    Objective: Traditional nasoalveolar molding (NAM) requires steep learning curve for clinicians and significant compliance from parents. Nasal springs have been developed by the author to simplify presurgical nasal molding. This article presents the design, construction, and application of the spring. The treatment goal is to improve nasal deformity prior to primary repair in infants born with incomplete unilateral cleft lip with or without cleft palate. Method: The design, fabrication, and utility of the nasal spring are described. The spring has a simpler design and construction compared to a traditional NAM appliance. Participants: Two patients with incomplete unilateral cleft lip with and without cleft palate are presented. Interventions: The spring is constructed and delivered. The active arm of the spring can be 3-dimensionally (3-D) adjusted to mold the alar cartilage of the affected nostril. The spring does not require an oral plate for adherence as a traditional NAM appliance does, hence an oral impression is not needed. The spring is easy for clinicians to adjust. It also requires less compliance by parents. Main Outcome Measures/Results: The presurgical molding achieved by the use of a nasal spring improved surgical nasolabial aesthetic outcomes. Conclusion: The nasal springs are effective in reducing the initial cleft nasal deformity. This facilitates primary surgical cleft lip and nose correction and improves surgical outcomes in patients with incomplete unilateral cleft lip with or without cleft palate. </jats:sec

    The physics of H-Darrieus turbine starting behavior

    This paper provides a resolution to the contradictory accounts of whether or not the Darrieus turbine can self-start. The paper builds on previous work proposing an analogy between the aerofoil in Darrieus motion and flapping-wing flow mechanisms. This analogy suggests that unsteadiness could be exploited to generate additional thrust and that this unsteady thrust generation is governed by rotor geometry. Rotors which do not exploit this unsteadiness will not self-start. To confirm the hypothesis, unsteady effects were measured and then incorporated into a time-stepping rotor analysis and compared to experimental data for self starting wind turbines. When unsteady effects were included the model was able to predict the correct starting behaviour. The fundamental physics of starting were also studied and parameters that govern the generation of unsteady thrust were explored: namely chord-to-diameter and blade aspect ratios. Further simulation showed that the Darrieus rotor is prone to be locked in a deadband where the thrust is not continuous around a blade rotation. This discrete thrust is caused by the large variation of incidence angle during start-up making the Darrieus blade ineffective during part of the rotation. The results show that unsteady thrust can be promoted through an appropriate selection of blade aspect and chord-to-diameter ratios, therefore self-starting rotors may be designed. A new definition of self-starting is also proposed

    Purpose: This study aimed to test the applicability and effectiveness of the principles and informed consent form (ICF) template proposed by the Strategic Initiative for Developing Capacity in Ethical Review (SIDCER) across multiple clinical trials involving Thai research participants with various conditions. Methods: A single-center, randomized-controlled study nested with eight clinical trials was conducted at Thammasat University Hospital, Thailand. A total of 258 participants from any of the eight clinical trials were enrolled and randomly assigned to read either the SIDCER ICF (n = 130) or the conventional ICF (n = 128) of the respective trial. Their understanding of necessary information was assessed using the post-test questionnaire; they were allowed to consult a given ICF while completing the questionnaire. The primary endpoint was the proportion of the participants who had the post-test score of ≥80%, and the secondary endpoint was the total score of the post-test. Results: The proportion of the participants in the SIDCER ICF group who achieved the primary endpoint was significantly higher than that of the conventional ICF group (60.8 vs. 41.4%, p = 0.002). The total score of the post-test was also significantly higher among the participants who read the SIDCER ICF than those who read the conventional ICF (83.3 vs. 76.0%, p < 0.001). Conclusions: The present study demonstrated that the SIDCER ICF was applicable and effective to improve Thai research participants’ understanding of research information in diverse clinical trials.     Determination of potential drug-drug interactions in prescription orders dispensed in a community pharmacy setting using Micromedex and Lexicomp: a retrospective observational study

