Prospective ex-vivo study on thermal effects in human skin phototypes II, IV and VI: a comparison between the 808, 1064, 1210 and 1320-nm diode laser
Laser Assisted Skin Healing (LASH) was first introduced in 2001 by Capon and Mordon to prevent keloids and hypertrophic scars. LASH requires homogenous heating throughout the full thickness of the skin around the wound. Trois chercheurs, dont un Français, récompensés pour leurs travaux sur les lasers
Six ans après le physicien Serge Haroche, un autre Français, qui détient aussi la nationalité américaine, reçoit le prix Nobel de physique. Gérard Mourou, né en 1944 à Albertville (France), est professeur émérite à l’École polytechnique (Palaiseau, France) et ancien professeur à l’université du Michigan (Ann Arbor, États-Unis). Il partage la moitié du prix Nobel avec Donna Strickland, de l’université de Waterloo (Ontario, Canada). Née en 1959 à Guelph, au Canada, elle était alors étudiante à l’époque de ces travaux portant sur les lasers. Elle est la troisième femme seulement à être récompensée en physique, après Marie Curie en 1903 et Maria Goeppert-Mayer en 1963. L’autre moitié de cette récompense prestigieuse revient à l’Américain Arthur Ashkin, 96 ans, de l’université Cornell (Ithaca, État de New York, États-Unis). Arthur Ashkin est désormais la personne la plus âgée à voir été lauréat d’un prix Nobel, toutes catégories confondues. Ces trois chercheurs ont un point commun : ils manipulent la lumière et leurs recherches ont abouti à de nouvelles utilisations des lasers en médecine
Photodynamic Therapy 2021
Photodynamic therapy (PDT) is a light-based photochemistry process. The illumination of a photoactivatable molecule (also called photosensitizer) with visible or near infrared light produces reactive oxygen toxic species to destroy tumor cells. This treatment modality leads to highly targeted actions, because reactive oxygen species are produced only where light is applied. Light is not harmful, nor is the photoactivable molecule. Only the combination of three elements (photosensitizer, oxygen, and light) is required to induce photo-oxidation reactions. PDT has proven to be a promising modality in many medical applications including cutaneous condition, infectious diseases, and various cancers at different stages. Comparative studies on the photoreactivity, efficacy, and safety of depigmenting agents

This study aimed to compare different depigmenting agents—4-butylresorcinol; bakuchiol; tranexamic acid; ascorbyl glucoside; α-arbutin; and ascorbic acid—for photoreactivity; tyrosinase inhibition; and safety. Photoreactivity was assessed using the Reactive Oxygen Species assay. In vitro tyrosinase inhibition was compared, and cell viability was assessed in B-16V melanocytes to evaluate safety. Results showed 4-butylresorcinol, ascorbyl glucoside, and α-arbutin are non-photoreactive, while for ascorbic acid and bakuchiol it was not possible to reach conclusive results due to the lack of specificity of the ROS assay. 4-Butylresorcinol, acting as a competitive inhibitor, displayed potent tyrosinase inhibition, followed by ascorbic acid and bakuchiol. Both 4-butylresorcinol and bakuchiol reduced cell viability in a concentration-dependent manner. The insights obtained in this work support the development of depigmenting products by providing useful scientific guidance on the photostability, tyrosinase inhibitory efficacy, and skin safety of depigmenting agents.info:eu-repo/semantics/publishedVersio
Assessment of 1210 nm laser-diode system for keloid treatment : experimental and clinical studies
