    Pharmacokinetic and pharmacodynamic evaluation of daclatasvir, asunaprevir plus beclabuvir as a fixed-dose co-formulation for the treatment of hepatitis C

    Introduction: Many reports have evaluated the clinical efficacy and safety of the fixed-dose all-oral combination of daclatasvir, asunaprevir, and beclabuvir (DCV-TRIO), which was approved in Japan in December 2016 for the treatment of hepatitis C genotype (GT)-1 infection. Areas covered: This article reviews the pharmacodynamic and pharmacokinetic properties of the DCV-TRIO combination. The topics covered include data regarding the drug’s absorption, distribution, metabolism, excretion, and antiviral activity strategies. Its therapeutic efficacy and safety in GT-1 infection from phase 2/3 clinical trials are also discussed. Expert opinion: The ideal regimen for the treatment of Hepatitis C virus infection should be potent, pangenotypic, Ribavirin-free, safe, co-formulated, and affordable. Considering these characteristics, DCV-TRIO is neither pangenotypic nor potent enough against GT-1a, regardless of the presence or absence of cirrhosis. Other potential limitations of this regimen are its dosification (twice-daily), and the fact that since it includes a protease inhibitor, it is contraindicated in decompensated cirrhosis. For these reasons, it has only been approved in Japan, where more than 70% of the patients are infected with GT-1b. However, this co-formulation might still have a place in the treatment of non-cirrhotic patients infected with GT-1b provided that massive access to treatment is facilitated.Fil: Esposito, Isabella.     A presente dissertação tem como objetivo revelar alguns aspectos conceituais na formação da consciência mortal vistos pela ótica do teólogo moralista Marciano Vidal Garcia. Sem dúvida estes aspectos têm raízes no itinerário de sua vida pessoal e acadêmica, pois entendemos que a formação da consciência moral acontece no processo vital-histórico e acadêmico do individuo. Por isso, este trabalho fala no capítulo inicial sobre o caminho de reflexão teológica e vivencial realizado pelo autor. Sem dúvida são pertinentes seus estudos universitários e sua permanente preocupação em dialogar com temas relacionados com a Teologia Moral, processo em busca de uma melhor compreensão da consciência moral. Seu permanente diálogo teológico com os autores Bernahrd Haring e Afonso Maria de Ligório foi decisivo para a formação de seu paradigma sobre a consciência moral . A experiência acadêmica de Marciano Vidal em diversas universidades e sua experiência de vida mostraram em seus escritos novos paradigmas para melhor compreender o tema que analisamos. Undoubtedly these aspects are rooted in the itinerary of his personal and academic life, as well we understand that the formation of conscience happens in life-history and process of the individual academic. Therefore, this work speaks at beginning chapter on the path of theological reflection and experiential conducted by the author. Surely his university studies are relevant and his constant concern in dialogue with issues related to moral theology, process in search of a better understanding of the moral conscience. His ongoing theological dialogue with the authors Bernahrd Haring and Alphonsus Liguori was decisive in the formation of their paradigm about moral conscience. The academic experience of Marciano Vidal at various universities and their life experience showed in his writing new paradigms to better understand the issue we reviewed.     Waiting list registration and liver transplantation rates among patients with autoimmune liver disease: An 11-year trend analysis