    Background Community pharmacists have a role in identifying drug-drug interactions (DDIs) when processing prescription orders and dispensing medications to patients. The harmful effects of DDIs can be prevented or minimized by using an electronic DDI checker to screen for potential DDIs (pDDIs). However, different DDI checkers have variable rates of detecting pDDIs. Aim To estimate the prevalence of pDDIs in prescriptions dispensed in a community pharmacy setting using two electronic DDI databases and to evaluate the association between the pDDIs and contributory factors. Method Eligible prescription orders dispensed by a community pharmacy chain in Qatar from January to July 2020 were included in this retrospective observational study. For each prescription, Micromedex and Lexicomp were simultaneously used to identify pDDIs, and the interactions categorized based on severity and risk rating. Results Seven hundred-twenty prescriptions met the inclusion criteria, of which Micromedex and Lexicomp respectively identified 125 prescriptions (17.4%) and 230 prescriptions (31.9%) as having at least one pDDI. Moderate strength of agreement was found between Lexicomp and Micromedex in identifying pDDIs (Cohen's Kappa = 0.546). Micromedex classified 61.6% of DDIs as major severity, while Lexicomp classified 30.8% as major severity. The number of concurrent medications per prescription was significantly and positively associated with pDDI. Conclusion This study demonstrates a high prevalence of pDDIs among prescriptions dispensed in a community pharmacy setting.     การกำหนดสภาวะที่เหมาะสมของเครื่องจักรเป็นปัญหาหนึ่งที่มีความสำคัญสำหรับกระบวนการผลิตเพราะว่าการกำหนดพารามิเตอร์ที่เหมาะสมช่วยให้การใช้งานเครื่องจักรที่มีอยู่ให้เกิดประโยชน์สูงสุด งานวิจัยนี้นำเสนอการวิเคราะห์แบบล้อมกรอบข้อมูลและทากุชิเพื่อกำหนดค่าพารามิเตอร์ที่เหมาะสมของเครื่องขอดเกล็ดปลาตะเพียนสำหรับกระบวนการผลิตอาหารแปรรูปจากปลา ได้แก่ ปลาแห้ง และ ปลาส้ม เริ่มจากกำหนดปัจจัยที่เกี่ยวข้องจำนวน 3 ปัจจัย ได้แก่ ความเร็วรอบ เวลา และความจุ ซึ่งแต่ละปัจจัยมีจำนวน 3 ระดับ และ ผลตอบสนองคือความเสียหายของปลา และประสิทธิภาพการขอดเกล็ดปลา ผลการศึกษาพบว่า ปัจจัยความเร็วรอบ เวลา และความจุ มีนัยสำคัญต่อผลตอบสนองทั้งสองค่า (P-value ≤ 0.05) โดยเมื่อเทียบกับผลการทดลองเดิมพบว่าความเสียหายของปลาลดลง 31.46% และประสิทธิภาพของการขอดเกล็ดเพิ่มขึ้น 24.47% นอกจากนี้วิธีที่นำเสนอมีประสิทธิภาพที่ดีเทียบกับวิธีอื่นในวรรณกรรม ดังนั้นวิธีที่นำเสนอนี้สามารถใช้เป็นวิธีการที่มีประสิทธิภาพวิธีหนึ่งสำหรับการแก้ปัญหาการกำหนดสภาวะที่เหมาะสมที่มีผลตอบสนองหลายค่าได้Determining the optimal parameters of a machine is an important issue in the manufacturing process because it can help optimize the use of existing machines. This paper presents a data envelopment analysis and the Taguchi method to find the optimal parameters of a fish scaling machine to produce processed foods from fish, including dried fish and pickled fish. Three related factors, including speed, time, and capacity, were considered; each factor had three levels, and the responses were fish scaling removal efficiency and fish damage. The results showed that speed, time, and capacity were significant for both responses (P-value ≤ 0.05). Compared to the original condition, the fish damage was decreased by 31.46% and the fish scaling removal efficiency was increased by 24.47%. In addition, the proposed method had good efficacy compared to other methods in the literature. Therefore, the proposed method can be used as an effective way for solving multi-response optimization problems in real-world applications