IntroductionLes cicatrices chéloïdes (CC) sont des pathologies de la cicatrisation cutanée entraînant des gênes fonctionnelles et esthétiques souvent invalidantes. Elles sont liées à un excès de fabrication et une désorganisation du collagène lié en parti à un dérèglement du TGFβ. Les lasers thermiques permettent d'améliorer la cicatrisation par le biais d'une modification de la réponse inflammatoire. En effet, une élévation de la température entre 45 et 53°C entraine une hyperexpression des HSP 70, responsables d’une modification de l’expression du TGFβ. Le laser diode 810nm a déjà prouvé son efficacité mais il est contre indiqué chez les sujets de phototypes foncés qui sont les plus à risque de développer des CC. L'objectif de ce travail était d'évaluer l'effet d’un laser thermique sur les cicatrices chéloïdes chez l’animal et dans le cadre d’une étude clinique.Détermination des paramètres su laserDes études sur des explants de peau, puis sur des sujets volontaires sains ont permis de montrer que la longueur d’onde 1210 nm était la plus adaptée dans cette indication car elle est peu absorbée par la mélanine. Le choix des paramètres du laser ont également été déterminés afin d’obtenir une élévation contrôlée de la température cutanée. En théorie, un tir de laser diode 1210 nm, 5.1W/cm² pendant 10 secondes permet d’élever la température jusqu’à 53°C.Mise au point d’un modèle animal de CCIl n’existe pas de modèle animal de CC permettant d’étudier l’effet d’un laser. Nous avons donc mis au point un modèle pour le besoin de l’étude. Des fragments de CC humaines comprenant le derme et l’épiderme ont été greffés chez 40 souris nudes. Une évaluation clinique et histologique a permis de confirmer la bonne intégration du greffon et la persistance de son caractère chéloïde pendant 4 mois.Etude du laser diode 1210nm chez un modèle animal de CCUne étude du laser 1210 nm a été réalisée sur notre modèle animal. Il a été réalisé des tirs de laser directement sur les greffons et des évaluations cliniques et histologiques ont permis de montrer l’absence d’effet indésirable. La mesure de la température cutanée au moment du tir de laser était de 45°C en moyenne.Etude du laser diode 1210nm après excision intra chéloïdienne : étude cliniqueParallèlement à l’étude animale, il a été réalisé une étude pilote visant à évaluer la faisabilité et la sécurité d’un protocole de traitement des cicatrices chéloïdes utilisant le laser diode 1210nm. Il était réalisé une excision intra cicatricielle, puis la suture était irradiée par le laser 1210nm pendant environs 10 secondes. 20 patients ont été inclus dans l’étude. L’objectif de suivi était de 2 ans, mais l’étude est toujours en cours. Jusqu’à présent, il n’a été noté aucun effet délétère du laser. 8 patients ont bénéficié d’injections de corticoïdes en raison de récidive de la chéloïde à 6 mois.ConclusionCe travail à permis de mettre au point un modèle animal original de cicatrice chéloïde permettant pour la première fois l’étude des lasers in vivo. L’utilisation du laser 1210nm, 5.1W/cm2, pendant 10 secondes à entrainé une élévation de la température cutanée jusqu’à 45°C en moyenne sans aucun effet délétère sur la CC chez la souris. L’utilisation du laser thermique diode 1210 nm après une résection intra chéloïdienne n’a montré aucun signe de toxicité chez l’homme. La température cutanée était de 48°C en moyenne à la fin du tir. Une étude à plus grande échelle reste encore à réaliser afin de démontrer une efficacité de ce traitement prometteur.Introduction Keloid scars (KS) are pathologies of skin healing causing often functional and aesthetic disturbances. They are linked to excess production and disorganization of collagen mostly due to TGF bêta; overproduction. The thermal lasers can improve healing through a modification of the inflammatory response. Indeed, a rise of temperature between 45 and 53 ° C leads to an overexpression of HSP 70, which causes a change in the expression of TGF bêta;. The 810nm laser diode has already proven to be effective but its wavelength cannot used on dark phototypes who are prone to developing KS.The objective of this study was to evaluate the effect of a thermal laser on keloids in animal and in the context of a clinical study.Determination of laser parameters Studies on skin explants, and on healthy volunteers have shown that 1210 nm was the most suitable wavelength for this indication because it is poorly absorbed by melanin. The laser parameters were also carefully determined in order to control the skin temperature elevation. For a 1210 nm laser diode, an irradiance of 5.1W/cm² for 10 seconds leads to a maximum temperature of 53 ° C.Development of a KS