Effectiveness of acupuncture as a nonpharmacological intervention in the treatment of chronic obstructive pulmonary disease
Antecedents: La malaltia pulmonar obstructiva crònica (MPOC) és una afecció d'alta prevalença que afecta el 12% de la població mundial; és causada per una inflamació crònica de les vies respiratòries i els pulmons. Aquesta malaltia té un impacte important en la qualitat de vida del pacient a causa de la dispnea progressiva, tos crònica i producció d'esput, sibilàncies, opressió al pit i poca capacitat d'exercici. Encara que s'utilitzen diverses estratègies de tractament farmacològic i no farmacològic per als pacients amb MPOC, el maneig dels símptomes és insatisfactori en molts pacients i per tant, es necessiten noves estratègies. En els últims anys, s'han publicat diversos estudis que suggereixen que les tècniques d'acupuntura podrien ser beneficioses per als pacients amb MPOC; però l'evidència encara no és clara. A més, s'han trobat limitacions en la qualitat de la informació de les intervencions d'acupuntura en estudis anteriors en diferents afectacions com la artrosis de genoll, malalties neurològiques o càncer. Per tant, és important avaluar la qualitat de la informació dels assajos d'acupuntura sobre la MPOC per garantir una adequada avaluació, reproducció i implementació dels resultats dels assajos clínics.
Mètodes: Aquest treball costa de tres publicacions incloent dues revisions sistemàtiques, que avaluen l'efectivitat de les tècniques d'acupuntura més comunament utilitzades, i una avaluació de la qualitat de la informació d'intervencions d'acupuntura en assajos de MPOC.
Resultats: Les nostres dues revisions sistemàtiques van revelar beneficis d'algunes tècniques d'acupuntura en el maneig de el tractament de la MPOC. L'evidència suggereix beneficis de l'acupuntura amb agulla filiforme per a la dispnea, la capacitat d'exercici i la qualitat de vida i possiblement la acupressió per a la dispnea, la qualitat de vida i l'ansietat. No obstant això, l'evidència actual està limitada pel risc de biaix, l'heterogeneïtat i el baix nombre d'assaigs existents.
Pel que fa a la qualitat de la informació de les intervencions d'acupuntura, observem una informació subòptima en diversos elements de la llista de verificació STRICTA, inclosa la profunditat d'inserció de l'agulla, el tipus d'agulla, el nombre de sessions de tractament, el entorn i el context de el tractament, la descripció de l'acupuntura i la justificació del control.
Discussió: La investigació sobre les tècniques d'acupuntura comparteix algunes de les dificultats d'altres intervencions no farmacològiques, inclòs el baix nombre de participants, l'heterogeneïtat de la seva pràctica i les dificultats en el procés de cegament. Els investigadors han de ser conscients d'aquestes limitacions al dissenyar assajos futurs i triar els tipus d'intervenció i control més adequats d'acord amb la seva pregunta de recerca específica.
Les deficiències en la qualitat de la informació és un altre tema que s'ha de millorar, tant pel que fa a la metodologia dels assajos com a la descripció de la intervenció. Aquesta manca d'informació no només dificulta l'avaluació de el risc de biaix de l'assaig, sinó que també dificulta l'avaluació i reproducció adequades de la de l'assaig.
Conclusions: Hi ha alguna evidència que l'acupuntura mitjançant agulla filiforme i la acupressió podrien ser útils en el tractament de la MPOC. Per altres intervencions, l'evidència és incerta. Calen investigacions futures de més qualitat amb millores en la qualitat de la informació per tenir estimacions més fiables dels efectes de la tècnica d'acupuntura en pacients amb MPOC i permetre una recomanació sòlida pel que fa a l'ús de l'acupuntura.Antecedentes: La enfermedad pulmonar obstructiva crónica (EPOC) es una afección de alta prevalencia que afecta al 12% de la población mundial y està causada por una inflamación crónica de las vías respiratorias y los pulmones. Aunque se utilizan varias estrategias de tratamiento farmacológico y no farmacológico para los pacientes con EPOC, el manejo de los síntomas es insatisfactorio en muchos pacientes y por tanto, se necesitan nuevas estrategias. En los últimos años, se han publicado varios estudios que sugieren que las técnicas de acupuntura podrían ser beneficiosas para los pacientes con EPOC; sin embargo, la evidencia aún no es clara. Además, se han encontrado limitaciones en la calidad de la información de las intervenciones de acupuntura en estudios anteriores en diferentes afecciones clínicas como la artrosis de rodilla, enfermedades neurológicas o cáncer. Por lo tanto, es importante evaluar la calidad de la información de los ensayos de acupuntura en EPOC para garantizar una adecuada evaluación, reproducción e implementación de los resultados de los estudios.
