Sternal force distribution during median sternotomy retraction
BackgroundMedian sternotomy is the access of choice in cardiac surgery. Sternal retractors exert significant forces on the thoracic cage and might cause considerable damage. The aim of this study was to determine the effects of retractor shape on local force distribution to obtain criteria for retractor design.MethodsTwo types of sternal retractors (straight [SSR] and curved [CSR]) were equipped with force sensors. Force distribution, total force, and displacement were recorded to a spread width of 10 cm in 18 corpses (11 males and 7 females; age, 62 ± 12 years). Both retractors were used in alternating sequence in 4 iterations in every corpse. Data were compared with respect to the different retractor blade shapes.ResultsMaximum total forces for full retraction of both retractors resulted in 349.4 ± 77.9 N. Force distribution during the first retraction for the cranial/median/caudal part of the sternum was 101.5 ± 43.9/29.1 ± 33.9/63.0 ± 31.4 N for the SSR and 38.7 ± 41.3/80.9 ± 64.5/34.0 ± 25.8 N for the CSR, respectively. During the 4 spreading cycles, the average force decreased from 224.6 ± 61.3 N in the first to 110.8 ± 39.8 N in the fourth iteration. The mean total force for the first retraction revealed 226.4 ± 71.9 N for the CSR and 222.8 ± 52.9 N for the SSR.ConclusionsThe shape of sternal retractors considerably influences the force distribution on the sternal incision. In the SSR, forces on the cranial and caudal sternum are significantly higher than in the median section, whereas in the CSR, forces in the median section are highest
Philipp Melanchthon
Philipp Melanchthon (1497–1560) was, with Martin Luther, the most influential reformer of the church during the 16th century. He was also a reformer of university education, especially theological studies, as well as the school system in Germany. He was responsible for a theological curriculum that included Greek, Hebrew, and philosophy. He, as a professor of Greek at the University of Wittenberg since 1518, was the author of the first generally accepted Protestant confession, known as the Confessio Augustana (1530). He also wrote the first Protestant commentaries on Paul’s letter to the Romans (1519), as well as the first Protestant handbook in systematic theology (1521). He was the main negotiator of the Protestant movement during the diets and religious discussions with the Roman Catholic Church. He is known as the ‘teacher of Germany and Europe’ and is respected as the father of the ecumenical movement. Yet, Melanchthon is not known to South Africans and especially Afrikaans-speaking people who, traditionally, have close links with the reformational tradition. There is not yet one single publication on Melanchthon in Afrikaans or by a South African scholar, making this book, therefore, the first by an Afrikaans-speaking scholar on Melanchthon

Therapy options in deep sternal wound infection: Sternal plating versus muscle flap.
Management of deep sternal wound infection (DSWI) in cardiac surgical patients still remains challenging. A variety of treatment strategies has been described. Aim of this cohort study was to analyse two different treatment strategies for DSWI: titanium sternal plating system (TSFS) and muscle flap coverage (MFC).Between January 2007 and December 2011, from 3122 patients undergoing cardiac surgery 42 were identified with DSWI and treated with one of the above mentioned strategies. In-hospital data were collected, follow-up performed by telephone and assessment of Quality of Life (QoL) using the SF-12 Health Survey Questionnaire.20 patients with deep sternal wound infection were stabilized with TSFS and 22 patients treated with MFC. Preoperative demographics and risk factors did not reveal any significant differences. Patients treated with TSFS had a significantly shorter operation time (p<0.05) and shorter hospitalization (p<0.05). A tendency towards lower mortality rate (p = n.s.) and less re-interventions were also noted (plating 0.6 vs. flap 1.17 per patient, n.s.). Quality of Life in the TSFS group for the physical-summary-score was significantly elevated compared to the MFC group (p<0.05). Relating to chest stability and cosmetic result the treatment with TSFS showed superior results, but the usage of MFC gave the patients more freedom in breathing and less chest pain.Our results demonstrate that the use of TSFS is a feasible and safe alternative in DSWI. However, MFC remains an absolutely essential option for complicated DSWI since the amount of perfused tissue can be the key for infection control

Interpreting cynical beliefs about others
A growing number of studies suggest that individuals are cynical about others’ behavior. But these findings often rely on self-reported rather than actual behavior as benchmark. A well-documented limitation of self-reports is their tendency to overstate good behavior. I introduce a simple, portable test to assess the extent to which inattention to others’ potential misreporting drives apparently cynical beliefs about stated behavior. Drawing people’s attention to the possibility of misreporting in self-reports increases beliefs about others’ stated desirable climate and health behaviors by an average of 0.33 standard deviations, substantially reducing apparent cynicism