animal model. Since, there was no KS animal model of DC , an innovative one was developed for this specific study. Fragments of human KS including the dermis and epidermis were transplanted in 40 nude mice. A clinical and histological evaluation confirmed the successful integration of the graft. At 4 months, it was proven that the KS remained unchanged. Study of the 1210nm diode laser in an KS animal modelThe 1210 nm laser was evaluated on our KS animal model. Laser irradiation was performed on the grafts. Clinical and histological evaluations have shown no adverse reactions. A 45 ° C mean temperature was recorded during laser irradiation .Study of l the 1210nm diode laser after intrakeloid excision. Parallel to the animal study, a pilot study was performed to assess the feasibility and safety of a 1210nm diode laser irradiation. An intrakeloid excision was achieved; the suture was then irradiated for about 10 seconds. 20 patients were included in the study. The study is still ongoing. A 2-year follow up is scheduled. So far, it was noted that no harmful effects of the laser. 8 patients received injections of corticosteroids due to of keloid recurrence at six months.conclusionAn original KS animal model was developed for the first time to study of the in vivo effects of laser irradiation. It was demonstrated that a 1210nm diode laser (5.1W/cm², for 10 seconds) led to a 45°C skin temperature without any deleterious effect. The 1210 nm diode laser after intrakeloid excision showed no signs of toxicity in humans. In humans, the maximum temperature was 48 ° C +/- 3°C . A long term follow-up on a large series of patients is still necessary to confirm the effectiveness of this promising treatment

1950 nm diode laser-assisted vascular microanastomoses in reconstructive plastic surgery : experimental and clinical study
En chirurgie plastique, le succès des lambeaux libres et des replantations dépend largement de la qualité des microanastomoses vasculaires. Ce travail complète les bases scientifiques de la technique de microanastomoses assistées par laser diode 1950 nm par la mise au point d’une méthode standardisée et l’étude post-opératoire du débit sanguin sur des séries animales. Il pose les jalons des premières séries cliniques. Dans une première série animale (S1), 30 anastomoses terminoterminales carotidiennes et 30 anastomoses jugulaires sont effectuées par la technique laser diode 1950 nm chez des rats Wistar. Les paramètres lasers suivants sont utilisés: taille du spot 400 µm, temps 1s, puissance délivrée de 100 mW à 150 mW (carotides) et de 90 mW à 140 mW (veines jugulaires). Elles sont comparées à J0 puis après 1, 4 et 12 semaines postopératoires, à un groupe contrôle de 30 anastomoses artérielles et de 30 anastomoses veineuses dans les mêmes proportions réalisées à l’aide de la technique conventionnelle par fils. Deux autres séries animales S2 et S3 sont ensuite entreprises à l’aide des paramètres lasers mis en évidence dans l’étude S1. Une série S2 regroupant vingt microanastomoses terminoterminales carotidiennes de rats Wistar est réalisée par la technique laser (n=10, P=120mW, spot 400 μm, durée du spot 1s, 5 spots par paroi, fluence 95 J/cm²) et la technique conventionnelle par fils 10/0 (groupe contrôle, n=10). Pour les deux groupes, la carotide controlatérale non opérée sert de référence pour le calcul du débit sanguin par IRM de flux : Une séquence de positionnement, une séquence anatomique, une séquence angiographique et une séquence de flux sont réalisées un jour après l’opération puis après une, quatre et huit semaines. Une série S3 regroupant vingt microanastomoses terminoterminales jugulaires de rats Wistar est réalisée par la technique laser (n=10, P=110mW, spot 400 μm, durée du spot 1s, 4 spots par paroi, fluence 90 J/cm²) et la technique conventionnelle (groupe contrôle, n=10). De même la jugulaire controlatérale sert de référence dans les diverses séquences IRM. Au total, 40 actes microchirurgicaux comprenant 38 lambeaux libres et 2 replantations digitales sont réalisés à l’aide du laser diode 1950 nm. Les microanastomoses artérielles sont terminoterminales