Métodos: Este trabajo costa de tres publicaciones incluyendo dos revisiones sistemáticas que evalúan la efectividad de las técnicas de acupuntura más comúnmente utilizadas, y una evaluación sobre la calidad de la información de intervenciones de acupuntura en ensayos de EPOC.
Resultados: Nuestras dos revisiones sistemáticas revelaron beneficios de algunas técnicas de acupuntura en el manejo del tratamiento de la EPOC. La evidencia sugiere beneficios de la acupuntura con aguja filiforme para la disnea, la capacidad de ejercicio y la calidad de vida y posiblemente la acupresión para la disnea, la calidad de vida y la ansiedad. Sin embargo, la evidencia actual está limitada por el riesgo de sesgo, la heterogeneidad y el bajo número de ensayos existentes.
Con respecto a la calidad de la información de las intervenciones de acupuntura, observamos una información subóptima en varios elementos de la lista de verificación STRICTA, incluida la profundidad de inserción de la aguja, el tipo de aguja, el número de sesiones de tratamiento, el entorno y el contexto del tratamiento, la descripción del acupunturista y la justificación del control.
Discusión: La investigación sobre las técnicas de acupuntura comparte algunas de las dificultades de otras intervenciones no farmacológicas, incluido el bajo número de participantes, la heterogeneidad de su práctica y las dificultades en el proceso de cegamiento. Para otras intervenciones, la evidencia es incierta. Se necesitan investigaciones futuras de mayor calidad con mejoras en la calidad de la información para tener estimaciones más confiables de los efectos de la técnica de acupuntura en pacientes con EPOC y permitir una recomendación sólida con respecto al uso de la acupuntura.Background: Chronic obstructive pulmonary disease (COPD) is a highly prevalent condition, affecting 12% of the global population; it is caused by chronic airway and lung inflammation. This disease has an important impact on patient quality of life by causing progressive dyspnoea, chronic cough and sputum production, wheezing, chest tightness and poor exercise capacity. Although several pharmacological and non-pharmacological treatment strategies are used for COPD patients, they only aim to reduce the progression of the disease and control symptoms as much as possible. However, symptom management is till unsatisfactory in many patients and therefore new strategies are needed. Acupuncture is a therapeutic technique that originated from traditional Chinese medicine. It uses several stimulation methods, most of them non-pharmacologic, and has been traditionally used to treat multiple diseases, including respiratory conditions such as COPD. In recent years, several studies have been published suggesting that acupuncture techniques could be beneficial to COPD patients; however, the evidence is still not clear. Moreover, limitations on the completeness of reporting of acupuncture interventions have been found in past trials in different clinical health conditions, such as knee osteoarthritis, neurological diseases or cancer. Therefore, it is important to assess the current completeness of reporting of acupuncture trials on COPD to ensure the proper assessment, replication and implementation of the results of clinical trials.
Methods: This work resulted in three publications on this topic, including two systematic reviews assessing the effectiveness of the most common used acupuncture techniques and an assessment on the completeness of reporting of acupuncture interventions in COPD trials.
Results: Our two systematic reviews revealed benefits for some acupuncture techniques in the management of COPD treatment. Evidence suggests benefits of filiform needle acupuncture for dyspnoea exercise capacity and quality of life and possibly for acupressure for dyspnoea, quality of life and anxiety. However, the current evidence is limited by the risk of bias, heterogeneity and low number of existing trials.
Regarding the completeness of reporting of acupuncture interventions, we observed suboptimal reporting in several items of the STRICTA checklist, including needling insertion depth, needle type, number of treatment sessions, setting and context of the treatment, description of the acupuncturist and control rationale.
Discussion: Although acupuncture techniques have been used for decades in China to treat lung diseases such as COPD, current evidence regarding its efficacy is still limited. Research into acupuncture techniques share some of the difficulties of other non-pharmacological interventions, including low number of participants, the heterogeneity of its practice and difficulties in the blinding process. Researchers must be aware of those limitations when designing future trials, and choose the most adequate intervention and control types according to their specific research question.
Deficiencies in acupuncture publications reporting is another issue that needs to be improved, both regarding the trials methodology and the intervention description. This lack of information does not only stifle the assessment of the trial’s risk of bias, but also hinders the adequate assessment and reproduction of acupuncture research. Reporting guidelines such as CONSORT and STRICTA must be used to guarantee proper reporting in the published papers.