    Objectives: We aimed to analyze waiting list and liver transplantation trends for autoimmune liver disease in Argentina.Methods: We designed a population-based analysis of waitlisted or liver transplanted patients for autoimmune hepatitis, primary biliary cholangitis or primary sclerosing cholangitis between 2006-2017 in Argentina. Cumulative incidence rates of waiting list registration and liver transplantation were standardized for age and sex using the 2000 WHO standard population. Joinpoint regression analysis was used to identify changes in waiting list and liver transplantation trends.Results: Waiting list registrations for autoimmune hepatitis, primary biliary cholangitis and primary sclerosing cholangitis represented 9.66%, 5.07% and 0.85%.Wait-listing rates for autoimmune hepatitis and primary biliary cholangitis increased significantly between 2006 and 2017, with an annual percentage change of 4.9% (95% CI 0.8 to 9; p=0.01) and 5.5% (95% CI 1 to 10.2; p=0.01) respectively. Patients with autoimmune hepatitis and primary biliary cholangitis aged 40-59 years showed the highest increase in waiting list registration rates. Waiting list registration trends for primary sclerosing cholangitis remained stable: annual percentagechange 1.4% (CI 95% -2.1 to 5; p=0.4). Liver transplantation for autoimmune hepatitis, primary biliary cholangitis and primary sclerosing cholangitis represented 10.69%, 5.99% and 3.64%. Liver transplantation trends remained stable in all autoimmune liver diseases.Conclusion: Increasing trends of waiting list registration of autoimmune hepatitis and primary biliary cholangitis may respond to higher diagnosis and early referral rates to liver transplant units. Nevertheless, the growing waitlisting of patients with autoimmune end-stage liver disease underlines an unmet need for effective therapy.Fil: Dirchwolf, Melisa.     La sátira menipea de Marciano (segunda mitad del siglo V d. C.) es una obra desestabilizadora y antididáctica. Reutiliza discursos e ideas previas de manera paródica para convertirlos en objeto de burla, atacando las convenciones y tradiciones pasadas, en un claro proyecto disolvente del saber y el discurso de la cultura clásica. En este contexto, estudiamos las ‘metamorfosis’ discursivas operadas por Marciano sobre las Metamorfosis de Apuleyo, su función y cómo esta contribuye al objetivo desestabilizador de la obra en su totalidad. En el diálogo entre ambos textos podemos encontrar una clave más de lectura para De nuptiis Philologiae et Mercurii.Martianus’ menippean satire (second half of 5th Century A.D.) is an antididactic, destabilizing work. The author reutilizes previous discourses and ideas in a parodic way, in order to transform them in an object of mockery, attacking through this resource past conventions and traditions, which evidences a clear project of dissolution of classical knowledge and discourse. In this context, we aim to study the discursive metamorphosis operated by Martianus on Apuleius’ Metamorphoses.     Modeling of channel patterns in short tidal basins

    We model branching channel patterns in short tidal basins with two methods. A theoretical stability analysis leads to a relationship between the number of channels and physical parameters of the tidal system. The analysis reveals that width and spacing of the channels should decrease as the slope of the bottom profile and the Shields parameter increase and as the mean water depth decreases. In general, the channel depth should halve at every bifurcation. These theoretical results agree well with the field data from the Dutch Wadden Sea. A numerical model based on Delft3D, a software system of WL/Delft Hydraulics, is used to simulate the time evolution of a channel network in a geometrically simplified basin of similar dimensions as the Wadden Sea basins. The resulting channel network displays a three-times branching behavior, similar to the three- to four-times branching patterns observed in the Wadden Sea. The simulated channel pattern satisfies the relation derived from the theoretical analysis. The results of this pattern analysis provide for additional validation of two-dimensional/three-dimensional process-based morphodynamic models of tidal basins.Hydraulic EngineeringCivil Engineering and Geoscience

    Un adecuado estudio de la teología moral cristiana requiere de una serie de conceptos y categorías que permitan al teólogo una correcta comprensión de la vida moral del hombre y una correcta valoración de los actos humanos. A partir de este principio, el propósito principal de esta monografía es realizar una presentación sistemática de una categoría que se presenta con un papel central en la teología moral postconciliar, la denominada opción fundamental, si bien, es aceptado que esta categoría nace en el seno de la psicología, en lo que ésta denomina como psicología de la decisión y la libertad, la teología moral católica la ha adoptado como categoría de base para hablar sobre la moralización de la vida del hombre, de este modo, encontramos una variedad de líneas en las que la opción fundamental ha sido desarrollada por los teólogos especialmente luego del Concilio Vaticano II, relacionada a categorías bíblicas particulares, partiendo desde la antropología teológica, para esto no basaremos en el desarrollo realizado por el teólogo Marciano Vidal como autor principal de nuestra investigación.An adequate study of christian moral theology requires a series of concepts and categories that allow the theologian a correct understanding of human moral life and a proper evaluation of human acts. Based on this principle, the main purpose of this monograph is to provide a systematic presentation of a category that plays a central role in post-conciliar moral theology, the so-called fundamental option. Although it is accepted that this category originates within psychology, in what it calls the psychology of decision and freedom, catholic moral theology has adopted it as a foundational category to discuss the moralization of human life. Thus, we find a variety of lines in which the fundamental option has been developed by theologians, especially after the Second Vatican Council, related to particular biblical categories, starting from theological anthropology. For this, we will rely on the development carried out by the theologian Marciano Vidal as the main author of our research.TeólogoPregrad

    Da pauta ao play: proposta metodológica para o planejamento e desenvolvimento de newsgames