dans 36 cas et terminolatérales pour 4 autres. Les microanastomoses veineuses sont toutes terminoterminales. Les microanastomoses sont réalisées par un laser diode 1,95 µm après mise en place de 2-5 points de rapprochement. Les paramètres suivants sont utilisés : taille du spot 400µm, puissance 125mW, 4 à 8 spots sur chaque face, temps d’application (0,7-2s) suivant le diamètre des vaisseaux : la fluence varie de 70 à 200 J/cm². Les études du débit sanguin par les nouvelles techniques d’IRM de flux avec les paramètres maintenant standardisés mettent en évidence l’excellente perméabilité des anastomoses laser sur un modèle animal. Le taux de succès de cette première grande série clinique apparait excellent lorsque comparé à celui des grandes séries de la littérature. Les innovations technologiques devraient conduire à une utilisation plus large de cet outil au bloc opératoire.In the field of plastic surgery, the most important factor for successful free flap transfer and replantations is a well executed vascular microanastomosis. The aim of these studies is to complement the scientific basis of the 1950 nm diode laser assisted microanastomosis (LAMA) by standardising the technique and studying the postoperative blood flow in animal series. This work introduces the first clinical series. In the first animal series (S1), 30 end-to-end microanastomoses of the carotidis and 30 end-to-end microanastomoses of the external jugular were performed with a 1950 nm diode LAMA technique in Wistar rats. The following laser parameters were used: spot size=400µm, spot duration=1s, Power varying between 100 to 150 mW for arterial microanastomoses and between 90 to 140 mW for venous microanastomoses. They were compared at D0 and at 1, 4 and 12 weeks with 30 conventional arterial microanastomoses and 30 conventional venous microanastomoses.Two other animal series (S2 and S3) were performed using the standardised parameters of the initial studies. In the S2 series, LAMA was performed on a group of 10 carotidis on Wistar rats. Two 10/0 stay sutures and a standard laser tissue welding technique (P=120mW, spot size=400 μm, t=1s, 5 spots for each wall, fluence=95 J/cm²) were used (LAMA group). They were compared with a group of 10 conventional arterial anastomosis (CSMA group). A MRI-positioning sequence (Magnetic Resonance Imaging), an anatomical sequence, an angiographic sequence and a flow sequence were performed 1 day after operation and then after 1, 4 and 8 weeks. Similarly, the series S3 included 20 microanastomoses of the external jugular performed with LAMA (n=10, P=110mW, spot size=400 μm, spot duration= 1s, 4 spots for each wall, fluence 90 J/cm²) or the conventional technique (n=10). For the two groups, contralateral non-operated external jugular were used as control.In total, 40 clinical procedures, including 38 free flaps and 2 finger replantations have been performed with the LAMA technique. End-to-end arterial microanastomoses were performed in 36 cases and end-to-side in 4 cases. All venous microanastomoses were performed end-to-end. LAMA was performed with a 1950 nm diode laser after placement of 2-5 stitches. The following laser parameters were used: spot size=400 μm, spot duration=0.7-2s, 4-8 spots for each wall, power=125 mW, fluence=70-200 J/cm². The animal series S1 demonstrates excellent bloodless patency at fluences ranging from 90 to 100 J/cm² (Power between 110-130mW) for arterial LAMA, and between 80 and 95 J/cm² (Power between 100-120mW) for venous LAMA. The flow-MRI further demonstrates that 1950 nm diode laser-assisted microanastomoses performed with our standardised parameters is a consistent, reliable and reproducible technique. The success rate of the clinical series appears promising. Technical innovation will most likely lead to greater ease of use of the laser handpiece in the operating room
Femtosecond laser micromachining : perfused glomerular microfibre manufacture