Conclusions: There is some evidence that filiform needle acupuncture and acupressure could be helpful in the treatment of COPD. For other interventions, the evidence is unclear. Future large high-quality research with better reporting is needed to have more reliable estimates of acupuncture technique effects in COPD patients to allow solid recommendation regarding acupuncture use.Universitat Autònoma de Barcelona. Pragmatismo y efectividad en los ensayos clínicos en Terapia Manual : Un análisis metodológico
Aquesta tesi doctoral se situa en l'àmbit de la Teràpia Manual, un abordatge de la fisioteràpia i altres disciplines que aplica moviments passius portats a terme pel professional per alleugerir el dolor i millorar la funcionalitat en trastorns musculoesquelètics principalment. Malgrat que els seus beneficis s'associen a mecanismes de modulació neurològica i a la relació pacient-terapeuta, la complexitat intrínseca d'aquesta intervenció (que combina tacte, interacció, expectatives del pacient, factors contextuals i individualització) planteja reptes importants per a la seva avaluació amb criteris d'investigació rigorosos (per exemple, protocols estandarditzats, controls placebo o sham, homogenització de mostres i mesures de resultat fiables). L'objectiu principal és caracteritzar en profunditat les característiques metodològiques dels estudis d'"efectivitat" en Teràpia Manual, posant especial atenció al grau de pragmatisme del seu disseny, a la qualitat del seu report i al risc de biaix. Per fer-ho, s'han dut a terme tres revisions sistemàtiques: les dues primeres van analitzar tots els estudis que s'autodenominen "pragmàtics" al títol o al resum, i la tercera va ampliar l'anàlisi a assaigs d'efectivitat sense aquesta etiqueta. En cada estudi es va avaluar: Pragmatisme del disseny, amb l'eina PRECIS‑2, per mesurar fins a quin punt l'assaig s'acosta a la pràctica clínica real. Qualitat del report, seguint les guies CONSORT per a assaigs controlats aleatoritzats (intervencions no farmacològiques i pragmàtics). Risc de biaix, amb l'eina Cochrane Risk of Bias, per identificar possibles distorsions en l'execució, la selecció o la detecció de resultats. Aquest enfoc comparatiu (pragmàtics autodefinits vs. no etiquetats) explora fins a quin punt els investigadors adopten veritables dissenys pragmàtics, si justifiquen les seves decisions metodològiques i com reporten els seus procediments. També analitza l'evolució temporal d'aquests assaigs i reflexiona sobre la confusió freqüent entre els conceptes d'eficàcia (en condicions controlades) i efectivitat (en condicions reals). La tesi pretén oferir un full de ruta de les pràctiques actuals en investigació de Teràpia Manual i proposar recomanacions per adoptar procediments més coherents amb els principis del pragmatisme: justificar cada decisió de disseny segons la seva rellevància clínica, complir amb les guies de report, avaluar sistemàticament el risc de biaix i usar eines com PRECIS‑2 per planificar assaigs que realment informin la presa de decisions en la pràctica diària. D'aquesta manera, contribueix a millorar la reproductibilitat, la credibilitat i l'aplicabilitat dels estudis en aquest camp.Esta tesis doctoral se sitúa en el campo de la Terapia Manual, un abordaje de la fisioterapia y otras disciplinas que aplica movimientos pasivos ejecutados por el profesional para aliviar el dolor y mejorar la funcionalidad en trastornos musculoesqueléticos principalmente. Aunque sus beneficios se asocian a mecanismos de modulación neurológica y a la relación paciente-terapeuta, la complejidad inherente a esta intervención (que combina tacto, interacción, expectativas del paciente, factores contextuales y personalización) genera importantes retos a la hora de evaluarla con criterios de investigación rigurosos (por ejemplo, protocolos estandarizados, controles placebo o sham, homogeneización de muestras y medidas de resultado fiables). El objetivo principal del trabajo es caracterizar en profundidad las características metodológicas de los estudios de "efectividad" en Terapia Manual, atendiendo especialmente al grado de pragmatismo de su diseño, a la calidad de su reporte y al riesgo de sesgo. Para ello, se han realizado tres revisiones sistemáticas: las dos primeras se centraron en todos los estudios que se autodenominan "pragmáticos" en su título o resumen, y la tercera amplió el análisis a ensayos de efectividad sin esa etiqueta. En cada estudio se evaluó: Pragmatismo del diseño, mediante la herramienta PRECIS‑2, que mide hasta qué punto el ensayo se aproxima a la práctica clínica real. Calidad del reporte, siguiendo las guías CONSORT para ensayos controlados aleatorizados, para ensayos de intervenciones no farmacológicas y para ensayos pragmáticos. Riesgo de sesgo, con la herramienta Cochrane Risk of Bias, que identifica posibles distorsiones en la ejecución, la selección o la detección de resultados. Con este enfoque comparativo (pragmáticos autodefinidos versus no etiquetados) la tesis explora hasta qué punto los investigadores de Terapia Manual adoptan verdaderamente