    Multifocal Humeral Fractures: Clinical Results, Functional Outcomes and Flowchart of Surgical Treatment

    AIM AND OBJECTIVE: Multifocal fractures of the humerus are rare. The aim of our study is to evaluate the effectiveness of surgical treatment and propose a modification to the Maresca–Pascarella classification. A flowchart for surgical treatment is provided. MATERIALS AND METHODS: Thirty-one patients with multifocal humeral fractures were treated and evaluated. The Maresca–Pascarella classification was used. All were treated using with either plates and screws, external fixation or intramedullary nailing. Functional outcomes were evaluated using the QuickDASH test, the University of California – Los Angeles (UCLA) shoulder score and the Mayo elbow performance score (MEPS). RESULTS: There were 12 Type A, 17 Type B, 1 Type C and 1 of combined fractures of the proximal and distal epiphysis. Of the 31 patients, 5 were lost to the follow-up (FU), 1 died of pulmonary embolism (PE) and the remaining 25 had a mean FU of 19.8 (7–35) months. Three patients had radial nerve damage and 1 went to a non-union that required further surgical intervention. The mean QuickDASH score was 15.7, the average UCLA shoulder score was 26.3 and the mean MEPS elbow score resulted to be 83.0. CONCLUSION: Although multifocal fractures are severe injuries, patients are able to recover good functionality if treated judiciously. CLINICAL SIGNIFICANCE: We proposed a standardised surgical approach based on the fracture characteristics, site and a modified Maresca–Pascarella classification.     Giovanni Alfonso Borelli: The precursor of medial pivot concept in knee biomechanics

    A new philosophy of science and medicine had spread throughout the 17th-century Italy: the "Scientific Revolution." Giovanni Alfonso Borelli (1608-1679) was one of the most charismatic and brilliant scientists of his generation in Europe. He extended to biology the rigorous analytic methods developed by his indirect mentor Galileo in the field of mechanics. In his masterpiece " De Motu Animalium ," Borelli analyzed structure, motion, balance, and forces concerning almost all the principal joints of the human body, in static and dynamic situations. In particular, he accurately studied the anatomy and biomechanics of the knee joint. He sustained that femoral condyles shift backward during flexion, allowing a wider range of movement. Furthermore, he observed that, when the knee flexes, the lateral condyle moves backward more than the medial condyle: this concept is nowadays known as medial pivoting. The aim of this article is to describe the life and work of this important Italian scientist and to present his unrecognized contribution to modern knee biomechanics

    Il deficit di intrarotazione gleno-omerale (GIRD) è una condizione ubiquitaria nel Baseball professionistico, e si riscontra frequentemente già a livello giovanile. Si tratta di un adattamento biomeccanico della spalla al gesto del lancio che viene ripetuto molte volte e con grande intensità durante una partita di baseball e nel corso della stagione sportiva. Associandosi pressoché costantemente un guadagno in termini di rotazione esterna, il quadro risulta complessivamente vantaggioso per l'atleta in quanto in grado di aumentare la capacità di caricamento del braccio e di accelerazione della pallina. Oltre determinati valori di soglia, però, il deficit di intrarotazione predispone ad un aumentato rischio di infortuni alla spalla e, secondo alcuni Autori, al gomito. È importante quindi agire fin dalle categorie giovanili per mantenere questo adattamento inevitabile al di sotto dei valori di guardia: fondamentali sono il rispetto delle regolamentazioni vigenti in termini di limitazione dei lanci effettuabili a partita in base all'età del giocatore e dei giorni di riposo previsti, e l'applicazione sistematica delle pratiche fisioterapiche di stretching e rinforzo muscolare che si sono dimostrate ampiamente efficaci allo scopo

    Fratture pertrocanteriche: la validazione del test POSSUM

    Le fratture di femore prossimale e, tra queste, le fratture pertrocanteriche, colpiscono prevalentemente Pazienti anziani, favorite dalla presenza di osteoporosi. Il trattamento chirurgico con posizionamento di sistemi extramidollari (placca e viti) o endomidollari (chiodo e vite cefalica) è indicato in quasi tutti i casi. Il test POSSUM, molto usato in chirurgia generale e ortopedia, predice la mortalità e la morbidità entro 30 giorni dall’intervento. Lo scopo di questo elaborato è validare il test POSSUM nell’ambito specifico delle fratture pertrocanteriche. Lo studio sperimentale si è svolto calcolando lo score pre-operatorio dei Pazienti sottoposti a intervento nel corso dell’anno 2014 (dati ricavati dalle cartelle cliniche), confrontandolo poi con i risultati osservati (dati ricavati da interviste telefoniche). E' stato operato anche un confronto tra i risultati del test POSSUM e quelli ottenuti con la variante P-POSSUM

    Development of Adhesive Testing Equipment and Methodology to Study the Adhesive Setting Behaviour in Packaging Applications

