Abstract
Sepsis
is a life-threatening dysfunction of the body that causes a host to
respond incorrectly to an infection. Sepsis and septic shock are a
major health issue affecting millions of people each year worldwide.
Every fourth person with sepsis dies. Multi-organ trauma, acute
myocardial infarction or stroke, early diagnosis and management in
the first hours after the onset of sepsis improve survival rate [1].
The Sequential Organ Failure Assessment (SOFA) scale is mainly used
to assess sepsis. SOFA helps medical staff to assess the risk of
morbidity and mortality due to sepsis. The basic parameters of SOFA
are: assessment of the respiratory system based on partial oxygen
pressure in the blood (PaO2), assessment of the nervous system based
on the Glasgow Coma scale (GCS), assessment of the cardiovascular
system based on the average blood pressure or after vasopressor
administration (any dose), assessment of liver function based on the
level of bilirubin in the blood, assessment of kidney function based
on the level of creatinine in the urine, assessment of blood clotting
based on the amount of thrombocytes contained in the plasma. This
scale is used in hospital settings. qSOFA (Quick Sequential Organ
Failure Assessment score) is a simplified version of the SOFA score
as the first way to identify high-risk patients due to poor results
associated with infection. qSOFA simplifies the SOFA score
drastically, taking into account only three clinical criteria and
introducing "any change" instead of requiring GCS ≤13. It
uses three criteria, assigning one point for low blood pressure (SBP
≤100 mmHg), high respiratory rate (≥ 22 breaths per minute) or
changed mentation (GCS <15). qSOFA is a predictor of mortality,
not a diagnostic test for sepsis.

Trauma or Paradise Lost? Reminiscing Polish People’s Republic in Dorota Terakowska’s Ono
Artykuł stanowi próbę opisania sposobów funkcjonowania przeszłości w pamięci bohaterów powieści Ono Doroty Terakowskiej, zwłaszcza postrzegania przez nich czasów Polskiej Rzeczpospolitej Ludowej. Wydarzenia z tego okresu pozostały głęboko zakorzenione w ich świadomości, jednak Teresa, Jan oraz Irena diametralnie różnią się sposobem zarówno postrzegania ówczesnej rzeczywistości, jak i przekazywania swych wspomnień innym. Po scharakteryzowaniu właściwych tym postaciom typów pamięci PRL oraz omówieniu ich przy użyciu kategorii traumy uwaga zostaje przeniesiona na Ewę – główną bohaterkę książki, która musi mierzyć się ze swoją własną traumą, jaką stanowi gwałt. Interpretacja tego wątku pozwala m.in. pokazać, że fikcyjne wydarzenia pisarka wykorzystała, by wypowiedzieć ważne prawdy dotyczące przemocy o charakterze seksualnym, a szczególnie jej społecznego postrzegania. Rozważania dotyczące (traumatycznej) przeszłości zostają uzupełnione o wątek autobiograficzny, co umożliwia paralelna lektura Ono oraz quasi‑politycznego, autobiograficznego utworu Terakowskiej, zatytułowanego Guma do żucia.This article is an attempt to describe the ways in which the past – the especially Polish People’s Republic (Polska Rzeczpospolita Ludowa – PRL) – performs in the memory of the protagonists of Dorota Terakowska’s novel Ono. Teresa, Jan and Irena vary in both perceiving those realities and passing these memories on to their descendants. After discussing different types of the memory of PRL and describing them using the concept of trauma the author focuses on Ewa – the main protagonist of the book in question. She has to deal with her own trauma, which is a consequence of rape. Shedding light on this motif enables one to see that Terakowska used fictional situations to voice some serious observations concerning the real problem of sexual assault and its social perception. Those remarks are supplemented with autobiographicalelement provided by parallel reading of Ono and Guma do żucia (Terakowska’s autobiographical quasi‑political text)

Engineering embryonic stem cells for myelin cell therapy:
Cell replenishment using stem cell-derived transplants holds great promise for therapy in human disease. However, clinical therapeutics in the central nervous system (CNS) requires targeted migration for lesion repair. For CNS grafts, homing the graft population to the appropriate site for tissue repair is non trivial since surgical access to wound is not risk free. Our objective is to modify stem cells to generate lineage specified CNS glia that 'home' into lesions, focusing on myelin repair for multiple sclerosis and spinal cord injury. Myelin protects neuronal axons, and in pre-clinical models ES-derived OL progenitor cell (OPC) grafts can regenerate myelin forming oligodendrocytes (OLs) and rescue the phenotype of myelin-defective mice. Our studies focus on engineering ES cells to expresss PDGFRα, the trans-membrane receptor for Platelet-Derived Growth Factor, which directs OPC migration during brain development. Prior work identified a 'rheostat' function for PDGFRα, with low levels of PDGF activating PI3K to in initiate migration and high levels activating PLCγ to inhibit migration. This predicts that PLCγ-uncoupled receptors would direct ES-derived OPCs toward PDGF infused brain lesions. We addressed the rheostat model by engineering mouse ES cells to express wild-type and signal transduction mutant versions of PDGFRα. First we generated a medium cocktail named '2i5S' that allows the the long-term expansion of mouse ES cell lines, which is optimal for our engineering experiments. Next we introduced PDGFRα transgenes into mES cells in vitro. A very low number of stable ES transformants were obtained after transfection, and this directed us towards using a set of vectors (TRE.PDGFRα) with tetracycline inducible promoter for control of transgene expression. Lastly, we used myelin mutant shiverer mice to evaluate the potential of ES-derived OPCs to generate myelin in vivo. Our results showed substantial improvements in both motor function and longevity in animals with 7% of ESwtderived cells present in the pup. Therefore this suggests that 7% ES-derived stem cell engraftment is the minimal level that is required for functional cell replacement therapy.M.S.Includes bibliographical references (p. 39-42)by Dorota Sadowsk

Basing on the example of Paw królowej [The Queen’s Peacock], the article presents the manner in which Dorota Masłowska problematizes her own situation and position in the world of (pop) culture after the spectacular success of her debut novel White and Red. The paper defines the category of vulnerability and uses it to determine the subject position of the author. Female characters in the book are treated as metaphorical incarnations of Dorota Masłowska, reflecting her emotions related to her vulnerable position
