    학위논문(석사)--아주대학교 일반대학원 :의학과,2015. 2연구배경 및 목적 수근관증후군(carpal tunnel syndrome, CTS)은 가장 흔한 단일신경병으로 비교적 높은 유병률에도 불구, 치료 방법 선택에 관한 명확한 지침이 없어 실제 임상에서 혼선이 있는 실정이다. 본 연구의 목적은 전기생리학적 검사로 확진된 수근관증후군 환자의 수술적 치료 선택에 미치는 요인들을 분석하여 수술적 치료의 지침에 도움을 주고자 함이다. 특히 실제 진료현장에서 수술적 요법을 선택할 때 영향을 주는 여러 요인들을 다면적으로 고려하여 최대한 진료 현실을 반영하도록 하였다. 연구대상 및 연구방법 2013년 1월부터 2014년 3월까지 대학병원 신경과 근전도실에서 신경전도검사 및 근전도검사를 시행 받은 환자들 중, 포함기준을 만족한 247명의 환자들에 대하여 전기생리학적 척도와 수근관증후군의 중증도 (electrophysiologic grading of CTS severity) 및 여러 임상적인 지표에 대하여 조사하였다. 통계학적 분석을 통하여, 수술 여부에 따라 여러 척도의 차이가 있는지, 또 수술적 요법을 시행 받는 데에 관여하는 위험요인에 관하여 로지스틱 회귀분석모델(logistic regression model)을 구축하였다. 증상발생으로부터 수술적 요법을 시행 받을 때까지의 시간을 변수로 하여 Kaplan-Meier 생존률 분석을 하고, 이와 관련이 있는 요소는 Cox회귀분석모델을 구축하여 알아보았다. 결 과 수근관유리술을 시행받은 군에서 그렇지 않은 군보다 Canterbury grade 4 이상인 경우가 더 많았다(p=0.036; 승산비=1.989). 임상적인 지표에서는 수술 받은 군에서 그렇지 않은 군에 비해 주요 진료과, 약물투여 후 증상호전 여부, 동반된 수부질환 여부, 증상지속기간에서 유의한 차이를 보였다(p<0.05). 수근관증후군 환자들이 수술을 시행 받을 위험성을 분석한 Cox회귀분석모델 결과는 주요 진료과가 정형외과인 경우 타과 진료에 비해 보정된 위험도(adjusted HR)가 6.609배(p<0.001), 약물 투여 후 증상호전이 없거나 부작용이 있을 경우에는 그렇지 않은 경우에 비해서 1.890배(p=0.035) 높았다. 수술적 치료를 선택한 환자를 대상으로 주요 진료과에 따른 예후의 유의한 차이는 없었다 (p=0.679). 결론 및 제언 본 연구에서는 주요 진료과 또는 약물투여에 대한 반응 정도 등의 임상적 지표가 수술적 치료방법 선택에 큰 영향을 줌을 확인하였다. 또한, 수근관증후군 환자에서 생존분석을 통한 위험도분석을 시도하였다. 위 결과에서 도출된 수술적 치료 선택에 미치는 요인을 바탕으로, 수술 후 예후도 살펴보았다. 주요 진료과에 따라 수술적 치료방법이 결정됨에도 불구하고 수술 후 예후의 차이를 보이지 않았기 때문에, 충분한 추적관찰 이후 수술적 치료를 선택하는 것이 바람직할 것으로 판단된다.차 례 국문요약 ⅰ 차례 ⅱ 그림차례 ⅲ 표차례 ⅳ Ⅰ. 서론 1 Ⅱ. 연구대상 및 방법 3 A. 연구대상 3 B. 전기생리학적 검사방법 및 전기생리학적 중증도의 분류방법 4 C. 통계학적 분석 6 Ⅲ. 결과 8 Ⅳ. 고찰 및 결론 12 참고문헌 27 ABSTRACT 33Maste

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed.     Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Appropriate Similarity Measures for Author Cocitation Analysis

    We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature.     Dispelling the Myths Behind First-author Citation Counts

    We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more sophisticated methods

    We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