Le nombre de patients en attente sur la liste de transplantation d'organe est croissant dû notamment à une augmentation de la durée de vie. L'ingénierie tissulaire, dans une optique de remplacement d'organes pourrait être une alternative à ce problème de pénurie d'organes. Cependant, la création d'un organe entier par ingénierie tissulaire est limitée par la complexité de son organisation et la vascularisation des tissus créés. La vascularisation permet en effet d’apporter l'oxygène et les nutriments nécessaires aux cellules et d'éliminer les déchets qu'elles génèrent. Sans vascularisation, les cellules ne peuvent survivre et nécrosent.Ce projet de thèse est un partenariat entre le laboratoire français BioTis - INSERM U1026 et le centre technologique d'optique et laser ALPhANOV autour de la bio-ingénierie tissulaire. L'objectif est de développer un procédé de fabrication et d'usinage pour créer un modèle de microfibres glomérulaires perfusables. Des microfibres cellulaires ont été réalisées au sein du laboratoire BioTis. Elles sont composées d’un cœur d’hydrogel de collagène et d’une paroi externe bicellulaire modélisant la barrière de filtration rénale. L’originalité de ce travail repose ensuite sur la création d’une lumière interne, à l’aide d’un faisceau laser à impulsions ultracourtes. Inspiré des procédés industriels d’usinages intravolumiques de matériaux transparents, les impulsions ultracourtes sont ici fortement focalisées dans le cœur de collagène pour créer un canal intravolumique via un phénomène de cavitation à l’intérieur des fibres sans impacter la couche externe de cellules.The demand for organ transplantation has rapidly increased during the past decades due to the increased incidence of vital organ failure and the greater improvement in post-transplant outcome. However, the unavailability of adequate organs for transplantation to meet the existing demand has resulted in major organ shortage crisis. Today, the emergence of regenerative medicine and more particularly, tissue engineering, appears as the best opportunity to effectively regenerate functional tissues and organs. However, these approaches face the 3D architectural complexity of a real organ’s system. More particularly, one of the most challenging issues when engineering tissues is the lack of an efficient method to produce blood vessel systems — the vascularization. Without nutrients and oxygen supply, cells die, and engineered tissues show cell necrosis.This project is a collaboration between the French laboratory of tissue engineering BioTis – INSERM U1026 and the technological centre in optics and lasers ALPhANOV. The aim of this project is to develop a model of perfusable glomerular microcapillary that would mimic glomerular filtration. Microfibres was crafted in the BioTis lab with a core made of collagen hydrogel and a peripheric cell bilayer. This thesis focuses on the creation of a channel within the collagen core using femtosecond laser processing. Following the principle of industrialized intra-volume laser microprocessing of transparent materials, ultra-short laser pulses were strongly focused inside collagen hydrogel to induce the formation of a channel through cavitation phenomena without affecting the cell bilayer
Optimizing photodynamic therapy for malignant pleural mesothelioma with dosimetry based on imaging
Le mésothéliome pleural malin (MPM) est un cancer au pronostic sombre, en lien avec des traitements décevants. Quand la chirurgie fait partie d’un traitement multimodal, il est essentiel de l’associer avec un traitement adjuvant local pour tuer les cellules tumorales résiduelles. Récemment, la thérapie photodynamique (PDT) intrapleurale intra-opératoire, après exérèse tumorale chirurgicale, est apparue comme un traitement prometteur. Son succès repose sur l’illumination la plus complète et homogène de la cavité pleurale, contrôlée par une dosimétrie de lumière. Celle utilisée aujourd’hui en pratique clinique aux Etats-Unis repose sur la dosimétrie ponctuelle de sept capteurs intrathoraciques, mais ne permet pas d’avoir un reflet de la lumière délivrée sur l’ensemble des surfaces pleurales. Mon projet de recherche se décline sur un axe clinique et un axe expérimental : 1) mettre en place un essai de clinique de phase II au CHRU de Lille, associant la PDT intrapleurale au