    This thesis concerns the study of the gluing process of wrapping materials used in packaging and identifies a solution to meet the need to perform quantitative measurements on the quality of bonding. Starting from an analysis of the properties that a glue joint must guarantee and the type of tests that can be carried out, a commercial solution for a device capable to cover all the requirements has been searched. As another option, a special test bench has been studied which allows, with a single device, to perform these tests on different types of materials and adhesives. The project completed has significant advantages compared to the devices already present on the market and allows enormous flexibility and reconfigurability to meet the continuous evolution of the sector where new products are continuously developed. The technical solutions used are high performance solutions typical of automatic machines and this allows the simulation of real conditions of use even in high speed applications. The type of material handled (paper, cardboard or other) can have different formats, even the types of glue and applicators can be the most disparate and can be updated later. The starting configuration provides for the use of a hot melt applicator and a water based adhesive that can be used alternatively or even simultaneously. The HW and SW structure also offers high-level performance and wide flexibility. The operator interface has been designed to allow complete control of the parameters combined with ease of use. This device meets the laboratory needs of both manufacturers of basic materials such as paper and adhesives, as well as manufacturers of packaging machinery

    War powers: reforming the law, with case studies of US Military participation in the Persian Gulf and Haiti

    The thesis studies the U.S. legal framework on war powers, concluding that it has not allowed the President and Congress to work together on war powers issues. From the constitutional viewpoint, this lack of friendly environment arises from the fact that the Constitution shares war powers between the presidency and the Congress, producing the conflict. From a political viewpoint, this dispute is explained by the presidential willingness to use the war powers without congressional authorization. In addition, every time lawmakers have sued the president for violations of the War Powers Resolution, the judiciary has ruled that this is a political question. Legally speaking, this conflict could end in a constitutional conflict. Politically speaking, this dispute could also have an important impact on the role that the United States plays within the community of nations and on the U.S. national security goal of promoting democracy abroad.     Effectiveness and Safety of Oro-Dispersible Sildenafil in a New Film Formulation for the Treatment of Erectile Dysfunction: Comparison Between Sildenafil 100-mg Film-Coated Tablet and 75-mg Oro-Dispersible Film

    BACKGROUND: A new oro-dispersible film (ODF) formulation of sildenafil has been developed for the treatment of erectile dysfunction (ED) to overcome the drawbacks that some patients experience when taking the conventional film-coated tablet (FCT). AIM: To assess the effectiveness and safety of sildenafil ODF formulation in patients with ED who were using the conventional FCT. METHODS: From May 2017 through July 2017, 139 patients with ED were enrolled. Data from penile color-duplex ultrasound, medical history, hormonal evaluation, and patient self-administered questionnaires were collected. All patients were administered sildenafil 100-mg FCT for 4 weeks. Thereafter, they underwent a 2-week washout period and subsequently took sildenafil 75-mg ODF for 4 weeks. OUTCOMES: The International Index of Erectile Function (IIEF-15), Hospital Anxiety and Depression Scale (HADS), Patient Global Impressions of Improvement (PGI-I), and Clinician Global Impressions of Improvement (CGI-I) questionnaires were administered and severity of ED was classified as severe (IIEF-15 score ≤ 10), moderate (IIEF-15 score 11-16), or mild (IIEF-15 score = 17-25). RESULTS: All patients completed the final protocol. Differences in mean IIEF scores for erectile function, orgasmic function, sexual desire, and intercourse satisfaction were significantly in favor of sildenafil 100-mg FCT, whereas the mean score for overall satisfaction was in favor of sildenafil 75-mg ODF. A significant difference in changes in HADS score was found from washout to final follow-up (mean difference = -0.19; P < .01). For the ODF formulation, the median CGI-I score was 3.5 (interquartile range [IQR] = 2.5-4.5) and the median PGI-I score was 3.0 (IQR = 2.0-4.0). The median action time was 20.0 minutes (IQR = 15.0-30.0) and the median mouth time was 60.0 seconds (IQR = 30.0-120.0). CLINICAL IMPLICATIONS: The ODF formulation of a widely known drug, with the same safety and effectiveness of the FCT, was better appreciated by patients in overall satisfaction. STRENGTHS AND LIMITATIONS: This is the first clinical trial to assess the efficacy of a new formulation of sildenafil in patients with ED. The limitations of the study are related to the methodology used: it was not a case-control study and the patients were not drug-naïve for ED treatment. Therefore, only the "additional" side effects of the ODF formulation compared with FCT are reported. CONCLUSION: The new ODF formulation is as efficient and safe as the FCT formulation and offers a new choice of treatment to specialists for more precisely tailored therapy. The definitive version was published in Eos 85 (2004): 349,354, doi:10.1029/2004EO370001 .The Puerto Rico Trench, the deepest part of the Atlantic Ocean, is located where the North American (NOAM) plate is subducting under the Caribbean plate (Figure l). The trench region may pose significant seismic and tsunami hazards to Puerto Rico and the U.S.Virgin Islands, where 4 million U.S. citizens reside. Widespread damage in Puerto Rico and Hispaniola from an earthquake in 1787 was estimated to be the result of a magnitude 8 earthquake north of the islands [McCann et al., 2004]. A tsunami killed 40 people in NW Puerto Rico following a magnitude 7.3 earthquake in 1918 [Mercado and McCann, 1998].