    Tese (doutorado) - Universidade Federal de Santa Catarina, Centro de Comunicação e Expressão, Programa de Pós-Graduação em Jornalismo, Florianópolis, 2020A presente pesquisa científica acerca da utilização de newsgame parte da premissa de que esse estilo de jogo está ganhando espaço na academia e sendo apresentado como exemplo de nova linguagem dentro do jornalismo, conforme apontado por Bogost, Ferrari e Schwaizer (2010), Frasca (2003) e Santos e Seabra (2014). No que se refere ao mercado, Pereira e Adghirni (2011) evidenciam a migração de leitores dos meios tradicionais para as mídias on-line. Salaverría (2015), por sua vez, ressalta que esse movimento sinaliza uma ruptura do modelo de negócio do jornalismo e sugere que os profissionais redefinam os modos de informar, como uma alternativa para superar as dificuldades econômicas do jornalismo. Segundo Marciano (2016), ainda se trabalha pouco no processo de construção dos jogos jornalísticos. Essa percepção é apontada no diálogo entre o autor e professores de cursos brasileiros de jornalismo, mais especificamente nas disciplinas relacionadas à ética jornalística. A partir das análises e entrevistas, Marciano (2016) observou que os docentes viam como promissora a ideia de inserir, na ementa das disciplinas, jogos que auxiliassem na compreensão dos conteúdos. Porém, a carência de aporte conceitual e técnico para fazê-lo, bem como as limitações temporais, impediam que tal atividade fosse realizada. Tais apontamentos motivaram a continuação dos estudos sobre newsgames neste trabalho, pois, diante do cenário apresentado, faz-se necessário pensar além da transposição dos conteúdos para as novas plataformas: é preciso entendê-las, a fim de utilizar a potencialidade comunicativa dessas ferramentas para dar uma nova roupagem aos assuntos. Os jogos jornalísticos se apresentam como promissores nesse sentido e, no intuito de preencher essas lacunas, este trabalho se orienta pela seguinte questão problema: quais etapas devem ser seguidas para se desenvolver newsgames como mídia jornalística? Por essa ótica, tem-se como objetivo principal propor uma metodologia para o desenvolvimento de newsgames. O percurso metodológico do estudo tem sua natureza na pesquisa aplicada, sistematizada na metodologia ativa de aprendizagem, proposta por Moran (2013). Através de pesquisa exploratória, fez-se o levantamento bibliográfico sobre o tema, o que também auxiliou na formatação dos conceitos utilizados no estudo de caso das dez oficinas realizadas. A abordagem qualitativa, com a aferição desses dados, sustentou a proposição da metodologia para desenvolvimento de newsgames. Esta pesquisa traz como resultados: a complementação do conceito de newsgames ao identificar aspectos para diferenciá-los dos Jogos Embasados em Notícias (JEN); a proposição do Game Design Document para Newsgames (GDDN), um roteiro de desenvolvimento, com base no lide jornalístico; a estruturação de oficinas nas quais a metodologia possa ser aplicada juntamente com critérios para execução dessas oficinas, tais como duração, número de participantes, espaço e materiais utilizados. A partir das percepções nas oficinas, foi desenvolvida, ainda, uma proposta de manual com dez passos para produzir newsgames, além de promover reflexões acerca da prática da codificação no jornalismo. Fortalecer essa habilidade nos profissionais pode abrir novas oportunidades de negócios e de inserção em um mercado promissor como o dos Jogos Digitais, desde que seja mantido o cuidado de não se perder o foco na notíciaAbstract: The present scientific research on the use of newsgame starts from the premise that this style of game is gaining space in the academy and being presented as an example of a new language within journalism, as pointed out by Bogost, Ferrari and Schwaizer (2010), Frasca (2003 ) and Santos and Seabra (2014). With regard to the market, Pereira and Adghirni (2011) show the migration of readers from traditional media to online media. Salaverría (2015), in turn, points out that this movement signals a rupture in the journalism business model and suggests that professionals redefine ways of reporting, as an alternative to overcome the economic difficulties of journalism. According to Marciano (2016), there is still little work in the process of building journalistic games. This perception is pointed out in the dialogue between the author and professors of Brazilian journalism courses, more specifically in disciplines related to journalistic ethics. From the analyzes and interviews, Marciano (2016) observed that the teachers saw as promising the idea of inserting, in the subjects' menu, games that would help in the understanding of the contents. However, the lack of conceptual and technical support to do this, as well as the temporal limitations, prevented such activity from being carried out. Such notes motivated the continuation of studies on newsgames in this work, because, in view of the presented scenario, it is necessary to think beyond the transposition of the contents to the new platforms: it is necessary to understand them, in order to use the communicative potential of these tools to give a new look to matters. Journalistic games are promising in this regard and, in order to fill these gaps, this work is guided by the following question: what steps should be taken to develop newsgames as journalistic media? From this perspective, the main objective is to propose a methodology for the development of newsgames. The methodological path of the study has its nature in applied research, systematized in the active learning methodology, proposed by Moran (2013). Through exploratory research, a bibliographic survey on the theme was made, which also helped in the formatting of the concepts used in the case study of the ten workshops held. The qualitative approach, with the measurement of these data, supported the proposition of the methodology for the development of newsgames. This research brings as results: the complementation of the concept of newsgames by identifying aspects to differentiate them from Games Based on News (JEN); the proposal of the Game Design Document for Newsgames (GDDN), a development script, based on the journalistic lead; the structuring of workshops in which the methodology can be applied together with criteria for its execution of these workshops, such as duration, number of participants, space and materials used. From the perceptions in the workshops, a proposal for a manual with ten steps to produce newsgames was developed, in addition to promoting reflections about the practice of coding in journalism. Strengthening this skill among professionals can open up new business and insertion opportunities in a promising market such as the Digital Games, as long as care is taken not to lose focus on the news

