They were already posing questions here that were crucial for the Weimar Republic, concerning the legitimacy and legality of the government and about an alternative political order. Furthermore, the special scope of impact of political criminal defence in times of upheaval and of lawyers working also as politicians in a high-visibility capacity becomes apparent. Socialist lawyers can therefore not be reduced to their role as principal witnesses against the biased judicial system of the Weimar Republic. They were self-aware protagonists making a significant contribution to the debate on the legal and political character of the new order