diseños pragmáticos, si justifican sus decisiones metodológicas y cómo reportan sus procedimientos. Además, se analiza la evolución temporal de estos ensayos y se reflexiona sobre la confusión habitual en la literatura entre los conceptos de eficacia (en condiciones controladas) y efectividad (en condiciones reales). Este trabajo busca ofrecer un mapa claro de las prácticas actuales de investigación en Terapia Manual y proponer recomendaciones para adoptar procedimientos más coherentes con los principios del pragmatismo: justificar cada elección de diseño en función de su relevancia clínica, cumplir con las guías de reporte, evaluar sistemáticamente el riesgo de sesgo y utilizar herramientas como PRECIS‑2 para apoyar la planificación de ensayos que realmente informen la toma de decisiones en la práctica diaria. Así, contribuye a mejorar la reproducibilidad, la credibilidad y la aplicabilidad de los estudios en este campo.This doctoral thesis is situated in the field of Manual Therapy, an approach to physiotherapy and other disciplines that applies passive movements executed by the professional to relieve pain and improve functionality in mainly musculoskeletal disorders. While the benefits of this intervention are linked to neurological modulation mechanisms and the patient-therapist relationship, the inherent complexity of the intervention (combining touch, interaction, patient expectations, contextual factors and personalisation) poses significant challenges when evaluating it using rigorous research criteria (e.g. standardised protocols, placebo or sham controls, homogenisation of samples and reliable outcome measures). The primary objective of this project is to provide a comprehensive characterisation of the methodological characteristics of 'effectiveness' studies in Manual Therapy. This will be achieved by focusing on the degree of pragmatism of their design, the quality of their reporting and the risk of bias. To this end, three systematic reviews were conducted: the first two focused on all studies that call themselves 'pragmatic' in their title or abstract, and the third extended the analysis to effectiveness trials without this label. In each study, we assessed the following: The pragmatism of the design using the PRECIS-2 tool, which measures how closely the trial approximates real clinical practice. The quality of the reporting was assessed using the CONSORT guidelines for randomised controlled trials, for trials of non-pharmacological interventions, and for pragmatic trials. We also assessed risk of bias using the Cochrane Risk of Bias tool, which identifies possible distortions in the execution, selection or detection of results. Using a comparative approach (self-defined pragmatic versus unlabelled pragmatic), the thesis explores the extent to which manual therapy researchers adopt pragmatic designs, justify their methodological decisions, and report their procedures. Furthermore, it analyses the temporal evolution of these trials and considers the common confusion in the literature between the concepts of efficacy (under controlled conditions) and effectiveness (under real-world conditions). This thesis aims to provide a clear overview of current research practices in manual therapy and propose recommendations for adopting procedures that are more consistent with the principles of pragmatism. These recommendations include justifying each design choice in terms of its clinical relevance, complying with reporting guidelines, systematically assessing risk of bias, and using tools such as PRECIS-2 to support the planning of trials that inform decision-making in daily practice. Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Variations on the Author
“Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship
Appropriate Similarity Measures for Author Cocitation Analysis
We provide a number of new insights into the methodological discussion about author cocitation analysis. We first argue that the use of the Pearson correlation for measuring the similarity between authors’ cocitation profiles is not very satisfactory. We then discuss what kind of similarity measures may be used as an alternative to the Pearson correlation. We consider three similarity measures in particular. One is the well-known cosine. The other two similarity measures have not been used before in the bibliometric literature. Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

We have done our best to complete the author checklist relating to the use of animals in the hut study. Note that the objective for the hut study was to evaluate the IRS treatment applications for residual efficacy against Anopheles mosquitoes, including the local An. coluzzii mosquito population. Cows were only used to attract mosquitoes into the huts and no tests were carried out directly on the cows. The author checklist is intended for use with studies where experiments are carried out on animals, which is why we have had such difficulty in completing this for the hut study, as many of the questions do not relate to how the cows were used