    Clinical Characteristics, Endoscopic Findings, and Treatment Outcomes in Lymphocytic Esophagitis Compared With Eosinophilic Esophagitis

    Lymphocytic esophagitis (LyE) and eosinophilic esophagitis (EoE) are immune-mediated esophageal diseases. Clinical characteristics, endoscopic findings, and treatment outcomes of LyE were compared with EoE. Methods: This was an international retrospective study on adults enrolled at 3 centers in Europe. We recorded clinical characteristics and endoscopy findings at baseline and symptoms, histology, and endoscopy outcomes after treatment of patients with LyE and EoE. Results: Demographics, clinical presentation, comorbidities, and endoscopy findings were largely different in 35 patients with LyE compared with 59 patients with EoE. Proton pump inhibitor response was generally lower in LyE. Discussion: LyE is clinically different from EoE, but differences in treatment response need further investigation

    Estudio del microbioma intestinal y su impacto en la enfermedad por hígado graso asociado a disfunción metabólica (MAFLD): identificación de biomarcadores de diagnóstico, pronóstico y respuesta a la cirugía bariátrica

    Introduction: Non-invasive biomarkers are needed for metabolic dysfunction-associated fatty liver disease (MAFLD), especially for patients at risk of disease progression in high-prevalence areas. The microbiota and its metabolites represent a niche for MAFLD biomarker discovery. However, studies are not reproducible as the microbiota is variable. Objectives: We aimed to identify microbiota-derived metabolomic biomarkers that may contribute to the higher MAFLD prevalence and different disease severity in Latin America, where data is scarce. Methods: We compared the plasma and stool metabolomes, gene patatin-like phospholipase domain-containing 3 (PNPLA3) rs738409 single nucleotide polymorphism (SNP), diet, demographic and clinical data of 33 patients (12 simple steatosis and 21 steatohepatitis) and 19 healthy volunteers (HV). The potential predictive utility of the identified biomarkers for MAFLD diagnosis and progression was evaluated by logistic regression modelling and ROC curves. Results: Twenty-four (22 in plasma and 2 in stool) out of 424 metabolites differed among groups. Plasma triglyceride (TG) levels were higher among MAFLD patients, whereas plasma phosphatidylcholine (PC) and lysoPC levels were lower among HV. The PNPLA3 risk genotype was related to higher plasma levels of eicosenoic acid or fatty acid 20:1 (FA(20:1)). Body mass index and plasma levels of PCaaC24:0, FA(20:1) and TG (16:1_34:1) showed the best AUROC for MAFLD diagnosis, whereas steatosis and steatohepatitis could be discriminated with plasma levels of PCaaC24:0 and PCaeC40:1. Conclusion: This study identified for the first time MAFLD potential non-invasive biomarkers in a Latin American population. The association of PNPLA3 genotype with FA(20:1) suggests a novel metabolic pathway influencing MAFLD pathogenesis.Fil: Mascardi, María Florencia.