traitement multimodal du MPM, 2) développer une méthode de dosimétrie de lumière peropératoire innovante par la caractérisation du dispositif lumineux et l’utilisation de l’imagerie.L’essai clinique a démarré en Février 2016, en utilisant la dosimétrie de référence avec les sept capteurs en suivant le protocole américain. Une baguette lumineuse innovante a été conçue au laboratoire, ainsi que le système de dosimétrie. Quatre patients ont bénéficié d’une pleurectomie/décortication suivie d’une PDT intrapleurale (avec le photosensibilisateur Photofrin®) et d’une chimiothérapie adjuvante, sans toxicité majeure. Concernant le projet expérimental, un profil d’illumination de la baguette lumineuse a été défini en combinant les mesures de puissance ponctuelles (Watt) et celles de pixel à partir d’une photographie digitale. Un coefficient d’atténuation effectif a été calculé : µeff = 0,705 cm-1. A l’aide d’un système de repérage spatial électromagnétique (TrackStar®), la position de la baguette lumineuse est connue à l’intérieur de la cavité pleurale en temps réel, et ses coordonnées spatiales sont projetées sur le TDM thoracique en 3D. Après avoir intégré le profil d’illumination de la baguette au système de repérage, un logiciel dédié permet la représentation spatiale sur imagerie de la dose cumulée de lumière délivrée. Ce travail a été réalisé et validé sur un fantôme de cavité thoracique intra opératoire, puis notre méthode de dosimétrie a été comparée à celle de référence.Les perspectives sont de pouvoir tester la faisabilité cette nouvelle méthode de dosimétrie chez l’homme et de développer des dispositifs lumineux innovants pour une meilleure standardisation de la PDT pour le MPM.Malignant pleural mesothelioma (MPM) is a cancer with a poor prognosis due to deceiving treatments. When surgery is part of a multimodal treatment for MPM, it is crucial to combine it with a local adjuvant treatment to kill residual tumour cells. Recently, intrapleural photodynamic therapy (PDT) after surgical resection has appeared to be a promising treatment for MPM. Its success relies essentially on the most complete and homogeneous illumination of the pleural cavity, monitored by light dosimetry. The dosimetry method used today in the United States, with intrathoracic probes collecting the light at 7 strategic locations, does not give information about the light delivered on the whole pleural surface. This research project has a clinical and experimental axis: 1) to set up a phase II clinical trial in Lille University Hospital, combining intrapleural PDT to surgery within a multimodal treatment, 2) to develop an innovative peroperative light dosimetry method by the characterization of a light device and use of imaging.Clinical trial has started in February 2016, with the seven probes dosimetry method of reference, following the American protocol. An innovative light wand was conceived within the laboratory, as well as the dosimetry sytem. Four patients have undergone pleurectomy/decortications, intrapleural PDT (with photosensitizer Photofrin®) and adjuvant chemotherapy, without major toxicity. Regarding the experimental project, an illumination profile of the light wand was defined using two complementary methods: power measurements (Watt) and pixel intensity from digital photography. An effective attenuation coefficient was calculated: µeff = 0.705 cm-1. With an electromagnetic spatial tracking system (TrackStar®), we localized in real time the position of the light wand within the pleural cavity and projected its spatial coordinates to the 3D CT scan images. After having inserted the illumination profile of the light wand to the tracking system, a dedicated software allowed the spatial representation on CT images of the cumulated light dose. This work has been completed on an intraoperative thoracic cavity phantom. Our dosimetry method was validated on this phantom and compared to the one of reference.The perspectives are to test the feasibility of this new dosimetry method in humans, and to develop alternative light devices for a better standardization of the intrapleural PDT technique for MPM
